Neurofilament light chain (NfL) as a surrogate outcome measure for GM2 gangliosidoses.

BACKGROUND: The GM2 gangliosidoses (GM2) are ultra-rare neurodegenerative disorders caused by deficient hexosaminidase A and/or B activity, leading to lysosomal GM2 ganglioside accumulation. Disease onset ranges from infancy to adulthood, with earlier onset associated with more rapid progression. Neurofilament light chain (NfL), a sensitive marker of axonal injury, has been extensively investigated as a biomarker for neurodegenerative disorders, including GM2. METHODS: To evaluate its clinical utility as a biomarker for GM2, NfL was measured in patients with GM2 enrolled in a Phase 2b, multin

N-acetyl-L-leucine normalizes Transcription Factor EB activity by stereospecific bidirectional modulation in a HeLa cell model of Niemann-Pick disease type C.

Levacetylleucine (Aqneursa™), an acetylated derivative and pro-drug of L-leucine, is the only FDA-approved monotherapy for Niemann-Pick disease type C (NPC). Its acetyl group enables transport via monocarboxylate transporters, supporting blood-brain barrier penetration and efficient cellular uptake. Inside cells, levacetylleucine is metabolised by acylases, generating elevated levels of L-leucine that enhance mitochondrial bioenergetics and is thought to ameliorate lysosomal dysfunction indirectly. Here, we describe a direct effect of levacetylleucine on lysosomal regulation through modulation

Branched-chain amino acids and gut microbiota: coregulation and impact on neurological function via the gut-brain axis.

Diseases that cause neurological dysfunction, such as Parkinson's disease (PD), Alzheimer's disease (AD), and maple syrup urine disease (MSUD), among others, are characterized by complex and multifaceted etiologies. There is growing evidence that branched-chain amino acids (BCAAs), regulated by the gut microbiota, play a critical role in the development of the central nervous system (CNS) disorders. This review focuses on the potential role of branched-chain amino acid metabolism in regulating brain function and gut microbiota. First, we summarize the current understanding of BCAAs, encompassi

Biological Mechanisms Underlying the Cardiovascular Effects of Branched-Chain Amino Acids: A Proteome-Wide Mendelian Randomization Study.

BACKGROUND: Ischemic heart disease (IHD) is the leading cause of morbidity and mortality. Branched-chain amino acids (BCAAs) are associated with higher IHD risk, but the underlying biological pathways remain unclear. OBJECTIVES: This study aims to explore these pathways using 2-step proteome-wide Mendelian randomization. METHODS: We examined the associations between genetic proxies for BCAAs and 2922 proteins in the United Kingdom Biobank Pharma Proteomics Project, supplemented by a meta-analysis with data from deCODE to identify proteins associated with BCAAs. Next, we tested their effects

Decreased Nonpolar Amino Acids in Cerebrospinal Fluid of Alzheimer's Disease Patients Revealed by Very High-Field NMR Spectroscopy.

Alzheimer's disease (AD) is characterized by metabolic dysfunctions, making cerebrospinal fluid (CSF) an ideal biological matrix for characterizing brain metabolism. Very high-field 900 MHz nuclear magnetic resonance (NMR) spectroscopy was used to profile CSF metabolites from 47 patients (n = 27 AD, n = 20 non-AD). A total of forty-eight metabolites were identified and quantified. CSF concentrations of four nonpolar amino acids were reduced in AD: l-alanine (p = 0.001), l-valine (p = 0.02), l-leucine (p = 0.04), and l-phenylalanine (p = 0.04) (two-way ANOVA, pathology and sex as factors). Full-spectrum cannabidiol-rich oil modulates behavior and neurochemical alterations in a rodent model of maple syrup urine disease.

Maple Syrup Urine Disease (MSUD) is caused by a genetic mutation in the branched-chain α-ketoacid dehydrogenase complex, resulting to accumulation of branched-chain amino acids (BCAAs) that affect the central nervous system and cause neurochemical alterations and behavioral changes. In this line, full-spectrum cannabidiol (CBD)-rich oil has emerged as a potential therapeutic strategy. Therefore, this study aims to evaluate the effects of two doses of the compound full-spectrum CBD-rich oil in a BCAA-induced MSUD rat model, against behavioral, cholinergic, inflammatory, and oxidative stress par

Impact of estimated total blood volume on NT-proBNP response to angiotensin receptor-neprilysin inhibition in acute heart failure: Insights from the PREMIER study.

BACKGROUND: Sacubitril/valsartan (Sac/Val) reduces N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels in acute heart failure (AHF), particularly in patients with reduced ejection fraction. However, whether estimated total blood volume (TBV), calculated using anthropometric equations, is associated with heterogeneity in biomarker response remains uncertain. METHODS: This post hoc exploratory sub-analysis of the PREMIER randomized trial evaluated whether baseline estimated TBV was associated with heterogeneity in NT-proBNP reduction after Sac/Val compared with angiotensin-converting e

Metabolic, Endocrine, and Appetite Responses to Carbohydrate Compared with Niacin Ingestion During Exercise in Healthy Females and Males.

BACKGROUND: The substrates used during exercise may influence the postexercise response of systemic metabolites and hormones, along with the drive to refeed following exercise. However, to our knowledge, no study has assessed whether manipulating substrate metabolism during exercise independent of carbohydrate ingestion alters these responses. OBJECTIVES: The objective of this study was to assess the effects of niacin and carbohydrate ingestion during exercise on postexercise metabolite and hormone concentrations, and ad libitum energy intake. METHODS: Fifteen participants (5 females) perfor

Branched-chain amino acid metabolism and macrophage polarization: potential regulatory nodes and therapeutic targets in diabetes-associated metabolic inflammation.

T2DM is characterized by chronic low-grade metabolic inflammation, and imbalanced macrophage polarization is a key immunological mechanism contributing to insulin resistance (IR), pancreatic β-cell damage, and diabetes progression. Recent studies in immunometabolism suggest that BCAA metabolism regulates macrophage polarization in a context-dependent manner. Physiological BCAA catabolism supports M2-like oxidative metabolism and repair functions, whereas chronic overload of BCAA and its catabolic metabolites in diabetes may promote pro-inflammatory responses through multiple pathways. Background: Regular biochemical monitoring is essential for the safe and effective dietary treatment of inherited disorders of amino acid metabolism. However, conventional venous blood sampling may limit monitoring frequency and increase the burden on families. This study evaluated the feasibility and clinical usefulness of home-collected dried blood spot (DBS) samples analyzed using tandem mass spectrometry and additional chromatographic methods for dietary monitoring in inherited disorders of amino acid metabolism other than phenylketonuria. Methods: Between 2022 and 2025, children, adolesce

The gut microbiome-cardiometabolic axis: insights into obesity, type 2 diabetes, and hypertension.

Alterations in gut microbial ecology have been linked to obesity, type 2 diabetes (T2D), and hypertension, but their biological significance remains difficult to separate from diet, medication use, adiposity, and other host factors. We synthesize evidence on intestinal barrier dysfunction, microbial translocation, low-grade inflammation, and microbiota-derived metabolites as interconnected mechanisms across these disorders. SCFAs, bile acids, trimethylamine N-oxide, tryptophan derivatives, branched-chain amino acid metabolites, and phenylacetylglutamine influence epithelial function, immune ac

Ultra processed microbiome: Effects of dietary Stable Carbonyl adducts (SCars) on developing microbiota of mice.

Consumption of ultra-processed food (UPF) is increasing and is causally linked with noncommunicable disease onset, however specific compounds within UPFs that influence disease risk remain poorly defined. The acronym 'SCars', for Stable Carbonyl Adducts, is introduced herein as an umbrella term encompassing advanced glycation end-products (AGEs), advanced lipoxidation end-products (ALEs), and other carbonyl-derived modifications. These molecules arise as products of spontaneous carbonyl chemistry, a reaction accelerated by industrial food processing. Dietary SCars have been associated with met

Effects of Probiotics and Vitamin D Deficiency Correction on Clinical, Inflammatory, Metabolic, and Microbiota Profiles in Older Adults with Oral Lichen Planus: A Multi-Omics Study.

Oral lichen planus (OLP) is a chronic inflammatory oral disease associated with immune dysregulation and malignant transformation risk. Vitamin D and probiotics may modulate immune and microbial pathways involved in OLP. In this study, we evaluated their effects on clinical outcomes and multi-omics profiles in 25 adult OLP patients (median age: 68 years). Vitamin D-deficient patients received 2000 IU/day vitamin D3, and all participants received a probiotic blend (Limosilactobacillus reuteri LRE11, Lacticaseibacillus rhamnosus LR04, and Lacticaseibacillus casei LC04) for 16 weeks. The role of intestinal microbial metabolites in polycystic ovary syndrome: mechanistic insights and intervention prospects.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a prevalent endocrine-metabolic disorder among women of reproductive age, defined by the core features of hyperandrogenism, ovulatory dysfunction, and polycystic ovarian morphology. PCOS is frequently complicated by insulin resistance, obesity, dyslipidemia, and chronic low-grade inflammation, which substantially impair patients' reproductive health, metabolic homeostasis, and quality of life. Although the etiology of PCOS remains incompletely elucidated, accumulating evidence indicates that gut microbiota and their metabolites participate in the
