In a one-year evaluation of the Obstetric-Gynecologic Services of the Los Angeles County University of Southern California Medical Center, bacteremia was confirmed on 144 occasions in 139 patients. This represented an over-all incidence of 7/1,000 admissions with gram-negative bacteremia observed in 3.1/1,000 admissions. There were four deaths in this series. The most frequently recovered aerobes were Escherichia coli, enterococci, and beta hemolytic streptococci, not Group A or D, while the most commonly isolated anaerobes were peptostreptococci, peptococci, and Bacteroides. These patterns of

Oxygen-sensitive anaerobic bacteria comprise the largest group of organisms among the human endogenous microflora. The oral cavity, the vagina, and the colon are the areas of the human body where the obligate anaerobes are predominant and can be isolated in very high numbers. Sepsis following surgery of any of these organs is frequently due to the escape of these endogenous bacteria during the course of the operative procedure. To enable the isolation and successful identification of most of these anaerobic microorganisms from clinical exudate, special collection and bacteriologic techniques a

The name Bacteroides is used to describe a group of Gram-negative bacilli which are non-sporing obligate anaerobes (Wilson and Miles, 1964). Their natural habitat is the large intestine, mouth, and vagina. Bacteroides septicaemia is becoming increasingly well recognized as a complication of gastrointestinal and gynaecological surgery. Patients with underlying malignancy or prior antibiotic therapy are predisposed to this condition (Bodner, Koenig and Goodman, 1970). In a survey by Okubadejo, Green and Payne (1973), twenty-nine strains of B. fragilis were tested against co-trimoxazole—all were

A study was made of 111 strains of plasma-negative spathylococci isolated from the blood, pleural fluid, urine, and exudate of the abdominal cavity of 30 patients. The studies were carried out by 18 criteria. A variety of biological properties and signs characteristic of pathogenic staphylococci (hemolytic activity, anaerobic splitting of mannite, the presence of phosphatase, lysozyme, protease, alpha-toxin, fibrinolysin) were noted. A high resistance to tetracycline and penicillin was found in the strains isolated from the blood and the pleural cavity.

Two bacteriologic systems for detecting bacteria in blood were compared; the automated radiometric BACTEC and the conventional method used in our laboratory for many years. BACTEC consisted of two bottles with 30 ml and the conventional method with 50 ml of media for aerobes and anaerobes. The BACTEC bottles were inoculated with 2 to 3 ml and the conventional with 4 to 5 ml of blood at the patient's bedside. Out of the 3,045 blood specimens cultured (804 patients), 262 (117 patients) were positive by one or both methods. The conventional system detected 5more cultures. The explanation of the d

Sera from eight patients with either septicemia or pyothorax caused by Bacteroides fragilis were assayed for specific antibody by the agglutination, immunofluorescence, and agar gel diffusion techniques. Antibodies were detected in the sera of all eight patients by all three serological tests employed; titers as high as 1:160 and 1:320 were detected by the agglutination test and the immunofluorescence technique, respectively, and all patients had precipitating antibody. Failure of B. fragilis sub-species thetaiotaomicron to react with any heterologous antisera from the other seven patients inf

To amplify recent interest in anaerobic infections following abdominal disease, trauma, or surgery, 512 consecutive patients subjected to emergency celiotomy had both aerobic and anaerobic cultures taken of peritoneal fluid as well as all complicating wound and intra-abdominal infections. Average time between peritoneal entry of abscess drainage and specimen incubating under anaerobic conditions was less than two minutes. During 4 of the seven study months, patients had antibiotic therapy randomized, with clindaymcin or cephalothin being sole parenteral agents and given intravenously prior to

Infection with Gram-negative bacteria is frequently the cause of death in human liver transplantation. In experimental liver transplantation in mongrel dogs we detected Gram-negative septicaemia in 73% of our first 15 cases; maximal survival was 26 days. In a second series of 27 orthotopic liver transplantations in tissue-typed littermate dogs and littermate pigs the standard surgical technique and aftercare were changed with regard to four factors: an end-to-end common bile duct anastomosis was made instead of a gallbladder to duodenum anastomosis; the continuous postoperative bacteriostatic

The frequencies of fever, parenchymal infiltration, and bacteremia were studied prospectively after 100 flexible fiberoptic bronchoscopies performed transnasally under topical anesthesia. Fever occurred after 16 per cent, and parenchymal infiltration, after 6 per cent of the procedures. Most complications were mild and transient; however, one patient developed rapidly progressive pneumonia and died. No organisms were isolated from cultures of blood drawn at the time of the procedure or during complications. The organisms most commonly isolated from the sputum of the patients who developed pneu

Septic complications of colonic perforation involve miltiple bacteria derived from the intestinal flora. This type of mixed intraabdominal infection was produced experimentally by insertion of a standardized inoculum of rat colonic contents into the peritoneal cavity of male Wistar rats. The respective roles of coliforms and anerobic bacteria were then studied by use of selective antimicrobial therapy (with gentamicin and clindamycin). Untreated rats had a two-stage disease. Initally, there was an acute peritonitis associated with a 37% mortality rate; all animals that survived developed indol

Anaerobic infections are reviewed with emphasis on management. Most anaerobic pulmonary infections respond to penicillin G, even when Bacteroides fragilis (penicillin-resistant) is present. Clindamycin is suitable in penicillin-sensitive patients. Intraabdominal infections have a complex flora usually involving anaerobes, especially B. fragilis. It is desirable to use antimicrobial therapy to cover potential pathogens of all types. Surgical drainage and debridement are extremely important considerations. Anaerobic bacteria were found in 72% of 200 patients with female genital tract infections

An unusual case report of a patient with bacteremia due to Succinivibrio dextrinosolvens is presented. Heretofore, Succinivibrio spp. were thought to occur only as organisms in the rumens of herbivorous animals. Succinivibrio is an anaerobic, Gram-negative, curved, spiral bacillus with a polar monotrichous flagellar pattern. Volatile acids produced from glucose metabolism include succinic, acetic, formic and lactic. Various carbohydrate substrates are fermented to produce strong or weak acid end-points. Succinivibrio spp. will grow in bile, will not hydrolyze esculin or starch, and do not prod
