Therapeutic Effects of Nizubaglustat in a Mouse Model of GM2 Gangliosidosis.

Nizubaglustat is a novel selective inhibitor of glucosylceramide synthase (GCS) and the non-lysosomal glucocerebrosidase (NLGase, GbA2) with brain penetrant properties. It is currently in clinical development as an oral treatment for rare lysosomal storage diseases with neurological involvement. One such disease group called GM2 gangliosidosis, to date, has no approved therapeutic treatment. To test the potential efficacy of nizubaglustat in a mouse model of GM2 gangliosidoses, we treated Sandhoff disease (SD) mice carrying a homozygous null mutation in the Hexb gene, as well as healthy hetero

Diagnostic Dilemma in an Infant With Sound-Triggered Motor Events: Reflex Epilepsy Versus Exaggerated Startle-A Case Report.

Auditory-triggered motor events in infancy present a significant diagnostic challenge due to overlap between epileptic and non-epileptic startle phenomena. We report the case of a term female infant with neonatal-onset seizures and subsequent development of reproducible sound-triggered jerky movements, raising diagnostic uncertainty between exaggerated startle reflex and reflex auditory epilepsy. The child initially presented with seizures on the third day of life and was treated with phenobarbital. During early infancy, brief motor events consistently precipitated by sudden auditory stimuli w

AAV-based gene therapy with modified HEXB confers lasting therapeutic benefits in GM2 gangliosidosis models.

GM2 gangliosidoses, including Tay-Sachs (TSD) and Sandhoff (SD) diseases, are lysosomal storage disorders with neurological manifestations caused by the excessive accumulation of GM2 ganglioside due to the deficiency of the β-hexosaminidase A (HexA). Although gene therapy approaches are underway, concerns regarding efficacy and safety remain. Here, we evaluate a tyrosine-mutant adeno-associated virus serotype 9 (AAV9/3) vector encoding modified HEXB (modHEXB) wherein nine amino acid residues are substituted from HEXA. The intracerebroventricular administration of AAV9/3-modHEXB in SD mice resu

Adult-onset Sandhoff disease presenting with a motor neuron disease phenotype: clinical and mechanistic insights from patient-derived models.

Sandhoff disease (SD) is a subtype of GM2 gangliosidosis caused by pathogenic variants in Hexosaminidase B (HEXB). It most frequently presents in infancy or early childhood, whereas adult-onset disease is rare and remains incompletely characterized. Here, we describe an adult-onset case of SD presenting as motor neuron disease and provide clinical and mechanistic insights using patient-derived models. The patient was a 34-year-old man with compound heterozygous HEXB variants (c.1598G > A, p.Arg533His and c.1645G > A, p.Gly549Arg) who developed progressive lower limb weakness. Muscle biopsy dem

An example for potentially underrated causes of recessive disease in the Greater Middle East: integrative long-read genome and transcriptome sequencing pinpoint a deep-intronic homozygous HEXB candidate founder variant in GM2-gangliosidosis.

BACKGROUND: Consanguinity provides shortcuts to identify homozygous recessive mutations. However, deep-intronic variants escape standard sequencing (panel; exome/WES), and their pathogenicity cannot be inferred from genomic data. We applied WES, long-read genome and long-read-RNA-sequencing (LR-WGS, LR-RNA-Seq) in a Syrian patient with biochemically evident GM2-gangliosidosis. RESULTS: No exonic HEXA, HEXB and GM2A mutations were found. LR-WGS/LR-RNA-Seq revealed a homozygous HEXB variant, c.771 + 985G > A, activating a 97 bp pseudo-exon. CONCLUSIONS: Integrative genome and transcriptome seq

Intraperitoneal administration of adenosine A2A receptor antagonist reduces neuroinflammatory and antioxidant responses in GM2 gangliosidosis mouse model.

Tay-Sachs disease (TSD) is a fatal lysosomal storage disorder characterized by neuronal GM2 ganglioside accumulation and progressive neurodegeneration. Neuroinflammation and oxidative stress are increasingly seen as key factors in disease progression. In this study, we evaluated the effects of istradefylline, a selective adenosine A2A receptor antagonist, in the Hexa-/-Neu3-/- mouse model, which closely recapitulates the neuropathological features of human TSD. Istradefylline treatment reduced the expression of several proinflammatory markers, including Ccl2, Ccl3, Ccl5, Cxcl10, Il1b, and Gfap

Intravenous gene therapy improves life span and clinical outcomes in a feline model of Sandhoff disease.

Sandhoff disease (SD) is a fatal neurodegenerative disorder caused by the absence of β-N-acetylhexosaminidase (Hex) and subsequent accumulation of GM2 ganglioside in lysosomes. Previous studies have led to the development of an adeno-associated virus (AAV) vector-delivered gene therapy for children with GM2 gangliosidosis in both expanded access and phase 1/2 clinical trials through intrathalamic and cerebrospinal fluid-based delivery. This study investigated intravenous delivery of a bicistronic AAV vector-based gene therapy that has not yet been tested in clinical trials to a feline model of

Retinal Pigment Epithelial Changes in Gordon Holmes Syndrome.

Purpose: To describe atypical peripapillary retinal changes and neuroimaging findings in a patient with Gordon Holmes syndrome associated with compound heterozygous PNPLA6 variants, expanding the ophthalmic phenotype of this rare disorder. Methods: A single case was reviewed. Results: A 31-year-old woman with Gordon Holmes syndrome was referred for ophthalmic evaluation. Fundus autofluorescence imaging demonstrated atrophic peripapillary retinal pigment epithelial changes without central chorioretinal atrophy, distinguishing it from the phenotype typically seen in Boucher-Neuhäuser syndrome. Similarities and differences in the late-onset GM2 gangliosidoses: Tay-Sachs and Sandhoff diseases.

Due to shared ß-hexosamindase A deficiencies and significant clinical overlap, the predominating subtypes of late-onset GM2 gangliosidosis, late-onset Tay-Sachs (LOTS) and late-onset Sandhoff disease (LOSD) have been considered essentially indistinguishable. However, growing evidence supports several distinctions between the two entities. We highlight these distinctions through the cross-sectional evaluation of 27 (21 LOTS and 6 LOSD) late-onset GM2 gangliosidosis participants. Study protocol included physical examinations, assessments of gait, balance, muscle strength, ataxia, and nerve condu

Brainstem Substructure Atrophy in Late-Onset GM2-Gangliosidosis Imaging Using Automated Segmentation.

Late-Onset GM2-Gangliosidoses (LOGG) are rare, neurodegenerative lysosomal disorders that include late-onset Tay-Sachs (LOTS) and Sandhoff disease (LOSD) subtypes. Cerebellar atrophy is common, even in the absence of clinical ataxia, particularly in LOTS. Recent reports have also described brainstem atrophy in LOTS. We assessed brainstem substructure atrophy in LOGG, including LOSD. 10 LOGG patients (7 LOTS, 3 LOSD) and 7 age-matched controls had structural MRI brain imaging. A FreeSurfer brainstem substructure module was used for automatic segmentation and included the pons, medulla, superior

Evaluation of the Landscape of Pharmacodynamic Biomarkers in GM1 and GM2 Gangliosidosis.

GM1 and GM2 gangliosidosis are inherited, progressive, neurodegenerative lysosomal disorders of variable onset and disease progression. GM1 gangliosidosis is a result of biallelic pathogenic variants in the GLB1 gene, which confer absent or reduced β-galactosidase enzyme activity and lead to the accumulation of glycoconjugates such as glycosphingolipid GM1-gangliosides. GM2 is caused by biallelic pathogenic variants in one of the three genes (HEXA, HEXB, and GM2A) which confer deficiency of β-hexosaminidase or the GM2 ganglioside activator protein, responsible for the catabolism of GM2 ganglio

Late-onset GM2 gangliosidosis: magnetic resonance imaging, diffusion tensor imaging, and correlational fiber tractography differentiate Tay-Sachs and Sandhoff diseases.

GM2 gangliosidosis is lysosomal storage disorder caused by deficiency of the heterodimeric enzyme β-hexosaminidase A. Tay-Sachs disease is caused by variants in HEXA encoding the α-subunit and Sandhoff disease is caused by variants in HEXB encoding the β-subunit. Due to shared clinical and biochemical findings, the two have been considered indistinguishable. We applied T1-weighted volumetric analysis, diffusion tensor imaging (DTI), and correlational fiber tractography to assess phenotypic differences in these two diseases. 51 T1-weighted and 40 DTI scans from 19 Late-Onset GM2 patients with e

Deep Learning Cerebellar Magnetic Resonance Imaging Segmentation in Late-Onset GM2 Gangliosidosis: Implications for Phenotype.

Late-onset Tay-Sachs (LOTS) disease and late-onset Sandhoff disease (LOSD) have long been considered indistinguishable due to similar clinical presentations and shared biochemical deficits. However, recent magnetic resonance imaging (MRI) studies have shown distinct cerebellar atrophy associated with LOTS. In this study, we furthered this investigation to determine if the cerebellar atrophy is globally uniform or preferentially targets certain cerebellar regions. We utilized DeepCERES, a deep learning cerebellar specific segmentation and cortical thickness pipeline to analyze differences betwe

GM2 activator deficiency: An ultra-rare disorder with a new case and review of 22 published cases.

GM2 activator deficiency (AB variant of GM2 gangliosidosis) is an ultra-rare autosomal recessive lysosomal storage disorder caused by pathogenic GM2A mutations. The loss of a functional GM2 activator protein disrupts GM2 ganglioside degradation, leading to progressive neurodegeneration. Although it shares clinical features with Tay-Sachs disease, GM2 activator deficiency remains a genetically and biochemically distinct disorder, with limited genotype-phenotype correlation due to the small number of reported cases. This report presents a 33-month-old male with an infantile-onset phenotype, char
