Management of acute infusion-related reactions in AAV gene therapy guided by mechanistic insight.

Infusion-related reactions (IRRs), serious immune events that occur either during or within six hours of an infusion, can occur with adeno-associated virus (AAV) gene therapy. IRRs may be attributed to IgE-mediated hypersensitivity, leading to treatment discontinuation due to concern about progressive, life-threatening allergic reactions. IRRs were observed in two participants who received an AAV8 gene therapy (DTX401) for glycogen storage disease type Ia and in one participant who received an AAV9 gene therapy (UX701) for Wilson's disease. All of these IRRs were associated with a rapid infusi

Bempedoic Acid Treatment in Four Adult Patients with Glycogen Storage Disease Type 1a: A Retrospective, Exploratory Case Series.

Background: Glycogen storage disease type 1a (GSD 1a) is an inborn error of metabolism due to glucose-6-phosphatase deficiency. Treatment consists of frequent, carbohydrate-rich meals, intake of glucose polymers like uncooked cornstarch, or drip feeding in small infants. Although dietary treatment is challenging, it has improved the long-term outcomes for patients with GSD 1a. Nevertheless, several complications, including dyslipidaemia, remain difficult to manage, and early atherosclerosis with risk for ischemic stroke is a potential long-term concern. We propose bempedoic acid (BA) as a pote

Generation and characterization of a human iPSC line (XACHi022-A) derived from an asymptomatic carrier of the GAA c.2051C > A (p.P684Q) variant.

Pompe disease is a lysosomal-store age disorder with GAA mutation. We collected a Pompe disease family with c.2051C > A (p.P684Q) and c.2024_2026 delACA (p. N675del) variants in GAA. In this study, we generated a human induced pluripotent stem cell (iPSC) line from an asymptomatic woman carrying c.2051C > A (p.P684Q) variant in GAA. The iPSC line exhibited characteristic stem cell morphology, robust expression of pluripotency and proliferation-associated proteins, a standard karyotype, and the capacity to differentiate into trilineage. The iPSC line can serve as a family-based control for the

Clinical, biochemical, and genetic perspectives in hepatic glycogen storage diseases: HEPA-GSD study.

Glycogen storage diseases (GSDs) are a group of inherited metabolic disorders resulting from genetic deficiencies in enzymes involved in glycogen synthesis or breakdown, transport proteins, or proteins that regulate glycogen metabolism. Liver GSDs are further subdivided into subtypes based on the specific enzyme deficiency. This multicenter, cross-sectional, descriptive retrospective study, conducted across 24 centers, aimed to document the clinical and genotypic findings in patients with hepatic GSD. The study included 282 patients (152 males, 53.9%) from 278 unrelated families. At the time o

A newborn with glycogen storage disease type Ib presenting with hypoglycemia and evidence of mitochondrial dysfunction.

Glycogen storage disease type Ib (GSDIb) is an inherited disorder of carbohydrate metabolism that commonly presents in infancy. Here, we describe a case of GSDIb who presented on the first day of life with severe hypoglycemia and lactic acidosis, prompting a broad differential diagnosis. Additional testing revealed hyperlipidemia, relatively low ketones, and elevations of TCA-cycle intermediates in urine suggestive of primary or secondary mitochondrial dysfunction. The diagnosis of GSDIb was established by genome sequencing showing homozygous likely pathogenic variants in SLC37A4. This case hi

INTRODUCTION: Non-alcoholic fatty liver disease (NAFLD) is a metabolic disorder characterized by lipid accumulation in hepatocytes. Our previous study demonstrated that the herbal pair (HS) of Astragalus Radix (HQ) and Dioscoreae Rhizoma (SY) reduces blood lipids and improves hepatic lipid metabolism in high-fat diet-fed mice. PURPOSE: This study aimed to investigate the effects and potential active ingredients of HS in treating NAFLD using 1H NMR urine metabolomics and molecular docking. MATERIALS AND METHODS: Forty-eight male C57BL/6 mice (4 weeks old, 18-20 g) were fed a high-fat diet for

Posterior simulation-based calibration tests of phylogenetic dating methods.

Simulation-based calibration (SBC) checking is a method to ensure that the inference machinery for a Bayesian statistical analysis is functioning in a correct and unbiased manner. Typically, SBC begins with sampling parameter values from the model priors (prior SBC). However, it has been shown that prior SBC can miss problems when these manifest only in certain regions of parameter space. In phylogenetics, this is relevant not only because of the vastness of tree and parameter space, but also because many phylogenetic analyses involve some degree of model misspecification. Posterior SBC is a r

Assessment of epithelial barrier integrity in cellular monolayer models using low cost, clinically approved fluorescent contrast agents.

Epithelial barriers serve as the interface between body tissue and the external environment, maintaining tissue homeostasis by regulating the transport of various cells, molecules and microbes between these compartments. In particular, the epithelial barrier of the gut is a key component in numerous physiological processes that collectively define human health, such as nutrient absorption, microbiota regulation and neuroimmune communication. As such, gut epithelial barrier dysfunction, also known as 'leaky gut' syndrome has been associated with a variety of different conditions such as inflamm

In vivo base editing alleviates hepatic iron accumulation and fibrosis in models of HFE-related hereditary hemochromatosis.

BACKGROUND & AIMS: HFE-related hereditary hemochromatosis is caused by loss-of-function mutations in the HFE gene, leading to excessive intestinal iron absorption and hepatic deposition. The C282Y variant in homozygosity accounts for 80-90% of diagnosed cases. If untreated, iron accumulation can cause liver fibrosis, cirrhosis, and hepatocellular carcinoma. METHODS: We employed lipid nanoparticles (LNPs) to deliver base editor mRNA and single-guide RNA for in vivo correction of the HFE C282Y mutation in a murine model under iron challenge. Additionally, patient-derived induced pluripotent ste

Network Pharmacology and Molecular Docking Combined with Experimental Validation: The Mechanism of Brusatol in Regulating Glucose Metabolism in Lung Adenocarcinoma.

INTRODUCTION: Lung cancer ranks first globally in cancer-associated mortality, with Lung Adenocarcinoma (LUAD) accounting for 35%-40% of all cases. Metabolic reprogramming is a hallmark of malignant cells, and aberrant glycolytic turnover acts as a major driver of LUAD progression. Natural small molecules have long attracted attention in anticancer research. Brusatol (Bru), an active ingredient extracted from the traditional Chinese medicinal plant Brucea javanica, exhibits robust antitumor activity. Nevertheless, its precise molecular mechanism of action in LUAD remains incompletely understoo

Anterior Osteophytes at L3-L4 as a Primary Predictor of Adjacent Segment Degeneration After Lumbar Instrumented Fusion.

INTRODUCTION: Degenerative lumbar spine disorder (DLSD) is a significant public health concern, presenting with low back pain, radicular leg pain, and reduced quality of life. Although conservative management is the first-line treatment, some patients require surgery due to persistent symptoms or progressive neurological deficits. Lumbar instrumented fusion is a commonly performed procedure for DLSD. Adjacent segment degeneration (ASD) is a frequent complication after spinal fusion, occurring at either the cranial or caudal side of the fusion mass. While the role of spinopelvic parameters in A

Short-chain fatty acids alleviate metabolic disturbances induced by high-carbohydrate diets in largemouth bass (Micropterus salmoides).

This study evaluated the effects of dietary short-chain fatty acids (SCFAs) on carbohydrate metabolism in largemouth bass fed a high-carbohydrate diet. Juvenile fish (15.33 ± 0.14 g) were fed six isonitrogenous and isolipidic diets for 55 days, containing 10% (C, control), 15% (HC, high carbohydrate), and 20% (EC, extremely high carbohydrate) corn starch and three SCFA-supplemented diets based on the 15% carbohydrate diet level (0.2% sodium acetate (HC + A), propionate (HC + P), butyrate (HC + B)). High-carbohydrate diets significantly reduced growth performance increased hepatic lipid deposit

Reliable two-stage location problem for mitigating flood impact on sustainable agricultural production and waste management.

This paper is an Italian Expert Consensus Document on multidimensional treatment of obesity and eating disorders. The Document is based on a wide survey of expert opinion. It presents, in particular, considerations regarding how clinicians go about choosing the most appropriate site of treatment for a given patient suffering from obesity and/or eating disorders: outpatient, partial hospitalization, residential rehabilitation centre, inpatient hospitalization. In a majority of instances obesity and eating disorders are long-term diseases and require a multiprofessional team-approach. In determi

Autophagy impairment is associated with enhanced satellite cell activation in muscle biopsies from younger late-onset Pompe disease patients.

Late-onset Pompe disease (LOPD) is caused by α-glucosidase (GAA) deficiency, leading to glycogen accumulation resulting in progressive muscular weakness and respiratory insufficiency. Glycogen overload, vacuolation, and autophagic accumulation are the histological hallmarks of the disease. However, markers capable of tracking the progression of LOPD across different disease stages remain insufficiently characterized. We performed a comprehensive myopathologic analysis of eleven LOPD muscle biopsies from untreated patients (age range 7-69 years) and compared them to eleven biopsies from histolo

Stability of alglucosidase alfa in 0.9% sodium chloride for enzyme replacement therapy in patients with Pompe disease: insights from enzyme activity and cellular uptake measurements.

OBJECTIVES: Enzyme replacement therapy (ERT) with alglucosidase alfa is the cornerstone of treatment for Pompe disease, a rare disorder caused by acid α-glucosidase (GAA) deficiency. Since 2008, home infusions have been provided in the Netherlands. However, the short shelf-life of the ready-to-administer infusions poses challenges for manufacturing and dispensing pharmacies. This study assessed the stability of ready-to-administer alglucosidase alfa infusions by determining enzyme activity and cellular uptake of two concentrations during 11 days of storage. METHODS: Alglucosidase alfa infusio

Pompe Disease: A Review of Diagnosis, Molecular Genetics, and Treatment Management.

INTRODUCTION: Pompe disease, a rare autosomal recessive lysosomal storage disorder, results from mutations in the GAA gene that lead to deficient acid alpha-glucosidase activity and glycogen accumulation in various tissues. METHODS: This review employs the diagnostic approach to the disease, encompassing enzymatic assays and molecular genetics, with a focus on genotype-phenotype correlations and regionspecific mutations. RESULTS: Over 500 mutations in the GAA gene, including missense, nonsense, insertions, deletions, and splicing defects, contribute to varying levels of enzyme deficiency, ac

Quantification of muscle glycogen distribution in Pompe disease using 7 Tesla 13C NMR spectroscopy.

BACKGROUND: Late-onset Pompe disease has a characteristic pattern of fat replacement and wasting of especially axial and hamstring muscles. This characteristic pattern of muscle degeneration is still to be explained but could relate to differences in how glycogen is deposited in the different muscles. This cross-sectional observational study investigates the glycogen levels of different muscle groups in young late-onset Pompe subjects and matched controls. METHODS: 13C-MR Spectroscopy at 7 Tesla field strength was used to quantify glycogen concentration in four muscle groups: the calf, hamstr
