Multi-omics analysis reveals the mechanism of verbenalin in treating gout via modulating purine metabolism, gut microbiota, and inflammatory pathways.

BACKGROUND: Gout is a prevalent metabolic disorder characterized by hyperuricemia and inflammation. Verbenalin, an iridoid glycoside from Verbena officinalis, possesses anti-inflammatory properties; however, its therapeutic potential and underlying mechanisms in gout remain underexplored. OBJECTIVE: This study aimed to evaluate the pharmacological effects and elucidate the molecular mechanisms of verbenalin in a rat model of gout. METHODS: Hyperuricemia and acute gouty arthritis were induced in rats using potassium oxonate/hypoxanthine and monosodium urate, respectively. Verbenalin was admin

Design, synthesis, and biological evaluation of pyridine-triazole derivatives as URAT1 inhibitors with potential application in hyperuricemia and gout.

Hyperuricemia is a major risk factor for gout and is closely associated with metabolic and cardiovascular disorders. Urate-lowering drugs targeting the urate transporter 1 (URAT1) represent an effective therapeutic strategy; however, the clinical application of benzbromarone is limited by its hepatorenal toxicity. In this study, a series of novel pyridine-triazole derivatives were designed through a scaffold-hopping strategy based on benzbromarone, aiming to improve safety while retaining urate-lowering efficacy. Among the derivatives, analog HYJ-2 showed marginally stronger inhibition of URAT

Integrated multi-omics mapping of the causal landscape of gout across the circulating-tissue axis.

BACKGROUND: Gout is a prevalent inflammatory arthropathy driven by monosodium urate crystal deposition, yet the causal relationships between circulating biomarkers and disease susceptibility remain incompletely characterized. Establishing robust causal associations and mapping them to specific effector genes and tissues is essential for identifying mechanistically informed therapeutic targets. METHODS: We conducted a comprehensive multi-omics Mendelian randomization study integrating a meta-analysis of three large-scale gout genome-wide association studies (N = 1,538,494) with genome-wide dat

Soluble uric acid enhances gouty inflammation via activating gene expression related to innate immune responses, M1 macrophage polarization and urate crystal phagocytosis in mouse macrophage-like cells: novel insights into its mechanisms from RNA sequencing.

Although elevated soluble uric acid (SUA) levels enhance the production of IL-1β in macrophages stimulated with monosodium urate (MSU) crystals, the underlying mechanism remains unelucidated. The aim of this study was to examine the effects of SUA on inflammatory response-related gene expression in lipopolysaccharide-primed and MSU crystal-stimulated macrophages using mouse macrophage-like J774.1 cells. Differential gene expression in SUA-pretreated and untreated group cells was analyzed by RNA sequencing and quantitative reverse transcription-polymerase chain reaction. SUA upregulated the gen

Reduced Serum Urate Concentrations Despite Unchanged Allopurinol Dosing in Gout Patients Commenced on Dialysis: A Retrospective Chart Review.

AIM: People with kidney failure are at increased risk of gout-related hospitalisations and mortality. Whilst dialysis enhances urate removal and reduces circulating oxypurinol (allopurinol's active metabolite), its effect on achieving target serum urate concentrations (< 0.36 mmol/L) and implications for gout management remain unclear. This study aimed to evaluate serum urate concentrations and allopurinol dosing following commencement of dialysis. METHODS: We conducted a retrospective chart review of adults prescribed allopurinol at a dialysis Centre in New South Wales, Australia, between 01

Spatiotemporal immune gradients in gout: immune response-driven activation of the NLRP3-IL-1β axis and its transition to trained immunity.

Gout is a crystal-associated autoinflammatory disease triggered by monosodium urate (MSU) crystals, clinically characterized by recurrent transitions between acute inflammatory flares dominated by innate immunity and a state of "trained immunity" during the remission phase. However, previous studies have mostly focused on single time points or local lesions. Such approaches fail to systematically explain the recurrent nature of acute gout flares and the mechanisms underlying multi-system involvement. By integrating evidence from single-cell and spatial transcriptomics as well as mechanistic in

Dendrophenol modulates the LRRK1/PIK3AP1 signaling axis: a novel strategy to mitigate inflammation and pyroptosis in gouty arthritis.

BACKGROUND: Gouty arthritis (GA) is caused by hyperuricemia and the articular deposition of urate crystals, for which current treatments remain suboptimal. Considering the anti-inflammatory properties of Dendrobium extracts, especially Dendrophenol (Den), this study conducted an in-depth investigation into its effects on GA. METHODS: Through methylthiazolyldiphenyl-tetrazolium bromide (MTT) test, the impact of Den on bone marrow-derived primary macrophages (BMDMs) was assessed to ascertain its non-toxic dosage range. The GA cell model was established, subjected to Den (0.5 µM) treatment, and

Is Gout an Autoinflammatory Disease?

PURPOSE OF REVIEW: This review recognizes gout as an autoinflammatory disease rather than a solely metabolic condition. It provides an overview of autoinflammatory diseases, the NOD-, LRR, and pyrin domain-containing protein 3 (NLRP3) inflammasome, and how soluble and crystalline forms of uric acid (UA) act as inflammatory triggers, prompting NLRP3 inflammasome formation and activation. It also highlights the genetics associated with the autoinflammatory features of gout and discusses NLRP3 inflammasome-targeted treatments. RECENT FINDINGS: Autoinflammatory diseases result from hyperactivity o

Global trends in age at onset of gout, 1990-2023: a sex-stratified and forecast analysis of the Global Burden of Disease Study 2023.

BACKGROUND: Global gout burden is rising, yet temporal shifts in age at onset remain insufficiently characterized. Understanding secular changes in incident age distribution is essential for anticipating healthcare demands in aging populations. METHODS: We conducted a secondary analysis of the Global Burden of Disease Study 2023 data to evaluate global incident gout trends from 1990 to 2023. Mean age at onset, proportion of young-onset cases (< 40 years), and sex-stratified patterns were analyzed using log-linear regression. Forecasting through 2040 was performed using time-series modeling.

A treat-to-target strategy versus symptom-driven management of gout in the Netherlands (GO TEST Overture): a multicentre, open-label, pragmatic, superiority, randomised controlled trial.

BACKGROUND: A treat-to-target approach is recommended for gout management, which involves titrating urate-lowering therapy to reach a serum urate target of less than 0·36 mmol/L, but evidence from pragmatic, head-to-head trials comparing a treat-to-target strategy with a symptom-driven approach is scarce. We aimed to address this gap by comparing these two approaches in patients with gout. METHODS: We did a multicentre, open-label, pragmatic, superiority, randomised controlled trial at eight secondary care rheumatology centres in the Netherlands. Participants aged 18 years or older with gout

Integrating crystal engineering with PEG6000 conjugation to formulate advanced solid dispersions of febuxostat for gout management.

OBJECTIVES: Febuxostat (FEB) is a poorly soluble xanthine oxidase inhibitor, which limits its in vitro dissolution performance. This study aimed to enhance the aqueous solubility and dissolution behavior of FEB through crystal engineering (salt and co-crystal formation) and the development of PEG6000-based amorphous solid dispersions. METHODS: Three FEB salts with p-toluenesulfonic acid (Tos1), tromethamine, and N-methyl-D-glucamine, as well as one FEB co-crystal with acridine (Acd1), were prepared together with their amorphous solid dispersions using PEG6000. Solubility, dissolution, and phy

The interplay between NLRP3 inflammasome and metabolic signals in gouty arthritis.

The pathogenesis of gouty arthritis (GA) begins with the deposition of monosodium urate (MSU) crystals in the joints. This crystal deposition triggers a critical inflammatory response by activating the NLRP3 inflammasome, which in turn drives the maturation and release of pro-inflammatory cytokines such as IL-1β. Beyond this well-defined inflammatory axis, metabolic dysregulation is increasingly recognized as a core component of GA pathogenesis. This paper systematically reviews the crosstalk between metabolic signaling and the NLRP3 inflammasome in GA, elucidating how MSU crystals serve as a

Case Report of Gouty Arthritis Complicated by Septic Arthritis.

Gouty knee arthritis shares numerous clinical similarities with septic knee arthritis. Core overlapping manifestations include sudden onset of unilateral knee symptoms such as redness, swelling, heat, and pain, accompanied by significant functional impairment and restricted mobility. Inflammatory markers like erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are often markedly elevated in both conditions. Early imaging studies typically fail to reveal bone destruction. Due to highly overlapping clinical presentations, misdiagnosis between the two conditions is common, often req
