Gout: a practical approach to diagnosis and management in low- and middle-income countries and resource-limited settings.

The prevalence of gout is increasing worldwide, with rapid rises reported in low- and middle-income countries (LMICs), particularly in parts of Asia, Latin America, and Africa. Several international organizations have published guidelines for optimal gout management, most of which are from developed countries with high-income settings. Therefore, implementing these recommendations in LMICs and other resource-limited settings (RLS) is often difficult due to several barriers. These include a shortage of rheumatologists and other relevant specialists, workforce constraints, limited access to adva

Multidimensional therapeutic advantages of Smilax glabra (Tufuling)-containing formulae in gout: an integrated Systematic Review and network pharmacology-based prediction.

BACKGROUND: Gout, an inflammatory form of arthritis triggered by monosodium urate (MSU) crystal deposition, poses a substantial global health burden with increasing prevalence and younger onset, particularly in China. In traditional Chinese medicine (TCM), dampness-heat accumulation is a predominant pattern associated with gout. Smilax glabra (Tufuling), a medicinal and edible herb with a history of use for detoxification and elimination of dampness, is also known to promote joint mobility. It is widely used for these purposes. This study aimed to systematically evaluate the clinical efficacy

Stigma, lost autonomy, reclaimed identity, and uncertainty around safe movement: a qualitative interview study of physical activity in gout.

To explore patients' experiences and perceptions of physical activity in the context of gout and gout flares. Interpretive description informed the design of this qualitative study. Individual interviews were conducted with 25 people with gout. An interview guide was used to lead discussions, focusing on participants' experiences of gout flares, physical activity engagement, and the perceived impact of flares on activity levels. Key questions were designed to capture culturally specific understandings and experiences related to gout and physical activity. Interviews were audio-recorded and tra

Ultrasound-activated piezocatalysis based on Bi4Ti3O12 for the wireless degradation of sodium urate: a new paradigm for gout treatment.

As a widespread inflammatory disorder, gout is characterized by the excessive deposition of monosodium urate (MSU) crystals. Despite substantial advances in clinical management, existing therapeutic approaches remain limited by the need for invasive procedures and delayed treatment efficacy. Here, leveraging the use of piezoelectric Bi4Ti3O12, we develop an ultrasound-driven piezocatalytic strategy for the wireless and rapid degradation of MSU. Several Bi4Ti3O12 samples are synthesized by varying the hydrothermal synthesis temperature, and the Bi4Ti3O12 sample obtained at 220 °C exhibits an op

An engineered sweeper with multi-stage intervention against uric acid, crystals, and inflammation for gouty arthritis therapy.

Gouty arthritis (GA) is an inflammatory joint disease driven by hyperuricemia and deposition of monosodium urate (MSU) crystals. Current therapeutic strategies primarily focus on the intervention of serum uric acid (UA) or inflammation, while neglecting the crucial process of MSU crystal formation. To address this gap, we employed a top-down therapeutic approach and developed a multifunctional "sweeper" termed UCCN. It synergistically reduced UA level while avoiding H2O2 accumulation through the degradable synergy of hollow cerium oxide nanostructures (hCeO2) and encapsulated urate oxidase. Mo

Investigating the causal role of smoking in gout: A triangulation approach combining NHANES data, genetic correlation, and Mendelian randomization.

The relationship between smoking and the development of gout is not well understood. To address this, we adopted a triangulation framework that integrates observational analysis, genetic correlation estimation, and two-sample Mendelian randomization (MR) to examine whether smoking confers a causal risk for gout. We first performed a cross-sectional analysis using information for 13,626 participants from the National Health and Nutrition Examination Survey between 2013 and 2018. The association of smoking with gout was subsequently assessed through logistic regression models. We next investigat

Causal determinants of gout in 614,000 adults: A two-sample Mendelian randomization study of dietary, lifestyle, and metabolic traits.

Gout affects over 55 million people worldwide, with prevalence projected to rise by 70% by 2050. Although observational studies have implicated several dietary, lifestyle, and metabolic risk factors, causal relationships remain uncertain because of confounding and reverse causation. Univariable and multivariable two-sample Mendelian randomization (MR) analyses were performed using genome-wide association data from 2 European cohorts: UK Biobank (6543 self-reported gout cases and 456,390 controls) and FinnGen (3576 cases and 147,221 controls). Genetic instruments for 12 exposures, including dri

mFABIO: An integrative multi-tissue TWAS fine-mapping approach to prioritize potentially causal genes and tissues underlying binary traits.

Recent advances in transcriptome-wide association study (TWAS) fine-mapping have enabled the joint modeling of multiple genes to improve causal gene prioritization. However, existing methods have been developed primarily for quantitative traits and most of them rely on gene expression data from a single tissue. Here, we present mFABIO, a multi-tissue TWAS fine-mapping method specifically designed for binary traits. mFABIO employs a probit model to directly link genetically regulated expression (GReX) of genes within a locus across multiple tissues to a binary outcome, while accounting for corr

Bempedoic acid and increased serum uric acid levels: a matter of concern?

AIMS: Several observational and genetic studies have found an association between high urate serum levels and hypertension, heart failure, and cardiovascular mortality. Bempedoic acid is a recently approved hypocholesterolemic agent acting by inhibiting the ATP citrate lyase and thus the hepatic cholesterol biosynthesis. Despite its optimal safety profile, a minor and reversible increase of serum uric acid levels has been observed. This effect is due to the inhibition by bempedoic acid of organic anion transporters 2 and 3 (OAT2 and OAT3) which mediate the renal excretion of urate. DATA SYNTH

[Gout of the manubriosternal joints: A case report].

Gout is a metabolic disorder caused by abnormalities in purine metabolism and/or impaired uric acid excretion. It is characterized by the deposition of monosodium urate crystals in joints or connective tissues, leading to recurrent episodes of acute arthritis. Gout can be categorized into two major types based on its etiology: primary and secondary. The majority of gout cases are associated with impaired uric acid excretion. The natural course of gout can be divided into three stages: (1) asymptomatic phase; (2) acute arthritis phase and intercritical phase; (3) chronic gouty arthritis and top

Targeting of NLRP3 Gln624/Ser658 by Carabrone attenuates inflammatory diseases.

The NOD-like receptor family pyrin domain-containing protein 3 (NLRP3) inflammasome plays a crucial role in host defense; however, its aberrant activation leads to excessive release of pro-inflammatory cytokines, triggering inflammatory responses and tissue damage in human diseases. In this study, the inhibitory effect and anti-inflammatory potential of carabrone on the NLRP3 inflammasome were systematically evaluated. Carabrone suppressed lipopolysaccharide (LPS) + ATP/Nigericin-induced IL-1β secretion, Caspase-1 activation, and apoptosis-associated speck-like protein (ASC) speck formation. M

Increased Cerebrospinal Fluid Adenosine Deaminase Activities in a Gouty Patient With Varicella-Zoster Virus Meningitis.

AIMS/BACKGROUND: Increased adenosine deaminase (ADA) activity in cerebrospinal fluid (CSF) has been used as a biomarker of tuberculous meningitis and other etiologies, including viral, fungal, and neoplastic meningitis. Gout can influence inflammatory pathways, altering biochemical markers such as ADA. This report describes a unique case of varicella-zoster virus (VZV) meningitis with gout associated with elevated ADA levels. CASE PRESENTATION: A 37-year-old man presented with a one-week history of headache, fever, and chills. His medical history included gout, diagnosed three years earlier.

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

[Immobilizing back pain in an elderly patient: Differential diagnostic considerations and diagnosis].

A 77-year-old patient presents with acute back pain. Due to elevated CRP and other lab abnormalities, bacterial spondylodiscitis is initially suspected, and blood cultures are obtained early. However, a biopsy reveals uric acid crystals and cultures remain negative, leading to the diagnosis of gout spondylodiscitis. Treatment includes renal-adjusted anti-inflammatory therapy, urate-lowering therapy and flare prophylaxis.

CHCHD2 as a prognostic biomarker and therapeutic target in lung adenocarcinoma: Insights from pan‑cancer multi‑omics and functional studies.

Helminths include both parasitic nematodes (roundworms) and platyhelminths (trematode and cestode flatworms) that are abundant, and are of clinical importance. The genetic characterization of parasitic flatworms using advanced molecular tools is central to the diagnosis and control of infections. Although the nuclear genome houses suitable genetic markers (e.g., in ribosomal (r) DNA) for species identification and molecular characterization, the mitochondrial (mt) genome consistently provides a rich source of novel markers for informative systematics and epidemiological studies. Prognostic factors for colchicine prophylaxis-related adverse events when initiating allopurinol for gout: retrospective cohort study.

OBJECTIVES: Colchicine is commonly used to prevent flares when starting urate-lowering therapy for gout. Patients with gout are frequently concurrently prescribed other medications (such as statins) that may interact with colchicine, increasing the risk of adverse events. The aim of this study was to describe potential prognostic factors for adverse events in patients prescribed colchicine when initiating allopurinol. METHODS: We conducted a retrospective cohort study in linked UK Clinical Practice Research Datalink and Hospital Episode Statistics datasets. Adults initiating allopurinol for g

An objective diagnosis of gout and calcium pyrophosphate deposition disease with machine learning of Raman spectra acquired in a point-of-care setting.

OBJECTIVE: Raman spectroscopy is proposed as a next-generation method for the identification of monosodium urate (MSU) and calcium pyrophosphate (CPP) crystals in synovial fluid. As the interpretation of Raman spectra requires specific expertise, the method is not directly applicable for clinicians. We developed an approach to demonstrate that the identification process can be automated with the use of machine learning techniques. The developed system is tested in a point-of-care-setting at our outpatient rheumatology department. METHODS: We collected synovial fluid samples from 446 patients

Target Serum Urate Achievement and Chronic Kidney Disease Progression in Patients With Gout and Kidney Disease.

IMPORTANCE: Clinicians often approach urate-lowering therapy (ULT) cautiously in patients with gout and impaired kidney function because they are concerned about the risk of progression to severe or end-stage kidney disease. However, evidence from randomized clinical trials of this association remains inconclusive. OBJECTIVE: To evaluate the association between achieving target serum urate level with ULT and progression of chronic kidney disease (CKD) to severe or end-stage in patients with gout and impaired kidney function.
