As life expectancy increases, the older adult population is expected to grow significantly. As a result, gout cases continue to escalate in both incidence and prevalence. Gout is a chronic inflammatory condition that disproportionately affects older adults, certain racial and ethnic groups, and underserved populations. Risk factors for gout include renal impairment, genetics, select medications, obesity, and lifestyle factors. It is also associated with numerous comorbidities, such as cardiovascular disease, metabolic syndromes, chronic kidney disease (CKD), depression, and anxiety. This artic

We present the case of a male in his early 70s who died after receiving adjuvant chemotherapy, probably due to an undiagnosed dihydropyrimidine dehydrogenase (DPYD) deficiency. Despite normal results from standard DPYD genotyping prior to treatment, he developed severe gastrointestinal toxicity that progressed to bowel ischaemia and ultimately death. This case underscores the potential limitations of current genotyping approaches, which may not detect rare or functionally significant DPYD variants. It also highlights the need for a cautious and comprehensive approach to fluoropyrimidine therap

The management of gout, a condition historically laden with stigma and misconceptions about its underlying causes, stands at a critical juncture. Despite significant advances in understanding its pathophysiology and treatment, gout management remains inconsistent and at odds with prevailing clinical guidelines and evidenced-based standards. This study investigates how inaccurate physician beliefs about gout can bias clinical judgment and contribute to compromised patient care. Across two independent samples (Pilot Study, N = 106; Main Study, N = 321), we examined how practicing rheumatologists

BACKGROUND: Allopurinol, the most prescribed urate-lowering drug, is a known cause of severe cutaneous adverse reactions. We aimed to develop and validate a model to assess the risk of allopurinol-induced severe cutaneous adverse reactions in adults newly prescribed allopurinol.
METHODS: In this retrospective new-user cohort study, we developed and validated a prognostic model using primary care, hospitalisation, and mortality data extracted from the UK Clinical Practice Research Datalink (CPRD) primary care database, for the period Jan 1, 2001, to March 29, 2021. Data from CPRD Aurum was use

BACKGROUND: A plethora of studies have demonstrated that the level of uric acid (UA) and gout are the risk factors for erectile dysfunction (ED). However, the causal effect of UA level and gout on ED is still unclear. This Mendelian randomization (MR) study aims to examine the bidirectional causality between ED and UA levels as well as gout.
METHODS: We performed a bidirectional MR analysis using summary statistics from genome-wide association studies (GWAS) to investigate the causal association between ED and UA levels as well as gout. We meticulously selected single nucleotide polymorphisms

Gout is the oldest recorded form of inflammatory arthritis to affect humankind, with roots stretching back to 2640 BC and known in Greece by 1700 BCE. It is due to deposition of sodium monourate driven by hyperuricemia. The association of humours with causation stems from Hippocrates (460-370 BCE). More specifically, a toxic humour was suspected by Celsus (25 BCE-50 CE) and Rufus of Ephesus (98-138 CE), and confirmed by Alfred Garrod in 1849. Its therapy has been based on colchicine since Severus Iatrosophista, Theodosius the Philosopher, and Jacobus Psychrestos, introducing Colchicum as an in

Fluoropyrimidine derivatives can cause severe toxicity in patients with DPD deficiency. Regulatory agencies, such as the European Medicines Agency (EMA), recommend pre-emptive genotyping of the HapB3 haplotype, along with other variants. Historically, the two main HapB3 variants, the benign c.1236G>A and the pathogenic c.1129-5923C>G, have been assumed to be in complete linkage disequilibrium. Recent findings contradict this assumption, questioning the reliability of the HapB3 analysis through c.1236G>A, which could directly impact patient safety. The aim of this study is to assess the linkage

INTRODUCTION: Monosodium urate (MSU) crystals are the primary cause of gout, however the knowledge on MSU crystals exposure and inflammatory changes found in immune cells is diverse, with reports ranging from very limited inflammatory effects to substantial reprogramming of the cells induced by MSU crystals alone. We examine the IL-1β production patterns and the transcriptomic signature in response to MSU crystals alone or in combination with TLR ligands in freshly isolated primary human peripheral blood mononuclear cells (PBMCs).
MATERIALS AND METHODS: PBMCs were isolated by density gradient

The treatment of gout remains a huge challenge, and currently, the mainstream Western medicine treatment methods still have some drawbacks. This study aims to elucidate the potential mechanism of modified Simiao decoction (MSMD) in the treatment of gout through network pharmacology analysis, Mendelian randomization (MR), and molecular docking. We hope to contribute to the research on the targets of action of traditional Chinese medicine and the exploration of the mechanism of gout. We employed network pharmacology to screen the potential targets of MSMD in relation to gout. MR was utilized to

Gout, a prevalent rheumatic disorder, arises from purine metabolic dysfunction characterized by elevated uric acid levels, which culminates in hyperuricemia, crystallization of sodium urate, and subsequent deposition of these crystals in joints, synovium, and other tissues or organs. Spontaneous resolution of gout is defined as the self-resolution of acute gouty arthritis observed in some patients within 7-10 days without pharmacological intervention, with no residual sequelae. The mechanistic basis underlying this spontaneous resolution remains incompletely understood, involving a spectrum of

BACKGROUND: CA72-4 is an important tumor marker, crucial for detecting and monitoring gastrointestinal, ovarian, and other cancers. However, colchicine, used for gout and other diseases, has been found to abnormally elevate serum CA72-4 levels. This may lead to misinterpreting CA72-4 elevation as a cancer signal, causing unnecessary anxiety and further tests.
METHODS: In this study, a 75-year-old male patient was selected. He was admitted for dizziness, with a history of gout and recent colchicine use. His CA72-4 level was found to be extremely high at 298.5 IU/mL, far above the normal range

The critical role of the purine nucleotide cycle (PNC) in human health is underscored by the fact that mutations in adenylosuccinate synthetase (ADSS) and adenylosuccinate lyase (ADSL), two of its three enzymes, are associated with severe inborn errors of purine metabolism. We use Caenorhabditis elegans to investigate the biological roles of the PNC and to characterize developmental and behavioral functions of ADSS and ADSL. Here we report that adss-1, which encodes ADSS in C. elegans, is required for fertility and locomotion. adss-1 loss-of-function mutants also have small body size, and thei

OBJECTIVES: Gout is a prevalent condition characterised by high serum urate levels, leading to crystal deposition in the joints. Although many studies have explored the mechanisms underlying gout flares, the dynamics of tophus formation remain unknown. This study provides the first transcriptomic profile of the tophaceous gout joint and investigates immune-stromal cell interactions in the pathogenesis of tophus formation.
METHODS: Single-cell transcriptomic profiling was conducted on 44,221 synovial tissue cells from patients with intercritical gout (without tophi) and tophaceous gout. Spatia

Gout, one of the most prevalent inflammatory arthropathies, arises from hyperuricemia and is increasingly recognized as a condition extending beyond the joints. Hyperuricemia, the core risk factor for gout, is also an independent risk factor for cardiovascular disease (CVD), complications, and mortality. Multiple conditions that predispose to gout and hyperuricemia, including obesity, metabolic syndrome, type 2 diabetes, hypertension, and chronic kidney disease (CKD), also elevate the risk of atherosclerosis, the central contributor to CVD and the leading cause of mortality in Western societie

For the first time, German recommendations for diagnosis and treatment of gout on an interdisciplinary S3 guideline level are available. For the diagnosis of a typical gout attack in a primary care setting, the typical clinical picture, medical history, and an elevated serum uric acid level are usually sufficient; gout might be diagnosed with the aid of the so-called gout calculator. For acute joint inflammations not typical of gout, specialist medical professionals may utilize additional diagnostic procedures such as joint sonography, joint aspiration with microscopic crystal detection, and d

BACKGROUND: Gout, a disease already described in the "Anonymus Parisinus Darembergii sive Fuchsii", one of the two surviving Greek medical manuscripts of the first century CE, is reviewed in an effort to trace the timeline of the knowledge of the disease between the Corpus Hippocraticum and the Renaissance.
METHODS: The treatise exists in four manuscripts of varying lengths: two are located in Paris, one in Vienna, and one in London. The study was conducted using the 1997 Leiden critical edition by Ivan Garofalo, which unifies all four manuscripts ("Anonymi Medici. De Morbis acutis et chronic

Goose gout, a metabolic disorder characterized by systemic urate deposition in visceral organs (e.g., liver, kidneys) and joints of goslings, is strongly associated with goose astrovirus (GAstV) infection and environmental stressors such as high humidity. This disease severely compromises growth performance and survival rates, yet no licensed prophylactic or therapeuti interventions are currently available. Here, we investigated the efficacy of SIA, a Chinese herbal formula comprising Saposhnikovia divaricata, Isatis indigotica and Astragalus membranaceus against GAstV-induced gout in goslings

Gouty arthritis (GA) is an immune disease caused by the deposition of monosodium urate (MSU) crystals. As living standards improve and lifestyles change, GA exhibits a high incidence rate, low cure rate, and multiple complications. However, the pathophysiological mechanism of GA remains incompletely understood, underscoring the importance of identifying effective therapeutic targets. Neutrophil extracellular traps (NETs) are complex network structures composed of DNA, histones, and various granule proteins, possessing antimicrobial and pathogen clearance functions.
