Serum Metabolomic Profiling Reveals Potential Mechanisms and Biomarkers of Seladelpar Treatment in Patients With Primary Biliary Cholangitis.

BACKGROUND: Seladelpar, a potent and selective peroxisome proliferator-activated receptor-δ (PPARδ) agonist, is an approved second-line therapy for primary biliary cholangitis (PBC) in patients with an inadequate response or intolerance to ursodeoxycholic acid (UDCA). In clinical studies, seladelpar significantly improved biochemical markers associated with disease progression and pruritus. Our objective was to explore the metabolic mechanisms underlying seladelpar's effects and identify potential biomarkers. METHODS: Untargeted serum metabolomic analyses were conducted using UHPLC-MS/MS on f

Artesunate-based macromolecular therapy attenuates neutrophilic asthma by targeting the BCAT1/Succinate/NLRP3 metabolite-immunity cascade.

Asthma is a chronic inflammatory respiratory disease worldwide. Its severe phenotypes, particularly the glucocorticoid-resistant ones, pose a major challenge, with neutrophilic inflammation being the key driver. However, no effective treatments are currently available. Herein, we report a macromolecular therapeutic strategy capable of targeting multiple key cells involved in the pathogenesis of neutrophilic asthma. The macromolecular therapy named PMART is constructed by conjugating artesunate and polyethylene glycol to the triazine scaffold. It is highly water-soluble and can self-assemble in

Perceived water safety and elevated liver enzymes in rural West Virginia: links between drinking water, beverage habits, and liver health.

INTRODUCTION: In rural communities, aging infrastructure and environmental challenges can contribute to mistrust in tap water, potentially influencing hydration behaviors and health outcomes. The relationship between perceived water safety and liver health remains underexplored, particularly in low-resource Appalachian settings in the US. METHODS: This cross-sectional study examined data from 272 adult patients in a primary care clinic in rural West Virginia, US. Participants rated their home tap water safety on a seven-point scale and reported beverage consumption patterns in a survey. Clini

Estrogen Production in Pancreatic Cancer Shapes a Tumor-Suppressive Stromal Microenvironment.

UNLABELLED: Pancreatic ductal adenocarcinoma (PDAC) is a leading cause of cancer-related death due to a lack of effective therapeutic interventions. PDAC tumors are highly fibrotic, and stromal abundance is hypothesized to contribute to patient outcomes. However, stromal fibroblasts can be tumor promoting and tumor restraining, which could explain the disappointing clinical results of stromal-targeting therapies. In this study, we observed that serum levels of stromal biomarkers associated with favorable outcomes were significantly higher in female patients with PDAC compared with male patient

GRSF1 Protects Against Heart Failure by Maintaining BCAA Homeostasis.

BACKGROUND: Imbalances in cardiac branched-chain amino acid (BCAA) metabolism and mitochondrial homeostasis are implicated in the onset and development of heart failure. However, the mechanisms triggering the downregulation of cardiac BCAA metabolism in heart failure remain unclear. Here, we identify a novel role of the RNA-binding protein GRSF1 (guanine-rich RNA sequence binding factor 1) in post-transcriptionally regulating cell-intrinsic BCAA metabolic pathways, ultimately contributing to the pathogenesis of heart failure. METHODS: We examined GRSF1 expression in the heart tissues of patie

Angiotensin Receptor and Neprilysin Inhibitors in CKD: Opportunity or Concern?

Combined inhibition of neprilysin (NEP) and the angiotensin II receptor through angiotensin receptor-neprilysin inhibitors (ARNIs) has transformed heart failure management and is garnering increasing attention in nephrology. This article explores the renal role of NEP and critically reviews preclinical and clinical evidence on the use of ARNIs in CKD, following a bench-to-bedside approach. Experimental data show that NEP inhibition improves proteinuria, fibrosis, and endothelial function, with enhanced benefits when combined with renin-angiotensin-aldosterone system blockade. Randomized clinic

The Association Between Amino Acids and the Onset and Progression of Type 2 Diabetes Mellitus: A Comprehensive Analysis Based on UK Biobank Database.

BACKGROUND: Recent studies have demonstrated an association between amino acids (AAs) and the occurrence of Type 2 diabetes mellitus (T2DM). However, whether there is an underlying causal relationship between AAs and T2DM, as well as their potential links to T2DM progression, complications, and treatment selection, still lacks sufficient clinical evidence. METHODS: This study included 205,208 participants from the UKB database, with 14,066 diagnosed with T2DM. We used LASSO regression, supplemented by Mendelian randomization (MR), to explore the causal relationship between AA levels and T2DM.

Alteration in Amino Acid Metabolism After Isocaloric, Energy-Restricted Ketogenic Diet in Women with Overweight and Obesity: Randomized KETO-MINOX Trial.

Background/Objectives: Circulating amino acid concentrations and their excretion can provide insights into dietary protein intake and metabolism. Alterations in amino acid homeostasis occur in various disorders due to nutritional imbalances or metabolic changes, including obesity. A ketogenic diet (KD) has gained popularity for weight management; however, its metabolic effects are not fully known. Therefore, the aim of this study was to evaluate the effect of an eight-week, energy-restricted Mediterranean-type KD on the amino acid metabolism in women with overweight and class I obesity. Branched-chain amino acid transferase 2 (BCAT2) deficiency: A case series and systematic review.

BACKGROUND: Branched-chain amino acid transaminase 2 (BCAT2) deficiency is an autosomal recessive disorder that impairs branched-chain amino acid (BCAA) catabolism. Its clinical and metabolic features remain poorly understood due to limited reports in the literature. METHODS: We report three novel cases of BCAT2 deficiency from Slovenia: one diagnosed following symptom onset, one through cascade screening of parents, and one by newborn screening. Diagnosis was established through metabolic evaluation and confirmation of pathogenic variants in the BCAT2 gene. In addition, we performed a system

Untargeted Metabolomics Analysis of Human Milk from Breastfeeding Bangladeshi Women Reveals Amino Acid Metabolic Pathways Associated with Maternal Nutritional Status and Infant Growth.

BACKGROUND: Human milk (HM) contains bioactive constituents that continuously change throughout lactation and that are critical for optimal infant development. However, the temporal variation of the HM metabolome, its maternal nutritional determinants, and its links to infant outcomes remain insufficiently characterized in low-middle-income countries (LMIC) where childhood disease burden and HM exposure are substantial, and maternal nutritional status is poor. OBJECTIVES: We aimed to 1) characterize temporal changes in the HM metabolome, 2) quantify maternal nutritional status-related differe

Dietary Nutrients, Gut Microbiota, and Cardiac Function: From Metabolic Mechanisms to Clinical Applications.

BACKGROUND: The heart depends on a continuous and flexible energy supply from fatty acids, glucose, and other substrates. Emerging evidence shows that gut microbiota-derived metabolites-such as trimethylamine-N-oxide (TMAO), short-chain fatty acids (SCFAs), secondary bile acids, indoles, phenylacetylglutamine (PAGln), and branched-chain amino acids-modulate cardiac metabolism and function. Although clinical evidence linking these metabolites to cardiovascular outcomes is expanding, most data remain associative, with limited causal or interventional proof. METHODS: A comprehensive narrative re

Elevated branched-chain amino acids linked to hippocampal volumes and cognitive improvement in mild cognitive impairment.

Branched-chain amino acids (BCAAs) have been associated with cognitive function, with conflicting evidence suggesting both potential benefits and risks in neurodegenerative diseases such as Alzheimer’s disease (AD) and mild cognitive impairment (MCI), highlighting the need for further investigation. This study aimed to explore the relationship between total BCAAs, cognitive function, and brain structure, specifically examining hippocampal volumes and their potential mediating effects in individuals with AD, MCI, and cognitively normal (CN) individuals. Cognitive function was assessed using the

Causal role of genetically predicted impairment of branched-chain amino acid catabolism on insulin secretion and insulin resistance in type 2 diabetes.

BACKGROUND: Elevated branched-chain amino acids (BCAAs; leucine, valine, isoleucine) are linked to type 2 diabetes (T2D) risk, characterised by defective insulin secretion in pancreatic β-cell and peripheral insulin resistance. Causative interaction between BCAA metabolism and these two diabetic pathogenesis remains unclear. METHODS: Using publicly available datasets from the European population, we conducted a meta-analysis of genome-wide association studies (GWAS), followed by multi-trait analysis of GWAS (MTAG), to identify genetic loci associated with BCAAs and their catabolites. Plasma branched-chain amino acids in chronic kidney disease: associations with atherogenic lipids and mortality.

Branched-chain amino acids (BCAAs) are essential nutrients that promote muscle protein anabolism but also associate with cardiometabolic diseases; however, their role in chronic kidney disease (CKD) remains unclear. We investigated plasma BCAA levels and their associations with metabolic parameters and survival in CKD patients. Plasma BCAA levels, along with clinical and laboratory parameters, were measured in 328 patients with CKD stage 5 (median age 54 years, 60% males). BCAA concentrations in 83 community-dwelling controls (median age 51 years, 66% males) served as comparators. Multivariate

Metabolomic analysis of arterial blood near intracranial occlusions reveals biochemical signatures of acute ischemic stroke.

BACKGROUND: Acute ischemic stroke is a major cause of death and disability, and timely assessment of symptom onset is essential for appropriate treatment. Conventional imaging often lacks accuracy in estimating the timing of ischemic injury, especially in patients with unclear or unwitnessed onset. Metabolomic profiling may provide biochemical information reflecting tissue damage, collateral circulation, and clinical progression. METHODS: We performed an untargeted metabolomic study of arterial blood collected during mechanical thrombectomy from one hundred sixty-one patients with acute ische

Substrate l-isoleucine facilitates the hyper inhibition of reveromycin A on human cytoplasmic isoleucyl-tRNA synthetase.

Aminoacyl-tRNA synthetases (AARSs) are essential for decoding the genetic code by accurately attaching amino acids to their corresponding tRNAs, making them attractive drug targets for treating various diseases. The natural product reveromycin A (RMA) demonstrates therapeutic potential for osteoporosis and other osteoclast-related disorders by selectively inducing osteoclast apoptosis, with human cytoplasmic isoleucyl-tRNA synthetase (HcIleRS) identified as its putative functional target. In this study, recombinant HcIleRS was expressed and characterized, and RMA was demonstrated to potently i

Cathepsin B promotes asthma potentially via macrophage-associated autophagy and apoptosis.

Asthma is a chronic airway disease driven by type 2 immune responses, a core mechanism shared across allergic conditions. Cathepsins (CTSs), lysosomal proteases that regulate immune processes such as autophagy, antigen presentation, and cytokine modulation, have been implicated in allergy, but whether specific CTSs-particularly cathepsin B (CTSB)-causally contribute to asthma and through which mechanisms remains unclear. Here, we integrated genetic, transcriptomic, and experimental approaches to determine whether CTSB contributes to asthma pathogenesis. Mendelian randomization analyses using l

Plasma metabolite association profiles for type 2 diabetes genetic clusters in Finnish men.

AIMS/HYPOTHESIS: A recent study has suggested eight clusters of genetic variants associated with type 2 diabetes. We aimed to characterise metabolite associations for these eight clusters. METHODS: We constructed type 2 diabetes overall and cluster-partitioned polygenic risk scores (PRSs) in 10,015 Finnish men with 979 named plasma metabolites measured in Metabolon HD4 mass spectrometry platform. We evaluated metabolite-PRS associations using linear regression. We also performed a mediation analysis to examine whether metabolites statistically accounted for part of the association between gen

Metabolomic Profile of Genetic Liability to Type 2 Diabetes Among 125,000 Mexican Adults: A Mendelian Randomization Study.

OBJECTIVE: The Mexican population experiences a notably high prevalence of type 2 diabetes (T2D) and high T2D-associated disease risks. We used targeted plasma nuclear magnetic resonance metabolomics data within a Mendelian randomization framework to characterize the metabolomic profile of genetically predicted liability to T2D in this population. RESEARCH DESIGN AND METHODS: Between 1998 and 2004, 50,000 men and 100,000 women aged ≥35 years were recruited from Mexico City. Mendelian randomization analyses used a genetic risk score (GRS) comprising 1,055 established T2D-associated risk varian

The ability of branched-chain amino acid-to-tyrosine ratio (BTR) to assess preoperative liver function of patients with hepatocellular carcinoma.

Assessment of hepatic functional reserve is critically important for preventing serious complications after hepatectomy such as liver failure. While the indocyanine green clearance test (ICG) is a liver-specific test that is not affected by other organs and is commonly used to evaluate liver reserve capacity. We lack knowledge regarding what test should be performed for patients with jaundice, portal shunts, or intolerance whose liver function cannot be accurately assessed by ICG. To close this gap, we focused on changes in amino acid metabolism associated with impaired liver function. The bra
