Relation of Colchicine to Total Joint Arthroplasty Among People With Gout in a Population-Based Cohort Study.

OBJECTIVE: To evaluate the relation of colchicine use to the risk of total joint arthroplasty (TJA) in individuals with gout, the most common form of arthritis for which colchicine is prescribed. METHODS: This new-user design, time-stratified, population-based cohort study used a UK primary care database (2000-2021). We identified participants with gout who were newly prescribed colchicine after their gout diagnosis. Each colchicine initiator was propensity score matched with a noninitiator using one-year cohort accrual blocks. The effect of colchicine on the risk of TJA was assessed using Co

Response to Allopurinol and Febuxostat According to the Fractional Excretion of Urate in Men With Gout.

OBJECTIVE: Body mass index (BMI), glomerular filtration rate (GFR), and pretreatment urate levels have been reported to influence the urate-lowering response to allopurinol. We investigated whether the fractional excretion of uric acid (FEUA) also modulates this response and relates to oxypurinol concentrations. We further evaluated its potential influence on febuxostat, not as a direct comparison, but to determine whether the effect of FEUA was specific to allopurinol. METHODS: The data are from n = 1,547 and n = 296 patients starting allopurinol and febuxostat, respectively. The relationshi

Dietary Diversity and Risk of Developing Gout: A Cohort Study of 161,213 Participants.

OBJECTIVE: Various epidemiological studies have shown the effects of certain foods on the risk of developing gout. However, these associations have not been comprehensively confirmed in prospective studies. We aim to investigate the association between dietary diversity and the incidence of gout in a large prospective cohort. METHODS: Using data from 161,213 participants (54.32% female) with a mean age of 56.5 years in a large cohort, we examined the association between the food intake of five major groups (grain, vegetables, fruits, meat, and dairy product) and gout. The diet diversity score

Crystallization and inhibition of monosodium urate monohydrate: advances in mechanistic understanding and perspectives for gout management.

Gout is a severe and prevalent pathological mineralization disorder characterized by hyperuricemia and recurrent arthritis, driven by deposition of urate crystals in joints and surrounding tissues. Clinical management of gout primarily targets inflammation and pain during acute attacks, as well as a long-term urate-lowering therapy. Nevertheless, achieving optimal control of gout remains a significant clinical challenge. As a crystal-induced disease, gout is fundamentally driven by monosodium urate monohydrate (MSUM) crystals, which represent the critical nexus between hyperuricemia and inflam

Mast cell-expressed Mrgprb2/MRGPRX2 mediates gout pain and inflammation via a neuroimmune axis.

Acute severe joint pain is a major symptom in gouty arthritis (GA), and its adequate treatment represents an unmet medical need. Mrgprb2, a specific mast cell receptor, has been implicated in the generation of chronic pain by mobilizing mast cell degranulation, yet its significance in GA pain and joint inflammation is still not well defined. Here, we found that Mrgprb2 was expressed in mouse synovial mast cells. In a murine model of GA, acute blockade or genetic deletion of Mrgprb2 significantly attenuated arthritis pain and hyperexcitability of joint nociceptors with significant reductions in

Renal Biopsy as a Diagnostic Salvage Modality in Gouty Nephropathy Coexisting With Amyloid A Amyloidosis.

Gouty nephropathy is an uncommon renal complication arising from chronic hyperuricemia, and its coexistence with Amyloid A (AA) (secondary) amyloidosis is exceedingly rare. We describe a 60-year-old man with a long-standing history of untreated polyarticular gout who presented with progressive joint pain, nephrotic-range proteinuria, and renal impairment. Laboratory evaluation showed elevated serum uric acid levels and impaired renal function. Renal biopsy revealed urate crystal deposits (tophi) with associated inflammatory changes consistent with gouty nephropathy, along with Congo red-positi
