Transanastomotic tube following surgery for congenital duodenal obstruction: its effect on nutrition and clinical outcomes.

PURPOSE: Congenital duodenal obstruction (CDO) is a common neonatal intestinal obstruction associated with prematurity, low birth weight, and additional anomalies. Postoperative enteral feeding delays, caused by proximal duodenal dysmotility and anastomotic edema, prolong the need for total parenteral nutrition (TPN) and the use of a central venous catheter (CVC). The transanastomotic tube (TAT) bypasses the dysfunctional segment for direct jejunal feeding. This study evaluated effects of TAT on outcomes, TPN/CVC requirements, complications, and hospitalization duration in neonates undergoing

A novel central line securement vest reduces line trauma and improves quality of life in patients with intestinal failure.

OBJECTIVE: We sought to assess the impact of a novel central line securement vest on the rate of line complications (trauma, infections, and replacements), and measures of quality of life (QOL) in pediatric patients with intestinal failure. METHODS: We enrolled patients at a single tertiary pediatric center. We randomized 23 intestinal failure patients to a vest (n = 12, vest group) or traditional securement methods (n = 11, control group). Eleven patients in the vest group and 10 patients in the control group completed the study. Rates of trauma, infections, and replacements were calculated

Vascular Preservation for Long-term Central Venous Access in Infants: The Facial Vein as an Underutilized Option.

Patients in the neonatal intensive care unit (NICU) may require long-term intravenous access for antibiotic administration, parenteral nutrition, resuscitation, and fluid support. In certain populations, particularly those with short bowel syndrome or with intestinal failure, preservation of vascular access is critical, as progressive loss of central venous access remains a major indication for intestinal transplantation and is associated with significant morbidity and mortality. The common facial vein has been sparsely described as an alternative route for central venous access and clinical d

Rare Pyloric Stenosis in Mitochondrial Disease Treated with Hand-Assisted Laparoscopic Pyloromyotomy.

Mitochondrial diseases are rare genetic disorders that affect the body's ability to produce energy at the cellular level. Because many organs rely heavily on mitochondrial function, these diseases can cause a wide range of symptoms involving the nervous system, muscles, heart, and gastrointestinal tract. Gastrointestinal complications are common and may include difficulty swallowing, gastroparesis, and intestinal motility disorders. Although pyloric stenosis has rarely been reported, mitochondrial dysfunction affecting gastrointestinal smooth muscle and enteric neuronal function provides a pla

Peripheral parenteral nutrition improves short-term nutritional status and inflammatory reaction after colorectal resection surgery: A randomized clinical trial.

BACKGROUND: Major colorectal surgery induces a substantial metabolic and inflammatory response, which contributes to postoperative catabolism and delayed recovery. This study aims to investigate the impact of early postoperative peripheral parenteral nutrition on inflammatory response and short-term nutritional status after colorectal resection compared with conventional saline treatment. METHODS: This is an open-label, randomized controlled superiority trial. Adult patients undergoing elective or emergency colorectal resection were allocated in a 1:1 ratio to receive either peripheral parent

Lipoprotein X as a source of analytical and interpretative interference in routine laboratory testing.

Lipoprotein X (LpX) is an abnormal lipoprotein-like particle occurring mainly in cholestasis, lecithin-cholesterol acyltransferase deficiency, post-transplant cholestatic liver dysfunction, hepatic graft-versus-host disease, and selected intravenous lipid emulsion or parenteral nutrition settings. Enrichment in phospholipids and unesterified cholesterol, paucity of cholesteryl esters and triglycerides, and usual absence of apolipoprotein B (apoB) challenge assumptions underlying routine laboratory testing. This review distinguishes analytical interference, in which LpX biases measurement proce

Choline and Choline-Related Metabolites in Pediatric Short Bowel Syndrome.

Background: Choline is an essential nutrient crucial for liver function. It is required for bile and lipoprotein secretion and the synthesis of both phosphatidylcholine (PC) to ensure tissue homeostasis and betaine as a methyl donor. Choline deficiency has been implicated in the pathogenesis of intestinal failure-associated liver disease (IFALD), with the strongest evidence for its contribution to hepatic steatosis in patients with short bowel syndrome (SBS). Contributing factors are (1) an impaired recycling of choline from bile PC, leading to fecal choline losses; (2) small bowel bacterial o

Hyperinsulinemia After Extensive Small Bowel Resection and Total Parenteral Nutrition in a Neonate: A Case Study.

BACKGROUND: Neonatal hyperinsulinemia is a rare condition, but poses a significant increase in the risk of recurrent and persistent hypoglycemia in newborns, which can lead to long-term neurodevelopmental complications if not identified early. This condition may be due to a congenital disorder, or arise transiently due to perinatal stress, and may develop as a consequence of interventions such as total parenteral nutrition (TPN). Infants diagnosed with SBO following extensive intestinal resection are particularly at higher risk. CASE PRESENTATION: This report is of a 2-week-old male neonate,

Preoperative underweight is associated with a more complicated perioperative course and impairs recovery in Hirschsprung's disease: The pivotal role of weight-for-age z-score.

BACKGROUND & AIMS: Low weight-for-age z-score (WAZ) is prevalent in children with Hirschsprung's disease (HD), yet its prognostic value for surgical outcomes is unclear. This study evaluated the impact of preoperative WAZ on perioperative course complexity and postoperative recovery in children undergoing pull-through surgery. METHODS: A retrospective cohort study was conducted on 256 children aged ≤10 years who underwent pull-through. Patients were stratified by preoperative WAZ: underweight (WAZ < -2), at risk of underweight (-2 ≤ WAZ < -1), and well-nourished (WAZ ≥ -1). Demographic, clini

Incidence and risk factors of refeeding syndrome in critically ill patients: A systematic review and meta-analysis.

BACKGROUND & AIMS: Refeeding syndrome (RFS) is characterized by electrolyte and metabolic disturbances that may occur following the initiation of nutritional support. Given the potential for these disturbances to contribute to clinically important complications, there is substantial interest in ensuring timely recognition of RFS and identifying patients at increased risk. This systematic review and meta-analysis aimed to systematically evaluate the incidence and risk factors of RFS in critically ill patients to support risk stratification and optimize nutritional management. METHODS: This stu
