Clinical infectious diseases : an official publication of the Infectious Diseases Society of America2025eng
BACKGROUND: Bloodstream infections (BSIs) caused by carbapenem-resistant gram-negative bacilli (CR-GNB) are difficult-to-treat infections associated with high mortality. Outcomes of BSI due to CR-GNB may vary in different countries.
METHODS: Prospective observational, multicenter study, including consecutive hospitalized index patients aged ≥18 years, with a positive blood culture for CR-GNB, between July 2020 and March 2022 in Argentina. Among patients with Klebsiella pneumoniae BSIs, logistic regressions adjusted by propensity score (PS) were used to identify variables associated with 30-da

The host immune system uses reactive oxygen species (ROS) and reactive nitrogen species to build up an oxidative response as a host defense mechanism against infectious agents. However, uncontrolled ROS causes excessive inflammation and oxidative stress, which is counteracted by host antioxidant responses. Oxidative stress is recognized as an imbalance between the production of oxidants such as ROS and antioxidant defenses, which account for organ damage following severe infection with bacterial pathogens. Ehrlichia is an obligatory intracellular Gram-negative bacterial pathogen that causes hu

BACKGROUND: A fifth of blood cultures from patients with infections acquired in a healthcare setting in South Africa will yield an organism. Carbapenem-resistant Enterobacterales (CREs), specifically Klebsiella pneumoniae are the most predominant Gram-negative bacteria (GNB) isolated among bloodstream infections (BSI). Additionally, the multidrug-resistant nature of these organisms is not only a threat to patients but it also poses a big public health challenge to current treatments and highlights the need for the development of new antimicrobials. Therefore, CRE have been placed on the critic

Late-onset neonatal sepsis (LOS) in very low birth weight (VLBW) preterm infants varies in severity from mild to fatal and often presents with cardiorespiratory instability. We hypothesized that heart rate (HR) and systemic oxygenation (SpO2) patterns would differ based on pathogen type and mortality. We analyzed HR and SpO2 changes before LOS diagnosis for Gram-negative, Coagulase-negative Staphylococcus, and other Gram-positive bacteria (GN, CONS, OGP) and compared survivors and nonsurvivors. Using continuous every-2-second vital sign data from 365 VLBW infants in four neonatal intensive care

BACKGROUND: Immunocompromised patients with sepsis face higher mortality than immunocompetent individuals. However, data on bloodstream infections (BSIs) and sepsis among critically ill immunocompromised (CII) patients remain limited. We aimed to describe the epidemiology, outcomes, and mortality risk factors of BSIs in this population.
METHODS: We conducted a retrospective cohort study of CII patients admitted to the medical ICU between January 2022 and December 2023 with suspected sepsis or septic shock. Patients with BSIs confirmed by positive blood cultures were identified. Propensity sco

Background Neonatal sepsis remains a significant cause of morbidity and mortality among infants ≤ 28 days of age, especially in developing and underdeveloped countries, ranking as the third major cause of death among neonates. Klebsiella pneumoniae is a Gram-negative bacterium of the family Enterobacteriaceae, a frequent pathogen in sepsis, and often exhibits multi-drug resistance (MDR), thereby complicating therapeutic options and prolonging hospital stay. This study aimed to evaluate the current trends in the bacteriological profile of neonatal sepsis and assess antimicrobial susceptibility

BACKGROUND: Bloodstream infections caused by bacteria with carbapenem resistance are associated with high mortality. Cefiderocol has broad in vitro activity against carbapenem-resistant Gram-negative bacilli. We aimed to assess whether cefiderocol was non-inferior or superior to standard-of-care antibiotics in preventing mortality in patients with bloodstream infection caused by Gram-negative bacilli.
METHODS: The GAME CHANGER trial was an open-label, parallel-group, randomised clinical trial in which patients with health-care-associated or hospital-acquired Gram-negative bloodstream infectio

BACKGROUND: Critically ill patients admitted to ICUs are at high risk of severe infections that may progress to sepsis and multi-organ dysfunction. Microbial toxins, such as endotoxins from Gram-negative bacteria and exotoxins from Gram-positive organisms, play a major role in the pathogenesis of systemic inflammation and tissue injury. While biochemical markers are routinely used to monitor organ function, their direct relationship with microbial toxin levels in ICU patients is not well established.
OBJECTIVES: To determine the correlation between circulating microbial toxin levels and bioch

Currently available rapid blood culture diagnostics detect few gram-negative resistance determinants, limiting their clinical utility. We prospectively evaluated the prototype BIOFIRE FILMARRAY Antimicrobial Resistance (AMR) Panel, a rapid multiplex PCR test that detects 31 AMR genes, on residual positive blood culture broths from patients with gram-negative bacteremia due to five target organisms at a New York City hospital. Predicted antimicrobial resistance based on the AMR Panel was compared to results from broth microdilution testing of bloodstream isolates recovered in culture. A simulat

The VITEK REVEAL antimicrobial susceptibility testing (AST) system is a fast in vitro diagnostic device that determines the antimicrobial susceptibility profile of gram-negative organisms from positive blood culture after independent organism identification. The objectives of this study were to (i) compare the performance of VITEK REVEAL to disk diffusion (DD) and (ii) evaluate implementation of VITEK REVEAL into a laboratory workflow that employs matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) as the primary method for organism identification. Chromobacterium violaceum is a Gram-negative bacillus known to be a rare cause of human infection. The organism enters the body through skin injury and usually presents as a localized skin infection that can progress to multiple abscesses and multiorgan failure. To our knowledge, this is the first reported case of skin and soft tissue infection caused by C. violaceum in the northeastern state of India, Mizoram. The organism was isolated from a pus sample obtained from a septic wound located on the foot and thigh of a pediatric patient. Though considered rare, this case report indicates that C.

BACKGROUND: Solid organ transplant (SOT) recipients are vulnerable to infections with multidrug-resistant organisms, and they often do not receive adequate empiric antimicrobials for serious Gram-negative infections. Knowledge of differences in antimicrobial resistance rates in this specific population can help guide empiric antimicrobial choices.
METHODS: We performed a retrospective cohort study comparing antimicrobial susceptibility patterns of Gram-negative organisms isolated from the adult SOT recipients with documented sepsis with the hospital-wide inpatient antibiogram. To evaluate emp

BACKGROUND: Sepsis is one of the leading causes of death globally. Standard antimicrobial susceptibility testing (AST) takes around 48 hours to get results from a positive blood culture bottle. Advanced diagnostic tools for AST are not available in most of the laboratories in developing countries. Therefore, performing a direct susceptibility testing (DST) might help to reduce turnaround time by 24 hours, reducing high mortality in patients with septicemia.
OBJECTIVE: The study aimed to evaluate the accuracy of DST from positive blood culture broth by comparing categorical agreement and error

Klebsiella aerogenes is a Gram-negative bacterium commonly associated with urinary tract infections (UTIs); however, it can occasionally cause bloodstream infections and, rarely, infective endocarditis (IE). Severe complications related to K. aerogenes bloodstream infections, such as systemic embolization, have been described in the literature. In this report, we describe a case of K. aerogenes IE in an elderly male patient who initially presented to urgent care with a UTI. He had been diagnosed and prescribed a course of oral antibiotics; however, the treatment failed due to antimicrobial res

Sepsis is a leading cause of neonatal mortality. Current knowledge of etiology, antimicrobial susceptibility, and outcomes provides evidence for judicious antimicrobial use. The aim for the present study was to identify etiologic organisms, antimicrobial susceptibility, and treatment outcomes at a tertiary hospital in Jos, Nigeria. A retrospective case review of neonates hospitalized for sepsis was conducted between August 25, 2017 and December 31, 2020. Clinical and laboratory data were collected from 1,984 neonates admitted, of whom 516 (26%) were diagnosed with neonatal sepsis (NNS). The cl

Sepsis involves an unregulated response to bacterial infection, the inflammatory response leading to persistent hypotension, leading to a precipitous fall in arterial blood pressure. The major trigger for the hypotension in sepsis and septic shock is the release of lipopolysaccharide (LPS) from the cell wall of gram-negative bacteria, which generates a major inflammatory response. LPS activates toll-like receptors (TLR4), leading to the release of an array of inflammatory mediators, which most notably activate inducible nitric oxide (NO) synthase and release copious amounts of NO. This is resp
