IntroductionCoronary artery bypass graft (CABG) surgery is performed globally around 400,000 times annually. Despite its benefits, CABG can lead to complications, including chylothorax, a rare condition where chyle accumulates in the pleural cavity due to thoracic duct trauma. Currently, there are no international guidelines for traumatic chylothorax management post-CABG. This is the first systematic review to provide a comprehensive overview of the current state of management for chylothorax post-CABG.MethodsThis systematic review was conducted by searching EMBASE, Cochrane, Ovid and PubMed d

BACKGROUND AND AIMS: Intestinal strictures are a common and serious adverse event of inflammatory bowel disease (IBD). Endoscopic balloon dilation (EBD) is the first step in treatment to avoid surgery. The aim of this study was to assess the prevalence, risk factors, and clinical sequelae of difficult EBD.
METHODS: This retrospective, multicenter study included 861 patients with IBD treated with EBD due to strictures. Risk factors for difficult EBD, defined as ≥3 per year, were evaluated.
RESULTS: During a median (IQR) of 23.00 (14.00-42.00) months of follow-up, there were 392 (45.5%) easier

BACKGROUND: Nasoenteral tube (NET) use is common in critically ill patients but is associated with significant complications, including accidental dislodgement, malpositioning in the bronchial tree or mechanical failures, which can impede nutritional therapy. These complications often lead to adverse events that increase hospital stay, costs, and patient morbidity.
OBJECTIVE: This study aimed to reduce complications related to the placement and maintenance of NETs in critically ill patients using multifaceted strategies.
METHODS: We conducted an observational cohort study between February 20

Background: In recent years, progress in the field of care has made prematurity an increasingly frequent phenomenon. The premature newborn, due to incompetence, is often subjected to parenteral nutrition (PN) for prolonged periods, and there may be several complications associated with it, first and foremost metabolic complications. Methods: In particular, the aim of this study was to evaluate how specific risk factors and/or auxological parameters influenced plasma variations in calcium, phosphorus, and magnesium levels. This is because, although little analyzed in the past, these electrolyte

With advancements in medical technology, biochemistry, and clinical practices, the modern approach to total parenteral nutrition (TPN) has been focused on precision, safety, and the optimization of metabolic and nutritional parameters based on the patient's needs. In the last decade, TPN mixtures have been transitioning from a lifesaving intervention for patients unable to receive enteral nutrition to a highly specialized therapy aimed at improving clinical outcomes, reducing complications, and personalizing care. Total parenteral nutrition has attracted great interest, and its adaptation to t

BACKGROUND: Postoperative pancreatic fistula (POPF) presents a significant challenge to oral intake after pancreaticoduodenectomy (PD). Strategies such as enteral feeding via nasojejunal tube, feeding jejunostomy (FJ), and total parenteral nutrition (TPN) are commonly used to optimize postoperative nutrition. However, the routine use of FJ in PD remains controversial. This study assesses the effectiveness of enteral feeding versus TPN in the management of POPF.
METHODS: A prospective observational study was conducted on 100 patients undergoing classical PD at a tertiary care centre in eastern

This study investigates the effects of fat emulsion-based early parenteral nutrition in patients following hemihepatectomy, addressing a critical gap in clinical knowledge regarding parenteral nutrition after hemihepatectomy. We retrospectively analysed clinical data from 274 patients who received non-fat emulsion-based parenteral nutrition (non-fatty nutrition group) and 297 patients who received fat emulsion-based parenteral nutrition (fatty nutrition group) after hemihepatectomy. Fat emulsion-based early parenteral nutrition significantly reduced levels of post-operative aspartate aminotran

BACKGROUND: Intestinal volvulus arises from the twisting of an intestinal loop around a fixed axis. This condition is distinguished by both the obstruction and constriction of the intestinal lumen, as well as the entrapment of intestinal blood vessels. Consequently, the intestinal obstruction resulting from volvulus is often of the strangulating variety, which poses a risk for swift intestinal necrosis. Mesenteric tumors are a notable precipitant of volvulus. However, certain nontumor lesions originating from the mesentery can also lead to severe intestinal volvulus.

This study aimed to detect the causal association of overweight and obesity on severe COVID-19 infection with respiratory failure through a two-sample Mendelian randomization (MR) method based on the genome-wide association studies datasets. All genome-wide association studies summary data of exposures and outcome used in this study were obtained from the IEU database derived from Europeans. The study mainly used the inverse variance weighted method to test causal relationship. Simultaneously, MR-PRESSO and MR-EGGER were used to detect the pleiotropy, and sensitivity analysis was performed usi

Clinical nutritional support is recognized by Klinefner's Surgery as one of the four pivotal advancements in surgical practice during the 20th century. Surgeons regard clinical nutrition as a "life-saving" discipline, pivotal in preserving the lives of numerous critically ill patients and facilitating the success of many surgical procedures. Parenteral nutrition (PN) support serves as a crucial component of clinical nutritional therapy, while a range of complications associated with total parenteral nutrition (TPN) can significantly undermine the efficacy of patient treatment. Impaired intesti

BACKGROUND: Intestinal transplantation is the treatment for patients with irreversible intestinal failure and complications of parenteral nutrition. Five-year graft survival is only 56%, possibly due to an imbalance in immunosuppression, aiming to prevent rejection while maintaining protection against pathogens. Studying the graft's mucosal cell populations and regulation of donor and recipient cells posttransplantation offers a unique opportunity to address this (im)balance leading to rejection.
METHODS: We performed single-cell mRNA sequencing of longitudinally sampled ileal graft biopsies

A woman in her 20s presented to the emergency department with severe hypokalaemia, resulting in a need for high-dependency unit care. She had a 8-month history of effortless regurgitation of food and significant weight loss. She denied any nausea or retching. A CT angiogram, OGD, enteroscopy, barium meal, MRI abdomen and flexible sigmoidoscopy were normal. The diagnosis of rumination syndrome was suspected from the history and witnessed regurgitation; the diagnosis was confirmed by high resolution oesophageal impedance manometry. The patient was managed with total parenteral nutrition (TPN), b

INTRODUCTION: Chylothorax following esophagectomy is a relatively rare but significant complication, with incidences ranging from 1.1% to 3.8%. It typically arises from damage to the thoracic duct or associated lymphatic vessels during extensive lymph node dissection and thoracic surgery. Initial management usually relies on conservative approaches such as dietary modifications, drainage, and pharmacotherapy. If these methods are ineffective, surgical intervention becomes necessary. However, standardized guidelines for the optimal management of thoracic duct injuries are as yet lacking, with d
