A Rare Presentation of Invasive Nontypeable Haemophilus influenzae With Systemic Involvement: A Case Report.

Haemophilus influenzae (H. influenzae) is a common organism that causes noninvasive infections in the respiratory tract. Lately, there has been an increasing incidence of invasive diseases with nontypeable strains not covered by Hib vaccines. We discuss a case of a middle-aged male with sickle cell trait and gout who presented with altered mentation and polyarthralgia. On investigations, he had H. influenzae bacteremia on two sets of blood cultures with dissemination to meninges and joints, causing acute encephalopathy and polyarthralgia. The initial results of arthrocentesis and lumbar punctu

Bacterial isolates, antibiotic susceptibility, and determinants of positive blood culture in children with community-acquired pneumonia in Central Tanzania.

INTRODUCTION: despite the widespread coverage of Haemophilus influenzae type b (Hib) and pneumococcal conjugate vaccines, the mortality of children under five years due to community-acquired pneumonia (CAP) in Tanzania remains unacceptably high. This study aimed to identify bacterial isolates, antibiotic susceptibility, and factors associated with blood culture positivity in children under five with CAP. METHODS: a cross-sectional study involving 195 children under-five years of age who were clinically diagnosed with CAP was conducted at a referral hospital in central Tanzania from October 20

Pneumococcal meningitis and endotoxemia: A cross-sectional clinical study.

INTRODUCTION: Lipopolysaccharide (LPS) is a major virulence factor during both meningococcal and Haemophilus influenzae meningitis. Pneumococcus does not produce LPS but could be responsible for bacterial digestive translocation as a consequence of sepsis. We addressed this question in the context of pneumococcal meningitis. METHODS: A cross-sectional study on 24 patients with pneumococcal meningitis (20 (83%) admitted in intensive care unit, 4 (17%) with septic shock) and 34 prospectively-enrolled healthy volunteers. Interleukin 6 and C-reactive proteins plasma concentrations were measured a

Do we need to reconsider our antibiotic regimen for preterm premature rupture of membranes? Pathogens and perinatal outcome in PPROM.

BACKGROUND: The antibiotic regimen currently recommended in most guidelines in case of preterm premature rupture of membranes (PPROM) with an aminopenicillin and a macrolide is mainly based on studies performed more than 25 years ago. In view of the changing pathogens in neonatal and perinatal infectious morbidity and their increasing aminopenicillin-resistance in recent decades, the question arises as to whether these data are still applicable to our current situation and whether an adjustment of our antibiotic standard regimen with amoxicillin and azithromycin is necessary. METHODS: We retr

Infections in sickle cell disease.

Sickle cell disease (SCD) is one of the commonest severe inherited disorders in the world. Infection accounts for a significant amount of the morbidity and mortality, particularly in sub-Saharan Africa, but is relatively poorly studied and characterized. Patients with SCD have significant immunodeficiency and are more likely to suffer severe and life-threatening complications of infection, and additionally infections can trigger complications of SCD itself. Those with more severe forms of SCD have functional asplenia from a very early age, which accounts for much of the morbidity in young chil

Increase in invasive Haemophilus influenzae serotype A infections during the COVID-19 pandemic in New South Wales, Australia.

Haemophilus influenzae, a causative agent of severe invasive infections such as meningitis, sepsis and pneumonia, is classified into encapsulated or typeable (represented by serotypes A to F) and non-typeable varieties (NTHi) by the presence or absence of the polysaccharide capsule. Invasive disease caused by H. influenzae type B (HIB) can be prevented through vaccination which remains the main disease control intervention in many countries. This study examined the genomic diversity of circulating H. influenzae strains associated with invasive disease in New South Wales, Australia, before and

An Unusual Case of Haemophilus influenzae Associated Polyarthritis: Diagnostic and Therapeutic Challenges in Concurrent Septic and Reactive Arthritis.

Septic arthritis and reactive arthritis are both recognized as distinct causes of swollen joints; however, they can, at times, overlap as causes of acute polyarthritis. Septic arthritis is an orthopedic emergency, typically caused by bacterial infection, and requires urgent antibiotic treatment and joint drainage to prevent irreversible joint damage. In contrast, reactive arthritis is a sterile, immune-mediated arthritis that occurs following infections and is managed with anti-inflammatory treatments such as corticosteroids and non-steroidal anti-inflammatory drugs (NSAIDs). We report the cas

Rapid and sensitive detection of Haemophilus influenzae using multiple cross displacement amplification combined with CRISPR-Cas12a-based biosensing system.

Haemophilus influenzae (H. influenzae, Hi) is an opportunistic bacterium that colonizes the upper respiratory tract of humans and frequently causes meningitis, pneumonia, sepsis, and other severe infections in children. Early and accurate detection of H. influenzae is essential for effective diagnosis and treatment. In this study, we established a novel diagnostic method by integrating the CRISPR-Cas12a detection platform with multiple cross-displacement amplification (MCDA), termed the Hi-MCDA-CRISPR assay. This method offers an efficient and highly precise diagnostic tool for the identificat

Longitudinal assessment of immunoglobulin response and disease progression in critically ill patients with community acquired pneumonia.

BACKGROUND: Low endogenous immunoglobulin(Ig)-levels are common in critically ill patients with sepsis, but it is unknown whether low Ig-levels are associated with poor outcome, and in which patients Ig-replacement therapy (IgRT) improves outcome. Given the crucial role of immunoglobulins in eliminating certain encapsulated pathogens, we examined the relationship between serial Ig-levels and disease course in critically ill patients with community acquired pneumonia (sCAP) caused by encapsulated or other pathogens. METHODS: We included a cohort of consecutive critically ill patients with CAP,

Optimizing recovery of Haemophilus influenzae from vaginal-rectal specimens and determining carriage rates in pregnant women.

PURPOSE: Haemophilus influenzae (HINF), primarily non-typeable H. influenzae: (NTHi), is an important cause of neonatal sepsis and meningitis. The goal of this study was to investigate the point prevalence of HINF vaginal-rectal carriage in pregnant women, which could impact neonatal health. METHODS: Simulated vaginal-rectal swabs were cultured and tested to establish optimal recovery methods for HINF. These methods were then applied to vaginal-rectal swabs from a prospective cohort of pregnant women (n = 300) undergoing routine Group B Streptococcus: (GBS) screening. Both culture and PCR wer

The importance of microbiology reference laboratories and adequate funding for infectious disease surveillance.

Microbiology reference laboratories perform a crucial role within public health systems. This role was especially evident during the COVID-19 pandemic. In this Viewpoint, we emphasise the importance of microbiology reference laboratories and highlight the types of digital data and expertise they provide, which benefit national and international public health. We also highlight the value of surveillance initiatives among collaborative international partners, who work together to share, analyse, and interpret data, and then disseminate their findings in a timely manner. Microbiology reference la

Hemophilus Septicemia Meningitis in an 11-Month-Old Vaccinated Infant.

Haemophilus influenzae is a gram-negative pleomorphic coccobacillus associated with many diseases, such as meningitis, pneumonia, septicemia, cellulitis, and otitis media. The most virulent and most common serotype is H. influenzae type b (Hib), which was responsible for the majority of meningitis cases until the development of vaccines that led to a decrease in its incidence worldwide. Here, we report the case of an 11-month-old female infant who was previously healthy and fully vaccinated against Hib and developed sepsis and meningitis. The patient was managed as a case of partially treated
