Juvenile neuronal ceroid lipofuscinosis (CLN3) is a rare neurodegenerative disorder lacking effective therapies. This study aimed at developing a drug repurposing approach to identify potential therapeutic candidates for CLN3 using its protein expression profile (CPEP) constructed from proteomics data. Differentially expressed proteins were identified and applied to query the iLINCS database, resulting in 60 FDA-approved drugs with reversal effects on CPEP. These candidates were further prioritized based on regulation strength, coverage, and blood-brain barrier permeability. Top candidates inc

Neuronal ceroid lipofuscinosis type 3 (CLN3) disease is a rare, life-limiting pediatric neurodegenerative disorder with no approved disease-modifying therapy. We conducted a prospective case report from October 2023 to April 2025 involving two female siblings with genetically confirmed CLN3 disease (homozygous for the common 1 kb deletion). Both patients were treated with oral, weight-based miglustat for 18 months. Miglustat was supplied as off-label use in the absence of a therapeutic alternative for this severe neurodegenerative disorder. Clinical outcomes were assessed using comprehensive o

Neuronal ceroid lipofuscinosis type 2 (CLN2) disease is a rare neurodegenerative condition that rapidly progresses with language regression, loss of ambulation, blindness, intractable seizures, and premature death in childhood. Enzyme replacement therapy has transformed the clinical trajectory of CLN2 disease, and early genetic testing is crucial because enzyme replacement therapy cannot reverse clinical progression. Lack of clinician awareness of early clinical symptomatology, initially normal language development, and history of provoked or treatment-responsive seizures may contribute to dia

The classical late-infantile form of neuronal ceroid lipofuscinosis type 2 (CLN2 disease) is a fatal lysosomal disorder characterized by progressive motor decline and premature death. A CLN2 cynomolgus macaque (Macaca fascicularis) model closely replicates the late-stage clinical and pathological features of human CLN2. To evaluate motor deterioration in this model, we conducted two behavioral assessments-the CLN2 Macaque Clinical Rating Scale and the Apple test-from the pre-onset stage in one macaque. Hypokinesia was observed as the first symptom at 22 months of age, followed by a marked decl

Proteomics of laser-dissected lipofuscin from aged, healthy brains reveals Palmitoyl-Protein Thioesterase 1 (PPT1) and other CLN proteins as constituents. PPT1 is increasingly sequestered to lipofuscin during ageing. Protein sequestering into lipofuscin may contribute to physiological neuronal ageing.

Juvenile neuronal ceroid lipofuscinosis (JNCL) is a neurodegenerative lysosomal storage disease caused by the loss of the endolysosomal transmembrane protein, CLN3. The main protein component of lysosomal storage material in JNCL is subunit c of mitochondrial ATP synthase (SCMAS), which is normally degraded within the lysosome by tripeptidyl-peptidase 1 (TPP1) during mitophagy. Previous studies have shown that TPP1 expression is elevated in JNCL, a potential compensatory response, while reduced levels of TPP1 exacerbate disease in a JNCL mouse model. These observations suggest a role for TPP1

Background: Neuronal ceroid lipofuscinosis type 2 (CLN2) is a rare neurodegenerative disorder caused by CLN2 gene mutations, leading to ceroid lipofuscin accumulation and progressive neurodegeneration. Cerliponase alfa, a recombinant tripeptidyl peptidase-1, may improve aspects of disease progression, but common hypersensitivity and infusion reactions, along with a limited number of studies and lack of controlled trials, constrain the generalizability of these findings. Purpose: This systematic review and single-arm meta-analysis aims to evaluate the efficacy and safety of cerliponase alfa in

OBJECTIVE: Cerliponase alfa is an enzyme replacement treatment for neuronal ceroid lipofuscinosis type 2 (CLN2), administered via biweekly intracerebroventricular infusions. Typically, infusions are delivered via a head-sited access device; however, subcutaneously tunneled chest-sited devices could offer an alternative for improved ease of access and patient comfort. The aim of this study was to evaluate the safety, efficacy, and user experience of chest-sited versus head-sited intracerebroventricular access devices in patients with CLN2.
METHODS: This single-center prospective observational

Inherited biallelic mutations in the CLN7 gene result in the variant late infantile onset neuronal ceroid lipofuscinosis, a subtype of Batten disease (BD), a severe and fatal childhood neurodegenerative disease. Intriguingly, CLN7 genetic variants have also been associated with retinopathies, amyotrophic lateral sclerosis, and frontotemporal dementia. CLN7 encodes a transmembrane protein localizing to endolysosomal membranes with outward-facing chloride channel activity. Loss of CLN7 function results in cortical neurons accumulating swollen lipofuscin-containing lysosomes, leading to neuroinfl

BACKGROUND: Neuronal ceroid lipofuscinosis (NCL) is a heterogeneous group of lysosomal disorders characterized by progressive psychomotor regression, visual impairment, and intractable seizures. Genetically, NCL type 3 (CLN3) is associated with variants in the gene encoding a lysosomal transmembrane protein. To date, few Japanese patients with CLN3 have been reported. Thus, their neurodevelopmental and clinical features remain unclear. Here, we report the clinical course of a genetically confirmed Japanese patient with CLN3.
CLINICAL REPORT: A 17-year-old Japanese boy was diagnosed with retin

PURPOSE: To report the finding of a hyperreflective ganglion cell layer band (HGB) in a genetically confirmed case of isolated NCL3-associated retinal degeneration.
METHODS: Multimodal imaging, including ultra-widefield fundus photography and optical coherence tomography (OCT) was used to assess findings.
RESULTS: A 21-year-old male with progressive vision loss since age 6 presented with light perception vision, nyctalopia, and photophobia. Genetic testing revealed compound heterozygous NCL3 variants (Glu295Lys and a splice-site mutation), consistent with NCL3-associated retinal dystrophy wi

The calcium channel TRPML1 regulates key functions in lysosomes and has emerged as a promising therapeutic target for lysosomal storage diseases and neurodegenerative diseases. While pharmacologically, TRPML1 activation has been shown to reduce storage burden, its impact on synaptic function and network activity is unknown. In this study, we examined how modulation of TRPML1 activity with agonist and antagonist affects synaptic transmission in mice of both sexes. In addition, we evaluated its therapeutic potential in a disease model of juvenile neuronal ceroid lipofuscinosis (JNCL or Batten di

In the last two decades, many gene mutations have been identified that when homozygous, lead to childhood neurological disorders, but when heterozygous, result in adult-onset neurodegenerative disease. A shared feature linking these genes? They encode proteins residing in or impacting the function of the lysosome, a key organelle in macromolecular degradation and recycling whose loss leads to the inability to manage proteostatic stress. Here, we propose that lysosomes connect a subset of genetic neurological and neurodegenerative disorders as they occur in two distinct life epochs-development

Juvenile neuronal ceroid lipofuscinosis (JNCL) is a rare neurodegenerative disorder caused by mutations in the CLN3 gene and characterized by early vision loss and a progressive neurological decline. To characterize the progression of retinal pathology, we conducted a 15-month study using the Cln3Δex7/8 mouse model which carries the most common causative mutation of JNCL. Retinal function was assessed every three months from the age of 3-15 months using electroretinography (ERG), optical coherence tomography (OCT), fundus imaging, and immunohistochemistry. OCT and fundus imaging in the Cln3Δex

BACKGROUND/OBJECTIVES: A Petit Bleu de Gascogne (PBDG) dog presented with a progressive neurological disorder characterized by hind-limb weakness, anxiety and hallucinatory episodes, lip smacking, progressive vision loss, muscle atrophy, and ataxia. Magnetic resonance imaging revealed diffuse brain atrophy. The dog was euthanized at approximately 23 months of age due to the progression of neurological signs. A study was undertaken to identify the molecular genetic basis of the disorder in this dog.
METHODS: Microscopic analyses were performed to characterize the disease pathology and whole-ge

BACKGROUND: There is currently limited information about sensory and perceptual abilities across the progression of CLN3 disease (Juvenile Neuronal Ceroid Lipofuscinosis; Batten disease), a recessively inherited lysosomal storage disorder and a leading cause of childhood neurodegeneration. Clinical symptoms include vision loss, motor impairments, and cognitive challenges, making it difficult to accurately assess neurocognitive and perceptual abilities. Thus, there is a critical need to identify objective biomarkers that can be used to inform disease progression and track treatment response in

BACKGROUND: Neuronal ceroid lipofuscinoses (NCLs) are rare, progressive lysosomal neurodegenerative disorders. Among late-infantile onset forms (after 18 months of age), type 2 (CLN2 disease) is the most frequent. This study assessed the incidence and disease burden at diagnosis of CLN2 disease in France.
METHODS: This was a nationwide, population-based, retrospective study including all patients identified with CLN2 disease in the French Hospital Discharge Database (Programme de Médicalisation des Systèmes d'Information, PMSI) from January 2015 to December 2023.
RESULTS: After a 2-year wash

Neuronal ceroid lipofuscinoses (NCLs) are rare, inherited lysosomal storage disorders (LSDs) of the brain and retina. Gastrointestinal (GI) manifestations are reported in clinical practice but remain largely underexplored. Lysosomal dysfunction affects both the central and the peripheral nervous systems, including the enteric nervous system (ENS) where accumulation of storage material and neuronal loss may directly contribute to GI dysmotility. These insights, combined with emerging gene therapies targeting both brain and ENS pathology, underscore the need for validated GI endpoints in NCL res

Severe gastrointestinal (GI) symptoms occur in people with CLN3 disease, a neurodegenerative disorder. If left untreated these GI symptoms compromise life quality and may contribute to death. We hypothesized GI symptoms in CLN3 disease are at least partially due to neurodegeneration in the enteric nervous system (ENS), the master regulator of bowel function. We examined the integrity of the ENS in human CLN3 autopsy small bowel and colon, and in CLN3 deficient (Cln3Δex7/8) mice. We performed detailed immunohistological analyses of enteric neurons and glia and assessed bowel transit times at mu

Neuronal ceroid lipofuscinosis type 7 (CLN7) is a devastating paediatric neurodegenerative disorder with no cure and limited natural history data to guide therapeutic development. Here, we present the first multimodal characterization of prodromal and early-stage CLN7 disease in Japanese macaques carrying a spontaneous CLN7 -/- mutation. Using structural T2-weighted MRI for volumetry, [18F]FDG PET for glucose metabolism, and [11C]PBR28 PET for neuroinflammation, we observed region-dependent patterns in volumetric, molecular, and metabolic alterations. MRI confirmed the presence of disease-asso
