Effectiveness of subdural evacuating port system (SEPS) and middle meningeal artery embolization (MMAE) for chronic subdural hematomas - a multicenter experience.

BACKGROUND: Craniotomy for subdural hematoma (SDH) in elderly patients with comorbidities can be challenging. The Subdural Evacuating Port System (SEPS; Medtronic, Minneapolis, MN) offers a less invasive alternative, while middle meningeal artery embolization (MMAE) has shown effectiveness in preventing SDH recurrence. We evaluated the combined effectiveness of SEPS+MMAE for chronic SDH (cSDH) treatment. METHODS: Retrospective database reviews were conducted. Demographic, comorbidity, procedural, and outcomes data were analyzed. cSDH resolution was tracked by measuring hematoma volumes on non

Clinical Characteristics and Disease Progression among Nondiabetic Patients with Apolipoprotein L1-Mediated Kidney Disease and Patients with Other CKD.

KEY POINTS: Patients with nondiabetic apolipoprotein L1-mediated kidney disease progressed to dialysis and transplant faster than patients with other forms of nondiabetic CKD. Although patients with apolipoprotein L1-mediated kidney disease were younger with fewer comorbidities, kidney decline was accelerated underscoring the aggressive nature of apolipoprotein L1-mediated kidney disease. These findings highlight the need of targeted therapies for apolipoprotein L1-mediated kidney disease and earlier identification through genetic testing to improve patient outcomes. BACKGROUND: Apolipoprotei

Multi-omics integration reveals shared genetic architecture between metabolic markers and gray matter atrophy in Alzheimer's Disease.

BACKGROUND: Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by widespread gray matter volume (GMV) reductions. Emerging evidence links glucose and lipid metabolic dysregulation to AD pathophysiology. However, the extent to which AD-related GMV alterations and metabolic traits share a common genetic basis remains poorly understood. OBJECTIVES: To explore the shared genetic architecture between GMV alterations in AD and metabolites related to glucose and lipid metabolism, aiming to provide biological insights into the prevention and treatment of AD. DESIGN: T

Evaluation of plasma p-tau217 biomarkers in detecting amyloid pathology and predicting cognitive outcomes: Observations from Japanese Alzheimer's disease neuroimaging initiative cohort.

BACKGROUND AND OBJECTIVES: Plasma phosphorylated tau 217 (p-tau217) has shown strong potential as a blood-based biomarker for detecting amyloid pathology in Alzheimer's disease. This study evaluated the diagnostic and prognostic utility of plasma biomarkers, including p-tau217, in participants from the Japanese Alzheimer's Disease Neuroimaging Initiative (J-ADNI) cohort. METHODS: We analyzed paired plasma and CSF samples from 172 J-ADNI participants. CSF and plasma biomarkers were quantified using the LUMIPULSE platform, and the same plasma samples were analyzed using the Simoa platform. The

Association of circulating proteasome activity with Alzheimer's pathology and cognitive functions in APOE ε4 carriers.

BACKGROUND: The proteasome is a major intracellular protease complex, but the significance of circulating proteasome activity in Alzheimer’s disease (AD) is not well established. Because APOE ε4 is the strongest genetic risk factor for AD, we examined whether plasma proteasome activity is associated with AD-related pathology, neurodegeneration, and cognitive decline, focusing on APOE ε4 carriers. METHODS: In this observational study, participants were classified as cognitively normal (CN), mild cognitive impairment (MCI), and dementia. All underwent 3.0-T MRI, [18F]flutemetamol PET for amyloid

Oxylipin composition of high-density lipoprotein is altered in men and Hispanic adults with mild cognitive impairment.

High-density lipoprotein (HDL) oxylipins are potent inflammatory mediators. HDL dyshomeostasis and inflammation increase mild cognitive impairment (MCI) and dementia risk, which vary by gender and race/ethnicity. It is unknown whether and how HDL oxylipin profiles differ between non-MCI and MCI individuals, or if potential differences are gender- and/or race/ethnicity dependent. In this targeted lipidomics study, we profiled plasma HDL oxylipins in older (70+) adults (N = 222) with or without MCI to determine how HDL oxylipin composition relates to cognitive impairment status. HDL oxylipin con

Emulated Effects of Glucagon-Like Peptide 1 Receptor Agonist Therapy in the General Population.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) reduce cardiovascular risk in obese individuals with established cardiovascular disease (CVD), potentially through modulating key risk factors. However, their benefit in primary prevention remains unclear. OBJECTIVES: This study aims to emulate GLP-1RA use for primary prevention by applying risk factor changes observed in the SELECT (Semaglutide and Cardiovascular Outcomes in Obesity without Diabetes) trial to obese individuals at risk of but without established CVD. METHODS: In 610,789 CVD-free individuals from European and No

Genetic Insights on Hypertriglyceridaemia-Induced Acute Pancreatitis in Pregnancy: A Case Series and Literature Review.

CONTEXT: Hypertriglyceridaemia-induced pancreatitis in pregnancy (HTG-IPP) is a rare but serious condition. There is a paucity of evidence-based guidelines and recommendations for screening and management of HTG-IPP. Individual genomics can predispose certain populations to a higher risk of developing HTG-IPP. OBJECTIVE: To report on a case series of the management of four individual pregnancies complicated by HTG-IPP, subsequently found to be associated with pathogenic genetic variants involved in triglyceride (TG) metabolism. METHODS: The medical records of four individual pregnancies from

Systematic assessment of obesity-related risk factors in renal cancer etiology: A longitudinal risk and Mendelian randomization analysis.

BACKGROUND: Excess body adiposity is an established cause of renal cancer, but underlying molecular pathways mediating this relationship remain unclear. This study aimed to systematically evaluate a panel of obesity-related risk factors as potential mediators in renal cancer etiology. METHODS AND FINDINGS: We used two complementary approaches to evaluate obesity-related risk factors in renal cancer etiology: (i) direct risk factor assessment in longitudinal cohorts and (ii) genetically proxied risk factors through two-sample Mendelian randomization (MR). Direct risk-factor association-analyse

Antipsychotic plasma concentration as predictor of movement disorders and cardiometabolic side-effects: A comparison with prescription dose.

The clinical evidence for antipsychotic (AP) therapeutic drug monitoring (TDM) in evaluating AP-related movement disorders and cardiometabolic side-effects remains inconsistent. This study evaluates how AP plasma concentrations associate with movement disorders and cardiometabolic side-effects over time, and compares its predictive value to prescription dose in first-episode psychosis (FEP) patients. We included 200 remitted FEP patients from the HAMLETT trial. AP plasma concentrations were standardized using robust z-scores to accommodate different AP types. The St. Hans Rating Scale and Barn

Lithium chloride suppresses ferroptosis of induced pluripotent stem cells with ApoE4/E4 from a sporadic Alzheimer's disease patient.

Alzheimer's disease (AD), particularly its sporadic form (SAD, 95 % AD patients), is strongly associated with the apolipoprotein E4 (ApoE4) genotype and characterized by oxidative stress, iron dysregulation, and increased susceptibility to ferroptosis. Lithium, a well-established neuroprotective agent, has shown potential to mitigate several pathological mechanisms in AD, including ferroptosis. This study investigates the therapeutic potential of lithium chloride in human induced pluripotent stem cells (iPSCs) derived from a SAD patient with ApoE4/E4 genotype and compared effects with those of

Barriers and Emerging Strategies to Bridge Gaps in Implementation of Evidence-Based Lipid-Lowering Therapies.

Atherosclerotic cardiovascular disease (ASCVD) is a leading cause of morbidity and mortality globally. Low-density lipoprotein cholesterol is a causal risk factor for atherosclerotic cardiovascular disease, with multiple classes of cholesterol-lowering therapies effective at reducing atherosclerotic cardiovascular disease risk. Despite robust efficacy data from randomized trials, real-world implementation of guideline-recommended lipid-lowering therapies is suboptimal. Barriers to the implementation of guideline lipid-lowering therapy recommendations exist at the healthcare systems level, the

Fenofibrate plus statin and ASCVD risk by triglyceride-rich lipoprotein cholesterol in 70,000 patients.

Fenofibrate may offer cardiovascular benefit in statin-treated individuals with elevated triglyceride-rich lipoprotein cholesterol (TRL-C), a measure reflecting cholesterol carried in triglyceride-rich lipoproteins, yet its role remains uncertain. We investigated whether fenofibrate added to statin therapy is associated with lower risk of atherosclerotic cardiovascular disease (ASCVD), particularly in individuals with high TRL-C. We analyzed 67,662 statin-treated adults without baseline ASCVD from the Korean National Health Insurance Service cohort. Participants were stratified by median TRL-C
