Phototherapy for different types of cutaneous porphyria: a case series and systematic review.

Porphyrias are a group of rare metabolic disorders characterized by defects in heme synthesis. Cutaneous photosensitivity is a common manifestation, posing significant challenges in its management. Photo-hardening accomplished using narrow-band ultraviolet radiation (NB-UVB) has been evaluated in the setting of erythropoietic protoporphyria (EPP) in small case series; however, data is scarce regarding other types of porphyria. We aimed to assess the effect of NB-UVB treatment on patients with different types of cutaneous porphyria. A case series of five patients with cutaneous porphyria who we

Porphyria cutanea tarda: cutaneous manifestations and management.

Porphyria cutanea tarda (PCT) is the most common porphyria and is caused by inhibition of hepatic uroporphyrinogen decarboxylase in the setting of iron overload and oxidative stress. Clinically, PCT presents with cutaneous photosensitivity, skin fragility, blistering and erosions on sun-exposed skin, hypertrichosis, dyspigmentation, milia, and dark urine. Because the skin findings may resemble other blistering disorders, including pseudoporphyria and other cutaneous porphyrias, recognition of the characteristic clinical pattern is important to prompt appropriate biochemical testing. Diagnosis

Stability of plasma and erythrocyte porphyrins: implications for diagnosis and monitoring of erythropoietic protoporphyria.

Diagnosing and identifying the type of porphyria in a patient requires specialist analysis of porphyrins and/or precursors in blood, urine and faeces. Correct sample storage and handling prior to analysis is essential to minimise preanalytical error. We evaluated the impact of light exposure, time and temperature on erythrocyte and plasma porphyrins in samples from patients with erythropoietic protoporphyria (EPP) and porphyria cutanea tarda (PCT) stored as whole blood for up to 96 hours, and in addition, the effect of freeze-thaw on plasma porphyrins in EPP, PCT and hereditary coproporphyria p

Genetic traits and diet triggering the iron-induced hepatic model of the idiopathic disorder sporadic porphyria cutanea tarda.

Metabolic disorders can be the consequence of external factors and individual susceptibility. Sporadic porphyria cutanea tarda (sPCT) is an idiopathic disorder of liver heme synthesis exhibiting inhibition of uroporphyrinogen decarboxylase, characterised by dermal and hepatic deposition of uroporphyrins from oxidation of sensitive uroporphyrinogens (uroporphyria). sPCT is associated with alcohol, estrogenic drugs, HIV and hepatitis C, as well as a poorly understood influence of iron. Hexachlorobenzene (HCB) and reputably 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) cause a similar disorder. The

From photosensitivity to autoimmunity: the role of AI metabolomics in PCT-celiac disease overlap.

Porphyria cutanea tarda (PCT), caused by hepatic uroporphyrinogen decarboxylase deficiency, may rarely overlap with undiagnosed celiac disease (CD), complicating diagnosis due to shared hepatic and dermatologic features. Both conditions present with skin manifestations such as photosensitivity, blistering, or dermatitis herpetiformis-like lesions, often leading to misclassification. AI-driven metabolomics provides a novel strategy to identify metabolic biomarkers, predict CD with high accuracy through machine learning, and uncover hidden comorbidities in PCT patients. Despite challenges includ

Understanding the Clinical Spectrum of the Cutaneous and Acute Hepatic Porphyrias.

Porphyrias are rare metabolic disorders caused by inherited or acquired enzymatic defects in the heme biosynthesis pathway, resulting in the accumulation of heme precursors or toxic porphyrin intermediates. The cutaneous porphyrias arise from enzymatic defects in later steps of the heme biosynthesis pathway, which lead to the build-up of photoactive porphyrins in the skin and liver, such as coproporphyrins, protoporphyrins, and uroporphyrins. These photoactive porphyrins generate reactive oxygen species that drive the characteristic cutaneous manifestations, including painful photosensitivity,

Inhibiting Wnt/β-catenin-mTOR signaling enhances autophagy and porphyrin clearance in murine porphyrias with hepatic involvement.

Porphyrias are rare metabolic disorders arising from defects in heme biosynthesis, leading to accumulation of toxic porphyrin intermediates, mitochondrial dysfunction, and liver injury. Current therapies are limited in efficacy, emphasizing the need for novel treatments. Prior studies showed hepatocyte-specific β-catenin deletion attenuates porphyrin accumulation and liver injury in 3,5-diethoxycarbonyl-1,4-dihydrocollidine (DDC)-induced porphyria. We hypothesized that inhibiting components of the Wnt-β-catenin-glutamine synthesis (GS) pathway reduces heme synthesis and also enhances porphyrin

Oxidation of Uroporphyrinogens During Heme Synthesis: Role of Iron, Susceptibility and Consequences.

In the biosynthesis of heme the tetrapyrrole hydroxymethylbilane is converted enzymatically to uroporphyrinogen III whereas conversion to uroporphyrinogen I occurs spontaneously. Both are substrates for uroporphyrinogen decarboxylase (UROD) but only the III isomer is a precursor of heme. These porphyrinogens are easily oxidised to the respective uroporphyrins and trace amounts occur in the urine of healthy humans and animals. Large quantities of uroporphyrins I and III, as well as other oxidation products, occur in the liver and urine of patients with some porphyrias and after poisoning of peo

Acute Intermittent Porphyria with a Secondary Porphyria Cutanea Tarda-like Biochemical Pattern in a Patient with Co-Morbid Human Immunodeficiency Virus Infection.

INTRODUCTION: Porphyrias are rare inherited disorders of heme synthesis caused by reduced activity of one of the eight enzymes in the pathway. When early precursors accumulate, acute hepatic porphyrias occur, which present as severe neurovisceral attacks. When later, light-sensitive porphyrins accumulate, they cause cutaneous forms with chronic photosensitivity, blistering and skin fragility. Although these patterns are usually distinct, acute, and cutaneous features may appear together. This may reflect two separate genetic defects, but more often occurs when oxidative stress, such as from ir

[Cutaneous presentation of digestive tract disorders and liver disease].

A variety of skin conditions may be important diagnostic indicators for disorders of the digestive tract and liver. In inflammatory bowel diseases, the most common cutaneous manifestations are erythema nodosum and pyoderma gangrenosum. Autoimmune liver diseases are often associated with cutaneous manifestations such as hidradenitis suppurativa, vitiligo, psoriasis and alopecia areata. Lichen planus or porphyria cutanea tarda can also be induced by infectious hepatitis. Enteral malabsorption can lead to deficiencies and characteristic skin changes, such as celiac disease, acrodermatitis enterop

Case Report: HCV-triggered porphyria cutanea tarda in a patient with SEC23B-mutated congenital dyserythropoietic anemia type II.

Porphyria cutanea tarda (PCT) is a hepatic porphyria often triggered by hepatitis C virus (HCV) infection, iron overload, or environmental exposure. Congenital dyserythropoietic anemia type II (CDA II) caused by SEC23B mutations leads to ineffective erythropoiesis and secondary iron accumulation. We report a 34-year-old man with genetically confirmed SEC23B-mutated CDA II who developed photosensitive bullae associated with chronic HCV genotype 1b infection, hyperbilirubinemia, and severe iron overload. Urinary porphyrins were positive, and skin biopsy showed subepidermal bullae with PAS-positi

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The risk of skin infections in end-stage renal disease patients with porphyria cutanea tarda: A retrospective cohort study.

BACKGROUND: Porphyria cutanea tarda (PCT) is one of many conditions characterized by increased photosensitivity, manifesting as blistering lesions on sun-exposed skin. Due to the increased photosensitivity and skin fragility, PCT patients are more susceptible to infections. Individuals with end-stage renal disease (ESRD), a potential complication of PCT, also exhibit an increased risk of infection. Due to these combined risks, this retrospective cohort study sought to determine if PCT is an independent risk factor for certain infections in the ESRD population. METHODS: This study examined dia

Treatment of Porphyria Cutanea Tarda Scarring With Combination Laser Treatment and a Pilot Use of Artificial Intelligence to Quantify Laser Results.

BACKGROUND: Porphyria cutanea tarda (PCT) is the most common subtype of porphyria and results from a deficiency of the enzyme uroporphyrinogen decarboxylase. Even after successful treatment, patients can be left with significant scarring, and there is little published data on the safety and efficacy of light-based or laser-based therapies. METHODS: This report examines a case of a 47-year-old male with PCT secondary to HCV, treated with a combination of fractionated erbium-doped yttrium-aluminum-garnet (Er:YAG), intense pulsed light (IPL), and carbon dioxide (CO2) lasers to address significan

Sporadic Porphyria Cutanea Tarda, Cutaneous Sarcoidosis, and Compound Heterozygosity of HFE Mutations Cys282Tyr and His63Asp-A Case Report.

Porphyria cutanea tarda (PCT) is caused by inherited or acquired defects of uroporphyrinogen decarboxylase (UROD) in the heme biosynthetic pathway. Altered iron homeostasis via hemochromatosis gene (HFE) mutations is one of many susceptibility factors associated with the sporadic form of PCT. Though sarcoidosis is not commonly associated with PCT, prior reports of hepatic sarcoidosis postulated that hepatic granulomas affect UROD activity by direct interference or immunological mechanisms. Here we describe a case of acquired PCT in the setting of (HFE):c.845G>A (p.Cys282Tyr) and (HFE):c.187C>G

Porphyria cutanea tarda and systemic lupus erythematosus: a case report.

BACKGROUND: Systemic lupus erythematosus is characterized by multiorgan involvement and the presence of autoantibodies. Porphyria cutanea tarda is a condition that affects the liver and skin by downregulating and inhibiting the enzyme uroporphyrinogen decarboxylase in erythrocytes. The presence of the two diseases simultaneously is rare, so we present this case report and a panoramic review of this uncommon association with scarce description found in the reviewed literature, in a patient with history of kidney transplant who is a user of immunosuppression and immunomodulation medications also
