BACKGROUND: Bloodstream infections (BSI) are a significant contributor to morbidity and mortality among hospitalized patients. Rapid diagnosis of BSI is critical for timely and appropriate clinical management, but current diagnostic strategies have a prolonged turnaround time or only identify select pathogens with minimal prognostic information.
METHODS: We examined serial serologic samples from 77 intensive care unit (ICU) patients at time points before, during, and after diagnosis of presumptive BSI to identify host-derived protein biomarkers associated with specific organisms and with clin

Gram-negative bacteria are clinically significant pathogens responsible for life-threatening infections, including respiratory infections. These can be acute or persistent and can exacerbate existing chronic diseases, such as cystic fibrosis, COPD and lung cancer. In this review, we use Pseudomonas aeruginosa as a model organism that demonstrates the molecular complexity of host-pathogen interactions during lower airway infection. Specifically, we focus on RNA modifications and show that they, on the one hand, regulate bacterial fitness and pathogenicity, and on the other control the execution

Neutropenic patients colonized with extended-spectrum β-lactamase-producing Enterobacterales (ESBL-E) frequently develop ESBL-E bacteremia while receiving fluoroquinolone prophylaxis. However, the impact of colonization with ESBL-E and AmpC β-lactamase-producing Enterobacterales (AmpC-E) in neutropenic patients who do not receive fluoroquinolone prophylaxis is unclear. To investigate this, we conducted a prospective study of patients with acute leukemia or myelodysplastic syndrome (MDS) receiving induction chemotherapy without antibacterial prophylaxis between November 2015 and January 2020. BACKGROUND AND OBJECTIVE: Sepsis is defined as SIRS (systemic inflammatory response syndrome) in the presence of confirmed or suspected infection. Neonatal sepsis remains a major cause of morbidity and mortality and is frequently complicated by septic shock, resulting in impaired tissue perfusion and organ dysfunction. Hemodynamic alterations in septic neonates are heterogeneous and often under-recognized. The primary objective of this study was to characterize the hemodynamic profile of neonates with sepsis for early detection of circulatory dysfunction using functional echocardiography. The

OBJECTIVES: Francisella tularensis is a Gram-negative coccobacillus causing tularemia. Due to its high infectivity and potential for aerosol transmission, it is considered a potential biological weapon. Although tularemia is rare in most regions, it can cause severe community-acquired infections, particularly in individuals with environmental exposure, and may be life-threatening.
PATIENTS AND METHODS: We report a confirmed case of F. tularensis infection diagnosed despite diagnostic difficulties. The patient developed pulmonary tularemia complicated by sepsis and lung abscess formation. The

BACKGROUND: This study provides the first prospective, genomically validated dataset integrated into the global MultiDrug-Resistant Organism (MDRO) Network from the Middle East and North Africa region. To address the substantial gap in antimicrobial resistance (AMR) data, the American University of Beirut Medical Center (AUBMC) investigated carbapenem-resistant Enterobacterales (CRE), Acinetobacter baumannii (CRAB), and Pseudomonas aeruginosa (CRPA).
METHODS: Consecutive, hospitalized patients whose clinical cultures were positive for CRE, CRAB, or CRPA were prospectively enrolled between Jun

INTRODUCTION: Carbapenem-resistant, Gram-negative bacteria (GNB) are a significant cause of neonatal sepsis, threatening survival. There is a scarcity of robust data on their burden and outcomes in low- and middle-income countries (LMICs). We aimed to describe clinical and microbiological characteristics, outcomes and mortality risk factors in neonates infected with GNB and assess the impact of carbapenem resistance.
METHODS: Prospective observational cohort study from 1 February 2021 to 30 June 2023 conducted in a tertiary neonatal unit in Johannesburg, South Africa, in infants <90 days with

Early-onset neonatal sepsis (EONS) is predisposed by maternal colonisation and neonatal risk factors. We present two cases of preterm neonates with vertically transmitted extended-spectrum β-lactamase (ESBL)-producing Escherichia coli sepsis. In the United Kingdom (UK), the common organisms causing EONS episodes are susceptible to the routinely used first-line empirical antibiotics, i.e., benzylpenicillin and gentamicin. These cases observed in a local neonatal unit highlight the growing concern of drug-resistant E. coli infections in this highly vulnerable age group. Gram-negative bacterial s

BACKGROUND: Neonatal sepsis is a significant cause of morbidity and mortality, especially in developing countries. Periodic assessment and documentation of clinical and bacteriological profiles and antibiotic susceptibility patterns of neonatal sepsis across geographic regions is essential to guide rational management.
METHODOLOGY: This observational, cross-sectional study was conducted at the neonatal intensive care unit (NICU) of Mahatma Gandhi Mission (MGM) Medical College and Hospital, Aurangabad, from September 2022 to June 2024. A total of 200 neonates between 28 and 42 weeks of gestati

Sepsis caused by Gram-negative bloodstream infection is characterized by dynamic host responses, but the longitudinal and incremental prognostic value of calprotectin remains uncertain. We conducted a multicenter retrospective observational study of 82 adults with Gram-negative bloodstream infection, sepsis, and acute respiratory failure. Serum calprotectin, routine biomarkers, cytokines, and growth factors were measured at diagnosis before antibiotic therapy (T0), day 3 (T1), and day 7 (T2), and compared according to 28-day survival. Sixty-four patients survived, and 18 died. Calprotectin dec

In an international cohort of hospitalized patients included in the Multi-Drug-Resistant Organism Network with carbapenem-resistant Gram-negative infections, increased age-adjusted Charlson Comorbidity Index scores were associated with increased mortality particularly in those with respiratory and less so in those with bloodstream infections. Cirrhosis, diabetes, and peripheral vascular disease were important contributors.

BACKGROUND: Bloodstream infections (BSIs) require rapid identification and susceptibility testing to guide timely antimicrobial therapy. Although paired aerobic and anaerobic blood culture bottles are routinely used, the incremental benefit of anaerobic bottles in settings with low anaerobic bacteremia prevalence remains uncertain. This study compared paired aerobic bottles with paired aerobic anaerobic bottle combinations in terms of pathogen isolation rate, time-to-positivity (TTP), and contaminant recovery.
METHODS: This 12-month descriptive study included 1009 adult inpatients with suspec

Diabetic foot infection (DFI) is a common complication of diabetes mellitus and a major cause of preventable morbidity and mortality in individuals with diabetes. Superficial DFI can lead to contiguous spread to soft tissue and bone as well as hematogenous spread and sepsis. It frequently involves typical pathogens such as Staphylococcus aureus and Pseudomonas aeruginosa, or it may be polymicrobial, involving Gram-negative bacilli and anaerobes. Atypical organisms can also be responsible, especially when environmental exposures are present. We report a rare case of Pasteurella multocida osteom

Background Bloodstream infections contribute to morbidity and mortality. A wide variety of microorganisms can be identified and recovered from these infections, and blood culture is the primary method for detecting them and determining their antimicrobial susceptibility. This helps clinicians confirm the efficacy of the initial empiric therapy or select an appropriate alternative based on the susceptibility of the isolated microorganism, thereby reducing the risks associated with antimicrobial misuse. We aimed to describe and analyze the bacteria isolated in blood culture samples, their antimi

BACKGROUND: Bloodstream infections (BSIs) are a major cause of morbidity and mortality among critically ill patients, particularly in intensive care units (ICUs). Increasing antimicrobial resistance among bloodstream pathogens further complicates management and limits effective empirical therapy.
METHODS: This retrospective observational study was conducted in the Department of Microbiology at a tertiary care teaching hospital in central India. Blood culture samples received from various ICUs over a one-year period (July 2024 to June 2025) were analyzed. Organisms were identified using standa

INTRODUCTION: Central venous catheters (CVCs) are commonly inserted in critically ill patients. However, their essential use increases the risk of complications, notably central line-associated bloodstream infections (CLABSIs). In recent years, CLABSIs have garnered growing attention due to their significant impact on patient outcomes.
AIMS: The aim of this study was to determine the incidence of CLABSIs in the intensive care units (ICUs) of a tertiary care hospital.
MATERIALS AND METHODS: This prospective study was carried out in the Department of Microbiology, KIMS Hospital, Amalapuram, Ko

BACKGROUND: Early-onset sepsis (EOS) is a major cause of neonatal morbidity and mortality, but its diagnosis remains challenging because early clinical manifestations are nonspecific and blood culture results are not immediately available. Consequently, many neonates receive empirical antibiotics despite subsequently having negative blood cultures, increasing unnecessary antibiotic exposure and prolonging neonatal intensive care unit (NICU) stay. Readily available laboratory markers such as complete blood count (CBC) parameters and C-reactive protein (CRP) are commonly used to support clinical
