Lowering of Lipoprotein(a) with Olpasiran.

Elevated lipoprotein(a) [Lp(a)] is associated with an increased risk of aortic stenosis and Atherosclerotic Cardiovascular Disease (ASCVD), including myocardial infarction. Therefore, Lp(a) has emerged as a potential target for reducing residual cardiovascular risk. Conventional lipid-lowering interventions, including lifestyle modifications such as diet and exercise and statin therapies, have not been shown to effectively reduce Lp(a) levels. Ezetimibe, which inhibits cholesterol absorption and can reduce LDL-C, does not significantly affect Lp(a) levels, even when used in combination with st

A rare case of familial chylomicronemia in infancy due to homozygous LMF1 variant.

Familial chylomicronemia syndrome (FCS) is a rare autosomal recessive disorder of triglyceride (TG) metabolism, with an estimated prevalence of 1 in 100 000 to 1 in 1 000 000. It typically presents in childhood or adolescence, while presentation in infancy is exceedingly rare. Loss-of-function variants in lipoprotein lipase gene account for 60-80% of FCS cases. Other causes of FCS include loss-of-function variants in glycosylphosphatidylinositol-anchored high-density lipoprotein-binding protein 1, apolipoprotein A-V, apolipoprotein C-II, and lipase maturation factor 1 (LMF1) genes. LMF1 varian

Biallelic ABCG8 Variants or Biochemical Evidence of Sitosterolemia: A Case Report.

Sitosterolemia is a rare lipid disorder characterized by xanthomas, premature atherosclerotic cardiovascular disease (ASCVD), and abnormally elevated circulating low-density lipoprotein cholesterol (LDL-c) measurements. Pathogenic variants in adenosine triphosphate (ATP)-binding cassette transporters lead to the accumulation of phytosterols. A child presented to the pediatric cardiology clinic with an incidentally noted presumed pathogenic variant and a variant of unknown significance in a sitosterolemia gene but had normal fasting lipids and no accumulation of circulating phytosterols. This c

Elevated Lipoprotein(a) in Asian Populations and its Implications for Cardiovascular Disease.

Elevated lipoprotein (a) (Lp[a]) is becoming increasingly recognized as a cardiovascular disease risk factor. The relationship of Lp(a) to cardiovascular disease is well characterized in European populations; however, data are lacking in the Asian region. In this scoping review, we aimed to characterize the distribution and prevalence of elevated Lp(a) in the Asian region and its relationship to coronary artery disease, cerebrovascular disease, and peripheral vascular disease. Thirty-one Asian studies met the inclusion criteria and were compared with 6 representative European studies. The grea

Genome-wide synthetic lethality screen of Bam complex-associated genes in Escherichia coli.

Biogenesis of the bacterial outer membrane is key to bacterial survival and antibiotic resistance. Central to this is the β-barrel assembly machine (Bam) complex and its associated chaperones, which are responsible for transport, folding, and insertion of outer membrane proteins (OMPs). The Escherichia coli Bam complex is composed of two essential subunits, BamA and BamD, and three non-essential accessory lipoproteins, BamB, BamC, and BamE. Optimal Bam function is further dependent on the non-essential periplasmic chaperones DegP, Skp, and SurA. Despite intensive study, the specific function o

Trends in Modifiable Risk Factors Contributing to Ischemic Heart Disease Mortality in Afghanistan: Estimates From the Global Burden of Disease Study 2023 (1999-2023).

BACKGROUND: Ischemic heart disease (IHD) is the leading cause of death, and the majority of this burden can be attributed to modifiable risk factors. Although surveys show that these risk factors are prevalent in Afghanistan, no study has used Global Burden of Disease (GBD) methodology to describe their contribution to IHD mortality. The study examined trends (1999-2023) in 14 selected behavioral, dietary, and metabolic risk factors for IHD mortality using the GBD 2023 estimates. METHODOLOGY: This retrospective analysis used GBD 2023 estimates of deaths from IHD in Afghanistan attributable to

A Review of the Vascular Pathophysiology of Migraine: Should We Consider Statins as a Treatment Option?

INTRODUCTION/OBJECTIVE: Migraine is a primary headache disorder that affects more than a billion people worldwide and can severely impact quality of life. Inflammation and endothelial dysfunction in the trigeminovascular system are the suggested underlying mechanisms. Statins, beyond their lipid-lowering effects, exhibit pleiotropic properties including anti-inflammatory, antioxidative, and endothelial-modulating effects, which may be relevant to migraine pathophysiology. This narrative review evaluates current evidence on the efficacy of statins in migraine prophylaxis, treatment, and prevent

Prenatal Use of Sirolimus in Fetal Therapy: Current Evidence, Clinical Applications, and Future Directions.

Prenatal imaging, genomic diagnosis, and pathway-directed pharmacology increasingly permit treatment of selected fetal disorders before birth. This narrative review evaluates maternally administered sirolimus as transplacental fetal therapy, emphasizing cardiac rhabdomyomas associated with tuberous sclerosis complex and severe vascular or lymphatic malformations. Sirolimus inhibits mechanistic target of rapamycin complex-1, a pathway central to cell growth, angiogenesis, and lymphangiogenesis. Published fetal case reports and small series describe regression of symptomatic cardiac rhabdomyomas

Large Language Model-Based Identification of Subjective Cognitive Decline in Electronic Health Records Among Older Adults With Normal Cognitive Testing: Retrospective Cohort Study.

BACKGROUND: Subjective cognitive decline (SCD) typically refers to self- or informant-reported decline in cognition despite the absence of objective impairment on standardized testing. Older adults with documented normal cognitive test performance provide a pragmatic anchor cohort for electronic health record (EHR)-based SCD phenotyping. However, cognitive concerns are primarily recorded in unstructured notes and are inconsistently documented, making it unclear how often and in whom concerns are captured in routine care. OBJECTIVE: This study aims to operationalize EHR-based SCD phenotyping i

Metabolic benefits of ketohexokinase inhibition in individuals with MASLD: a randomized placebo-controlled crossover trial.

High fructose consumption, particularly from sugar-sweetened beverages, contributes to cardiometabolic disease. Here we study the metabolic effects of 6-week pharmacological inhibition with PF-06835919 of ketohexokinase, the enzyme that catalyses the first committed step of fructose metabolism, in a randomized placebo-controlled crossover trial in 15 participants with metabolic dysfunction-associated steatotic liver disease. The primary outcome was hepatic insulin sensitivity, measured by insulin-stimulated suppression of endogenous glucose production. Secondary outcomes included fat distribut

Myasthenia gravis: from recognition of heterogeneity to a paradigm shift toward precision therapy.

Myasthenia gravis (MG) is a heterogeneous neuroimmune disorder in which pathogenic autoantibodies impair neuromuscular junction transmission and produce fluctuating skeletal muscle weakness. Although conventional therapy with acetylcholinesterase inhibitors, corticosteroids, and broad immunosuppressants remains effective for many patients, its limitations are increasingly evident, including delayed onset, cumulative toxicity, and inadequate control in refractory disease. These challenges have accelerated the transition from non-selective immunosuppression to biologic and other targeted therapi

Degradable MMP9-responsive high-density lipoprotein nanoparticles enable efficient siRNA-PGRN and quercetin delivery to remodel the fibrotic microenvironment and attenuate renal fibrosis.

Renal fibrosis is the final common pathway of chronic kidney disease and remains a challenge due to the complex fibrotic and inflammatory microenvironment. This study aimed to develop an enzyme-responsive, fibrosis-targeted nanotherapeutic system capable of simultaneously delivering siRNA against progranulin (siPGRN) and quercetin (QT) to remodel the fibrotic microenvironment and effectively attenuate renal fibrosis. The TIMP-HDL-Nano@siPGRN&QT nanoparticles prepared in this study exhibit uniform nanoscale size (~ 100 nm), high encapsulation efficiency, excellent stability, and favorable bioco

Testosterone Inhibition and Risk of Laryngeal and Hypopharyngeal Squamous Cell Carcinoma in Men.

IMPORTANCE: Laryngeal and hypopharyngeal squamous cell carcinoma most commonly occur in men. It has been hypothesized that testosterone explains this sex difference, but there is a lack of research on the topic. 5α-Reductase inhibitors (5-ARIs) are common medications used for men with benign hyperplasia that inhibit the conversion of testosterone to the bioactive form dihydrotestosterone and, thus, have anti-androgenic properties. OBJECTIVE: To examine whether use of 5-ARIs is associated with lower risk of laryngeal and hypopharyngeal squamous cell carcinoma in men. DESIGN, SETTING, AND PART

Comparative efficacy and safety of orally administered PCSK9 inhibitors in patients with Hypercholesterolaemia: a dose-stratified systematic review and network meta-analysis.

BACKGROUND: Injectable PCSK9 inhibitors are effective but remain under-used because of cost, the subcutaneous route, cold-chain storage and patient preference. Orally administered PCSK9 inhibitors, comprising two peptide-based agents and one small molecule, have recently completed phase 3 evaluation. METHODS: We searched four databases from inception to 1 May 2026 for randomised trials of orally administered PCSK9 inhibitors versus placebo. A frequentist random-effects network meta-analysis treated each drug-dose combination as a separate node. The primary outcome was the mean percentage chan

Intraprocedural Hemodynamics During Concomitant Rheocarna Therapy and Maintenance Hemodialysis: Effects of Dialyzer Membrane Type and Circuit Sequence.

Chronic limb-threatening ischemia (CLTI), a severe manifestation of peripheral artery disease, is particularly prevalent among patients receiving maintenance hemodialysis (HD) and is associated with poor limb and survival outcomes. Low-density lipoprotein (LDL) apheresis, including Rheocarna, is used as adjunctive therapy for refractory CLTI; however, intraprocedural hypotension remains a major limitation, especially when combined with HD. We examined the effects of dialyzer membrane type and extracorporeal circuit sequence on hemodynamic stability during concomitant therapy. This case series

Cardiometabolic diseases in transgender populations.

The number of people seeking gender-affirming medical care and gender-affirming hormone therapy (GAHT) is increasing. As sex steroids shape cardiometabolic physiology, GAHT is expected to meaningfully alter cardiometabolic risk. The aim of this review was to systematically determine and narratively synthesize evidence on the cardiometabolic effects of GAHT and identify clinically relevant knowledge gaps. We searched PubMed for English-language primary human studies published through November 26, 2025. Of 346 screened records, 96 unique original investigations were included. Most studies had le
