Interplay between age, APOE Ɛ4 and the metabolome in plasma and brain in Alzheimer's disease.

Age and the ε4 variant of the apolipoprotein E gene (APOE ε4) are two major drivers of Alzheimer's disease (AD). APOE is also the major determinant of longevity. How age and APOE interact in the development of AD is largely unknown. In this study we integrate metabolomics (N = 274,259) and proteomics (N = 54,219) data in plasma from the UK Biobank with the metabolomics (N = 514) and proteomics (N = 618) data in brain from the Religious Orders Study and the Rush Memory and Aging Project (ROSMAP) to understand the interplay of age, APOE ε4 and metabolome in the development of AD. We find that le

Thiamine-responsive maple syrup urine disease missed by newborn screen: A case report.

Maple syrup urine disease (MSUD) is a rare autosomal recessive metabolic disorder caused by a deficiency of the branched-chain α-ketoacid dehydrogenase (BCKAD) complex. It is classified into four subtypes: classic, intermediate, intermittent, and thiamine-responsive. We report a case of a female infant who presented with global developmental delay at 8 months of age. Plasma amino acid analysis revealed markedly elevated levels of leucine (1863 μmol/L), isoleucine (790 μmol/L), valine (1011 μmol/L), and alloisoleucine (427 μmol/L). The patient demonstrated marked improvement in biochemical mark

Valbenazine Sprinkle: An Alternative Formulation of Valbenazine for Oral Administration in Patients With Dysphagia.

Valbenazine is a vesicular monoamine transporter 2 (VMAT2) inhibitor approved for the treatment of tardive dyskinesia and Huntington's chorea. Dysphagia, pill aversion, and fear of swallowing are common in patients with these conditions. A new valbenazine sprinkle capsule, to be swallowed whole or the granule contents sprinkled onto soft food, was developed and approved based on the studies presented here. Study 1 evaluated the stability of valbenazine granules in soft foods. Recovery of valbenazine granules within 2 h after addition to applesauce, pudding, and yogurt was 95.6%-100.2%, indicat

Two Countries, One Metabolic Dilemma: Nutritional Management of Concurrent Maple Syrup Urine Disease and Type 1 Diabetes Mellitus.

Maple Syrup Urine Disease (MSUD) and Type 1 Diabetes Mellitus (T1DM) are two distinct metabolic disorders with unique dietary management requirements. While MSUD necessitates strict restriction of branched-chain amino acids (BCAAs), T1DM requires precise carbohydrate counting to maintain optimal glycemic control. We report two cases of patients diagnosed with both MSUD and T1DM, highlighting the challenges and strategies in dietary management. Case 1, a 5-year-old girl, was diagnosed with T1DM after presenting with hyperglycemia and metabolic acidosis, despite previously stable MSUD management

The gut-adipose/pancreas axis: a novel perspective on glycolipid metabolism dysregulation in MAFLD and T2DM pathogenesis.

Metabolic-associated fatty liver disease (MAFLD) and type 2 diabetes mellitus (T2DM) frequently co-exist on the pathological basis of dysregulated glucose and lipid metabolism, forming a bidirectional causal relationship. The upstream mechanisms underlying this association require further elucidation. Recent studies suggest that the interactive network comprising the "gut-adipose axis" and "gut-pancreatic axis" represents a core component of the comorbidity mechanism. This network initiates with gut microbiota dysbiosis, which alters short-chain fatty acids (SCFAs), lipopolysaccharide (LPS), b

Minocycline Protects Against Oxidative Stress in a Model of Maple Syrup Urine Disease.

Branched-chain amino acids (BCAA) leucine, isoleucine, and valine are metabolized by complex branched-chain ketoacids dehydrogenase (BCKDH). In Maple Syrup Urine Disease (MSUD), the BCKDH complex has its activity blocked by a genetic mutation, compromising the BCAA metabolism and leading to the accumulation of these BCAA, related to neurological damage in this disease. Thus, minocycline is a broad-spectrum antibiotic, bacteriostatic, and studies have shown benefits in neurodegenerative disease progression, like reduction of oxidative stress, inflammation, and downregulation of molecular pathwa

Functional archetypes in the human gut microbiome reveal metabolic diversity, stability, and influence disease-associated signatures.

BACKGROUND: Understanding the functional diversity of the gut microbiome is critical for elucidating its roles in human health and disease. While traditional approaches focus on taxonomic composition, functional configurations of the microbiome remain understudied. This study introduces a deep-learning framework combined with archetypal analysis to identify and characterize functional archetypes in the adult human gut microbiome. This approach aims to provide insights into interindividual variability, function-driven microbiome stability, and the potential confounding role of functional divers

From Crisis to Continuum: Redefining Survivorship in Neurometabolic Care.

Advances in newborn screening, molecular diagnostics, and targeted therapies have markedly improved survival for individuals with inborn errors of metabolism (IEM), allowing many to live well into adulthood. While some may remain asymptomatic, their underlying metabolic vulnerabilities persist and can be unmasked by illness, stress, or other triggers. As a result, a growing population of adults with IEM now faces unique and evolving challenges related to long-term disease management, continuity of care, and quality of life. Despite these shifting demographics, there remains limited literature

Transcriptomic and Metabolomic Mechanisms Underlying Adaptive Differentiation of Black Soldier Fly Larvae Induced by Regional Food Waste Domestication.

Black soldier flies (Hermetia illucens) are widely used in organic waste bioconversion, and their adaptive capacity to region-specific food waste is critical for efficient application. This study aimed to explore the molecular mechanisms underlying the adaptation of black soldier fly larvae to long-term domestication on regional food waste from Kunming and Qujing. Integrated transcriptomic and metabolomic analyses were performed to identify differences in gene expression and metabolite profiles between the two groups of larvae. The results showed significant divergence in gene expression netwo

Multi-matrix metabolomics in rare monogenic diabetes syndromes: Analysis of oral fluids and serum in carriers of pathogenic variants in the ALMS1/BBS genes.

Metabolomic profiling enables the identification of specific biochemical alterations in various diseases, including rare monogenic diabetes and obesity syndromes such as Alström syndrome (ALMS) and Bardet-Biedl syndrome (BBS). These disorders are characterized by early-onset obesity, insulin resistance, diabetes mellitus, retinodystrophy and other symptoms, but some features may also occur in heterozygous carriers of pathogenic or likely pathogenic variants in ALMS1 and BBS genes. The aim of the study was to compare the metabolomic profiles of saliva, gingival crevicular fluid (GCF) and serum

Adenine base editing rescues disrupted BCKDH function and reduces BCAAs toxic accumulation in maple syrup urine disease patient iPSC-hepatic organoids.

BACKGROUND: Maple syrup urine disease (MSUD) is an inherited metabolic disorder caused by a deficiency in the activity of the hepatic branched-chain α-ketoacid dehydrogenase (BCKDH) complex, which leads to the toxic accumulation of three branched-chain amino acids (BCAAs) and their respective α-ketoacid, resulting in severe neurotoxicity, coma and even death without effective therapeutic measures. METHODS: In this study, we established the patient induced pluripotent stem cells (iPSC)-derived hepatic organoids (HOs), analyzed the characteristics, and applied adenine base editor (ABE8e) to cor

Cinnamic acid improves insulin sensitivity in prediabetic mice through branched-chain amino acid metabolic reprogramming.

BACKGROUND: Insulin resistance is a key pathological feature in the early stages of type 2 diabetes mellitus (T2DM), often associated with obesity and metabolic dysfunction. Cinnamic acid (CA) is a natural compound found abundantly in cinnamon and other dietary sources, has demonstrated antidiabetic potential. However, its preventive role in improving insulin sensitivity during the prediabetic stage and the underlying mechanisms remain poorly understood. METHODS: This study evaluated the preventive effects of cinnamic acid in a high-fat diet (HFD)-induced prediabetic mouse model. Male C57BL/6

Dietary amino acid intake and the risk of hypertension: a systematic review and meta-analysis.

INTRODUCTION: Hypertension (HTN) is a major global health burden. While dietary protein is a known modifiable risk factor, the role of specific amino acids remains controversial. This systematic review and meta-analysis investigates the association between dietary amino acid intake and the risk of hypertension. METHODS: Three electronic databases (PubMed, Scopus, Web of Science) were systematically searched from inception until September 30, 2025 for observational studies examining dietary amino acid intake and hypertension. Data extraction and quality assessment were performed. Where applica

Low-dose mixtures of dietary nutrients ameliorate behavioral deficits in multiple mouse models of autism.

Autism spectrum disorder (ASD) is a group of heterogeneous, behaviorally defined neurodevelopmental conditions influenced by both genetic and environmental factors. Here, we show that supplementation of multiple low-dose nutrients-an important environmental factor contributing to ASD-can modulate synaptic proteomes, reconfigure neural ensembles, and improve social behaviors in mice. First, we used Tbr1+/- mice, a well-established model of ASD, to investigate the effect of nutrient cocktails containing zinc, branched-chain amino acids (BCAA), and serine, all of which are known to regulate synap

Sniffing Out Skin Disease: Odors in Dermatologic Conditions.

Although dermatology relies primarily on sight and touch, clinicians often overlook smell despite encountering diagnostic odors. We reviewed PubMed and Google Scholar for articles on disease-related cutaneous odors. Bacterial and fungal infections emit characteristic malodors, while genetic and metabolic disorders (eg, Darier disease, trimethylaminuria, phenylketonuria, maple syrup urine disease) and organ dysfunction (renal and hepatic failure) produce unique scents. Preliminary evidence also suggests melanoma may have a distinct odor detectable by canines. Although objective odor measurement

Microbiome-mediated crosstalk between T2DM and MASLD: a translational review focused on function.

Type 2 diabetes mellitus (T2DM) and metabolic dysfunction-associated steatotic liver disease (MASLD) frequently co-occur and aggravate one another through shared pathways of insulin resistance, low-grade inflammation and disordered lipid handling. Framing their interaction through the gut-liver-pancreas axis, this review synthesizes recent progress with a function-first emphasis, moving beyond taxonomic lists to the microbial outputs most consistently linked to dual metabolic-hepatic endpoints. We summarize how short-chain fatty acids (SCFAs), bile acids (BAs), lipopolysaccharide (LPS) and oth

A risk-based, QTPP-driven framework for semi-solid extrusion 3d printing of personalized medicines: Integrating hospital compounding and clinical trial regulation.

Semi-solid extrusion (SSE) 3D printing enables precise, patient-specific oral medicines, yet its uptake is constrained by fragmented regulatory pathways. This study develops an applied, risk-based framework grounded in empirical data from hospital compounding and early-phase clinical trial implementation, integrating both within a unified regulatory model through the concept of the printable ink as a pharmaceutical intermediate. Drawing from three pediatric use cases involving 5 different active pharmaceuticals ingredients (APIs), trimethoprim-sulfamethoxazole (Bactrim®), cyclophosphamide, tam

Plasma amino acid profile in obese children, adolescents, and adults: similarities or differences - a narrative review.

Obesity, regardless of age, is associated with various metabolic disorders. Amino acids, as essential food ingredients, play a critical role in regulating glucose and energy metabolism. Thus, their plasma concentrations may reflect the metabolic state of the individual. This narrative review aims to find out similarities in plasma amino acid profiles across the selected obese individuals (children, adolescents, and adults). An analysis of 21 original studies revealed similar amino acid alterations regardless of the age group- in particular, elevated levels of branched-chain amino acids (BCAAs:
