Aim: Treatment options for late-onset Pompe disease (LOPD) include enzyme replacement therapy (ERT) with alglucosidase alfa (alg), cipaglucosidase alfa plus miglustat (cipa + mig) and avalglucosidase alfa. However, only one randomized controlled trial (RCT) directly compared cipa + mig and alg and had relatively few ERT-naive patients. A multilevel network meta-regression (ML-NMR) integrated individual patient data and aggregate data into indirect treatment comparisons, with relative effects adjusted to any target population, to compare the efficacy of cipa + mig and alg. Glycogen storage disease 1b (GSD1b) typically presents in early infancy with poor fasting tolerance, hepatomegaly, and neutropenia. We report two siblings who were diagnosed with GSD1b in adulthood. Both had a normal fasting tolerance throughout childhood and, as adults, were able to fast for at least 16 h without developing hypoglycaemia. The older sibling developed nodular cirrhosis during adolescence. The younger sibling exhibited a more pronounced metabolic phenotype, including hyperuricaemia leading to recurrent gout and nephrolithiasis. He experienced occasional episodes of mild neutrope

Glycogen storage diseases (GSDs) are a group of inherited disorders caused by enzyme or transport protein defects that affect glycogen metabolism, leading to glycogen accumulation in tissues and overlapping clinical symptoms. This study aimed to identify pathogenic genetic variants in Iranian patients suspected of having GSDs and to characterize novel variants. Whole-exome sequencing was performed on 20 patients from consanguineous families, revealing 12 known and three novel pathogenic variants. Novel variants included a homozygous frameshift variant in SLC37A4 in a GSD-Ib patient, a homozygo

BACKGROUND: Glycogen storage disease type Ia is a rare inherited metabolic disorder often accompanied by renal complications; however, the dynamic progression and its renal pathology remain poorly understood.
CASE PRESENTATION: We report a genetically confirmed Chinese Han female with glycogen storage disease type Ia followed up from childhood to early adulthood (over a 17-year period), documenting the complete natural history of renal involvement. Proteinuria emerged in her childhood (age 6 years), followed by the onset of estimated glomerular filtration rate decreasing and overt Fanconi syn

Both Sodium Taurocholate Cotransporting Polypeptide Deficiency (NTCPD) and Glycogen Storage Disease Type VI (GSD-VI) are autosomal recessive (AR) genetic disorders that affect liver metabolism in newborns. NTCPD is primarily caused by mutations in the Solute Carrier Family 10 Member 1 (SLC10A1) gene, resulting in decreased bile acid transport function, while GSD-VI is caused by mutations in the phosphorylase glycogen liver (PYGL) gene, leading to a deficiency of liver glycogen phosphorylase. Both diseases are rare, and there have been no previous reports of their coexistence in a single patien

We report herein the design of multivalent pharmacological chaperones (PCs), based on D- and L-deoxynojirimycin (DNJ), to treat Pompe disease (PD), a lysosomal storage disorder caused by mutations in the acid alpha-glucosidase (GAA) enzyme. Multivalent DNJ platforms were built on a pentaerythritol core by Copper Azide-Alkyne Click (CuAAC) ligation. Moreover, new phosphorus-containing dendrimers were decorated with DNJ by Strain Promoted Azide-Alkyne Click (SPAAC) reaction. Our main objective was to systematically study how the chirality and the hydrophobicity of these constructs could modulate

BACKGROUND: Allergic rhinitis (AR), an IgE mediated inflammatory disease, significantly impacts quality of life of a considerable proportion of the general population. Omalizumab, a humanized monoclonal antibody against IgE, has been evaluated for both seasonal and perennial AR. We aimed to assess the efficacy and safety of omalizumab in randomized controlled trials (RCTs) in inadequately controlled AR.
METHODS: We conducted a systematic literature search of RCTs evaluating the safety and efficacy of omalizumab in AR. We synthesized evidence for clinical improvement of AR symptoms, quality of

Atherosclerosis reflects a chronic inflammatory process of arteries. The origin of chronic vascular inflammation has been associated over a long time primarily with lipid disorders, but evidence from the past years has suggested that lipid-independent pathways are also involved. Recent research has demonstrated that the gastrointestinal microbiota has an impact on the development of atherosclerosis. Many clinical studies have revealed that there exist altered gut microbiota and increased intestinal abundance of bacteria from the oral cavity in atherosclerosis-related disorders such as cardiova

BACKGROUND AND OBJECTIVES: Late-onset Pompe disease (LOPD) causes progressive muscle weakness, locomotor dysfunction, and postural instability, severely affecting quality of life. Longitudinal data on these changes remain limited. We quantified changes in muscle strength, locomotor performance, and postural control in adults with LOPD vs controls, aiming to identify the most sensitive motor parameters and determine key muscle contributors to locomotor and postural decline. We also defined clinically meaningful thresholds (minimal clinically important difference [MCID]) for sensitive parameters

Glycogen storage disease type Ia (GSD Ia) is a rare autosomal recessive inherited disorder of carbohydrate metabolism, caused by a deficiency in glucose 6-phosphatase-α (G6PC1). Patients primarily suffer from failure to thrive, hepatomegaly, and severe fasting intolerance, biochemically characterized by hypoketotic, hypoglycemia, and hyperlipidemia. Because of clinical and biochemical heterogeneity, identifying biomarkers is imperative for prognosis and monitoring. An untargeted proteomics workflow was employed for identifying protein changes in liver and plasma from hepatocyte-specific G6pc k

BACKGROUND: HyperCKemia, defined as elevated serum creatine kinase, commonly reflects muscle injury but may also indicate underlying metabolic disease. Metabolic aetiologies, including glycogen storage disorders, fatty acid oxidation defects, mitochondrial cytopathies, and purine metabolism disorders, are clinically important owing to diagnostic complexity, therapeutic implications, and potential reversibility.
OBJECTIVE: To summarise current evidence on metabolic causes of hyperCKemia in adults, with emphasis on disorders of carbohydrate, lipid, and purine metabolism and mitochondrial diseas

McArdle disease (McA) is a rare metabolic disorder of autosomal recessive inheritance caused by pathogenic variants in the PYGM gene, which lead to a deficiency of the myophosphorylase enzyme. This enzymatic defect impairs muscle glycogenolysis, typically resulting in exercise intolerance, premature fatigue, and exertional cramps triggered by anaerobic or high-intensity physical activity starting in childhood or adolescence. However, the diagnosis is frequently delayed due to the heterogeneous and non-specific presentation of these symptoms. The authors report a case of a 61-year-old woman wit

Glycogen Storage Disease Type IIIA (GSD IIIA) is a rare autosomal recessive metabolic disorder affecting the liver, skeletal muscle, and heart. The physiological changes of pregnancy exacerbate the underlying metabolic disturbances of this condition, complicating maternal and fetal outcomes. We present the case of a 22-year-old woman with GSD IIIA who experienced a successful pregnancy and delivery through comprehensive multidisciplinary management, including metabolic, obstetric, dietary, anaesthetic, and neonatal care. This case contributes to the limited but growing body of literature on pr
