The HO-1/bilirubin axis in cardiovascular protection: critical evidence for modulation by plant metabolites and traditional multi-component formulations used in Chinese medicine.

Bilirubin was long regarded primarily as a waste product of haem catabolism, but accumulating experimental and epidemiological evidence suggests that, within an appropriate concentration range, it may contribute to cardiovascular protection through antioxidant, anti-inflammatory, endothelial, metabolic, and mitochondrial effects. Haem oxygenase-1 (HO-1) is a key inducible enzyme in bilirubin formation because it converts haem to biliverdin, which is subsequently reduced to bilirubin. However, HO-1 activation also produces carbon monoxide and ferrous iron, and increased HO-1 expression alone do

Hepatitis A-associated oxidative haemolysis in a patient with glucose-6-phosphate dehydrogenase deficiency with co-inherited sickle cell trait.

An adult male patient in his late teens presented with pruritus, nausea and progressive jaundice after travel to West Africa. He did not get a pre-travel hepatitis A vaccination or malaria chemoprophylaxis. Laboratory testing demonstrated hepatocellular injury with alanine aminotransferase (ALT) 4002 U/L (reference <60), marked hyperbilirubinaemia (total bilirubin 632 µmol/L, reference <20) and a mildly prolonged international normalised ratio (INR). Concurrently, he developed non-immune haemolysis with undetectable haptoglobin, elevated lactate dehydrogenase and reticulocytosis, as well as bi

Regulation of Diverse Endogenous Metabolic and Signaling Pathways by UDP-Glucuronosyltransferases (UGTs) Revealed by Metabolomics of Knockout Mice.

Uridine 5'-diphospho-glucuronosyltransferase (UGT) enzymes play a critical role in hepatic and other metabolism of endogenous and exogenous small molecules. The addition of glucuronic acid via UGT increases the polarity and subsequent solubility of small molecules, helping direct them for biliary excretion and renal excretion by organic anion transporter OAT3 (SLC22A8). Mutations in UGTs are associated with disorders in biliary metabolism (e.g., Gilbert's syndrome, Crigler-Najjar syndrome). Here, we used comprehensive serum metabolomic profiling of pan Ugt1-knockout (Ugt1-/- ) mice to define t

Development of a dosing algorithm integrating pharmacogenetic markers and inflammation for individualization of initial voriconazole maintenance doses.

Achieving early therapeutic trough concentrations (Cmin) of voriconazole (VRC) is crucial for treatment efficacy and to avoid side effects. Since VRC Cmin are influenced by numerous determinants, this multicenter retrospective study aimed to develop a multiparametric dosing algorithm for individualizing initial VRC maintenance doses. Individual data regarding VRC Cmin from adult patients and associated parameters (age, sex, underlying disease [presence of hematological disease/other], weight, dose, route of administration, indication: curative/prophylactic, cytochrome [CYP]2C19 and 3A genotype

Heme, copper, and a new way to kill cancer cells.

INTRODUCTION: Unfractionated heparin (UFH) is the traditional anticoagulant of choice in critically ill COVID-19 patients requiring extracorporeal membrane oxygenation (ECMO). Nadroparin, a low molecular weight heparin, potentially offers advantages such as predictable pharmacokinetics and reduced bleeding risks compared to UFH, with complex pharmacokinetics, influencing activated partial thromboplastin and causing substantial haemorrhagic risks. Bleeding, the most common adverse event during ECMO, is associated by many with increased activated partial thromboplastin time. MATERIAL AND METHOD

Low serum pseudocholinesterase levels are associated with mortality in patients with hepatocellular carcinoma.

BACKGROUND: There is no consensus scoring system for staging and prognosis in hepatocellular carcinoma (HCC), which is the fourth leading cause of cancer-related mortality worldwide. Commonly used systems include the albumin-bilirubin (ALBI) score, the Barcelona staging classification (Barcelona Clinic Liver Cancer, BCLC), the Model for End-Stage Liver Disease (MELD), and the model to estimate survival in ambulatory HCC patients (MESIAH). Liver secretion of pseudocholinesterase (PCHE) has been linked to liver function but is poorly studied in the natural history of HCC. We evaluated whether se

Association between systemic redox balance and osteoporosis: prospective evidence, polygenic modification, and proteomic and inflammatory mediation in the UK Biobank.

PURPOSE: Redox signaling governs bone remodeling, but whether systemic redox balance is associated with incident osteoporosis, genetic susceptibility, and proteomic/inflammatory pathways at population scale remains unclear. METHODS: We analyzed UK Biobank participants free of osteoporosis at baseline. Serum redox balance score (SRBS) combined albumin, total bilirubin, and γ-glutamyl transferase. Cox proportional hazards models adjusted for prespecified covariates. Effect modification by an osteoporosis polygenic risk score (PRS) was tested on multiplicative and additive scales. Mediation was

Leveraging polygenic risk scores to infer causal directions in genotype-by-environment interactions between complex traits.

Most existing genotype-by-environment interaction (G×E) methods assume a known causal direction as an assumption that often does not hold and can lead to biased estimates and spurious findings. To address this, we introduce the Genetic Causality Inference Model (GCIM), a novel approach designed to infer causal directions in G×E studies. GCIM integrates polygenic risk scores (PRS) for both the exposure and the outcome to strengthen causal inference and reduce spurious interaction signals. We evaluated GCIM using simulated data across varying genetic and residual correlation settings and compare

Comparison of Two Artificial Liver Treatments for Acute Liver Failure: A Retrospective Observational Study.

BACKGROUND: Liver failure can lead to severe disorders in the body's metabolism, detoxification, synthesis and other functions, with a high mortality rate of 60%-90%. The dual plasma molecular adsorption system (DPMAS) is a new artificial liver technology that has received much attention in recent years. However, it cannot provide coagulation factors, albumin or other substances during the treatment process and may even consume albumin during the adsorption process. The DPMAS typically needs to be used in combination with plasma exchange (PE). Few studies have compared PE alone and DPMAS + PE.

Gait deviations in adolescent patients with Wilson disease and their possible connections with biochemical markers: preliminary results.

UNLABELLED: The study aims to evaluate the gait pattern of Wilson Disease adolescents, receiving pharmacological treatment, without clinical signs of gait problems, with instrumented gait analysis. The gait changes could be the first signs of neurological manifestations, providing information about which patients are at greater risk of developing neurological problems. Another aim of the paper was to find possible relationships between gait disturbances and biochemical markers. Twenty-seven patients underwent gait analysis with the VICON system. Captured data were processed and averaged. From

Binding and transport functions of human serum albumin and its clinical implications in liver disease: a narrative review.

BACKGROUND AND OBJECTIVE: Human serum albumin (HSA), a multifunctional plasma protein derived from the liver, plays a crucial role in the pathophysiology and management of liver diseases. Increasing research reveals that the non-colloid functions of HSA, especially its binding and transport of both endogenous and exogenous substances, are clinically important, beyond its well-characterized colloid effects such as maintaining oncotic pressure. In chronic liver diseases such as cirrhosis, impaired function of HSA disrupts its ligand-binding and detoxification processes, thereby leading to variou

Fluorescent Yellow Urine Induced by Vonoprazan Fumarate: A Case Report of a Benign Phenomenon.

An abnormal color in urine may be caused by various factors, including foods with high levels of vitamins, certain medications that metabolize in the body to produce fluorescent substances, or certain metabolic disorders. Although abnormal urine, such as black, green, pink, or purple urine, is commonly reported, fluorescent urine is rare and typically observed under ultraviolet light. This case reports a healthy participant with persistent fluorescent yellow urine after receiving vonoprazan fumarate (TAK-438) under visible lights. A 25-year-old healthy female participated in a Phase I clinical

Progressive Decline in Interferon-γ Release is Associated with Cirrhosis Progression and Severity: A Prospective Preliminary Report.

Cirrhosis progresses through distinct stages, with increasing severity and susceptibility to infections due to cirrhosis-associated immune dysfunction. Lymphocyte-based functional assays are considered the gold standard for assessing immune function, although none have been evaluated across different cirrhosis stages. We conducted a pilot study to assess whether performing interferon-γ (IFN-γ) release assays (IGRA), in response to a non-antigenic mitogen (phytohemagglutinin [PHA]), could help classify immune function and disease severity in 70 patients with cirrhosis awaiting liver transplanta
