Liver disease and protein needs.

Protein deficiency is often associated with liver disease. The principal cause of protein deficiency is decreased dietary intake. Deficiencies in digestion and absorption that are common in alcoholics contribute to protein deficiency in alcoholic liver disease. The protein requirements in most patients with compensated chronic liver disease are not different from normal, but increase during episodes of hepatocellular deterioration. An increased demand for protein after liver injury drains nitrogen from other organs such as muscle. Aromatic amino acids released from muscle in increased amounts

Adaptation of body protein metabolism in adult and aging man.

Aging causes a selective loss in lean body mass, muscle being most extensively eroded. This presentation is an analysis of the consequences of this age-related loss for protein and amino acid metabolism. The use of stable isotopes allows us to measure the dynamics of protein metabolism in the intact human subject, while the output of 3-methylhistidine permits estimates of muscle turnover to be made which show a smaller reduction in body protein turnover per kg body weight in elderly subjects. This is due to the reduced contribution by muscle protein turnover, which decreases from 30 per cent o

Exchange transfusion in acute episodes of maple syrup urine disease. Studies on branched-chain amino and keto acids.

Two neonates with maple syrup urine disease were treated by exchange transfusion. Within 15 h blood leucine and KICA concentrations were lowered from 2.6 mM to 1.1 mM using 570 to 620 ml blood per kg body weight. The other branched-chain amino acid/keto acid pairs fell to normal. During exchange transfusion the patient's nitrogen balance seems to be negative. Further exchange transfusion was useless. More importantly the patient should be forced into an anabolic state by high caloric supply or insulin plus glucose treatment. More KICA than leucine was eliminated, however, KICA blood levels rem

Cerebral palsy associated with maple syrup urine disease.

Maple syrup urine disease (MSUD) is a rare heritable enzyme defect associated with mental retardation. A diet deficient in the branched-chain amino acids is essential for survival. Patients with MSUD are at risk of ketoacidotic metabolic crises brought on by catabolic states, including simple infection or fasting. Delayed diagnosis and therapy can predispose these patients to loss of the gag reflex, lethargy, seizures, and feeding problems. Ultimately, this may result in aspiration and respiratory arrest, which, in turn, can cause cerebral palsy. Of seven cases of MSUD reviewed at The Hospital

Decreased essential amino acid requirements without catabolism in phenylketonuria and maple syrup urine disease.

The normal infant requirement for essential amino acids includes requirements for growth, obligatory catabolism, and other minor losses. The fraction required for obligatory catabolism was estimated by the difference between the normal requirement and the dietary tolerance of infants lacking the enzymes needed for catabolism of phenylalanine and leucine. The average dietary tolerance for phenylalanine in infants with classical phenylketonuria is approximately 42, 31, and 23 mg/100 kcal at 0 to 4, 4 to 12, and 12 to 24 months, respectively. This is equivalent to 68, 58, and 50% of the estimated

Oxidation and metabolic interconversion in malignant cachexia.

Some patients with progressive malignant disease have an increase in basal metabolic expenditure as well as total caloric expenditure, but the findings are neither striking nor consistent. Studies of specific metabolic changes in such patients have shown the following. (a) The predominant substrate for energy in these subjects is fatty acid, as in normal humans. Comparative studies show, however, that greater proportions of the oxidative metabolism in patients with cancer are from fatty acids, particularly when exogenous glucose is available. While the free fatty acid levels in plasma decrease

Inherited disorders of 3-methylcrotonyl CoA carboxylation.

The clinical course of 4 patients who had reduced activities of 3-methylcrotonyl CoA carboxylase (also called 3-methylcrotonylglycinuria) is described. Two children presented with a metabolic acidosis, one in the neonatal period and the other with episodes of acidosis that started in the second year of life. In the other 2 children neurological symptoms were prominent, one having infantile spasms and the other developmental regression with a skin rash and alopecia. Three of the children responded well to oral biotin and dietary protein restriction but the fourth, despite a biochemical response

Plasma tryptophan and five other amino acids in depressed and normal subjects.

Decreases in brain serotonin levels have been implicated in the pathogenesis of human depression. While circulating levels of tryptophan (TRP) might possibly reflect concentration of brain serotonin, a more relevant peripheral measure may be the ratio of plasma TRP to five other neutral amino acids (TRP/5aa ratio). In depressed subjects and normal controls, plasma TRP (free and total), phenylalanine (PHE), tyrosine (TYR), leucine, isoleucine, and valine were measured on three days. When depression was most severe, depressed patients had lower TRP/5aa ratios and total TRP levels and higher PHE

Leucine metabolism in patients with malignant disease.

Studies of the metabolism of the amino acid, leucine, have been carried out in six patients with progressive cancer and compared to similar studies in a group of control subjects. U-14C leucine was given by the single injection technique and the disappearance of radioactive leucine from the plasma as well as the appearance of the label in expired air was determined over a 3-hour interval in both the overnight fasting state and during glucose ingestion. Plasma leucine levels in the basal state were higher in the patients with malignant disease (193 +/- 17 mumol/L vs 135 +/- 16 mumol/L) as was t

Effect of glucose and/or branched chain amino acid infusion on plasma amino acid imbalance in chronic liver failure.

The characteristic amino acid pattern observed in chronic liver failure with high aromatic and low branched chain amino acid levels is considered to be consequent to increased muscle protein catabolism. The main catabolic stimulus has been attributed to hyperglucagonemia and to a reduced insulin/glucagon molar ratio. Intravenous administration of a solution containing branched chain amino acids and glucose to patients with chronic liver cirrhosis rapidly normalizes the plasma amino acid pattern. This effect may result from either a change in the insulin/glucagon ratio, induced by glucose, or f

Amino acids in retinitis pigmentosa.

A survey of fasting whole blood amino acids in 65 patients with various subtypes of retinitis pigmentosa performed. Eight X-linked recessive patients showed decreased taurine and aspartate. Nineteen autosomal recessive patients, and to lesser extent 10 autosomal dominant patients, showed reduced levels of whole blood threonine and histidine. Branched-chain amino acids and arginine were present in increased amounts in 2 patients with Laurence-Moon-Bardet-Biedl syndrome. These findings in LMBB patients are probably related in part to their obesity and emphasise that appropriate controls are requ

Oral ammonia tolerance test in patients with portal hypertension.

Eighty patients with portal hypertension due to various hepatic diseases were subjected to the oral ammonia tolerance test (OATT). Blood samples were collected before and at 30-min intervals after the administration of ammonium chloride (50 mg/kg). The ammonia levels, and the OATT curve patterns and sigma NH3-N values were determined and it was investigated whether there are correlations between these values and various other parameters of hepatic function. The results of OATT correlated with the plasma disappearance rate of indocyanine green and the molar ratio of branched chain amino acids t

Perspectives in the control of chronic pain by nutritional manipulation.

The role of cental neurotransmitters in mood, behavior and emotion are briefly reviewed. Some neurotransmitters, such as norepinephrine and serotonin, have been implicated in both the determination of pain threshold and the behavioral responses to painful; stimuli. A number of experiments have indicated that brain serotonin can be increased through administration of tryptophan, the dietary precursor of serotonin. Other dietary amino acids have also been found to alter pain tolerance thresholds. This paper reviews the evidence for, and the possible future use of, dietary manipulation to control

[Branched amino acids. Role and value in parenteral nutrition].

The authors undertook a general review of the principal studies concerning branched amino acid (B.A.A.) and justifying their use in large doses during parenteral alimentation. Certain particular features emerge concerning valine, isoleucine and leucine which render these three amino acids particularly valuable:--their metabolism is essentially mascular and they escape hepatic control in great part, in contrast to other amion acids;--broken down by muscle, they represent a significant source of energy, sometimes the only available, but may also be used in numerous metabolic processes;--particul

L-dopa competes with tyrosine and tryptophan for human brain uptake.

Tyrosine and tryptophan have been assayed spectrofluorometrically in postmortem human brain areas of patients with Parkinson's disease treated orally with or without 3,4-dihydroxyphenylalanine (L-dopa) plus the peripherally acting decarboxylase inhibitor benserazide. Tyrosine as well as tryptophan decrease significantly after treatment with L-dopa, thus showing a competitive action of L-dopa to other aromatic amino acids on human brain uptake. It is suggested that some of the side effects of L-dopa treatment in Parkinson's disease are due to a disturbance in the brain and neural uptake of othe

Gyrate atrophy of the choroid and retina: amino acid metabolism and correction of hyperornithinemia with an arginine-deficient diet.

Four patients with gyrate atrophy of the choroid and retina were studied, all of whom exhibited the hyperornithinemia characteristic of this disorder. Elevated plasma histidine and diminished plasma lysine and branched-chain amino acids were also noted. The renal clearances of these four amino acids were not sufficiently elevated to explain their low plasma levels. In one subject, an arginine-deficient diet led to progressive reduction in plasma ornithine from 13 times normal to the upper limits of normal, along with the disappearance of ornithinuria and lysinuria. Decarboxylation of alpha-ketoisovaleric acid after oral administration in man.

The keto analogues of essential amino acids represent a promising therapeutic modality in hereditary and acquired disorders of nitrogen metabolism. The utilization of these substances in humans has been assayed primarily by nitrogen balance studies. A simple and accurate breath excretion test for 14CO2 enabled us to measure the decarboxylation of 1-14C-alpha-ketoisovaleric acid (KIV, the keto analogue of valine) in two normal and six diseased subjects. Normal volunteers as well as patients with gastrectomy, hepatic failure, renal failure, and myotonic dystrophy were tested in 5-g protein diets

Nucleoside excretion in breast cancer: comparison with other biochemical tumour-index-substances.

[We have measured four urinary nucleosides (dimethylguanosine, 1-methylinosine, pseudouridine and beta-aminoisobutyric acid)in patients with benign breast disease and patients with early and advanced breast cancer in order to assess their value as tumour-index-substances. We compared the results with other biochemical indices of breast cancer and sought and correlations between these indices. The results indicate that few abnormalities occurred in patients without overt metastases and these did not predict early relapse. In those with metastatic disease, dimethylguanosine excretion was most fr

Plasma neutral amino acids in mania and depression: variation during acute and prolonged treatment with L-tryptophan.

The free tryptophan and plasma neutral amino acids including kynurenine have been determined before treatment, after a single load, and during prolonged treatment with L-tryptophan on a bipolar manic-depressive patient who has shown resistance to current treatments. The data of the patient were compared with the data of healthy control subjects in order to evaluate the availability of tryptophan to the brain. A relative deficiency of tryptophan in the plasma, as measured by the ratio of tryptophan to those amino acids which compete with tryptophan during transport processes, was found in the p
