Ultra-early versus early adjunctive vasopressin initiation after norepinephrine escalation in septic shock: a target trial emulation.

PURPOSE: The optimal timing of adjunctive vasopressin after norepinephrine escalation in septic shock remains uncertain. We emulated a target trial comparing ultra-early vasopressin initiation within 0-3 h with early initiation within > 3-6 h after the norepinephrine infusion rate reached 0.25 μg/kg/min or higher. METHODS: We emulated a target trial using electronic health record databases (MIMIC-IV 2008-2022 and eICU-CRD 2014-2015). Adults with septic shock who reached the norepinephrine threshold before vasopressin use were eligible. We used the clone-censor-weight method to estimate cumula

A consensus of international experts on definition, sampling, treatment, and prevention of peripheral extracorporeal membrane oxygenation cannula-site infection obtained by the Delphi method: the SAVECMO study.

BACKGROUND: Nosocomial infections are common in patients receiving extracorporeal membrane oxygenation (ECMO), with ECMO cannula-site infections (ECMO-CSI) being the most frequent infections directly related to the ECMO run. These infections can significantly impact patient outcomes. Currently, no adult guidelines exist for the prevention, diagnosis, and/or treatment of peripheral ECMO-CSI, resulting in heterogeneity in both clinical practice and research findings. METHODS: We conducted a Delphi study involving 39 international experts in ECMO management. The experts participated in four Delp

Validation of the ICD-10 Diagnoses of Early-Onset Neonatal Infection in the Danish National Patient Register from 2010 to 2018.

PURPOSE: To examine the accuracy of the diagnoses of early-onset neonatal infection in the Danish National Patient Register. PATIENTS AND METHODS: All Danish neonates born after 35 weeks of gestation with an ICD10 diagnosis of sepsis or meningitis within the first week of life between 2010 and 2018 were identified. To examine the validity of the diagnoses, medical records were reviewed for 500 neonates diagnosed with sepsis and all 63 neonates diagnosed with meningitis. Proven infection was defined by bacterial pathogens identified from blood or cerebrospinal fluid. Probable sepsis was define

Can dynamic changes in preoperative urinary white blood cells and nitrite predict infectious complications after percutaneous nephrolithotomy in culture-positive patients?

BACKGROUND: Percutaneous nephrolithotomy (PCNL) frequently encounters postoperative infectious complications, notably fever and urosepsis. Although preoperative urine culture guides antibiotics, delayed reporting hampers timely risk stratification. Urinalysis is simple, cost-effective, and repeatable; however, most existing studies focus on single static measurements and lack systematic evaluation of temporal trends. OBJECTIVE: To evaluate the association between temporal changes in preoperative urinary white blood cells (uWBC) and nitrite (NIT) and the risk of postoperative fever and uroseps

Survey to evaluate clinical benefits and acceptance in international tele-intensive care unit collaboration.

OBJECTIVES: Tele-intensive care unit (TICU) networks have evolved quickly in recent years and have proved their value in providing care for the critically ill. Objective measures to assess performance and quantify success e.g. clinical and economic outcome parameters are desirable but not easily accessible in international collaboration with very diverse clinical settings and legal prerequisites. Therefore, we initiated a study to evaluate the performance in our tele-intensive care unit network. METHODS: We conducted an online survey addressing clinical staff at ten spoke sites in four countr

Sepsis associated with Clostridioides difficile infection carries similar mortality to sepsis of other origin: a propensity score-matched analysis.

Severe Clostridioides difficile infection (CDI) is associated with high mortality, partly due to comorbidities. We aimed to assess whether sepsis due to CDI leads to worse outcomes compared to sepsis from other infections and whether CDI is associated with a distinct host immune profile. In this retrospective analysis, patients with CDI and sepsis, included in two prospective multi-center CDI studies, (one conducted during 2015-2021 and one during 2022-2023), were compared to comorbidity- and severity-matched patients with sepsis of other cause [community acquired pneumonia (CAP), hospital-acq

Dose escalation with intraoperative radiotherapy in newly diagnosed glioblastoma (INTRAGO-II): an open-label, multicentre, randomised, controlled, phase 3 trial.

BACKGROUND: Despite maximal safe resection and chemoradiotherapy, most glioblastomas recur locally. We aimed to evaluate whether additional intraoperative radiotherapy compared with standard of care improves outcomes in patients with newly diagnosed glioblastoma. METHODS: INTRAGO-II was an open-label, multicentre, randomised, controlled, phase 3 trial enrolling patients aged 18-80 years with supratentorial glioblastoma amenable to resection of the contrast enhancing tumour with Karnofsky performance score (KPS) of ≥60%. Patients from 18 centres in Brazil, Canada, China, Germany, Spain, South

Association between pravastatin use and short-term mortality in ICU patients with sepsis: a retrospective propensity score-matched cohort study.

BACKGROUND: Sepsis is a life-threatening condition with persistently high mortality in the ICU, highlighting an urgent need for effective pharmacological interventions. Statins have been proposed as potential adjunctive therapies owing to their pleiotropic properties, but evidence specifically supporting the use of pravastatin-a hydrophilic statin with distinct pharmacokinetics-in this critically ill population remains insufficient. METHODS: This retrospective cohort study utilized data from the MIMIC-IV database (version 3.1). Adult patients with sepsis at their first ICU admission who recei

Early Identification of a Cytokine-Low Immune Phenotype in Suspected Sepsis Using a Point-of-Care Whole-Blood TNF-α Release Assay.

BACKGROUND/OBJECTIVES: Routine biomarkers and clinical scores do not directly assess functional immune responsiveness in suspected sepsis. We evaluated whether the TNF-α Release Assay (TARA), a whole-blood LPS-induced functional assay, is associated with an early cytokine-low immune phenotype in Emergency Department (ED) patients. METHODS: In this prospective single-center cohort, adults with suspected sepsis and NEWS2 ≥ 3 were sampled at presentation, at 4 h and, when available, at 24 h. Immune phenotypes were defined independently of TARA by unsupervised k-means clustering of 12 circulating

Day-7 lactate-to-albumin ratio as a prognostic marker in sepsis patients requiring prolonged mechanical ventilation: a multicenter retrospective landmark analysis of the Korean Sepsis Alliance registry.

BACKGROUND: Prognostication in patients with sepsis requiring prolonged mechanical ventilation (MV) is challenging. While the lactate-to-albumin ratio (LAR) predicts early sepsis outcomes, its value during prolonged MV remains undefined. We evaluated the prognostic performance of the MV day-7 LAR in this population. METHODS: This multicenter study retrospectively analyzed prospectively collected data [2019-2023] from the Korean Sepsis Alliance (KSA) registry. By a pre-specified day-7 landmark design, the analysis was restricted to adult sepsis patients who survived on MV to day 7 (requiring M

Expert Perspective: Diagnosis and Treatment of Castleman Disease.

Castleman disease (CD) is a major diagnostic challenge for rheumatologists. Unicentric CD (UCD) involves one enlarged lymph node region, whereas multicentric CD (MCD) involves multiple enlarged lymph node regions. Both UCD and MCD may exhibit a wide range of symptoms that overlap with other immune-mediated conditions. MCD can be associated with excessive cytokine production due to a plasma cell neoplasm (MCD-polyneuropathy, organomegaly, endocrinopathy, monoclonal paraprotein, skin changes) or uncontrolled human herpesvirus-8 infection (HHV-8) (HHV-8-positive MCD), but more than half of cases

Left Ventricular Ejection Fraction and Previous Cardiac Revascularization: Impact on Patient Survival, Graft Survival, and Complications in Kidney Transplant Recipients.

BACKGROUND: Kidney transplant physicians believe that the cardiac status of kidney transplant recipients influences posttransplant outcomes. However, the Scientific Registry of Transplant Recipients (SRTR) does not include cardiac variables in its risk-adjustment model, raising the question of whether it fairly risk adjusts recipients. METHODS: This study conducted a retrospective analysis of the prospectively collected National Surgical Quality Improvement Program Transplant database to assess the impacts of pretransplant cardiac revascularization and left ventricular ejection fraction (LVEF

Secondary Cardiorenal Syndrome in a Cohort of Septic Patients Treated in a Medical Intensive Care Unit: A Single-Center Experience.

Context Sepsis is a life-threatening condition caused by an abnormal host response to infection, resulting in multiple organ failure and high mortality rates. Secondary or type five cardiorenal syndrome (CRS) emerges as a consequence of systemic infection, resulting in both cardiac and renal dysfunction. The synergistic impact of inflammation and hemodynamic instability results in an adverse cycle of vital organs damage during which renal impairment further aggravates cardiac dysfunction and vice versa. Objectives The objective of this study is to assess the influence of specified demographic,

Shorter or biomarker-guided antibiotic durations for common serious neonatal infections: a collection of non-inferiority meta-analyses.

BACKGROUND: Unnecessary antibiotic prolongation promotes antimicrobial resistance. Evidence-based guidance on antibiotic durations for treating neonatal infections is lacking. Noninferiority meta-analyses evaluating shorter durations or biomarker-guided durations have not been conducted. METHODS: We conducted systematic reviews and meta-analyses using a unified search strategy (MEDLINE, EMBASE, Cochrane Library, January 1990-December 2024) to determine whether short-course antibiotics are noninferior to "standard" courses for culture-positive sepsis, culture-negative sepsis, UTIs, uncomplicat

The Role of Double J Stenting in Nonobstructive Acute Pyelonephritis With Acute Kidney Injury.

Background and objective There has been a paradigm shift in the clinical manifestations and renal outcomes of patients with nonobstructive acute pyelonephritis (NO-APN), with acute kidney injury (AKI) being a common complication. While the standard of care remains parenteral antibiotics, minimally invasive urologic procedures is still uncertain. This study aimed to assess the effectiveness of double J (DJ) stenting in patients with NO-APN with AKI in promoting renal recovery, assessed by improvements in creatinine and inflammatory markers. Methods This retrospective study examined the renal ou

[Unsolved dental problems in children with congenital hyperinsulin].

The review of modern literature on congenital hyperinsulinism in children is presented, highlighting the features of the clinical course, nutrition treatment and dynamic monitoring of children. Only a few publications have hypothesized about possible dental problems in children with congenital hyperinsulinism, without specifying and describing them. The risk factors for caries, the specifics of dental care, and ways to improve the quality of life have not been sanctified. The general condition of the child can be so severe that dental problems are not taken into account by specialists of other

Cefazolin vs. antistaphylococcal penicillins for the treatment of methicillin-susceptible Staphylococcus aureus bacteraemia: a systematic review and meta-analysis.

BACKGROUND: There is debate on whether cefazolin or antistaphylococcal penicillins should be the first-line treatment for methicillin-susceptible Staphylococcus aureus (MSSA) bacteraemia. Ongoing trials are investigating whether cefazolin is non-inferior to (flu)cloxacillin, but it remains uncertain whether these findings apply to other antistaphylococcal penicillins. OBJECTIVES: We conducted a systematic review and meta-analysis comparing cefazolin with each of the individual antistaphylococcal penicillins for MSSA bacteraemia. METHODS: Data sources: We updated a 2019 systematic review but
