A clinical description of an apparently classical case of type 1 GM1 gangliosidosis is presented. The patient was the first-born child of first cousins. The prevalence of lactose malabsorption (LM) among Bangladeshi village children has been determined using the recent developed breath hydrogen test. Initial hospital-based comparison studies showed general agreement between the breath hydrogen test and a modified lactose tolerance test. Eight cases of acute gastroenteritis, two caused by enteropathogenic "E. coli", with secondary cow's milk protein intolerance were studied. Diagnosis was established clinically and by means of intestinal biopsy, before and after a milk challenge. The role of acute gastroenteritis as a trigger of cow's milk protein intolerance and other factors contributing to the appearance of that complication are discussed. The mechanisms leading to intractable diarrhoea in a situation of prolonged gastroenteritis are also commented.

Dihydroergotamine (DHE), an alpha-adrenergic blocking agent, rapidly improved 121 out of 123 diarrheal patients. A hypotonic sigmoid and a hyperreactive rectum were found in these patients. Manometric studies of the distal colon showed that DHE counteracts the rectal hyperactivity and increases sigmoidal tone. On the other hand, anticholinergic drugs and/or emotional stimuli accentuate the rectal hyperactivity of diarrheal patients. Both features could be due to an unbalanced neurologic control of the gastrointestinal tract with dominance of the alpha-adrenergic over the cholinergic activity.
