OBJECTIVE: To compare the performance of remission definitions for gout in an observational treat-to-target patient cohort with 5 years of follow-up.
METHODS: Inclusion criteria were crystal proven gout with increased serum urate levels and a flare. Remission was determined according to the 2016 preliminary gout remission definition, a modified preliminary definition with more lenient thresholds for the individual variables pain due to gout and patient global assessment of gout disease activity, and the simplified definition without patient reported outcomes. Linear mixed models were used to

Gout is characterized by deposition of monosodium urate (MSU) crystals in or around joints, tendons, bursae, and other tissues, resulting in painful recurrent flares and tissue damage. Gout is the most common form of inflammatory arthritis, with a prevalence of 5.1% in the United States, affecting 12.1 million adults. When urate levels exceed the limit of solubility (6.8 mg/dL [400 μmol/L]), MSU crystals may form or grow. Gout flares are the result of inflammatory responses to MSU crystals. The primary method to prevent and reduce gout flares, tophi, chronic inflammatory arthritis, and joint d

Elevated serum uric acid levels are the essential pathophysiology of gout. Although gout rarely develops in childhood, chronic persistent hyperuricemia can induce precipitation and deposition of sodium urate crystals, leading to the development of gout. Hyperuricemia is caused by increased uric acid production and/or decreased uric acid excretion capacity of the kidneys and/or intestinal tract. Increased production of uric acid, the final metabolite of purine, is associated with an increase of phosphoribosyl pyrophosphate, the key compound in the purine synthesis pathways, as observed in hypox

PURPOSE OF REVIEW: The co-existence of gout and psoriatic disease (PD) is long standing but more recently frequently encountered in clinical settings due to increased awareness of their shared comorbidities and clinical phenotypes, often posing diagnostic and management challenges. Here we review the overlap in gout and PD focusing on shared clinical features, common inflammatory pathophysiology and comorbidities which may prompt a diagnosis of 'Psout' and lead to changes in management.
RECENT FINDINGS: Several epidemiological studies have highlighted the increased incidence of hyperuricemia

Gout is a common inflammatory arthritis with well-described imaging characteristics. Radiography depicts erosive change and in advanced cases, tophus deposition near joints and in association with tendons or bursae. Computed tomography demonstrates the same features but may also use dual-energy or photon-counting techniques that allow for tissue composition analysis and the specific identification of monosodium urate deposition. Magnetic resonance imaging (MRI) is useful in identifying tophi and the damage associated with gout, such as bone erosion and cartilage loss in advanced cases. MRI als

Gout is one of the most prevalent forms of arthritis that is usually accompanied by other comorbidities. For this reason, multiple drugs are routinely prescribed for gout patients, which may affect the clinical course outcomes, and increase the risk of drug-drug interactions. This work presents a novel, simple, and sensitive high performance liquid chromatography (HPLC) method for the simultaneous determination of lesinurad (LES) and other co-administered drugs that are subject to potential pharmacokinetic interactions such as etoricoxib (ETC), eplerenone (EPL), and amiodarone (AMD) in rat pla

Febuxostat, a selective xanthine oxidase inhibitor, is regarded as the first-line therapeutic option for gout and hyperuricemia. With its extensive application, there has been a growing number of reports regarding associated adverse reactions. In this case, a 71-year-old male patient, who had been under long-term febuxostat treatment for gout, was admitted to the hospital approximately one year later due to persistent upper abdominal pain. Abdominal CT imaging revealed thickening of the gastric antrum wall, accompanied by peripheral exudation and the presence of free gas in the abdominal cavit

Hyperuricemia and monosodium urate induced nod-like receptor family, pyrin domain-containing 3 (NLRP3) inflammasome activation is the major pathogenesis for gouty arthritis, and urate transporter 1 (URAT1) is a proven target for hyperuricemia. In this study, scaffold hopping modification with tranilast led to the identification of HNW005, an NLRP3 inflammasome and URAT1 dual-target inhibitor, which exhibited notable inhibitory potency against NLRP3 inflammasome activation (KD = 204.6 nM, IC50 = 1.7 μM) and uric acid transmembrane transportation (IC50 = 6.4 μM). Importantly, HNW005 displayed si

Despite significant advancements in disease diagnostics, the development of portable, highly selective, and low-cost electrochemical sensors for real-time, noninvasive detection of chronic kidney disease (CKD) and gout biomarkers remains a challenge. In this work, we demonstrate an inexpensive CKD and gout diagnostic platform with the incorporation of biomimetic polymer matrix nanocomposites based on creatinine (CRE) and uric acid (UA) imprinted conducting polythiophene (MIP) and graphitic carbon nitride nanotubes (NTs). These nanocomposites are termed CRE-MIPNT or UA-MIPNT, depending on the t

We report on a woman with palmoplantar whitish-yellowish aggregated, partly ulcerated papules on a slightly erythematous base. Histological examination showed pale amorphous nodular deposits in the dermis. On radiological examination, calcifications could be ruled out. Laboratory tests showed marked hyperuricemia and chronic renal insufficiency. We diagnosed miliary gout (MG) and initiated uric acid-reducing therapy with allopurinol 100 mg/day. First described in 2007, MG presenting with multiple whitish-yellowish papules is a rare intradermal variant of classic gout that is characterized by s

BACKGROUND: Gout is a metabolic disorder caused by the deposition of urate crystals in the joints and surrounding tissues. Prolonged hyperuricemia leads to the formation and accumulation of urate crystals, often resulting in acute and chronic joint damage. Spinal gout is a rare type caused by the deposition of urate crystals in spinal tissues. It leads to local inflammation, pain, and, in severe cases, neurological symptoms. The clinical presentation of spinal gout is varied and lacks specificity, making it easily confused with other spinal disorders, especially in elderly patients.

Gout is the commonest form of inflammatory arthritis. Flares are the commonest presentation of gout. Typically, gout flares present as acute monoarthritis, and most often affect the first metatarsophalangeal joint ('podagra'). Other joints in the lower limbs are affected more often than those in the upper limbs. Joint aspiration followed by examination of the synovial fluid using a polarised light microscope is the gold standard for a definite diagnosis of gout. Gout may be diagnosed without recourse to joint aspiration if there is podagra, elevated serum urate and no suspicion of infection. U

INTRODUCTION: Proton pump inhibitors (PPIs) used for gastrointestinal-related disorders are associated with increased insulin resistance, a risk factor for worsening hyperuricemia. However, lansoprazole has shown potential in reducing insulin resistance by increasing the expression of peroxisome proliferator-activated receptor gamma and CCAAT/enhancer-binding protein alpha mRNA in adipogenesis. This study aims to investigate the effects of lansoprazole compared to other PPIs in reducing the risk of gout.
METHOD: We conducted a retrospective cohort study on patients using lansoprazole from 200
