Preliminary evaluation of ciprofloxacin, a new 4-quinolone antibiotic, in the treatment of febrile neutropenic patients.

Twenty-four episodes of fever in neutropenic patients with haematological malignancy were treated with ciprofloxacin. In 13 episodes ciprofloxacin was used after failure of first-line antibiotic therapy, and in 11 episodes because of a history of allergy to the proposed first line antibiotics. Improvement or temporary improvement was seen in 64% of patients with evaluable infection. Fifty per cent of patients with bacteraemia improved. Resistance to ciprofloxacin developed in strains of staphylococci and streptococci, but Gram-negative organisms were generally extremely sensitive to the antibi

Imipenem/cilastatin as secondary therapy for infections in cancer patients.

Imipenem/cilastatin was used to treat 68 documented infections in patients who had failed to respond to other antibiotic regimens. The overall response rate was 68% and was higher among patients in whom the infecting organism could be identified (71% vs. 60%). The response rates were 73% in the 37 cases of septicaemia and 54% in the 13 cases of pneumonia. The response rate in Gram-negative bacillary infections was 69%, but was 50% for those caused by Pseudomonas aeruginosa. The response rate did not correlate with patients' neutrophil counts, but was higher if the neutrophil count increased du

Clinical comparison of a new automated infrared blood culture system with the BACTEC 460 system.

A new blood culture instrument, the BACTEC (Johnston Laboratories, Inc., Towson, Md.) NR-660, which utilizes infrared detection of carbon dioxide from microbial metabolism, was compared with the radiometric BACTEC 460 system. There were 1,554 isolates from 18,785 paired aerobic blood cultures. Of these isolates, 1,303 were isolated from the radiometric 6B medium, and 1,259 were isolated from the NR6A medium (P = 0.06). Analysis of the data indicated no significant differences in recovery when any individual species was considered. When organisms were considered as groups, there were no signifi

Aminoglycosides in gynecologic infections.

Aminoglycosides have always had a major role in the treatment of gynecologic infections. This therapeutic emphasis has been based upon the bactericidal activity of the aminoglycosides against gram-negative aerobes. This group of organisms is important to the gynecologist because of their frequent isolation from soft tissue, urinary sites, and the bloodstream in women with nosocomial or community-acquired pelvic infections. In the past decade, there has been increasing clinical awareness of the multi-bacterial nature of these soft tissue pelvic infections. A major therapeutic change directed ag

Aztreonam.

Aztreonam is a new, totally synthetic beta-lactamase agent--the first monobactam. It is highly resistant to hydrolytic inactivation caused by plasmid-mediated (except PSE-2 enzyme found in some Pseudomonas species) or chromosomally mediated beta-lactamases (except for K1 produced by rare strains of Klebsiella oxytoca). Accordingly, aztreonam remains active against many pathogens that are resistant to other beta-lactam antibiotics. The drug exhibits directed antibacterial activity against gram-negative organisms and is effective as monotherapy against most Enterobacteriaceae and Hemophilus and

Bacteremia and fungemia in patients with the acquired immunodeficiency syndrome.

Forty-nine episodes of bacteremia and fungemia occurred in 38 of 336 patients with the acquired immunodeficiency syndrome seen at our institution since 1980. There were five types of infections. Infections commonly associated with a T-cell immunodeficiency disorder comprised 16 episodes and included those with Salmonella species, Listeria monocytogenes, Cryptococcus neoformans, and Histoplasma capsulatum. Infections commonly associated with a B-cell immunodeficiency disorder included those with Streptococcus pneumoniae and Haemophilus influenzae. Infections occurring with neutropenia were caus

Sepsis and cholestasis: the in vitro effects of bacterial products on 14C-taurocholate uptake by isolated rat hepatocytes.

Bacterial endotoxins are known to be an important cause of cholestasis, yet not all organisms that cause cholestasis produce endotoxins. In order to determine whether bacterial products other than endotoxins may be involved in the cholestasis process, 14C-taurocholate (TC) uptake by isolated rat hepatocytes was measured in the presence of mid-log, stationary and mid-death phase bacterial broth supernatants from eight common bacterial pathogens. The results were then correlated with a quantitative assessment of endotoxin production by each organism. Supernatants from Haemophilus influenzae, Pse

Septic shock (Part 1).

Septic shock is a distinct clinical entity with an overall mortality of 30% to 40%. Gram-negative organisms are the most frequently identified causal agents. The shock syndrome appears to result from activation of complement, coagulation, kinin, endorphin, and other hormonal systems. Physiologic abnormalities include hypotension, metabolic acidosis, increased cardiac index, and decreased tissue extraction of oxygen. Clinicians must entertain the diagnosis of septic shock in a variety of settings, as prognosis is affected by early institution of therapy. Treatment is directed toward control of

Efficaciousness of immunoglobulin G prophylaxis on mortality and physiological variables in gram-negative peritonitis in rodents.

A rapidly expanding role involving the use of immunoglobulin preparations in conditions other than the classical immunodeficiency syndromes is evident in recent years. Treatment with immunoglobulin may be especially important during infection in which specific antibody to pathogens has been consumed, degraded, or not produced and in cases in which antibiotic treatment is ineffective. Recent studies in animals and humans support this concept, although further studies are needed to fully develop it. This study was completed to explore possible beneficial effects of prophylactic treatment of rode

Capnocytophaga species: infections in nonimmunocompromised and immunocompromised hosts.

Retrospective review of isolates of Capnocytophaga, a genus of capnophilic gram-negative bacilli, referred to the Massachusetts State Laboratory Institute in Boston revealed 31 patients with infection due to Capnocytophaga, 16 in nonimmunocompromised hosts. These infections included empyema (three patients), lung abscess (one), sinusitis (one), conjunctivitis (three), subphrenic abscess (one), wound (three), osteomyelitis (one), and bacteremia (three). Two of the wound infections were closed-fist injuries involving bone or soft tissue. Capnocytophaga was frequently isolated as part of a polymi

Cefotaxime (CTX) was administered to 117 pediatric patients. Although 26 of these patients were excluded from the clinical evaluation of the study because other antimicrobial agents were given concomitantly with CTX or because no infectious diseases were proved, these cases were evaluated for adverse effects of the drug. The remaining 91 cases were evaluated for clinical effect; pneumonia in 56 cases, septicemia in 5, suspected septicemia in 5, meningitis (aseptic cases included) in 3, urinary tract infection in 5 and other diseases in 17. No pathogenic organisms were identified in any of the

[Infections caused by intravascular catheters].

Microbiological investigations showed a positive culture in 430 out of 1540 venous catheters and shunts for dialysis. In 21 cases, a mixed culture with two different specific organisms was present. Of the total of 451 isolated organisms, 362 were gram-positive cocci and only 56 gram-negative bacillaceae. Staphylococcus epidermidis was by far the most frequent pathogen (n = 228), staphylococcus aureus in second place (n = 94). Among the gram-negative organisms, germs of the Klebsiella-Enterobacter group and Pseudomonas aeruginosa dominated. Staphylococcus aureus represented 31% of all germs iso

Current state of infectious diseases--potential areas of directed therapy with aztreonam.

Gram-negative bacillary organisms have continued to be a major source of hospital infections. Beta-lactamase-producing species have rendered many older beta-lactam antibiotics impotent. Aztreonam is a monocyclic beta-lactam antibiotic that inhibits aerobic gram-negative bacilli, including beta-lactamase-producing species. The agent has been used in the treatment of infections due to Enterobacteriaceae and Pseudomonas aeruginosa. Potential areas of use of aztreonam are as sole therapy for urinary tract infections, gram-negative pneumonias, and bone and joint infections. It can be used in combin

Antimicrobial therapy of bacterial sepsis in the newborn infant.

Septicemia continues to be an important cause of neonatal morbidity and mortality. The bacteria most commonly responsible are group B beta-hemolytic streptococci and Escherichia coli, but regional differences exist. Recently sepsis caused by Staphylococcus epidermidis has occurred with increasing frequency in several neonatal intensive care units. Other organisms are less commonly responsible. The choice of antibiotics for suspected sepsis is based on the possible organisms involved and their antibiotic susceptibility patterns, which vary from hospital to hospital and at different times in the

Overview of first-line amikacin therapy for urologic infections.

Severe infections in urologic patients are frequently and effectively treated with aminoglycoside medications. Because of the frequency of nosocomial gram-negative infections in urologic patients, antimicrobial therapy with broad-spectrum antibiotics, such as aminoglycosides, is an integral part of management of urinary infections. Amikacin, because of its activity against infections caused by Pseudomonas, Serratia, and other frequently resistant bacteria, as well as its ability to achieve high blood and tissue levels, provides a significant advantage over other aminoglycoside agents in hospit

Identification and treatment of infections in multiply traumatized patients.

Trauma is the leading cause of death among young adults, and infection is a leading complication in multiply traumatized patients. All antibiotic use and all infections among 1,009 patients admitted to the Maryland Institute for Emergency Medical Services Systems over a six-month period were reviewed. The vast majority of patients had sustained high-speed automobile trauma and had blunt injuries. All antibiotics were given by the infectious diseases consultants under predetermined protocols. During this time period, 175 infections and 76 bacteremias were identified. Thirty-three percent of the

Efficacy of type-specific and cross-reactive murine monoclonal antibodies directed against endotoxin during experimental sepsis.

To study the role of antibodies in promoting survival during gram-negative bacterial sepsis, we have developed several murine monoclonal antibodies (MAbs). One MAb (5B10) reacted in an enzyme-linked immunosorbent assay with only a single organism (Escherichia coli 0111:B4), while the other (8A1) reacted to all gram-negative whole-cell and lipopolysaccharide (LPS) antigens examined. Either 5B10 MAb, 8A1 MAb, or sterile saline solution was administered intravenously to outbred male Swiss-Webster mice immediately before one of three challenges: (1) viable bacteria intravenously, (2) viable bacter

Ceftazidime in the treatment of nosocomial sepsis.

Ceftazidime has broad antibacterial activity against many gram-positive and most clinically significant nosocomial gram-negative bacillary pathogens. Many studies have been undertaken both in this country and in western Europe to determine the clinical effectiveness of ceftazidime in seriously ill patients. Differentiating between nosocomial and community-acquired infections is difficult in many reports, but high cure rates, usually exceeding 80 percent, have been reported for documented gram-negative bacillary infections. In non-neutropenic patients, response rates have also been in a compara
