Hermansky-Pudlak Syndrome Type 1 Presenting with Interstitial Lung Disease: A Report of a Rare Case from Saudi Arabia.

BACKGROUND Hermansky-Pudlak syndrome (HPS) is a rare autosomal recessive disorder characterized by oculocutaneous albinism, bleeding diathesis, and potential multi-organ involvement such as pulmonary fibrosis and granulomatous colitis. While its incidence is globally low, it may be under-reported, particularly in regions with high rates of consanguinity, such as the Middle East. By reporting this case, we aim to increase awareness of this condition as well as its association with ILD. CASE REPORT Herein, we present the case of a 48-year-old Saudi man of Arab descent with a 2-year history of pr

The Impact of Oxytocin on Stimulus Discrimination of Zebrafish Albino and Non-Albino Models.

Zebrafish have the ability, to a certain extent, to distinguish between different types of stimuli, including distinguishing between videos of conspecifics and non-conspecifics, a skill known as stimulus discrimination. In this study, we investigated the effects of oxytocin on this ability in albino and non-albino zebrafish models, focusing on the correlations between albinism, sensory deficiencies, and socio-emotional behaviors. Our hypothesis is based on the premise that oxytocin influences socio-emotional behaviors in zebrafish, with varying effects depending on phenotype (albino vs. non-al

Mutations in the albinism gene oca2 alter vision-dependent prey capture behavior in the Mexican tetra.

Understanding the phenotypic consequences of naturally occurring genetic changes, as well as their impact on fitness, is fundamental to understanding how organisms adapt to an environment. This is critical when genetic variants have pleiotropic effects, as determining how each phenotype impacted by a gene contributes to fitness is essential to understand how and why traits have evolved. Here, we characterized the effects of mutations in the oca2 gene, which underlie albinism and reductions of sleep in the blind Mexican cavefish Astyanax mexicanus, on larval prey capture. We found that when sur

Amblyopia in a Young Child With an Atypical Response to Treatment: A Case Report.

Amblyopia is a prevalent and treatable cause of visual impairment in children, often responding well to timely intervention. Clinicians should investigate potential subtle structural abnormalities when expected improvements are not achieved. We present the case of a three-year-old blonde male with decreased vision and high astigmatism, identified during routine screening. Initial visual gains with spectacle correction suggested isometropic amblyopia; however, limited improvement over time led to further investigation. Optical coherence tomography (OCT) revealed abnormal foveal contour, and lig

Primary Angle Closure in a 7-Year-Old Child: Evaluation and Findings With Conventional Ultrasound and Ultrasound Biomicroscope.

Primary angle closure (PAC) is rare in pediatric patients, typically associated with persistent hyperplastic vitreous, congenital cataracts, ocular surgery, retinopathy of prematurity, topical or oral medications, and ocular albinism. This case report describes the youngest documented case of unilateral subacute PAC in a seven-year-old female without these underlying conditions. The patient underwent evaluation using A-scan ultrasound and ultrasound biomicroscopy (UBM), revealing a shallow anterior chamber and a short axial length (20.45 ± 0.02 mm in the left eye). The angle closure was caused

Arcuate pattern of retinal ganglion cell axons in oculocutaneous albinism has implications for axon pathfinding.

Humans have a characteristic arcuate pattern of retinal ganglion cell (RGC) axons on the retina, indicating an unknown axon-repellent factor in the macula during development. This arcuate pattern has implications for diseases such as glaucoma. Albinism is an interesting group of conditions with loss of the radial foveal architecture (foveal hypoplasia) and abnormal RGC axon pathfinding at the optic chiasm. Here, we achieved optical coherence tomography imaging in a patient with oculocutaneous albinism type 1 (OCA1), which is challenging due to nystagmus. These scans showed a normal pattern of

Foveal Hypoplasia Grading with Optical Coherence Tomography: Agreement and Challenges Across Experience Levels.

Background/Objectives: The diagnosis and prognosis of arrested foveal development or foveal hypoplasia (FH) can be made using the Leicester grading system for FH and optical coherence tomography (OCT). In clinical practice, ophthalmologists and ophthalmic health professionals with varying experience consult patients with FH; however, to date, the FH grading system has only been validated amongst experts. We compare the inter-grader and intra-grade agreement of healthcare professionals against expert consensus across all grades of FH. Methods: Handheld and table-mounted OCT images (n = 341) wer

An 18-month-old girl with Vici syndrome: A case report study.

We report an 18-month-old female diagnosed with Vici syndrome, a rare congenital disorder characterized by developmental delay, albinism, cataracts, agenesis of the corpus callosum, hypotonia, and immunological anomalies. The patient, the second child of healthy first-cousin parents, presented with hypotonia at birth and subsequently developed bilateral cataracts, feeding difficulties, and multiple systemic manifestations, including kidney, bladder, and gallstones, as well as cardiomegaly and seizures. Genetic testing identified homozygous mutations in the EPG5 gene, confirming the diagnosis o

Periungual squamous cell carcinoma in childhood: a case report.

Bowen's disease (BD) is the second most common skin cancer worldwide, often linked to sun exposure, arsenic, and immunosuppression. Though rare in pediatrics, it is the third most common pediatric skin cancer, associated with conditions like albinism and immunosuppression. Recent years have seen an increase in Squamous Cell Carcinoma (SCC) cases among children, potentially related to immunosuppression and human papillomavirus (HPV) infection, particularly subtype 16. We present the case of an 11-year-old girl from Mexico City with a five-year history of a slowly growing, asymptomatic, dark-bro

Case report: Inflammatory bowel disease in Hermansky-Pudlak syndrome type 3 due to novel variant in HPS3.

BACKGROUND: Hermansky-Pudlak syndrome (HPS) is a rare autosomal recessive disorder with phenotypic and genetic heterogeneity, characterized by oculocutaneous albinism, bleeding diathesis, and other specific subtypes such as colitis. HPS3 is caused by biallelic mutations in HPS3. Patients with HPS3 have milder symptoms and were rarely reported to be involved in digestive disorders. CASE SUMMARY: We report a case of an 11-year-old male patient who experienced chronic diarrhea and abdominal pain for a duration of 1 year, in the absence of identifiable predisposing factors. Colonoscopy and histop

Unusual Autosomal Dominant Inheritance of Oculocutaneous Albinism Type 4 (OCA-4): Clinical and Functional Features From A Chinese Family.

Oculocutaneous albinism (OCA) is a complex genetic disorder characterized by reduced or absent pigmentation in the skin, hair, and eyes. Among the eight known subtypes, OCA-4 is caused by a mutation in SLC45A2, which plays a crucial role in melanin biosynthesis. While autosomal recessive inheritance is the most common pattern for all OCA subtypes, autosomal dominant cases are extremely rare. We report three patients from a Chinese family exhibiting autosomal dominant OCA-4. Clinical assessments evaluated pigmentation and ocular features in affected family members. Next-generation sequencing wa

A Narrowband 635 nm Autofluorescence Peak in Albino Mouse Eyes Found With Multi-Modal Imaging Reveals the Presence of Protoporphyrin IX in the Choroid.

PURPOSE: To investigate differences in fundus autofluorescence (AF) spectra of pigmented (C57Bl/6) and albino (Balb/c) mouse retinas. METHODS: AF spectra were measured with a scanning laser ophthalmoscope (SLO) with a high-resolution spectrometer. The action spectrum of a 635 nm AF "spike" in albino mice was measured to estimate the underlying absorption spectrum. Optical coherence tomography (OCT) and OCT angiography were used to determine the most likely depth location of the source of the AF feature. RESULTS: Two narrowband emission peaks centered at 635 nm and ∼705 nm were observed in th

Hermansky-Pudlak syndrome (HPS): a rare genetic interstitial lung disease.

Hermansky-Pudlak syndrome pulmonary fibrosis (HPS-PF) is a rare cause of genetic interstitial lung disease (ILD). A man in his 40s who has oculocutaneous albinism (OCA) since birth presented with respiratory failure to the intensive care department. On further evaluation, high-resolution CT was suggestive of non-specific interstitial pneumonia (NSIP). In view of the early age of presentation of ILD and OCA, genetic testing was done, which confirmed the diagnosis of HPS. He was treated with oxygen, steroids and antifibrotics. Lung transplantation is the definitive treatment option, and he is be

Thirteen-year-old Child Develops Squamous Cell Carcinoma Without Underlying Causes.

Histologically, only 1%-2% of excised skin tumors are malignant. Cutaneous squamous cell carcinoma is rare in children. Several cases have been reported in the literature. In most cases, there are underlying predispositions in children, such as xeroderma pigmentosum, albinism, and immunosuppression. Presently, the patient did not have any underlying medical conditions or a family history of cutaneous squamous cell carcinoma.

Multiplexed Assays of Variant Effect and Reclassification of TYR Variants in Chinese Patients with Oculocutaneous Albinism.

Oculocutaneous albinism type 1, caused by mutations in the tyrosinase gene TYR gene, is the most common form of albinism worldwide. Owing to the variants of uncertain significance (VUSs) classified by the American College of Medical Genetics and Genomics guidelines, patients who carry these VUSs do not receive a definitive molecular diagnosis. To clarify the pathogenicity of the VUS variants, we conducted multiplexed assays of variant effect to reclassify the TYR VUS variants. Protein expression, melanin production, enzyme activity, and subcellular localization were applied to 46 selected vari
