Infusion-related reactions (IRRs), serious immune events that occur either during or within six hours of an infusion, can occur with adeno-associated virus (AAV) gene therapy. IRRs may be attributed to IgE-mediated hypersensitivity, leading to treatment discontinuation due to concern about progressive, life-threatening allergic reactions. IRRs were observed in two participants who received an AAV8 gene therapy (DTX401) for glycogen storage disease type Ia and in one participant who received an AAV9 gene therapy (UX701) for Wilson's disease. All of these IRRs were associated with a rapid infusi

Background: Glycogen storage disease type 1a (GSD 1a) is an inborn error of metabolism due to glucose-6-phosphatase deficiency. Treatment consists of frequent, carbohydrate-rich meals, intake of glucose polymers like uncooked cornstarch, or drip feeding in small infants. Although dietary treatment is challenging, it has improved the long-term outcomes for patients with GSD 1a. Nevertheless, several complications, including dyslipidaemia, remain difficult to manage, and early atherosclerosis with risk for ischemic stroke is a potential long-term concern. We propose bempedoic acid (BA) as a pote

Pompe disease is a lysosomal-store age disorder with GAA mutation. We collected a Pompe disease family with c.2051C > A (p.P684Q) and c.2024_2026 delACA (p. N675del) variants in GAA. In this study, we generated a human induced pluripotent stem cell (iPSC) line from an asymptomatic woman carrying c.2051C > A (p.P684Q) variant in GAA. The iPSC line exhibited characteristic stem cell morphology, robust expression of pluripotency and proliferation-associated proteins, a standard karyotype, and the capacity to differentiate into trilineage. The iPSC line can serve as a family-based control for the

Glycogen storage diseases (GSDs) are a group of inherited metabolic disorders resulting from genetic deficiencies in enzymes involved in glycogen synthesis or breakdown, transport proteins, or proteins that regulate glycogen metabolism. Liver GSDs are further subdivided into subtypes based on the specific enzyme deficiency. This multicenter, cross-sectional, descriptive retrospective study, conducted across 24 centers, aimed to document the clinical and genotypic findings in patients with hepatic GSD. The study included 282 patients (152 males, 53.9%) from 278 unrelated families. At the time o

Glycogen storage disease type Ib (GSDIb) is an inherited disorder of carbohydrate metabolism that commonly presents in infancy. Here, we describe a case of GSDIb who presented on the first day of life with severe hypoglycemia and lactic acidosis, prompting a broad differential diagnosis. Additional testing revealed hyperlipidemia, relatively low ketones, and elevations of TCA-cycle intermediates in urine suggestive of primary or secondary mitochondrial dysfunction. The diagnosis of GSDIb was established by genome sequencing showing homozygous likely pathogenic variants in SLC37A4. This case hi

Simulation-based calibration (SBC) checking is a method to ensure that the inference machinery for a Bayesian statistical analysis is functioning in a correct and unbiased manner. Typically, SBC begins with sampling parameter values from the model priors (prior SBC). However, it has been shown that prior SBC can miss problems when these manifest only in certain regions of parameter space. In phylogenetics, this is relevant not only because of the vastness of tree and parameter space, but also because many phylogenetic analyses involve some degree of model misspecification. Posterior SBC is a r

History An asymptomatic 45-year-old man with sickle cell disease and coinherited α-thalassemia was referred to the hepatology department for workup of an indeterminate hepatic mass discovered incidentally at screening cardiac MRI. The patient was diagnosed with sickle cell disease in childhood and required several hospitalizations with blood transfusions. During adulthood, disease management was suboptimal, with infrequent transfusion therapy and undertreated chronic hemolytic anemia. There was no history of malignancy, hepatitis B or C infection, alcohol consumption, metabolic syndrome, diabe

INTRODUCTION: Degenerative lumbar spine disorder (DLSD) is a significant public health concern, presenting with low back pain, radicular leg pain, and reduced quality of life. Although conservative management is the first-line treatment, some patients require surgery due to persistent symptoms or progressive neurological deficits. Lumbar instrumented fusion is a commonly performed procedure for DLSD. Adjacent segment degeneration (ASD) is a frequent complication after spinal fusion, occurring at either the cranial or caudal side of the fusion mass. While the role of spinopelvic parameters in A

This paper is an Italian Expert Consensus Document on multidimensional treatment of obesity and eating disorders. The Document is based on a wide survey of expert opinion. It presents, in particular, considerations regarding how clinicians go about choosing the most appropriate site of treatment for a given patient suffering from obesity and/or eating disorders: outpatient, partial hospitalization, residential rehabilitation centre, inpatient hospitalization. In a majority of instances obesity and eating disorders are long-term diseases and require a multiprofessional team-approach. In determi

Late-onset Pompe disease (LOPD) is caused by α-glucosidase (GAA) deficiency, leading to glycogen accumulation resulting in progressive muscular weakness and respiratory insufficiency. Glycogen overload, vacuolation, and autophagic accumulation are the histological hallmarks of the disease. However, markers capable of tracking the progression of LOPD across different disease stages remain insufficiently characterized. We performed a comprehensive myopathologic analysis of eleven LOPD muscle biopsies from untreated patients (age range 7-69 years) and compared them to eleven biopsies from histolo

OBJECTIVES: Enzyme replacement therapy (ERT) with alglucosidase alfa is the cornerstone of treatment for Pompe disease, a rare disorder caused by acid α-glucosidase (GAA) deficiency. Since 2008, home infusions have been provided in the Netherlands. However, the short shelf-life of the ready-to-administer infusions poses challenges for manufacturing and dispensing pharmacies. This study assessed the stability of ready-to-administer alglucosidase alfa infusions by determining enzyme activity and cellular uptake of two concentrations during 11 days of storage.
METHODS: Alglucosidase alfa infusio

INTRODUCTION: Pompe disease, a rare autosomal recessive lysosomal storage disorder, results from mutations in the GAA gene that lead to deficient acid alpha-glucosidase activity and glycogen accumulation in various tissues.
METHODS: This review employs the diagnostic approach to the disease, encompassing enzymatic assays and molecular genetics, with a focus on genotype-phenotype correlations and regionspecific mutations.
RESULTS: Over 500 mutations in the GAA gene, including missense, nonsense, insertions, deletions, and splicing defects, contribute to varying levels of enzyme deficiency, ac

BACKGROUND: Late-onset Pompe disease has a characteristic pattern of fat replacement and wasting of especially axial and hamstring muscles. This characteristic pattern of muscle degeneration is still to be explained but could relate to differences in how glycogen is deposited in the different muscles. This cross-sectional observational study investigates the glycogen levels of different muscle groups in young late-onset Pompe subjects and matched controls.
METHODS: 13C-MR Spectroscopy at 7 Tesla field strength was used to quantify glycogen concentration in four muscle groups: the calf, hamstr

A male infant born at 38 weeks of gestation was found to have myocardial hypertrophy on neonatal screening echocardiography, with a left ventricular mass index (LVMI) of 102.1 g/m2 (Z-score 3.1). No skeletal muscle weakness was noted. At 2 months, he developed feeding difficulty and progressive myocardial hypertrophy and was rehospitalized at 3 months with severe cardiomegaly (LVMI 419.5 g/m2, Z-score 12.3) and respiratory distress. Based on reduced GAA enzymatic activity and genetic testing revealing a pathogenic GAA mutation, infantile-onset Pompe disease (IOPD) was confirmed. Enzyme replace

Brain glycogen has long been regarded uniquely as a source of energetic support in situations of emergency or heightened activity. Recently, brain glycogen was found to contain a significant amount of glucosamine, which is used to sustain protein glycosylation. In this update, we highlight that glucosamine synthesis through the hexosamine pathway would subtract glutamine, which is indispensable for glutamate and GABA recycling. Brain glycogen seems then to serve an additional role. By providing glucosamine and, through it, inhibiting the hexosamine pathway, glycogen avoids glutamine depletion.

Kidney disease in glycogen storage disease type Ia (GSD-Ia), deficient in glucose-6-phosphatase-α (G6Pase-α), is associated with acute kidney injury (AKI) and renal fibrosis. During AKI, autophagy is typically activated to eliminate protein aggregates and damaged organelles; however, sustained autophagy can contribute to maladaptive repair and fibrosis. Using a GSD-Ia mouse model, we demonstrate that renal G6Pase-α deficiency results in heightened autophagy activation, as indicated by increased expression of multiple autophagy-related components and enhanced autophagic flux. Notably, both posi

BACKGROUND: Pompe disease (PD), caused by acid α-glucosidase (GAA) deficiency, leads to glycogen accumulation in various tissues including the central nervous system. While enzyme replacement therapy (ERT) is lifesaving, it does not cross the blood-brain barrier. CNS manifestations including sensorineural hearing loss, dysarthria, and cognitive delay-persist despite ERT. Hence, CNS-specific biomarkers are needed to identify at-risk patients and guide monitoring and treatment. This clinical, retrospective study evaluated the utility of plasma glial fibrillary acidic protein (GFAP) as a potentia

Genetic disorders in neonates often present with overlapping clinical features, posing significant diagnostic challenges. Glycogen storage diseases (GSD) disrupt glycogen metabolism, leading to energy deficits. Pathogenic variants in the Wilms tumor 1 (WT1) gene represent a rare but significant cause of early-onset steroid-resistant nephrotic syndrome (SRNS), associated with a broad range of both kidney and extrakidney phenotypic manifestations. The coexistence of these genetic diseases in a single patient has not been previously reported. Herein, we present a case of a newborn with symptomati
