OBJECTIVES: To compare the causes of deaths recorded on death certificates with findings at autopsy and to identify other deficiencies in the completion of death certificates.
DESIGN AND SETTING: The causes of death (Part I and II) recorded on the death certificates of all patients undergoing autopsy at St Vincent's Hospital, Melbourne, in 1992 were compared with the autopsy findings. Additional clinical information about previous medical and surgical history and antemortem investigations was obtained from the clinical summary in the autopsy reports, and from hospital charts in some cases.

During 1983-1988, hospitalizations of patients with a diagnosis of human immunodeficiency virus (HIV) infection increased from 1.3 to 33.7 per 100,000 persons. We used the National Hospital Discharge Survey, which is based on a representative sample of discharges from nonfederal short-stay hospitals, to describe illnesses among hospitalized patients with HIV infection. Of 222,200 such hospitalizations during 1983-1988, most occurred among persons who were 25-44 years of age (79%), white (66%), and male (90%). Among men 25-44 years of age, HIV admissions increased from 8.5 to 148.6 per 100,000

Clinical experience assumes major importance as the source of data on cultured epidermal autograft (CEA), since a large controlled study is probably not forthcoming. Among the general questions to be answered are: Which patients are candidates for grafting with CEA? Do selection criteria specify size of total body surface area burn or age? Is CEA more suitable for certain body areas than others? Does CEA close the wound as quickly, as well, and as safely as other available methods of coverage? Fresh or frozen allograft, the gold standard, is presumed to be safe, but the current concern about t

Infective endocarditis is a serious disease with a continuing mortality of approximately 20%. Risk factors include a variety of congenital and acquired heart diseases. Infection follows an episode of bacteraemia which is most commonly due to oral bacteria, notably streptococci. Less commonly bacteraemia may arise from surgical procedures or diseases of the gastrointestinal and genitourinary tracts or from sepsis at other body sites, including intravenous drug abuse. Several societies and associations have published recommendations for the prevention of bacteraemia in those at risk from endocar

PURPOSE: Streptococcal infection has increasingly become a problem in neutropenic patients. We report on an outbreak of Streptococcus mitis sepsis in six bone marrow transplant patients receiving oral antimicrobial prophylaxis.
PATIENTS AND METHODS: We performed an epidemiologic study of all patients in our bone marrow transplant program from 1986 to 1988. The hospital and microbiology records for all patients were reviewed. All bone marrow patients were treated according to specified protocols, including an oral prophylactic antimicrobial regimen that was changed in late 1987 from vancomycin

A solid-phase radioimmunoassay (RIA) was developed for the detection of yellow fever (YF) virus in infected cell culture supernatant fluid and clinical samples. The test employed a flavivirus group-reactive monoclonal antibody attached to a polystyrene bead support and a radiolabeled type-specific antibody probe in a simultaneous sandwich RIA format. Optimal assay conditions specified a 16-h incubation at high temperature (45 degrees C). Monoclonal antibody to tetanus toxoid was added to the radiolabeled probe to inhibit nonspecific binding. The sensitivity of the assay for cell culture-propag

Parting surfaces between areas having different standards of hygienic requirements, represent naturally the weak points in the hygienic conception of operating theatres. These boundary areas between the operation room on the one hand and the adjoining entrance lock for the staff on the other are of specific interest in this connection. While opening the connecting doors, dust particles may be whirled up due to turbulence, thermic, constructional and other effects, which are difficult to be identified and may then settle down directly on the operating area after reaching there from the entrance

The authors evaluated whether depth of needle insertion as determined by needle length is an important factor in sclerotherapy-associated bacteremia. In 18 consecutive sclerotherapy sessions in which sodium morrhuate was used, blood cultures were positive in 11% when using an injector with only 3 to 4 mm of needle protruding beyond the sheath, compared with the 39% incidence of bacteremia previously observed when an injector was used which permitted 6 to 8 mm of needle to protrude (0.01 less than p less than 0.05). Twenty-five additional sclerotherapy sessions in which the shorter needle was u

Laboratory test results and procedures can be evaluated at four levels:1. Analytic analysis of laboratory test: precision, technical sensitivity, technical specificity; 2. Diagnostic analysis of laboratory test: diagnostic sensitivity, diagnostic specificity, Youden index, likelihood ratio, etc.; 3. Operational analysis of laboratory test: predictive value of positive result, predictive value of negative result, efficiency, discriminant function, etc.; 4. Medical decision-making analysis of laboratory test: threshold probability, cost-benefit analysis, solving the decision tree. Analysis of re

It is intended to summarize the latest research in the complement field. After a short comment on the biochemical behaviour of proteins participating in the complement reaction, biochemistry of the complement reaction itself and its biological activities are described. The question of participation of the complement system in disease is examined by specifying the possibilities to analyse the complement system in clinical problems, and, on the other hand, by giving a review on our latest knowledge of modifications of the complement system in disease.
