INTRODUCTION: Early and targeted therapy is pivotal for the outcome of bloodstream infection, a life-threatening condition with high mortality. EUCAST developed a protocol for rapid antimicrobial susceptibility testing (RAST) that allows testing directly from positive blood cultures (BCs). Recently, fully automated minimum inhibitory concentration (MIC)-based systems were introduced. This is the first study to compare two of these systems with agar-based RAST and genotypic resistance testing.
METHODS: Ninety-seven Gram-negative BCs were prospectively analyzed using five different RAST assays.

OBJECTIVE: Antibiotic therapy is typically initiated and continued for 36 or 48 hours in neonatal patients while awaiting blood culture results. Our study aims to determine time to positivity (TTP) of positive blood cultures in a level IV neonatal intensive care unit (NICU) to determine the appropriate duration of empiric antibiotics.
PATIENTS AND METHODS: This is a retrospective study of all patients admitted to the NICU with a positive blood culture from January 2021 to March 2025. Only the first positive blood culture per patient was included in the analysis unless a new organism was ident

Catheter-related bloodstream infections are a significant complication in haemodialysis patients. Water-associated Gram-negative organisms are increasingly recognised in this setting. We report a case of catheter-related bloodstream infection due to Aeromonas dhakensis in a 52-year-old woman on maintenance haemodialysis. The organism was identified using MALDI-TOF MS and showed in vitro susceptibility to ceftazidime. However, the clinical response to the antibiotic remained suboptimal until catheter removal. Repeat blood cultures after removal were sterile. This case highlights the emerging cl

Objective Late-onset neonatal sepsis (LONS) remains a significant cause of neonatal morbidity and mortality, especially in developing countries. Our objective was to identify vitamin D deficiency in LONS, its association with maternal vitamin D deficiency, and its relationship with microbial profile and mortality in late-onset sepsis (LOS). Study design We conducted an analytical observational study with two matched groups (Group A and Group B) over 29 months, from November 2018 to March 2021, in the level III outborn neonatal unit at a tertiary-care centre in New Delhi, India. A total of 320

Oligella urethralis is a gram-negative, non-fermentative coccobacillus that is considered part of the normal urogenital flora but can cause opportunistic infections in immunocompromised patients or those with underlying conditions. Coccobacilli such as O. urethralis are rarely encountered as urinary pathogens compared to typical rod-shaped bacteria, making their identification clinically significant. We report a case of O. urethralis bacteremia in a 74-year-old male long-term care facility resident with ureteral stones who presented with acute pyelonephritis and bacteremia. Blood and urine cul

BACKGROUND: Sepsis remains a leading cause of morbidity and mortality, particularly when caused by multidrug-resistant organisms (MDROs). Early identification of colonising or infecting pathogens may inform initial antimicrobial selection. Surveillance cultures, providing microbiological data prior to infection onset, could guide timely and targeted therapy. This retrospective study analysed routine surveillance culture results from patients with bloodstream infection (BSI) episodes, assessing pathogen prevalence, resistance phenotypes, and concordance with specimen type in haemato-oncology (H

OBJECTIVES: The duration of neutropenia is prolonged in cord blood transplantation, rendering infection management critical. Carbapenems have traditionally been employed as last-resort antibiotics; however, carbapenem-resistant bacteria have emerged as major concerns worldwide. Cefiderocol (CFDC) is a novel antimicrobial agent that exhibits activity against carbapenem-resistant nonfermenting gram-negative bacilli, such as resistant Pseudomonas aeruginosa and Stenotrophomonas maltophilia, and has thus attracted considerable attention.
DESIGN OR METHODS: Herein, we report a case in which a pati

Klebsiella pneumoniae is a Gram-negative species that is a leading cause of hospital-associated infections. Such infections can result in bacteremia, when bacteria disseminate to the bloodstream and colonize filtering organs. While interactions between alveolar macrophages and K. pneumoniae have been described in the context of pneumonia, less is known about interactions between K. pneumoniae and monocyte-derived macrophages, which are present during bacteremia across tissues. The antibacterial stress mechanisms used by innate immune cells and the genes K. pneumoniae utilizes to resist macroph

PURPOSE: Procalcitonin (PCT) is an acute-phase protein and widely used marker for diagnosing bacterial infection and sepsis, but its physiological role remains incompletely defined. Interleukin-1β (IL-1β) is a critical mediator of the immune response to infection, whose excessive release can drive remote organ injury and dysfunction. Its secretion is therefore tightly controlled. Because other acute-phase proteins have been shown to regulate IL-1β secretion, we investigated whether PCT exerts a similar immunomodulatory effect and whether this influences sepsis severity, particularly in Gram-ne

Introduction Sepsis is one of the leading causes of ICU admissions, with a substantial proportion of patients developing sepsis-associated acute kidney injury (S-AKI). In such cases, continuous renal replacement therapy (CRRT) is a cornerstone of renal supportive care; however, evidence from low-resource settings (LRS) remains limited. This study aimed to describe the demographic and clinical characteristics of patients with S-AKI treated with CRRT in an LRS medical ICU (MICU), as well as to identify predictors of mortality. Subjects and methods This retrospective, observational, consecutive,

Matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) offers fast identification to improve time to appropriate antibiotics in Gram-negative bloodstream infections (GN-BSIs). This retrospective study evaluated patients with GN-BSI PRE- and POST-implementation of rapid identification with and without direct antimicrobial susceptibility testing (AST). Patients aged 0-18 years with GN-BSI before and after implementation of direct blood culture broth MALDI-TOF MS and AST were evaluated. Cohorts were defined by methods of organism identification and AST: tradit

Chimeric antigen receptor T-cell therapy-related adverse events have become a focus of clinical attention. However, a comprehensive study of infection-related adverse events remains scarce, especially for high-risk, life-threatening infections. To provide real-world evidence from a pharmacovigilance perspective for monitoring infection-related adverse events. This study investigated the characteristics of infection-related adverse events using the Food and Drug Administration Adverse Event Reporting System database. The disproportionality analysis was used to calculate the reporting odds ratio

BACKGROUND: Difficult-to-treat resistant (DTR) Gram-negative infections show resistance to all first-line antibiotics (ie, β-lactams and fluoroquinolones) and have a 40% greater mortality rate than susceptible infections. New antibiotics are now available with improved safety and efficacy and with in-vitro activity against DTR infections; however, their influence on the outcomes of patients with DTR infections remains unclear. We aimed to evaluate whether and why mortality in patients with DTR infections has changed since the introduction of these newer antibiotics in the USA.

BACKGROUND: Timely active empirical therapy (AET) may improve outcomes following bloodstream infection. In high-income settings, low pediatric mortality following gram-negative bloodstream infection (GNBSI) limits its value as a sole endpoint. Composite measures that incorporate multiple clinically relevant outcomes may detect differences more readily. We assessed the effect of AET on clinical outcomes in pediatric GNBSI.
METHODS: We analyzed data from a prospective surveillance study of hospitalized Australian children with GNBSI. A ranked composite endpoint included 30-day all-cause mortali

Comamonas testosteroni is a gram-negative aerobic bacillus commonly found in environmental sources and known for its bioremediation potential. Although it is widely distributed in nature, human infection is rare, but it is increasingly being recognized as a nosocomial pathogen. A 68-year-old female with stage 5 chronic kidney disease (CKD V) on maintenance hemodialysis since 2017 presented with fever and decreased consciousness. On examination, a tunneled cuffed catheter (TCC) was noted in the right internal jugular vein. Two sets of blood cultures, one drawn from the TCC and the other from a

Pseudomonas aeruginosa is an opportunistic Gram-negative pathogen and a major clinical concern due to its multidrug resistance and extensive virulence repertoire. In this study, recombinant PcrV protein was formulated with Alum and chitosan nanoparticles (CS-NPs) as co-adjuvants to enhance immunogenicity. CS-NPs were prepared via ionic gelation and characterized for morphology and charge. BALB/c mice were immunized with formulations containing r-PcrV + Alum + Nanochitosan, r-PcrV + Alum, r-PcrV + Nanochitosan, or Nanochitosan + Alum or PBS (control). This was followed by burn induction, after

Background Sepsis and septic shock remain the major causes of mortality in intensive care units (ICUs), increasingly complicated by antimicrobial resistance. However, microbiological profiles and resistance patterns vary widely across regions, and data from North African ICUs remain limited. This study aimed to describe the distribution of pathogens according to infection site, to analyze antimicrobial susceptibility patterns, and to assess the burden of multidrug-resistant organisms among ICU patients with sepsis and septic shock. Methods We conducted a prospective observational study in a mi

Background and Objectives: Urosepsis is a common cause of sepsis in adults and is associated with substantial morbidity and mortality, particularly when urinary obstruction delays timely source control. The roles of diagnostic uncertainty at presentation, microbiological phenotypes (including multidrug resistance), and biomarkers in shaping management pathways and outcomes warrant further evaluation. Materials and Methods: This retrospective, single-center, observational study included 154 consecutive adult patients hospitalized for urosepsis. Sepsis was defined according to the Sepsis-3 crite

Chromobacterium violaceum is a rare but highly virulent Gram-negative organism found in tropical and subtropical environments, associated with rapidly progressive sepsis and high mortality. Musculoskeletal involvement, particularly septic arthritis, is exceedingly uncommon. We report a case of a previously healthy 23-year-old male who presented with progressive left ankle pain, swelling, and fever. Initial evaluation revealed thrombocytopenia, markedly elevated inflammatory markers, and positive serology for Dengue virus infection, suggestive of a possible concurrent infection. Blood cultures
