Nationwide newborn screening for mucopolysaccharidoses in Taiwan: Impact, early diagnosis, and clinical advances over the past decade.

PURPOSE: Taiwan launched a nationwide newborn screening (NBS) program for mucopolysaccharidoses (MPS) in August 2015. This study evaluated the diagnostic yield, timing of diagnosis, and downstream clinical management outcomes of this population-based NBS program. METHODS: As of March 31, 2025, over 838,585 infants were screened for MPS I, MPS II, MPS IVA, and MPS VI. First-tier screening was based on enzyme activity measurement from dried blood spots using liquid chromatography-tandem mass spectrometry. Screen-positive infants underwent comprehensive confirmatory evaluation, including quantit

Treosulfan-based conditioning for hematopoietic stem cell transplantation in mucopolysaccharidosis: a pilot study.

Treosulfan (Treo), a prodrug of a bifunctional alkylating agent, has been used in conditioning regimens to reduce the risk of hepatic sinusoidal obstruction syndrome. We aimed to establish a Treo-based conditioning regimen for mucopolysaccharidosis (MPS) as a phase 1 study. The regimen included thoracoabdominal irradiation, Treo, fludarabine, and antithymocyte globulin. Antithymocyte globulin was omitted in cord blood transplantation. Five patients with MPS II, aged 8 months to 21 years, were enrolled. Stem cell sources were HLA-C-mismatched unrelated bone marrow in one patient and HLA 1 to 3

Natural history of hearing loss in adults with mucopolysaccharidoses across phenotype and genotype.

BACKGROUND: Hearing loss is a recognised complication of mucopolysaccharidoses (MPS), but adult-specific data on long-term auditory outcomes remain limited. We aimed to characterise the adult natural history of hearing loss across MPS subtypes, including prevalence, severity, and longitudinal trajectory, and to explore associations with phenotype, genotype, and treatment exposure. METHODS: This retrospective study analysed clinical and audiological data from adults with confirmed MPS at a tertiary metabolic centre. Hearing loss was classified using standard thresholds. Cross-sectional analyse

Natural History of Morquio A Syndrome.

Mucopolysaccharidosis IVA (Morquio A) is a progressive lysosomal disorder caused by a deficiency of N-acetylgalactosamine-6-sulfate sulfatase, resulting in the accumulation of glycosaminoglycans, primarily in cartilage and bone. The condition leads to skeletal dysplasia, impaired ossification, and growth imbalance, causing cervical compression, tracheal obstruction, hip dysplasia, and cardiopulmonary complications. Although existing natural history studies have clarified several aspects of disease progression, objective methods to quantify skeletal pathology and its functional impact remain li

Infusion rate adjustment in enzyme replacement therapy with pabinafusp alfa for mucopolysaccharidosis II.

AIMS: Enzyme replacement therapy (ERT) for mucopolysaccharidosis II (MPS II) requires long-term, weekly intravenous infusions often lasting over 3 h each time, which can burden paediatric patients and caregivers and negatively affect their quality of life and treatment compliance. The aim of this study was to assess whether shortening infusion duration impacts the long-term efficacy and safety of ERT. METHODS: A post hoc analysis was conducted using 260 weeks of clinical data from 27 Japanese patients with MPS II who received pabinafusp alfa at a dose of 2.0 mg/kg/week during a Phase II/III c

RPE Abnormality Is a Potential Primary Cause for Retinal Degeneration in Mucopolysaccharidosis Type VI Patients and a Rat Model.

PURPOSE: Mucopolysaccharidosis type VI (MPS VI) is a rare autosomal recessive lysosomal storage disorder caused by pathogenic variants in ARSB, leading to deficiency of N-acetylgalactosamine 4-sulfatase and accumulation of glycosaminoglycans. Although enzyme replacement therapy (ERT) alleviates systemic symptoms, its efficacy for ocular complications is limited. Because ocular manifestations may require distinct therapeutic approaches, a precise understanding of the underlying retinal pathology is essential. This study aimed to characterize ocular and retinal involvement in MPS VI through clin

Molecular Modeling of N-Acetylglucosamine Binding to the I154R Mutant of NAGLU: Pathogenic Insights into Sanfilippo Syndrome Type B.

Sanfilippo syndrome type B, also known as mucopolysaccharidosis type IIIB (MPS IIIB), is a rare autosomal recessive lysosomal storage disorder caused by mutations in the N-acetyl-α-D-glucosaminidase (NAGLU) gene, which encodes the enzyme α-N-acetylglucosaminidase. This enzyme is essential for degrading heparan sulfate. The deficiency leads to toxic accumulation within cells. To investigate the impact of NAGLU mutations, mutational data were retrieved from public databases including NCBI, UniProt, and HGMD. A total of 162 variants were evaluated using sequence-based prediction tools to identify

An international observational study on transition of care from paediatric to adult services for patients with mucopolysaccharidosis II.

BACKGROUND: Treatment for mucopolysaccharidosis II (MPS II; Hunter syndrome) via enzyme replacement therapy with intravenous idursulfase (Elaprase) has been available for over 15 years. This treatment has led to survival of more patients into adulthood and a concomitant rise in the need for transition of care from paediatric to adult care teams; however, implementation of transition pathways is not standardized. METHODS: This cross-sectional, non-interventional, multi-country qualitative interview study recruited patients with MPS II (n = 13), caregivers of patients with MPS II (n = 23) and h

Advances in Therapeutic Options for Pulmonary and Sleep Disorders in Mucopolysaccharidosis (MPS) Patients: A Narrative Review.

Mucopolysaccharidosis (MPS) are a group of inherited lysosomal storage genetic disorders that affect the body's ability to break down glycosaminoglycans (GAGs) due to the deficiency of required enzymes. This leads to depositions of these GAGs in various tissues and organs resulting in multi-systemic manifestations including pulmonary and sleep related issues. In recent years, there have been significant advancements in therapeutic options and supportive management which have led to the overall improvement in respiratory care, culminating in improved quality of life for MPS patients. Management

Patient-driven surveys of mucopolysaccharidoses revealed patient-reported outcomes in the Japan MPS patient and family group.

BACKGROUND: Japan Mucopolysaccharidoses (MPS) Patient and Family Group (J-MPS) was founded in 1986. It is the largest patient group for all types of MPS in Japan and now includes some of the patients with lysosomal storage diseases. Patients receive medical care but suffer from the daily progression of symptoms, holding a different assessment of treatment effectiveness than that considered by the medical professionals. This study summarises the annual reports of the society and the results of periodic surveys conducted by their own. The 20-year annual reports are based on J-MPS membership reco

Metabolomic Insights into Lysosomal Storage Diseases: An Untargeted View.

Lysosomal Storage Diseases (LSDs) include roughly 70 inherited metabolic disorders, most of which are expressed in an autosomal recessive pattern. These conditions arise from mutations in genes encoding lysosomal enzymes, leading to intracellular buildup of substrates and subsequent lysosomal dysfunction. LSDs can be broadly classified based on the nature of the stored substrate, encompassing Sphingolipidoses, Mucopolysaccharidoses, Lysosomal Glycogen Storage Disease, Oligosaccharidoses, Mucolipidoses, and Lysosomal Proteinoses. Although individually rare, LSDs collectively affect approximatel

Advances in Therapies for Mucopolysaccharidoses.

Mucopolysaccharidoses (MPS) are severe, inherited metabolic diseases, classified among lysosomal storage diseases (LSDs). The presence of pathological variants of genes coding for enzymes involved in the degradation of glycosaminoglycans (GAGs) is a primary cause of each MPS type, and accumulation of these compounds is a characteristic feature of MPS. Depending on the kind of defective enzyme and the type of stored GAG(s), 12 classical types are distinguished, and a few other related diseases, whose classification is unclear. Although there is no fully effective cure for MPS, several kinds of
