Genetic evaluation in living liver donors: beyond individual risk to system responsibility.

PURPOSE OF REVIEW: Living donor liver transplantation (LDLT) increasingly depends on biologically related donors, particularly in Asia and the Middle East, where deceased donation remains limited. This review evaluates genetic risks in living donors and proposes a system-based approach to donor selection. RECENT FINDINGS: Heterozygous carriers exhibit variable functional capacity determined by enzyme thresholds and epigenetic mechanisms such as X-chromosome lyonization. In ornithine transcarbamylase deficiency (OTCD), maternal donors may have insufficient enzymatic activity, resulting in dono

Emerging plastic additives in umbilical cord serum, metabolomic profiles, and fetal growth restriction: A case-control study.

BACKGROUND: Fetal growth restriction (FGR) is associated with substantial perinatal and long-term health risks. As traditional plasticizers are being restricted, exposure to emerging plastic additives is increasing, yet their associations with FGR and related metabolic profiles remain poorly characterized. METHODS: This case-control study included 90 FGR cases and 100 controls from the "Early Life Plan" cohort. A total of 117 plastic additives were measured in umbilical cord serum, and untargeted metabolomics was performed using UHPLC-HRMS. Multivariable logistic regression, restricted cubic

Combining metabolomics and network pharmacology to clarify the mechanism of Zuogui Wan in treatment of oligoasthenospermia.

Zuogui Wan (ZGW), a classic traditional prescription used for nourishing kidney yin, has demonstrated considerable efficacy in the treatment of oligoasthenospermia (OAS) in both clinical and laboratory studies. However, the specific mechanism by which ZGW ameliorates OAS remains unclear. In this study, we integrated metabolomics and network pharmacology to explore the potential mechanisms underlying the therapeutic effects of ZGW in OAS. Pharmacodynamic studies revealed that ZGW increased body and testicular weights in mice and improved semen parameters. Furthermore, the testicular structure o

Colonic administration of baitouweng decoction alleviates ulcerative colitis via coordinated reprogramming of arginine and tryptophan metabolism.

BACKGROUND: Ulcerative colitis (UC) involves persistent colonic inflammation, loss of epithelial barrier integrity, and disturbed immune regulation. Although Baitouweng decoction (BTW) is widely used for intestinal inflammatory disorders, the effects and mechanisms of colon-targeted administration remain unclear. PURPOSE: This study evaluated the effects of transendoscopic enteral tubing (TET)-mediated colonic BTW administration (CBTW) in dextran sulfate sodium (DSS) induced colitis and examined its association with arginine and tryptophan metabolism. METHODS: CBTW efficacy was evaluated in

Isoform differentiation studies of arginase: From structural function to design strategies for specific ligands and their application prospects.

Arginase (ARG), a binuclear manganese metalloenzyme, hydrolyzes L-arginine to L-ornithine and urea, competitively regulating NO bioavailability with NOS. Mammals express two isoforms, ARG1 (cytosolic) and ARG2 (mitochondrial), that share a conserved catalytic core yet exert distinct physiological and pathological effects. ARG1 is essential for hepatic urea detoxification, as systemic Arg1 knockout mice develop hyperammonemia within 10-12 days after birth; in contrast, Arg2 knockout mice show only slightly elevated plasma arginine levels with no overt phenotypic abnormalities, this significant

Multi-omics reveals regulatory networks and critical early-warning factors for severe disease progression in diabetic patients infected with SARS-CoV-2.

BACKGROUND: Diabetic patients face elevated risks of severe COVID-19, yet the molecular underpinnings of disease progression, particularly for Omicron subvariants, which have predominated since late 2021, remain poorly defined. This observational study leverages a cohort recruited during China's Omicron peak (December 2022-February 2023) to delineate multi-omic signatures underlying severe diabetic COVID-19. METHODS: We enrolled 55 patients across five clinical strata: non-diabetic mild/severe, diabetic mild/severe/fatal. Integrated 4D-DIA proteomics and LC/GC-MS metabolomics were applied, ad

Evaluating the Educational Impact of an Image-Based Assessment Tool in a Gross Anatomy Laboratory Course: A Thematic and Quantitative Analysis of Student Experiences.

BACKGROUND: Learning anatomy remains challenging due to complex terminology, intricate 3D relationships, and the extensive amount of material students must master. While dissection laboratories provide valuable tactile and spatial learning, they often lack structured mechanisms to foster independent preparation and reflective consolidation. The Image-Based Assessment Tool was designed as a structured, student-centered adjunct to facilitate pre- and post-laboratory experience. METHODS: This mixed-methods study employed quantitative paired-sample analyses and qualitative thematic analysis to ex

Lysosomal cholesteryl ester hydrolysis drives white matter repair by reprogramming microglia into a novel reparative state.

White matter repair relies on microglial clearance of cholesterol-rich myelin debris. Microglia have been reported to predominantly depend on de novo sterol synthesis to support this repair process. Lysosomal acid lipase (LAL) is the only known lysosomal enzyme capable of hydrolyzing cholesterol esters. In stark contrast to previous studies, we demonstrate here that LAL-mediated lysosomal lipolysis—not de novo sterol synthesis—serves as a central determinant of the microglial capacity to drive white matter repair. Using single-cell RNA sequencing, we identified a novel reparative microglial st

PAH-associated health risk from ultrafine particles: an urban field study.

Reteplase (Retavase) is a plasminogen activator, mimicking endogenous tissue plasminogen activator (t-PA), a serine protease, converting plasminogen to plasmin and thereby precipitating thrombolysis. It is a third-generation recombinant form of t-PA that operates in the presence of fibrin (i.e. it is fibrin specific). Reteplase can be administered as a bolus dose (nonweight-based), rather than an infusion, which promotes rapid and safe administration. The ease of administration of this reteplase dosage regimen (two 10U bolus doses, each over 2 minutes, 30 minutes apart) is conducive to prehosp

Androgen Receptor Drives Polyamine Synthesis, Creating a Vulnerability for Prostate Cancer.

UNLABELLED: Supraphysiologic androgen (SPA) treatment can paradoxically restrict the growth of castration-resistant prostate cancer (CRPC) with high androgen receptor (AR) activity, which is the basis for the use of bipolar androgen therapy (BAT) for patients with this disease. Although androgens are widely appreciated for enhancing anabolic metabolism, how SPA-mediated metabolic changes alter prostate cancer progression and therapy response is unknown. In this study, we report that SPA markedly increased intracellular and secreted polyamines in prostate cancer models. AR binding at enhancer s

Advances in understanding, diagnosing, and treating hepatic encephalopathy: from epidemiology to emerging therapies.

This comprehensive review explores the multifaceted landscape of hepatic encephalopathy (HE), encompassing its epidemiology, pathophysiology, classification, and evolving therapeutic approaches. Delving into the prevalence and incidence of HE, it is found that the burden of this condition is on individuals and healthcare systems. The intricate mechanisms underlying HE, including abnormalities in synapse transmission, the ammonia hypothesis, the false neurotransmitter theory, the GABAergic theory, and the benzodiazepine theory, are scrutinized, providing a nuanced understanding of the disorder.

Polyamine pathway in neurological disorders: Potential therapeutic implications based on current evidences.

Polyamines (PAs) with a positive charge and low molecular weight, are a class of general biogenic amines that are involved in many important cellular processes, including maintaining nucleic acid stability, controlling ion channel activity, regulating transcription and translation, modulating cell cycle, influencing kinase activity, protecting oxidative damage, and maintaining membrane structure. Cumulative studies have shown that in various neurological disorders, the normal synthesis and metabolism of the polyamine pathway would be disrupted. And the changed polyamine system is believed to p

Urea cycle modulation by combined SGLT2 inhibitors and metformin.

BACKGROUND: Sodium-glucose co-transporter 2 inhibitors (SGLT2i), when combined with metformin (COMBI), offer multi-organ protective effects in patients with type 2 diabetes (T2D), particularly those at high risk of cardiovascular or renal complications. However, the underlying molecular mechanisms remain poorly understood. METHODS: We profiled 303 targeted serum metabolites in 1494 participants of the KORA study, including T2D patients treated with COMBI therapy, metformin monotherapy, or no glucose-lowering medication. Additionally, metabolomic profiling was quantified on seven tissues (plas

Synthesis-driven reverse metabolomics reveals 3-hydroxy N-acyl amides as gut microbial molecules.

3-hydroxy N-acyl amides are bioactive lipids with reported anti-obesity and glucose-regulating effects, yet they are rarely detected in untargeted metabolomics studies because they are largely absent from existing spectral reference libraries. To address this gap, we synthesized an MS2 spectral resource comprising 436 structurally diverse 3-hydroxy N-acyl amides, spanning 3- to 18-carbon chains with a wide range of amine headgroups such as ornithine, valine, and dopamine. Using a synthesis-driven reverse metabolomics approach, we found 161,626 spectral matches across 54,744 publicly available

Alpha-difluoromethylornithine suppresses angiogenesis via the FLI1-CLEC14A-VEGFC pathway in retinal endothelial cells.

Pathological angiogenesis is a leading cause of vision loss in blinding disorders such as age-related macular degeneration, in which anti-VEGF therapies are widely used, yet their limitations necessitate new therapeutic strategies. This study explores the molecular mechanisms through which alpha-difluoromethylornithine (DFMO), an inhibitor of polyamine synthesis, suppresses angiogenesis in human retinal microvascular endothelial cells (HRMECs). Transcriptome sequencing revealed that DFMO significantly reduced tube formation and downregulated 3839 genes, including C-type lectin domain-containin

A Melittin-Derived Lead Compound Ameliorates Severe Acute Pancreatitis by Restoring Oxidative Homeostasis and Macrophage Metabolism.

Severe acute pancreatitis (SAP) is a life-threatening inflammatory condition driven by macrophage-mediated oxidative stress and metabolic dysregulation. While bioactive peptides such as melittin show anti-inflammatory potential, their clinical application is limited by cytotoxicity and unclear mechanisms. In this study, we developed HMLT, a melittin-derived peptide with histidine substitutions designed to reduce cytotoxicity. Compared with native melittin, HMLT exhibited significantly lower cytotoxicity in RAW264.7 macrophages while maintaining potent anti-inflammatory activity, as demonstrate
