BACKGROUND: In recent times due to the advancement of haemodialysis techniques, there is increase in the life span of the patients with end-stage renal disease, which tends to increase the risk of complications of bacteraemia due to skin penetration for blood access and leading to propensity for more skeletal and spinal complications. Hence, early diagnosis of spine infection and early initiation of appropriate therapy are important in ensuring successful treatment and preventing further morbidity.
MATERIALS AND METHODS: The study included 22 males and 12 females who were known case of end-st

An 81-year-old ex-smoker with chronic cough and weight loss had imaging showing right upper-lobe nodules, tree-in-bud changes and bronchial obstruction. Bronchoscopy revealed a right upper lobe bronchus polypoidal mass with histological evidence of actinomycosis. This case highlights actinomycosis mimicking malignancy and the need to exclude wider differentials and cancer mimics.

Actinomyces species are facultative anaerobic bacteria that are part of the normal flora of the human oral cavity, gastrointestinal tract, and vaginal tract. These bacteria are known to cause actinomycosis, a rare chronic granulomatous infection that typically presents as a slowly progressive disease. Pulmonary actinomycosis, although rare, has long been recognized as a distinct form of pneumonia, typically affecting individuals with underlying risk factors such as poor oral hygiene, aspiration risk, pre-existing lung disease, or immunocompromising conditions. The recent surge in electronic ci

Pulmonary actinomycosis is a rare chronic infection that can closely mimic pulmonary tuberculosis (PTB) in both clinical presentation and radiological appearance, particularly in tuberculosis-endemic regions. Delayed or missed diagnosis may result in prolonged inappropriate therapy and disease progression. We report a case of pulmonary actinomycosis in a diabetic patient initially treated empirically for pulmonary tuberculosis, highlighting the importance of histopathological examination and anaerobic culture in patients with poor response to anti-tubercular therapy.

Actinomycosis is a rare infection caused by Gram-positive, facultative anaerobic bacteria of the Actinomyceataceae family. Pulmonary Actinomycosis has a strong clinical and radiological resemblance to tuberculosis, lung abscess, and lung cancer, leading to missed or delayed diagnosis. We present a case of an elderly male with a productive cough and weight loss with right upper lobe consolidation on imaging. Definitive diagnosis was by histopathology; drawing samples by fiber-optic bronchoscopy, broncho-alveolar lavage, and endobronchial ultrasound. Tuberculosis, fungal infection, and malignanc

Pulmonary actinomycosis and pulmonary nocardiosis are rare chronic respiratory infections that may closely resemble lung cancer due to their overlapping clinical, radiological, and bronchoscopic characteristics. This hybrid case series and narrative review aims to elucidate the presenting patterns, diagnostic pathways, management strategies, and practical implications of these infections in the differential diagnosis of lung cancer. We analyzed four consecutive cases managed at the Sotiria Thoracic Diseases General Hospital in Athens from 2021 to 2025, supplemented by eight additional cases id

Pulmonary actinomycosis is a chronic respiratory infection caused by Actinomyces. Actinomyces graevenitzii is a rare species of Actinomyces with rare cases of infections. Herein, we report a case of endobronchial actinomycosis due to foreign-body aspiration in which A . graevenitzii was identified. The patient was a 48-year-old woman with a prior history of childhood asthma. She presented with a three-month history of hemoptysis, green sputum, and cough. Plain computed tomography of the chest revealed an elongated foreign body at the left inferior lobar bronchus, with infiltrative opacities ex

BACKGROUND: Pulmonary actinomycosis is a rare chronic infection that frequently mimics lung malignancy, often leading to delayed diagnosis due to its non-specific clinical and radiological presentation. Given the diagnostic challenges associated with this condition, the aim of this study was to evaluate the clinical presentation, diagnostic pathways, treatment strategies, and outcomes of patients diagnosed with pulmonary actinomycosis in a single center.
METHODS: We retrospectively reviewed patients diagnosed with pulmonary actinomycosis at our institution between January 2014 and December 20

Pulmonary actinomycosis is a rare cause of granulomatous and suppurative lung disease that often presents with non-specific symptoms leading to misdiagnosis or delayed diagnosis. Complications include sepsis, infective endocarditis and rarely empyema necessitans. In our report, we present a case of a gentleman in his late 50s, with poor oral hygiene and a number of other risk factors who presented with severe weight loss and an expanding chest wall mass. Following a thorough physical examination and biochemical, radiological and microbiological investigations, the patient was diagnosed with an

BACKGROUND: Pulmonary actinomycosis is a rare, chronic, suppurative infection caused by anaerobic, Gram-positive Actinomycesspecies, characterized by a notoriously high misdiagnosis rate. Due to non-specific clinical and imaging presentations, it is frequently misdiagnosed. This study aims to establish the first CT-based classification system to improve diagnostic accuracy and guide individualized treatment.
METHODS: A retrospective analysis was conducted on patients with pathologically or molecularly confirmed pulmonary actinomycosis between February 2003 and April 2024. Their chest CT featu

Actinomyces (A.) hyovaginalis is a Gram-positive, rod-shaped, polymorphic bacterium. In domestic pigs, the infection is typically associated with pyogranulomatous pneumonia. In wild boars, this infection was never associated with such lesion. We report the gross, histopathological, and bacteriological findings in a case of pyogranulomatous pneumonia caused by A. hyovaginalis infection in a free-ranging wild boar. Fragments of the lungs, lymph nodes, and heart from a wild boar were sent by boar hunters for histopathological analysis with a presumptive tuberculosis diagnosis. Grossly, in the lun

Colloid adenocarcinoma of the lung is a rare subtype of lung adenocarcinoma characterised by the destruction of pre-existing airspaces caused by mucin produced by the tumour. Pulmonary actinomycosis is a chronic suppurative lung disease caused by Actinomyces that is sometimes difficult to differentiate from lung cancer. Here, we report a case of pulmonary colloid adenocarcinoma complicated by pulmonary actinomycosis. A 66-year-old male presented with complaints of cough and bloody sputum and was diagnosed with right upper lobe lung adenocarcinoma. During the preoperative waiting period, the le

Actinomycosis is a rare, chronic infectious disease caused by Actinomyces spp., characterized by an indolent and slowly progressive course. It represents a diagnostic challenge since its nonspecific clinical features often lead to misdiagnosis, mimicking pathologies such as solid neoplasms, active tuberculosis, nocardiosis, fungal infections, or other granulomatous diseases. This study describes a 56-year-old male with abdominal and thoracic pain, weight loss, fever, and dyspnea, over a two-week period. Imaging revealed the presence of hepatic abscesses and right-sided pleural effusion. Actino

Pulmonary actinomycosis is a rare manifestation of Actinomyces infection and is linked to aspiration, poor oral hygiene, and respiratory disease. Although timely treatment leads to favorable outcomes, pulmonary Actinomyces infection often presents with nonspecific symptoms and imaging findings, delaying appropriate interventions. Our 57-year-old female patient with a history of oral cancer and chronic pancreatitis presented to the emergency department with anorexia, weakness, nausea, and diarrhea. Chest and abdominal imaging raised concerns for pancreatic disease and, to our interest, an upper

We encountered a case of pulmonary actinomycosis diagnosed through bronchoscopy, initially suspected as lung cancer originating from the bullae, and the PET-CT findings did not rule out malignancy. Pulmonary actinomycosis is a rare lung infection that often mimics cancer. Its anaerobic nature and prolonged culture time make diagnosis challenging without strong clinical suspicion.

BACKGROUND: Actinomycosis is a rare, globally distributed disease with an annual incidence of 1 case per 300,000 persons. It classically manifests as a cervicofacial disease with sinus tract formation and purulent discharge with or without sulfur granules. Spinal compromise occurs in less than 5% of cases; however, it causes significant morbidity associated with neurological impairment.
OBSERVATIONS: This case report presents a patient with thoracic actinomycosis invading the spinal column who presented with symptoms and imaging findings mimicking malignancy. Surgery was performed, and frozen

The emergence of next-generation sequencing (NGS) has significantly improved the detection rate of Actinomyces species in pulmonary actinomycosis (PA), a rare and diagnostically challenging infectious disease. Here, we report a case of an elderly patient with a history of pyoderma gangrenosum who presented with pulmonary infiltrates and was subsequently hospitalized. Both histopathological examination of tracheoscopic biopsy specimens and NGS analysis of bronchoalveolar lavage fluid (BALF) confirmed infection with Actinomyces odontolyticus (A. odontolyticus). The patient exhibited marked clini

We present the case of a 63-year-old male patient with a history of chronic obstructive pulmonary disease (COPD), heavy smoking, and poor dental hygiene, who presented with progressive dyspnea, fever, and a productive cough. The patient was initially evaluated for pneumonia, but a chest radiograph revealed a right-sided pleural effusion. Further analysis of the pleural fluid showed an exudative effusion. Histopathological examination of a pleural biopsy sample identified gram-positive branching filamentous rods with yellow sulfur granules, consistent with a diagnosis of pulmonary actinomycosis

BACKGROUND: Actinomyces graevenitzii is a relatively uncommon Actinomyces species, which is an oral species and predominantly recovered from respiratory locations [1,2]. It is a gram-positive anaerobic bacteria or microaerobic filamentation bacteria, which can induce pyogenic and granulomatous inflammation characterized by swelling and concomitant pus, sinus formation, and the formation of yellow sulfur granules. All tissues and organs can be infected; the most common type involves the neck and face (55%), followed by the abdominal and pelvic cavities (20%). Chest involvement is the third most
