Intraprocedural Hemodynamics During Concomitant Rheocarna Therapy and Maintenance Hemodialysis: Effects of Dialyzer Membrane Type and Circuit Sequence.

Chronic limb-threatening ischemia (CLTI), a severe manifestation of peripheral artery disease, is particularly prevalent among patients receiving maintenance hemodialysis (HD) and is associated with poor limb and survival outcomes. Low-density lipoprotein (LDL) apheresis, including Rheocarna, is used as adjunctive therapy for refractory CLTI; however, intraprocedural hypotension remains a major limitation, especially when combined with HD. We examined the effects of dialyzer membrane type and extracorporeal circuit sequence on hemodynamic stability during concomitant therapy. This case series

Cardiometabolic diseases in transgender populations.

The number of people seeking gender-affirming medical care and gender-affirming hormone therapy (GAHT) is increasing. As sex steroids shape cardiometabolic physiology, GAHT is expected to meaningfully alter cardiometabolic risk. The aim of this review was to systematically determine and narratively synthesize evidence on the cardiometabolic effects of GAHT and identify clinically relevant knowledge gaps. We searched PubMed for English-language primary human studies published through November 26, 2025. Of 346 screened records, 96 unique original investigations were included. Most studies had le

Optimizing treatment in homozygous familial hypercholesterolemia: Monocentric observational clinical study on efficacy and safety of evinacumab and lomitapide combination.

BACKGROUND: Genetic variants of the low-density lipoprotein (LDL) receptor cause homozygous familial hypercholesterolemia (HoFH). Disease-specific treatment options include lipoprotein apheresis and lomitapide, and more recently, the angiopoietin-like 3 (ANGPTL3)-targeting monoclonal antibody evinacumab. OBJECTIVE: We evaluated the efficacy and safety of this therapeutic combination in patients with HoFH. METHODS: We report a single-center, real-world clinical experience including 6 patients with HoFH, 4 of whom were pediatric, with persistently elevated LDL-cholesterol (LDL-C) (>130 mg/dL),

Predictive modeling of vocal biomarkers for the diagnosis of Parkinson's disease.

Parkinson's disease (PD) is among the two most prevalent neurodegenerative disorders (NDDs), affecting about 2-3% of individuals aged 65 and older. This NDD exhibits characteristic motor symptoms and several other non-motor features. Vocal deficits have been identified as one of the earliest quantifiable indicators of PD, which makes speech evaluation a viable, painless diagnostic instrument. We aim to apply machine learning (ML) models to vocal biomarkers for the early detection of PD, and use explainable artificial intelligence (XAI) techniques to interpret the predictions. The dataset is fr

Cytokine-involved phenotyping identifies a high-risk inflammatory cluster in treatment-naïve Takayasu's arteritis.

OBJECTIVES: Takayasu's arteritis (TAK) is a clinically heterogeneous large-vessel vasculitis. Conventional inflammatory markers such as erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) may not adequately reflect underlying immune activation or predict long-term outcomes. We aimed to identify immune phenotypes based on serum cytokine profiles and evaluate their clinical relevance in treatment-naïve TAK. METHODS: We performed an unsupervised two-step cluster analysis in a cohort of 95 treatment-naïve patients with TAK. Serum levels of 12 cytokines were measured at baseline and

Rates of Lipoprotein(a) Oxidation Accelerate at Elevated Levels and Attenuate With Atorvastatin Active Metabolites Compared With Other Statins.

Lipoprotein(a) [Lp(a)] levels are associated with increased cardiovascular risk, possibly linked to its oxidized phospholipid content that may accelerate lipoprotein oxidation. Attenuating this process could reduce Lp(a) atherogenicity. Atorvastatin is metabolized into 2 hydroxylated metabolites, which account for low-density lipoprotein reduction and potential antioxidant activity. We compared these metabolites' effects to other active statins on oxidation of plasma samples enriched with various levels of Lp(a) or other apolipoprotein B particles, as well as lipid membranes enriched with chol

Lipoprotein (a) Testing and Changes in Lipid-Lowering Therapy in Patients With ASCVD: Findings From the cvMOBIUS-2 Registry.

BACKGROUND: Although the effectiveness of Lp(a) (lipoprotein[a])-lowering therapies remains under study, lipid guidelines now recommend Lp(a) testing in all adults to identify individuals who need more aggressive risk factor modification. Therefore, we evaluate contemporary patterns of Lp(a) testing and the impact of testing on LDL-C (low-density lipoprotein cholesterol) management. METHODS: Electronic health record patient data from 13 US health systems from January 1, 2021 to January 1, 2025, were utilized to assess the annual proportion of patients with atherosclerotic cardiovascular disea

Prognostic impact of residual inflammatory and triglyceride risk in statin-treated patients with well-controlled LDL cholesterol and atherosclerotic cardiovascular disease.

AIMS: Identifying alternative contributors to the residual risk of atherosclerotic cardiovascular disease (ASCVD) beyond LDL cholesterol (LDL-C) levels is crucial. We investigated the relative impact of triglycerides (TGs) and high-sensitivity C-reactive protein (hs-CRP) on outcomes in statin-treated patients with well-controlled LDL-C undergoing percutaneous coronary intervention (PCI) for established ASCVD. METHODS AND RESULTS: We included 9446 statin-treated patients with LDL-C < 70 mg/dL undergoing PCI between 2012 and 2022, stratified into four groups: (i) no residual risk (TG <150 mg/dL

Lifestyle and pharmacologic interventions for adults with hypertriglyceridemia: Critical to reduce risk of ASCVD and pancreatitis but often underutilized.

The 2026 American College of Cardiology (ACC)/American Heart Association (AHA)/Multisociety Guideline on the Management of Dyslipidemia (2026 Guideline) replaces the 2018 AHA/ACC/Multisociety Guideline on the Management of Blood Cholesterol. The 2026 Guideline expands the section on the management of hypertriglyceridemia (HTG) and acknowledges that triglyceride (TG)-rich lipoproteins contribute to atherosclerotic cardiovascular disease (ASCVD) risk. The 2026 Guideline emphasizes lifestyle interventions as the foundation of HTG management and recommends risk-based pharmacologic interventions fo

Ginkgolide as a Promising Multi-Target Therapeutic for Alzheimer's Disease: Targeting ApoE4 and Beyond.

INTRODUCTION: The progressive neurodegenerative disease known as Alzheimer's disease (AD) is typified by neuroinflammation, amyloid-beta buildup, and cognitive impairment. Current pharmacological treatments merely alleviate symptoms, despite extensive research, which underscores the need for innovative, multi-target medicines. Since apolipoprotein E4 (ApoE4) is a significant genetic risk factor linked to the development of AD, it is a potentially effective treatment target. With their neuroprotective qualities, natural substances like Ginkgolide may help treat some diseases. This study investi

Sex-specific disparities in lipid-based prediction of major cardiovascular events.

AIMS: Lipid-based markers have a profound prognostic potential concerning cardiovascular diseases. However, differences in lipid levels between women and men are evident, and it is unknown whether the differences also extend to the predictive power of the lipid markers. Therefore, this study aims to evaluate potential sex differences concerning the risk estimation of five topical lipid markers: low-density lipoprotein cholesterol (LDL-C), remnant cholesterol, apolipoprotein A-1 (ApoA-1), the triglyceride/HDL cholesterol (TG/HDL) ratio, and ceramide scores (CERT1 and CERT2). Targeting Triglycerides in Cardiovascular Disease Prevention: Evidence, Mechanisms, and Emerging Therapies.

PURPOSE OF REVIEW: The goal of this review is to evaluate the evolving role of triglycerides (TGs) and TG-rich lipoproteins (TRLs) in cardiovascular disease (CVD) risk and prevention. We examine the mechanistic rationale, genetic and epidemiological evidence, and therapeutic potential of targeting TGs in residual risk reduction, particularly in high-risk populations. RECENT FINDINGS: Emerging data from Mendelian randomization studies and large clinical cohorts support a causal link between elevated remnant lipoproteins and atherosclerotic CVD, in which apolipoprotein B may be the principal dr

Genome-wide association study identifies novel and confirms established loci associated with serum lipids levels in Brazilians.

Dyslipidemia is an important risk factor for cardiovascular diseases and can result from genetic and environmental influences. Most genome-wide association studies (GWAS) have been conducted in European populations, limiting our understanding of polymorphisms involved in lipid levels in admixed populations such as Brazilians. Therefore, this study aimed to identify genetic variants associated with lipid traits and develop polygenic risk scores (PRS) for dyslipidemia prediction in a large cohort of Brazilians. We performed GWAS of lipid phenotypes using 19,016 Brazilian individuals previously g

Genetic determinants of plasma lipids in Greenlanders.

PURPOSE OF REVIEW: Greenlanders differ from other populations in terms of traditional lifestyle and genetic architecture. This might have a great impact on lipid levels in the population and the spectrum of genetic variants associated with lipid traits. Here, we review recent advances in lipid genetics in Greenlanders and highlight the potential of moving from single lipid trait analyses to more comprehensive lipidomic profiling. RECENT FINDINGS: Genetic association studies in the Greenlandic population have identified variants, including PCSK9 (rs12117661), LDLR (rs730882082), and SI (rs7814
