Indobufen versus aspirin in dual antiplatelet therapy: a retrospective study of effects on serum uric acid levels and gout attacks in patients post-coronary stenting.

OBJECTIVE: This retrospective study compared the effects of indobufen versus aspirin, each combined with clopidogrel, on serum uric acid (SUA) levels and gout attacks in patients undergoing coronary stenting. METHODS: Among 213 enrolled patients, 156 were included after propensity score matching and assigned to either the indobufen plus clopidogrel group (n = 78) or the aspirin plus clopidogrel group (n = 78). SUA levels were measured at baseline and at the 1-month, 6-month, and 1-year follow-ups. Gout attacks and major cardiovascular events were recorded throughout the study. RESULTS: Basel

CA72-4 derived from synovial cells inhibits monosodium urate-induced macrophage M1 polarization by activating the TIGIT/SHP-1 axis through binding to Siglec-15.

Excessive M1 polarization of macrophages is a key driver of inflammatory processes in gout, while the regulatory mechanisms involved remain unclear. Herein, the role of carbohydrate antigen 72-4 (CA72-4), derived from human synovial cells (HFLS), in regulating macrophage M1 polarization during gout progression was investigated. PMA was used to induce differentiation of THP-1 cells into macrophages, and the macrophages were then treated with a conventional medium (CM), the CM was from Non-treated HFLS (NCM), or the CM from MSU-treated HFLS (MCM), and then incubated with MSU treatment for 24 h.

HIF-1α in Gout: A central regulator of metabolism, inflammation, and environmental stress.

Gout is a chronic inflammatory disease characterized by the deposition of monosodium urate crystals in the joints. However, growing research suggests a more complex pathophysiology involving genetic susceptibility, metabolic stress, and environmental factors. Among emerging contributors, hypoxia-inducible factor 1 alpha (HIF-1α) has been proposed as a potential regulator that links these processes, though its role in gout remains underexplored. This review examines the emerging evidence supporting HIF-1α's role in both urate production and inflammation, to offer a unifying framework connecting

Effects of Causal Beliefs About Gout on Public Views About Gout Management: A Randomized Experimental Study.

OBJECTIVE: Despite well-described biologic causes, the public views unhealthy lifestyle choices as the primary cause of gout. This study investigated how causal beliefs about gout influence the public's views about its management. METHODS: In a randomized study, 201 members of the public viewed one of two 2-minute videos about gout. The videos were identical except for a 45-second section describing the causes of gout using either a biologic explanation (including genetics, kidney disease, age, sex, and diuretic therapy) or a lifestyle explanation (including diet, alcohol, sugary drinks, and

Hyperthyroidism drives gout risk: A Mendelian randomization observational study.

The causal interplay between thyroid dysfunction and gout remains controversial because of confounding and reverse causation in observational studies. Using Mendelian randomization (MR), this study investigated the causal effects of hyperthyroidism and hypothyroidism on the risk of gout. Summary statistics from the UK Biobank and FinnGen genome-wide association study databases were analyzed using 2-sample MR. Genetic instruments for thyroid dysfunction were selected under stringent criteria (P < 5 × 10-8, r2 < 0.001). Causal estimates were derived using inverse-variance weighted regression, su

IL-38 reprograms macrophage metabolism to restrain NLRP3 inflammasome assembly and attenuate monosodium urate crystals induced inflammation.

OBJECTIVE: Although interleukin (IL)-38 is recognized as an anti-inflammatory cytokine, its precise mechanism in monosodium urate (MSU) crystals induced inflammation remains undefined, and its impact on macrophages metabolism during MSU crystals induced inflammation has not been explored. METHOD: RAW264.7 cells, THP-1 cells and mouse peritoneal macrophages were stimulated with MSU crystals with or without recombinant IL-38. Glucose uptake, Glucose Transporter Type 1 (GLUT1) expression, Reactive Oxygen Species (ROS) levels and NOD-like receptor family, pyrin domain-containing protein 3 (NLRP3)

Cluster-defined gout subtypes exhibit divergent chronic kidney disease trajectories: a machine learning approach in a Chinese prospective cohort.

OBJECTIVE: To systematically evaluate the contribution of clinical heterogeneity to chronic kidney disease (CKD) progression in gout patients using data-driven phenotyping, and to assess whether incorporating genetic risk improves prediction of renal outcomes. METHODS: In this prospective cohort study, 1497 Chinese gout patients were enrolled and followed for CKD progression. K-means clustering was applied to four core clinical variables: serum urate (SU), fractional excretion of uric acid (FEUA) (i.e. high- and low-excretion), gout duration and kidney stone burden. The primary outcome was th

DMARDs and gout therapy in patients with chronic arthritis and end-stage renal disease: a descriptive guide.

OBJECTIVES: The coexistence of chronic arthritis - here defined as rheumatoid arthritis, spondyloarthritis and gout - and end-stage renal disease (ESRD) is clinically relevant, underscoring the need for safe medication choices and dosage adjustments. However, drug use in dialysis patients is challenging, as it requires balancing treatment efficacy - potentially reduced by drug removal during dialysis - with the risks associated with impaired renal function, such as overdose and drug toxicity. In the absence of evidence-based guidelines, this paper aims to summarize the existing literature and

Accuracy and readability of LLM-generated responses for gout management: a benchmark study based on ACR guidelines.

BACKGROUND: Gout and hyperuricemia, linked to purine metabolism abnormalities or impaired uric acid excretion, are rising with lifestyle changes. Effective self-management and health literacy are crucial for gout management. While large language models (LLMs) show promise in enhancing health management, their potential on gout patient education remains underexplored. This study aimed to evaluate the accuracy and readability of responses generated by three LLMs, including DeepSeek-V3, DeepSeek-R1, and GPT-5, to questions based on the gout and hyperuricemia guidelines published by the American C

Discovery of C2: A Novel Lead Compound for the Treatment of Gout and Hyperuricemia via Multi-Pathway Inhibition.

Gout and hyperuricemia, prevalent metabolic disorders, are driven by elevated serum uric acid (UA) levels and subsequent monosodium urate crystal deposition, which provoke inflammatory responses and joint damage. Current therapeutic options remain unsatisfactory, underscoring the urgent need for novel agents with improved efficacy and safety profiles. In this study, we aimed to discover a new lead compound capable of concurrently addressing both hyperuricemia and associated inflammatory pain. Two novel compounds were designed and synthesized, among which C2 was identified as a promising candid

Firsekibart for steroid withdrawal in glucocorticoid-dependent refractory gout: a case report.

Glucocorticoid dependence presents a major therapeutic hurdle in refractory gout, often preventing successful steroid tapering and perpetuating a cycle of recurrent flares. We describe a 53-year-old male with severe tophaceous gout and glucocorticoid dependence, experiencing frequent flares upon reducing methylprednisolone below 16 mg/day despite urate-lowering therapy (ULT). After conventional anti-inflammatory agents proved inadequate, a single 200-mg subcutaneous dose of firsekibart (formerly known as genakumab), an anti-interleukin-1β (IL-1β) monoclonal antibody, was administered. Followin

Identification of the polyphenols from Lonicera flos-Lonicera Caulis compatibility in gut content and their implication in ameliorating gouty arthritis.

The combination of Lonicerae flos and Lonicerae Caulis, a traditional medicinal pair, is commonly used in folk medicine for the treatment of inflammatory conditions such as gouty arthritis (GA). However, their combined polyphenols (FP-CP) against GA have not yet been explored, the specific components responsible for its efficacy remain to be identified. This study unveils the protective mechanism of FP-CP against inflammatory resorption by regulating gut microbiota and metabolites in GA rat. Using a GA rat model and UPLC-MS/MS metabolomic analysis of intestinal contents, we identified seven pr

Exploring the inflammatory origins of gouty arthritis: mechanistic studies based on local lesion proteomics and validation.

BACKGROUND: The exact pathogenesis of gouty arthritis (GA) is unclear. However, it is believed that GA is characterized by recurrent episodes and a self-limiting nature. During episodes, the disease primarily manifests locally, making local omics research particularly relevant. OBJECTIVE: Based on the local proteomic analysis of GA patients during the chronic phase (with tophi) and acute phase (with joint cavity effusion), this study systematically screened for inflammation-related differentially expressed proteins to provide specific biomarkers and potential intervention targets for disease

Electroacupuncture alleviates acute gouty arthritis by inhibiting NLRP3 inflammasome activation via modulation of the circadian-inflammation axis.

Electroacupuncture (EA) has shown anti-inflammatory potential in gouty arthritis (GA). This study aimed to investigate its therapeutic effects and underlying mechanisms. Core acupoints (ST36 and SP6) were identified through data-mining. In MSU-induced GA rats, EA significantly alleviated joint swelling, improved gait, reduced serum TNF-α, IL-6 and IL-1β, and suppressed synovial inflammatory infiltration. Mechanistically, EA downregulated NLRP3, caspase-1 and IL-1β in joint tissue. Further analyses revealed that EA modulates sciatic nerve signaling, influences the central circadian clock in the

Rheumatology: What You May Have Missed in 2025.

Internal medicine physicians are often the first physicians from whom patients with rheumatic conditions seek guidance, and they collaborate with rheumatologists when caring for these patients. Here, we summarize 6 studies published in the rheumatology literature in 2025 of relevance to internal medicine specialists and subspecialists who are not rheumatologists. The first study concerns the treat-to-target urate strategy for gout. The second study compared the benefits of a very-low-calorie diet and exercise compared with exercise alone in patients with hip osteoarthritis. The investigators o
