BACKGROUND: The utility of coronary artery calcium (CAC) scoring in individuals with elevated lipoprotein(a) [Lp(a)] for atherosclerotic cardiovascular disease (ASCVD) risk assessment is currently unclear given the propensity of Lp(a) toward noncalcified plaque.
OBJECTIVES: The authors aimed to evaluate the interaction between elevated Lp(a) (>50 mg/dL) and CAC score, and the association of Lp(a) with ASCVD risk across strata of CAC.
METHODS: A pooled cohort of participants without known ASCVD from 4 U.S.-based prospective cohort studies with baseline Lp(a) and CAC measurements was used. The

INTRODUCTION: Understanding the risk factors that associate with early cognitive decline in Alzheimer's disease (AD) is important to identify high-risk individuals and initiate early intervention. Existing studies show that APOEε4, systemic inflammation, and diabetes may play roles in cognitive decline, but the extent to which these factors interact with each other remains unclear. Our objective was to examine the main effects and higher-order interactions between APOEε4, high sensitivity-C-reactive protein (hs-CRP) as a measure of systemic inflammation, and diabetes on domain-specific measure

AIMS: Lipoprotein(a) [Lp(a)] is an inherited cardiovascular risk factor. However, its association with coronary plaque characteristics beyond traditional risk enhancers remains unclear. We aimed to evaluate the association between Lp(a) levels and coronary plaque characteristics in asymptomatic primary prevention patients, and to compare its predictive value against other risk enhancers, including LDL particle concentration (LDL-P), high-sensitivity C-reactive protein (hsCRP), and coronary artery calcium (CAC) score.
METHODS AND RESULTS: We retrospectively analysed 547 asymptomatic patients u

BACKGROUND: Metabolism of docosahexaenoic acid (DHA), an omega-3 (Ω3) fatty acid (FA), differs between carriers of the epsilon 4 allele of the apolipoprotein E (APOE4)-the main genetic risk factor for late-onset Alzheimer's disease-and APOE3 carriers. Dietary DHA has been shown to prevent cognitive decline in APOE4 carriers. However, whether DHA must be consumed the whole life is unclear. We hypothesized that a DHA intake started later in life and for a shorter duration prevents cognitive decline in APOE4 mice.
OBJECTIVE: To investigate three dietary durations of DHA on the prevention of cogn

Familial hypercholesterolemia (FH) is the most common monogenic disorder predisposing individuals to premature atherosclerotic cardiovascular disease. Despite an estimated prevalence of 1 in 250 individuals, FH remains largely underdiagnosed and undertreated across Latin America, where more than 2 million people are expected to be living with FH. Early identification and intensive lipid-lowering therapy substantially improve clinical outcomes; however, multiple structural, socioeconomic, and educational barriers continue to limit optimal care throughout the region. FH detection rates remain lo

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognized as the hepatic manifestation of systemic metabolic dysfunction and is closely associated with obesity, dyslipidaemia, insulin resistance, and metabolic syndrome. However, evidence regarding its burden and metabolic correlates among apparently healthy young adults remains limited. This study evaluated the prevalence of MASLD and its association with clinical, anthropometric, metabolic, and hepatic parameters in a young adult population.
METHODS: This hospital-based cross-sectional study incl

Metabolic dysfunction-associated steatotic liver disease (MASLD) is among the leading causes of chronic liver disease, with few approved treatment options. Vitamin D supplementation has been proposed as a safe and efficacious supplement intervention for MASLD. The current study aimed to systematically evaluate the effect of vitamin D supplementation in any preparation on hepatic (histological, radiological, and biomarker) and metabolic parameters (glucose regulation, lipid profile, and indices of obesity) in patients diagnosed with steatotic liver disease. These effects were compared with pati

INTRODUCTION: Early identification of patients with high cardiorenal risk and timely targeted interventions are critical in managing type 2 diabetes (T2D). Although we previously developed a multivariable risk score to predict diabetic kidney disease (DKD), its ability to stratify cardiorenal risk in T2D patients with established high cardiovascular risk remains unknown.
METHODS: In this post-hoc analysis of the ELIXA trial, 2,635 T2D participants without baseline DKD were stratified into different risk groups (low-, moderate-, high-, and very high-risk group) using the previously developed r

Pharmacological treatment remains vital in the effective management of atherosclerotic cardiovascular disease (ASCVD). Low-density lipoprotein (LDL) cholesterol-lowering therapies, such as statins, have consistently demonstrated robust efficacy in the primary and secondary prevention of cardiovascular events. The introduction of ezetimibe, bempedoic acid, and proprotein convertase subtilisin/kexin type 9 inhibitors have further strengthened the effectiveness of LDL cholesterol management, particularly in patients who are statin intolerant or who remain at high risk despite maximal tolerated st

KEY POINTS: Patients with nondiabetic apolipoprotein L1-mediated kidney disease progressed to dialysis and transplant faster than patients with other forms of nondiabetic CKD. Although patients with apolipoprotein L1-mediated kidney disease were younger with fewer comorbidities, kidney decline was accelerated underscoring the aggressive nature of apolipoprotein L1-mediated kidney disease. These findings highlight the need of targeted therapies for apolipoprotein L1-mediated kidney disease and earlier identification through genetic testing to improve patient outcomes.

BACKGROUND: Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by widespread gray matter volume (GMV) reductions. Emerging evidence links glucose and lipid metabolic dysregulation to AD pathophysiology. However, the extent to which AD-related GMV alterations and metabolic traits share a common genetic basis remains poorly understood.
OBJECTIVES: To explore the shared genetic architecture between GMV alterations in AD and metabolites related to glucose and lipid metabolism, aiming to provide biological insights into the prevention and treatment of AD.

BACKGROUND: The proteasome is a major intracellular protease complex, but the significance of circulating proteasome activity in Alzheimer’s disease (AD) is not well established. Because APOE ε4 is the strongest genetic risk factor for AD, we examined whether plasma proteasome activity is associated with AD-related pathology, neurodegeneration, and cognitive decline, focusing on APOE ε4 carriers. METHODS: In this observational study, participants were classified as cognitively normal (CN), mild cognitive impairment (MCI), and dementia. All underwent 3.0-T MRI, [18F]flutemetamol PET for amyloid

High-density lipoprotein (HDL) oxylipins are potent inflammatory mediators. HDL dyshomeostasis and inflammation increase mild cognitive impairment (MCI) and dementia risk, which vary by gender and race/ethnicity. It is unknown whether and how HDL oxylipin profiles differ between non-MCI and MCI individuals, or if potential differences are gender- and/or race/ethnicity dependent. In this targeted lipidomics study, we profiled plasma HDL oxylipins in older (70+) adults (N = 222) with or without MCI to determine how HDL oxylipin composition relates to cognitive impairment status. HDL oxylipin con
