Efficacy and Safety of Oral Non-Statin Lipid-Lowering Therapies in Dyslipidaemia: A Systematic Review and Network Meta-Analysis.

INTRODUCTION: Dyslipidaemia remains a major contributor to atherosclerotic cardiovascular disease (ASCVD), and many patients fail to achieve recommended low-density lipoprotein cholesterol (LDL-C) targets despite statin therapy or are unable to tolerate statins. Oral non-statin therapies have emerged as important alternatives; however, their comparative efficacy and safety remain unclear. METHODS: We systematically searched PubMed, Embase, Web of Science, and Cochrane Central up to December 2025 for clinical trials evaluating these medications in adults with dyslipidaemia. Primary outcomes we

Hypermetabolic State Drives Platelet Dysfunction in Human and Mouse Metabolic Dysfunction-Associated Steatotic Liver Disease.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is a growing health concern given rising global prevalence, limited diagnostic and therapeutic tools, and strong association with cardiovascular disease. Platelets and oxidized phospholipids have been implicated in MASLD pathogenesis; however, the drivers and activities of platelets in MASLD remain overlooked. METHODS: We conducted a cross-sectional study to determine platelet metabolic and functional states in MASLD and to investigate whether oxidized phospholipids, which accumulate during MASLD, contribute to plate

Use of Coronary Artery Calcium Scoring in Individuals With Elevated Lipoprotein(a): A Multicohort Study.

BACKGROUND: The utility of coronary artery calcium (CAC) scoring in individuals with elevated lipoprotein(a) [Lp(a)] for atherosclerotic cardiovascular disease (ASCVD) risk assessment is currently unclear given the propensity of Lp(a) toward noncalcified plaque. OBJECTIVES: The authors aimed to evaluate the interaction between elevated Lp(a) (>50 mg/dL) and CAC score, and the association of Lp(a) with ASCVD risk across strata of CAC. METHODS: A pooled cohort of participants without known ASCVD from 4 U.S.-based prospective cohort studies with baseline Lp(a) and CAC measurements was used. The

Independent, but not synergistic, associations of APOEε4 and systemic inflammation on cognitive decline: findings from the Women's Health Initiative.

INTRODUCTION: Understanding the risk factors that associate with early cognitive decline in Alzheimer's disease (AD) is important to identify high-risk individuals and initiate early intervention. Existing studies show that APOEε4, systemic inflammation, and diabetes may play roles in cognitive decline, but the extent to which these factors interact with each other remains unclear. Our objective was to examine the main effects and higher-order interactions between APOEε4, high sensitivity-C-reactive protein (hs-CRP) as a measure of systemic inflammation, and diabetes on domain-specific measure

Lipoprotein(a) selectively associates with vulnerable coronary plaque phenotypes in comparison with other established risk markers.

AIMS: Lipoprotein(a) [Lp(a)] is an inherited cardiovascular risk factor. However, its association with coronary plaque characteristics beyond traditional risk enhancers remains unclear. We aimed to evaluate the association between Lp(a) levels and coronary plaque characteristics in asymptomatic primary prevention patients, and to compare its predictive value against other risk enhancers, including LDL particle concentration (LDL-P), high-sensitivity C-reactive protein (hsCRP), and coronary artery calcium (CAC) score. METHODS AND RESULTS: We retrospectively analysed 547 asymptomatic patients u

Clinical Factors and Biomarkers During Pregnancy and Risk of Cardiovascular Disease.

IMPORTANCE: Cardiovascular disease (CVD) is the leading cause of death among women worldwide. Pregnancy serves as a natural cardiovascular stress test and universal clinical encounter, yet few approaches leverage its insights to inform long-term cardiovascular risk. OBJECTIVE: To determine whether clinical measures and biomarkers obtained during pregnancy may identify women at risk of long-term CVD. DESIGN, SETTING, AND PARTICIPANTS: This was a registry-linked, population-based cohort study of all pregnancies reaching at least 22 weeks between June 2010 and October 2013 in Southern Denmark.

Short-term docosahexaenoic acid rich diet prevents cognitive deficits in human apolipoprotein E epsilon 4-targeted replacement mice.

BACKGROUND: Metabolism of docosahexaenoic acid (DHA), an omega-3 (Ω3) fatty acid (FA), differs between carriers of the epsilon 4 allele of the apolipoprotein E (APOE4)-the main genetic risk factor for late-onset Alzheimer's disease-and APOE3 carriers. Dietary DHA has been shown to prevent cognitive decline in APOE4 carriers. However, whether DHA must be consumed the whole life is unclear. We hypothesized that a DHA intake started later in life and for a shorter duration prevents cognitive decline in APOE4 mice. OBJECTIVE: To investigate three dietary durations of DHA on the prevention of cogn

Familial Hypercholesterolemia in Latin America: Unmet Needs, Barriers, and Challenges.

Familial hypercholesterolemia (FH) is the most common monogenic disorder predisposing individuals to premature atherosclerotic cardiovascular disease. Despite an estimated prevalence of 1 in 250 individuals, FH remains largely underdiagnosed and undertreated across Latin America, where more than 2 million people are expected to be living with FH. Early identification and intensive lipid-lowering therapy substantially improve clinical outcomes; however, multiple structural, socioeconomic, and educational barriers continue to limit optimal care throughout the region. FH detection rates remain lo

Clinical and Metabolic Associations of Metabolic Dysfunction-Associated Steatotic Liver Disease in Young Adults: A Cross-Sectional Analysis.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognized as the hepatic manifestation of systemic metabolic dysfunction and is closely associated with obesity, dyslipidaemia, insulin resistance, and metabolic syndrome. However, evidence regarding its burden and metabolic correlates among apparently healthy young adults remains limited. This study evaluated the prevalence of MASLD and its association with clinical, anthropometric, metabolic, and hepatic parameters in a young adult population. METHODS: This hospital-based cross-sectional study incl

Cardiorenal risk stratification in high-risk type 2 diabetes using a simple clinical score: findings from the ELIXA trial.

INTRODUCTION: Early identification of patients with high cardiorenal risk and timely targeted interventions are critical in managing type 2 diabetes (T2D). Although we previously developed a multivariable risk score to predict diabetic kidney disease (DKD), its ability to stratify cardiorenal risk in T2D patients with established high cardiovascular risk remains unknown. METHODS: In this post-hoc analysis of the ELIXA trial, 2,635 T2D participants without baseline DKD were stratified into different risk groups (low-, moderate-, high-, and very high-risk group) using the previously developed r

Adjunctive pharmacological strategies for residual risk reduction after myocardial revascularisation.

Pharmacological treatment remains vital in the effective management of atherosclerotic cardiovascular disease (ASCVD). Low-density lipoprotein (LDL) cholesterol-lowering therapies, such as statins, have consistently demonstrated robust efficacy in the primary and secondary prevention of cardiovascular events. The introduction of ezetimibe, bempedoic acid, and proprotein convertase subtilisin/kexin type 9 inhibitors have further strengthened the effectiveness of LDL cholesterol management, particularly in patients who are statin intolerant or who remain at high risk despite maximal tolerated st

Glucose-6-Phosphate Dehydrogenase Deficiency Reprograms Macrophage Metabolism in Hyperlipidemia.

Macrophages regulate inflammatory responses in hyperlipidemia and dysmetabolic states while performing a critical homeostatic function by clearing damaged or senescent red blood cells (RBCs) through erythrophagocytosis. This process delivers heme and iron that must be detoxified and recycled through NADPH-dependent pathways. Glucose-6-phosphate dehydrogenase deficiency (G6PDd), affecting ~6% of the global population, limits NADPH generation and predisposes RBCs to oxidative damage and hemolysis. Whether G6PD deficiency alters macrophage metabolic responses to RBC clearance in hyperlipidemic en
