OBJECTIVES: Dihydropyrimidine dehydrogenase (DPD) phenotyping through uracil and dihydrouracil determination is a well-established approach to identify (partial) DPD deficiencies prior to fluoropyrimidine chemotherapy. However, preanalytical stability has challenged this test for years. This study therefore investigated whether dried blood spots (DBS) can improve preanalytical stability.
METHODS: Uracil, dihydrouracil, and uridine were determined in 6 mm DBS sub-punch extracts by liquid chromatography-tandem mass spectrometry. Paired venous and capillary DBS were collected from 15 healthy vol

BACKGROUND: Gout is one of the most common inflammatory arthritides and represents a growing health burden worldwide. This study compares the disease burden of gout and its attributable risk factors between China and European Union (EU) countries from 1990 to 2023 using data from the Global Burden of Disease Study 2023 (GBD 2023). In addition, the study evaluates potential causal relationships between key risk factors and gout and projects future trends in disease burden.
METHODS: Using GBD 2023 data, we analyzed the epidemiology of gout in China and EU countries. Analyses included descriptiv

Gout and hyperuricemia, caused by high serum uric acid, require safer and more effective treatments due to the toxicity and limited efficacy of current drugs. Dual inhibition of URAT1 and GLUT9 may reduce renal toxicity compared to single-target approaches. Starting from lead compound F-5, we used scaffold hopping and structure-guided design to develop 46 novel polycyclic pyrimidine derivatives. Among these, compound 17 showed potent and balanced inhibition of URAT1 (IC50 = 4.01 μM) and GLUT9 (IC50 = 1.60 μM), greatly improving upon F-5. Additionally, 17 reduced serum uric acid levels by 82.4%

BACKGROUND: Gout is an inflammatory arthritis triggered by the deposition of monosodium urate crystals and characterized by activation of innate immune pathways. Human β-defensin-1 (hBD-1), encoded by the DEFB1 gene, is an antimicrobial peptide with immunomodulatory functions. However, its role in gout-related inflammation remains unclear. This study aimed to investigate the relationship between DEFB1 variants (rs11362 and rs1800972) and circulating hBD-1 levels in gout.
METHODS: A total of 81 individuals were enrolled, including 41 with gout and 40 healthy controls. Serum hBD-1 concentration

BACKGROUND: Although hyperuricemia (HUA) is required for gout development, only a fraction of individuals with elevated serum urate progress to the disease. Metabolomic profiling may help uncover biological mechanisms and improve prediction of gout onset.
METHODS: We conducted a prospective metabolomics analysis among 32,563 HUA individuals in the UK Biobank. Baseline plasma metabolites were measured, and differences between individuals who developed gout (n = 2,856) and those who did not were identified. Cox proportional hazards models with false discovery rate correction assessed metabolite

CASE: A 78-year-old Chinese man presented to our unit with chronic tophaceous gout complicated by osteomyelitis of the left first metatarsophalangeal joint (1MTPJ). The patient underwent multiple debridements, resulting in a critical bone defect. Reconstruction of the first ray with 1MTPJ arthrodesis was performed using a vascularized medial femoral condyle free flap (MFC-FF). At the 1-year follow-up, plain radiographs demonstrated satisfactory bony union and patient-reported outcome measures were optimal.
CONCLUSION: The MFC-FF can be an effective option for reconstruction of small bone defe

OBJECTIVE: Gouty arthritis (GA) is an inflammatory disease caused by abnormal uric acid metabolism, with its pathological mechanism involving inflammatory cell infiltration and abnormal expression of pro-inflammatory factors. Monosodium urate (MSU) crystals activate the NLRP3 inflammasome, promoting the abnormal release of pro-inflammatory cytokines such as interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6), as well as the expression of mediators like monocyte chemoattractant protein-1 (MCP-1) and high mobility group box 1 (HMGB1), thereby amplifying local inflam

Acute gouty arthritis (AGA) is a common inflammatory joint disease characterized by pain resulting from the deposition of monosodium urate (MSU) crystals into joints and surrounding tissues. RGFP966, a selective inhibitor of histone deacetylases 3 (HDAC3), can down-regulate the AIM2 inflammasomes. This study aimed to explore the mechanism of action of RGFP966 in MSU-induced AGA. MSU-induced AGA rats were treated with RGFP966 or colchicine, and physiological and pathological indicators were determined. The rat joint synovial tissues and kidneys pathological damage was observed by hematoxylin an

Gouty nephropathy is a common complication of gout, which is characterized pathologically by the abnormal deposition of urate crystals in the kidneys. This deposition can directly trigger local inflammatory responses and lead to renal tissue damage. Neutrophil extracellular traps (NETs), as a unique neutrophil-derived immune defense mechanism, play a key pro-inflammatory role in gouty nephropathy. Under conditions of long-term hyperuricemia, Monosodium urate (MSU) crystals can activate related inflammatory signaling pathways, promoting inflammatory cell infiltration, the release of pro-inflamm

OBJECTIVE: Pegloticase, an infused pegylated uricase enzyme, rapidly lowers serum urate (SU) in patients with uncontrolled gout. Preinfusion intravenous glucocorticoids (GCs) are typically administered before pegloticase to reduce risks of infusion reactions (IRs); however, they may exacerbate comorbidities in this patient population, and thus minimizing GC exposure is desirable. This study examined preinfusion GC dose reduction and elimination in patients with uncontrolled gout receiving pegloticase and methotrexate (MTX) co-therapy.
METHODS: This retrospective case series of medical data (J

Aims. For a study on the timeline of gout from the Corpus Hippocraticum to the Renaissance, encompassing some 26 authors, we have focussed on a Latin manuscript - "De podagra" - of the Middle Ages (posthumous edition dated to the 7th-8th century AD) of Rufus of Ephesus (98-117 AD), during the reign of Trajan. A multipurpose study has been devised to 1) translate into English and Italian the Medieval Latin treatise of the 7th-8th century AD of Rufus of Ephesus on "De podagra"; 2) define the role of Rufus in the historical timeline of gout from the Corpus Hippocraticum to the Renaissance ; 3) an

Mendelian randomization (MR) method was used to explore the causal relationship between the occurrence of arthritis disease and volume changes in specific cerebral cortical regions. By analyzing the genome-wide association studies summary data, single nucleotide polymorphism data related to arthritis and cerebral cortex volume were selected. Using the inverse variance weighted (IVW) method as the preferred method, MR-Egger, simple mode, weighted median, and weighted mode were used as auxiliary analyses to conduct a 2-sample bidirectional MR analysis. IVW analysis showed that arthritis was posi

Gout is traditionally monitored through serum urate levels (SUA). However, the inflammatory component of the disease may not fully parallel short-term urate fluctuations. We aimed to evaluate longitudinal within-patient dynamics of systemic inflammatory activation and to determine whether inflammation persists beyond clinically apparent flares and may not always reflect contemporaneous SUA. Patients with gout were assessed across three successive outpatient clinic visits. Systemic immune-inflammation index (SII), calculated from the values of the complete blood count, and SUA were measured at

Pacific peoples in Aotearoa New Zealand experience some of the highest rates of gout worldwide. Although understanding gout and urate-lowering therapy is a key component of ensuring uptake and adherence to medication, Pacific communities have identified a lack of resources and tools to support this. The objective of this qualitative study was to co-design education resources for Pacific patients and communities in Aotearoa New Zealand, aiming to increase awareness of gout and of urate-lowering therapy. A Pacific Research Collective, comprising Pacific community members, health professionals, a

BACKGROUND: Renal impairment and gout are mutually exacerbating conditions, yet the global burden of gout attributable to kidney dysfunction remains unquantified. This study examines trends in gout-related disability associated with renal impairment to inform global health strategies.
METHODS: Data were extracted from the global burden of disease (GBD) 2021. The age-standardized disability-adjusted life years (DALYs) rate and estimated annual percentage change (EAPC) were analysed. A Bayesian Age-Period-Cohort (BAPC) model was implemented to project disease burden. Health inequality analyses

The management of hyperuricemia and gout remains constrained by the narrow therapeutic index of current drugs. To improve the druggability of our previously identified URAT1 inhibitor T7, we replaced its carboxylic acid with a sulfonamide group. A total of 40 novel derivatives were synthesized, among which 23 derivatives exhibited robust in vivo serum uric acid-lowering activity. Lead compound 2 (bearing a p-bromobenzenesulfonamide) exhibited potent URAT1 inhibition (IC50 = 0.19 μM) and significantly lowered serum uric acid in hyperuricemic mice (96.8% reduction) and rats (91.9% reduction). Gout is an aseptic inflammatory disease caused by monosodium urate (MSU) crystal deposition. Its clinical signs go beyond the intense pain of acute arthritis to include structural damage such as chronic synovitis, bone erosion, and tophus formation. The long-term progression and recurring episodes of inflammation pose significant challenges in managing the disease. While hyperuricemia creates the metabolic basis, only a small percentage of patients develop gout, indicating that activation of the innate immune system is essential for its development. Recently, neutrophil extracellular traps (NE
