Minocycline Protects Against Oxidative Stress in a Model of Maple Syrup Urine Disease.

Branched-chain amino acids (BCAA) leucine, isoleucine, and valine are metabolized by complex branched-chain ketoacids dehydrogenase (BCKDH). In Maple Syrup Urine Disease (MSUD), the BCKDH complex has its activity blocked by a genetic mutation, compromising the BCAA metabolism and leading to the accumulation of these BCAA, related to neurological damage in this disease. Thus, minocycline is a broad-spectrum antibiotic, bacteriostatic, and studies have shown benefits in neurodegenerative disease progression, like reduction of oxidative stress, inflammation, and downregulation of molecular pathwa

From Crisis to Continuum: Redefining Survivorship in Neurometabolic Care.

Advances in newborn screening, molecular diagnostics, and targeted therapies have markedly improved survival for individuals with inborn errors of metabolism (IEM), allowing many to live well into adulthood. While some may remain asymptomatic, their underlying metabolic vulnerabilities persist and can be unmasked by illness, stress, or other triggers. As a result, a growing population of adults with IEM now faces unique and evolving challenges related to long-term disease management, continuity of care, and quality of life. Despite these shifting demographics, there remains limited literature

Adenine base editing rescues disrupted BCKDH function and reduces BCAAs toxic accumulation in maple syrup urine disease patient iPSC-hepatic organoids.

BACKGROUND: Maple syrup urine disease (MSUD) is an inherited metabolic disorder caused by a deficiency in the activity of the hepatic branched-chain α-ketoacid dehydrogenase (BCKDH) complex, which leads to the toxic accumulation of three branched-chain amino acids (BCAAs) and their respective α-ketoacid, resulting in severe neurotoxicity, coma and even death without effective therapeutic measures. METHODS: In this study, we established the patient induced pluripotent stem cells (iPSC)-derived hepatic organoids (HOs), analyzed the characteristics, and applied adenine base editor (ABE8e) to cor

Sniffing Out Skin Disease: Odors in Dermatologic Conditions.

Although dermatology relies primarily on sight and touch, clinicians often overlook smell despite encountering diagnostic odors. We reviewed PubMed and Google Scholar for articles on disease-related cutaneous odors. Bacterial and fungal infections emit characteristic malodors, while genetic and metabolic disorders (eg, Darier disease, trimethylaminuria, phenylketonuria, maple syrup urine disease) and organ dysfunction (renal and hepatic failure) produce unique scents. Preliminary evidence also suggests melanoma may have a distinct odor detectable by canines. Although objective odor measurement

Acrodermatitis dysmetabolica: lessons from two pediatric cases.

OBJECTIVES: Acrodermatitis dysmetabolica (AD) is a dermatologic manifestation associated with inherited metabolic disorders (IMDs), distinct from acrodermatitis enteropathica, which occurs solely due to zinc deficiency. CASE PRESENTATION: This report presents two pediatric cases: a 30-month-old girl with maple syrup urine disease (MSUD) experiencing AD secondary to severe isoleucine deficiency due to a protein-restricted diet, showing improvement with dietary adjustments, and a 2.5-month-old boy infant with propionic acidemia (PA) who developed AD alongside septic shock, which progressed desp

A risk-based, QTPP-driven framework for semi-solid extrusion 3d printing of personalized medicines: Integrating hospital compounding and clinical trial regulation.

Semi-solid extrusion (SSE) 3D printing enables precise, patient-specific oral medicines, yet its uptake is constrained by fragmented regulatory pathways. This study develops an applied, risk-based framework grounded in empirical data from hospital compounding and early-phase clinical trial implementation, integrating both within a unified regulatory model through the concept of the printable ink as a pharmaceutical intermediate. Drawing from three pediatric use cases involving 5 different active pharmaceuticals ingredients (APIs), trimethoprim-sulfamethoxazole (Bactrim®), cyclophosphamide, tam

Insights from Metabolomics Profiling of MSUD in Pediatrics Toward Disease Progression.

BACKGROUND: Maple syrup urine disease (MSUD) is a genetic disorder caused by mutations in the branched-chain α-ketoacid dehydrogenase (BCKDH) complex, leading to toxic buildup of branched-chain amino acids (BCAAs) and their ketoacid derivatives. While newborn screening (NBS) and molecular testing are standard diagnostic tools, they face challenges such as delayed results and false positives. Untargeted metabolomics has emerged as a complementary approach, offering comprehensive metabolic profiling and potential for novel biomarker discovery. We previously applied untargeted metabolomics to neo

A colorimetric strategy for quantifying amino acids using E. coli auxotrophs displaying gold-binding proteins.

We present a cell-based colorimetric assay for amino acid quantification using Escherichia coli auxotrophs engineered to grow exclusively in the presence of their corresponding target amino acids and to display gold-binding proteins (GBPs) on their surface. Upon the addition of gold nanoparticles (AuNPs), the surface-expressed GBPs mediate AuNP aggregation, resulting in a distinct color shift from pink to blue. This approach enables the detection of ten essential amino acids through characteristic colorimetric changes and allows for quantitative analysis via absorbance measurements with high s

Neuroprotective, antioxidant and anti-inflammatory effect of carnitine in patients with Maple syrup urine disease: branched-chain amino acids and branched-chain keto acids levels.

Maple syrup urine disease (MSUD) is a genetic disorder of the metabolism of branched-chain amino acids (BCAAs). We investigate the effects of treatment with dietary restriction and carnitine supplementation in MSUD patients. During treatment, patients were placed on a restricted diet and received a semi-synthetic formula enriched with carnitine. Our results revealed that treated patients showed elevated levels of neurodegeneration biomarkers (BDNF and PDGF-BB), while levels of TBARS and isoprostanes, indicators of lipid oxidative damage, were significantly decreased compared to diagnostic grou

Design and pharmacodynamic study of live biotherapeutic products with efficient degradation of branched-chain amino acids.

The homeostasis of branched-chain amino acids (BCAAs) plays a crucial role in maintaining health, and the accumulation of BCAAs can lead to various diseases. Therefore, exogenous degradation or conversion of excessive BCAAs may help alleviate diseases caused by BCAA accumulation, such as maple syrup urine disease. This study utilized synthetic biology approaches to engineer two strains for efficient BCAA catabolism successfully-ECN-Deg and ECN-Tra-by integrating specific metabolic pathways into the chassis strain, Escherichia coli Nissle 1917 (ECN). ECN-Deg integrates a metabolic module for BC

Inflammation in maple syrup urine disease.

Maple syrup urine disease (MSUD), also called leucinosis, is a rare inborn error of metabolism characterized by the deficiency of an enzyme complex: branched-chain alpha-keto acid dehydrogenase (BCKAD). This complex is located in the mitochondria and is responsible for metabolizing the three essential branched-chain amino acids (BCAA): isoleucine, leucine, and valine. As the patient with MSUD cannot directly break down these "building blocks" of protein, a harmful build-up of substances in the plasma, urine and tissues will follow. In this review, we overview available data of inflammation in

Domino Hepatectomy and Implantation With Innovative Outflow Reconstruction for an Adult Patient With Maple Syrup Urine Disease and Type 1 Diabetes Mellitus: Surgical Technique.

Maple syrup urine disease is characterized by deficient metabolism of branched chain amino acids leading to neurological and developmental delays. Because livers with maple syrup urine disease are histologically normal except for a single enzyme deficiency, which is also present in other organs, liver transplant provides adequate enzymes for metabolism of branched chain amino acids, and the explanted liver can be used as a domino graft for another person needing a liver transplant. We report on the case of a rare individual who survived into adulthood with ante-natal diagnosis and strict dieta

Pediatric liver transplant for maple syrup urine disease a single center experience.

INTRODUCTION: Maple syrup urine disease (MSUD) is an autosomal recessive inborn error of branched-chain amino acid metabolism caused by an inherited deficiency of branched-chain alpha-ketoacid dehydrogenase (BCKDH) activity that degrades isoleucine, leucine, and valine. Liver transplantation (LT) is a therapeutic option to treat the classical severe type of MSUD. This study aimed to assess and compare liver biochemical outcomes and amino BCAAs levels post-LT according to different types of donors. METHODS: Retrospective cohort analysis of 10 patient medical records diagnosed as MSUD who under

Navigating Adulthood with Maple Syrup Urine Disease: Patient and Caregiver Perspectives on Healthcare Transition and Independent Living.

BACKGROUND: Medical advances in diagnosis and management have increased the life expectancy of many pediatric-onset rare diseases, including inherited metabolic disorders (IMDs). Historically fatal childhood conditions are now manageable chronic illnesses, with over 90% of individuals with IMD reaching adulthood. As more individuals with IMDs reach adulthood (age 18 years), there is a new imperative for health care transition planning that promotes developmentally-appropriate skills for medical self-management and independent living. Knowledge gaps on the long-term disease-related outcomes lim
