Allopurinol-induced drug reaction with eosinophilia and systemic symptoms syndrome in a patient with previous nephrectomy: a case report.

BACKGROUND: Drug reaction with eosinophilia and systemic symptoms is a rare but serious hypersensitivity reaction, typically occurring 2-8 weeks after drug initiation. It presents with rash, fever, hematological abnormalities, and multi-organ involvement, most commonly affecting the liver, kidneys, and lungs. Allopurinol is a well-recognized trigger, particularly in patients with renal impairment. This case is notable owing to the patients' previous donor nephrectomy, rather than intrinsic renal disease. CASE PRESENTATION: A 76-year-old white British male with a history of donor nephrectomy a

An integrated machine learning framework for developing a transcriptomic analysis and machine learning-based diagnostic model of gout based on sleep disorder-related genes.

Gout, often comorbid with sleep disorders (SDs), is prevalent in inflammatory arthritis. This comorbidity is notable in patients with impaired renal function requiring blood purification, with unclear underlying mechanisms. This study integrated transcriptomic analysis and machine learning to identify shared genetic markers and develop a diagnostic model for gout based on SD-related genes. Transcriptomic datasets from the Gene Expression Omnibus, including those of gout and SD patients, were systematically analyzed to develop the model. Differentially expressed genes (DEGs) associated with bot

Phaseoloidin, a homogentisic acid glucoside from Entada phaseoloides, suppresses gout inflammation via NLRP3 inflammasome.

BACKGROUND: Gout is an arthritic disorder caused by the deposition of sodium urate crystals in the joints. Phaseoloidin (PHA), a β-glucoside of homogentisic acid isolated from the seeds of the legume "Entada phaseoloides (L.) Merr.", has previously showed anti-inflammatory properties, but its effects on gout and underlying mechanisms remain unexplored. PURPOSE: This study aimed to investigate the novel therapeutic potential of PHA in gout, with a focus on its multi-target actions against NLRP3 inflammasome activation and associated inflammatory cascades-areas not yet addressed for this compou

Rheumatoid arthritis and gout: a rare combination or overlooked coexistence?

BACKGROUND: Rheumatoid arthritis (RA) and gout are traditionally considered distinct diseases with differing pathogenic mechanisms, making their coexistence controversial. Emerging evidence suggests this overlap may be underestimated. This study aimed to evaluate their epidemiological association, genetic causality, and intersecting molecular features. METHODS: Epidemiological analyses used National Health and Nutrition Examination Survey (NHANES; 2007 ~ 2018, n = 19,705) data. Propensity score matching and weighted multivariate logistic regression assessed gout prevalence, temporal trends, a

Advances in the study of gut microecology and mechanisms of hyperuricemia and gouty arthritis.

Gouty arthritis is a metabolic disorder caused by purine metabolism dysregulation, characterized by monosodium urate crystal deposition in and around joints, triggering acute articular inflammation via NLRP3 inflammasome activation and IL-1β-mediated inflammatory cascades. While hyperuricemia represents a critical biochemical prerequisite for gouty arthritis development, elevated serum urate levels do not invariably lead to the disease. Mounting evidence suggests a significant relationship between gut microbiota and the pathogenesis of both gouty arthritis and hyperuricemia. The gut microbial

Proteomic footprint of serum urate concentration and urate transporter ABCG2 dysfunctional polymorphism: a cross-sectional study.

BACKGROUND: Existing proteomic studies have primarily focused on gout flares or symptomatic hyperuricaemia, and few have comprehensively investigated circulating proteomic profiles in serum urate (SU) concentration as a continuous quantitative trait. This cross-sectional study investigated the association between SU concentration and serum proteomic profiles. METHODS: We enrolled 176 Japanese individuals aged ≥50 years and measured serum proteins using the Olink Explore 3072. To analyse the association between SU concentration and serum proteins, we applied linear regression adjusting for age

[Causal relationship between genetically determined childhood obesity and serum uric acid levels and gout in adults: a two-sample Mendelian randomization study].

Obesity is a significant risk factor for developing hyperuricemia and gout. This study aims to exploring the causal relationship between childhood obesity and serum uric acid (SUA) levels and gout in adults in a Mendelian randomization (MR) framework. The Genome-wide association study (GWAS) data for childhood obesity were obtained from the Early Growth Genetics Consortium, published in 2012, involving 13 848 children. The GWAS data for SUA were sourced from a 2021 phenome-wide association study in the Medical Research Council Integrative Epidemiology Unit (MRC-IEU) GWAS database, comprising 3

[Multidisciplinary expert consensus on tophus damage grading and surgical treatment (2025 edition)].

Tophi, a severe complication of advanced gout, develop in approximately 12% to 53% of patients with gout. The treatment of bone and joint damages and chronic refractory wounds caused by tophi presents considerable clinical challenges. Currently, there is no unified standard for the optimal surgical timing, perioperative management, and treatment approaches for this condition. To address this, the Wound Repair Professional Committee of Chinese Medical Doctor Association convened a multidisciplinary panel of senior experts in wound repair, plastic surgery, orthopedics & joint surgery, rheumatolo

Nanoencapsulated Sirolimus Plus Pegadricase (NASP) in Uncontrolled Gout: Current Status and Future Directions.

Uncontrolled gout (UG) is a progressive arthropathy that develops as a consequence of sustained hyperuricemia. Recombinant uricases can profoundly lower serum urate (SU) and reverse the clinical manifestations of uncontrolled gout. However, therapeutic uricases are highly immunogenic and can provoke the development of anti-drug antibodies (ADAs), resulting in a loss of effect and potentially severe allergic reactions. Pegloticase is currently the only US Food and Drug Administration-approved uricase indicated for the treatment of UG. The Food and Drug Administration recently approved the co-ad

Mitonuclear Discordance and Gout, Type 2 Diabetes and Chronic Kidney Disease in Aotearoa New Zealand Indigenous Māori and Pacific People.

BACKGROUND: Mitochondrial proteins are encoded by both mitochondrial- and nuclear-encoded genes. Because mitochondrial DNA (mtDNA) is maternally inherited, admixed individuals may have different ancestral sources for their nuclear and mitochondrial genomes. The potential incompatibility between these genomic components may cause suboptimal mitochondrial function and result in energy-related pathologies. This incompatibility, or 'mitonuclear discordance', is defined as the proportion of the nuclear genome not derived from the same ancestral source as the mtDNA. METHODS: Based on this understan

Clinical response to capecitabine in a DPYD*2A homozygous patient: Case report and therapeutic guidance.

Complete dihydropyrimidine dehydrogenase (DPD) deficiency due to homozygous DPYD*2A is a contraindication for fluoropyrimidines because of the extreme risk of life-threatening toxicity. We report a 36-year-old male with metastatic rectal adenocarcinoma and complete DPD deficiency who received ultra-low-dose capecitabine in combination with oxaliplatin and cetuximab. Initial dosing of capecitabine 150 mg on Days 1, 6 and 16 of a 21-day cycle (0.76% of the BSA-based standard regimen) caused Grade 3 mucositis, but further dose reduction to capecitabine 150 mg on Days 1 and 8 of a 21-day cycle (0.

CRISPR-Cas9-mediated uATG introduction in the 5'UTR of the Uox gene for hyperuricemia mouse models: implications for gout and metabolic disorders.

Sequence-specific gene knockdown technologies are crucial for fundamental research and therapeutic applications. RNA interference and CRISPR interference, while extensively utilized for gene expression manipulation, face limitations due to their ectopic or transient expression. In this study, we developed a generalizable and efficient method to downregulate gene expression in human 293T cells by introducing de novo upstream ATGs (uATGs) of genes using CRISPR-Cas9-mediated genome editing. Through CRISPR library screening, in-depth sequencing, and flow cytometry analysis, we validated that the i

The Evolving Role of Musculoskeletal Ultrasound in Gout: From Diagnosis to Management-A Narrative Review.

Gout, a prevalent and treatable form of crystal-induced arthritis, results from monosodium urate (MSU) crystal deposition in articular and periarticular tissues. Early diagnosis is crucial to prevent progression to chronic gouty arthritis, tophus formation, and structural joint damage. Musculoskeletal ultrasound (MUS) has emerged as a sensitive, noninvasive, and cost-effective imaging modality that enables the visualization of urate crystal deposition, evaluation of treatment response, and prediction of disease flares. This narrative review summarizes recent advances in MUS for the diagnosis a

TFAP2A transcriptionally regulates B3GALT2 to affect gouty arthritis progression through pyroptosis: a study based on machine learning and multi-omics integration analysis.

BACKGROUND: Gouty arthritis (GA) is an inflammatory joint disease driven by monosodium urate (MSU) crystal deposition. The NOD-like receptor thermal protein domain associated protein 3 (NLRP3) inflammasome-mediated pyroptosis is central to GA pathogenesis, yet potential regulatory targets remain limited. The role of β-1,3-galactosyltransferase 2 (B3GALT2), a glycosyltransferase, is entirely unknown in GA. This study aims to identify novel biomarkers for GA and investigate the role and regulatory mechanism of B3GALT2. METHODS: Differentially expressed genes (DEGs) analysis was performed on the

Development and characterization of allopurinol-loaded transethosomal gel for topical gout management.

Gout refers to the painful inflammation of the joints due to deposition of monosodium urate (MSU) cystals. Treatment of gout commonly involve a biopharmaceutical classification system (BCS) class IV drug, allopurinol (ALLP), which is a xanthine oxidase inhibitor with poor solubility and permeability profile. The current study aimed at the development of ALLP-loaded transethosomes (ALLP-TEs) through thin film hydration technique. Design of experiment (DoE) was optimized via Box-Behnken design analysis and the effects of independent variables was examined over the dependent one's. Optimized ALLP

Gout Inflammation Time Programming: Molecular Clock from Crystal Triggering to Tissue Remodeling.

This review introduces and elaborates a novel temporal paradigm, the "Gout Inflammation Time Programming" model, conceptualized through the Gout-STAT™ framework. This model redefines gout inflammation as a dynamic continuum progressing through three precisely timed phases: an acute Perception phase (0-24 h) initiated by monosodium urate (MSU) crystal recognition, triggering the NOD-like receptor thermal protein domain associated protein 3 (NLRP3) inflammasome and neutrophil-driven burst; a critical Adaptation phase (24-72 h) where outcomes are determined by immunometabolic reprogramming of mac
