Influenza A infection typically causes mild hepatocellular liver enzyme elevations, while clinically significant cholestatic liver injury is rare. We report a case of severe intrahepatic cholestasis associated with Influenza A infection in a patient with pre-existing metabolic dysfunction-associated steatotic liver disease (MASLD). A 65-year-old female with MASLD and chronic prosthetic hip infection presented with confusion, hypoglycemia, and sepsis. One month prior, liver function tests were normal. On admission, laboratory evaluation demonstrated a cholestatic pattern with total bilirubin 3.

BACKGROUND: In recent years, improvements in living standards and shifts in dietary patterns have led to metabolic dysfunction-associated steatotic liver disease (MASLD) becoming the most common cause of chronic liver disease worldwide. MASLD is closely associated with obesity, insulin resistance, and metabolic syndrome, although its precise pathogenesis remains incompletely understood. The disease encompasses a spectrum of conditions ranging from non-alcoholic fatty liver (NAFL) to non-alcoholic steatohepatitis (NASH) . Global estimates indicate that the prevalence of MASLD is 25% and continu

Per- and polyfluoroalkyl substances (PFAS) and their alternatives are recognized environmental hepatotoxins, yet their combined effects with metals on liver health in human populations remain underexplored. This study measured serum levels of 13 PFAS (including legacy and emerging alternatives such as Cl-PFESAs) and plasma levels of 25 metals, and evaluated their individual and joint associations with five liver function biomarkers (ALT, AST, ALP, GGT, and DBIL) among 1263 adults in Guangzhou, China. Generalized additive models (GAMs) revealed that most individual pollutants were positively as

BACKGROUND: Skeletal fluorosis is a chronic bone disease caused by excessive fluoride exposure, leading to osteosclerosis, osteoporosis, and ectopic ossifications. Its radiological features and their links to age, joint function, and metabolism remain unclear, necessitating further study. This study aimed to develop a Fluorosis Imaging Reporting and Data System (FI-RADS) based on X-ray radiographs of multiple skeletal sites for the diagnosis of skeletal fluorosis in endemic regions.
METHODS: This cross-sectional study aimed to establish the FI-RADS grading system by analyzing radiographic dat

Liver cirrhosis causes substantial morbidity, and options beyond transplantation are limited. Umbilical cord-derived mesenchymal stem cells (UC-MSCs) may support liver repair, but their delivery via the hepatic artery has not been evaluated. This study assessed the safety and exploratory efficacy of allogeneic UC-MSCs infusion in cirrhosis. Twenty patients with liver cirrhosis entered a single-center open-label pilot trial in Hanoi, Vietnam, between 2020 and 2023; 17 completed follow-up and were included in exploratory analyses. All received a single hepatic artery infusion of UC-MSCs at 1 × 1

BACKGROUND AND OBJECTIVES: Neonatal conjugated hyperbilirubinemia is a pathological condition reflecting cholestasis and hepatobiliary dysfunction, associated with significant morbidity and mortality if not detected early. Early recognition is critical, especially for surgically correctable or treatable causes such as biliary atresia and inherited metabolic liver diseases. Limited region-specific data exist from Northern India. This study aimed to characterize the clinical and biochemical profile, delineate the etiological spectrum, and identify predictors of adverse outcomes among neonates ad

BACKGROUND: Choledocholithiasis (CDL) is a common indication for endoscopic retrograde cholangiopancreatography (ERCP). While guidelines use static hepatic biomarker values to estimate pre-test probability, the utility of trending these labs (bilirubin and alkaline phosphatase [ALP]) is unclear. It is unknown if improving biomarker trends reliably indicate stone passage and can help avoid unnecessary procedures.
OBJECTIVE: To evaluate whether serial trends in hepatic biomarkers prior to ERCP can predict the presence of suspected CDL in hospitalized patients and guide decisions on ERCP referra

The liver plays a central role in glucose and lipid metabolism. In human medicine, glucose metabolism alterations and insulin resistance are commonly associated with chronic liver disease (CLD), particularly in metabolic dysfunction-associated steatotic liver disease (MASLD). In veterinary medicine, however, the role of glucose metabolism in CLD remains poorly understood. We enrolled 67 client-owned dogs, including healthy controls, hepatopathic dogs with (metabolic hepatopathy [MH]) or without (non-metabolic hepatopathy [non-MH]) metabolic factors such as overweight and hyperlipidemia, chroni

Dubin-Johnson syndrome (DJS) and Rotor syndrome (RS) are rare, autosomal recessive disorders that result in chronic, predominantly conjugated hyperbilirubinemia without cholestasis or hepatocellular injury. Although both conditions are benign and non-progressive, they reflect distinct molecular defects in hepatocellular transport pathways. DJS arises from mutations in the ABCC2 gene encoding the canalicular transporter multidrug resistance-associated protein 2, leading to impaired biliary excretion of conjugated bilirubin and organic anions. In contrast, RS results from combined deficiencies o

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is a systemic disorder closely linked to cardiometabolic risk and dysregulated lipid metabolism. Apolipoprotein C3 (ApoC3), a key regulator of triglyceride-rich lipoproteins and mediator of cardiovascular disease, has been implicated in hepatic steatosis. However, its stage-specific role across the MASLD spectrum remains incompletely defined.
METHODS: In this cohort study, serum ApoC3 concentrations were quantified in 197 patients with MASLD across different disease stages (non-fibrotic MASLD, fibrosis, cirrhosis, an

BACKGROUND: Increasing evidence indicates that vitamin D has anticancer effects. However, there are very limited data regarding the relationship between vitamin D levels and the efficacy or prognosis by atezolizumab plus bevacizumab (ATZ/BEV) for hepatocellular carcinoma (HCC). We sought to investigate this.
METHODS: This study included 142 HCC patients who were treated with ATZ/BEV as the first-line of systemic chemotherapy. Their initial treatment responses were evaluated using dynamic computed tomography or magnetic resonance imaging. Serum levels of 25-hydroxyvitamin D (vitamin D) were me

BACKGROUND: Nonalcoholic fatty liver disease (NAFLD) is a key contributor to chronic liver diseases. It is defined as hepatic fat accumulation in ≥ 5% of hepatocytes in individuals with minimal or no alcohol consumption and no other identifiable causes of liver steatosis. Drug-induced hepatic toxicity is increasingly recognized as a key contributor to the development and progression of NAFLD. Among them, valproic acid (VPA), an antiepileptic drug, is well documented for inducing hepatic steatosis, in addition to interfering with folate metabolism and accelerating the progression of NAFLD. INTRODUCTION: Primary biliary cholangitis (PBC) has steadily increased in incidence, but its causes remain to be completely elucidated. Moreover, the potential for disease progression is not uncommon, especially when normalization of hepatobiliary enzyme levels is difficult, making the emergence of new treatments highly anticipated. We clarified the therapeutic effects of glucosyl hesperidin (G-Hes), which activates the antioxidant NRF2/KEAP1 pathway, on improving PBC pathophysiology.
METHODS: This randomized controlled trial evaluated changes in serum levels of hepatobiliary enzymes, lipids,

The Model for End-Stage Liver Disease (MELD) score is widely used to prioritize patients for liver transplantation and to estimate short-term mortality in end-stage liver disease. Inaccuracies in serum creatinine measurements, particularly interference from bilirubin, both key components of the MELD, may influence clinical decision-making. However, standardized approaches to address such analytical bias are lacking. Here we show that bilirubin-related interference leads to clinically relevant MELD distortions and associated outcomes. We developed a correction model using controlled in vitro ma

Acute heart failure (AHF) is a systemic condition characterised by multiorgan involvement that varies from subclinical to overt organ failure. The liver is particularly vulnerable due to its dual blood supply and sensitivity to haemodynamic and inflammatory disturbances. Abnormal liver function tests are highly prevalent in patients with AHF, reflecting major mechanisms: passive hepatic venous congestion from elevated central venous pressure, hepatocellular ischaemia from low cardiac output, systemic inflammation and poor metabolic status associated with cachexia. In patients with AHF, liver f

Elevated aminotransferases with concomitant jaundice in drug-induced liver injury (DILI) are associated with adverse outcomes, but the prognostic significance of very high aminotransferases without jaundice is unclear. We examined selected outcomes of patients with DILI and markedly elevated aminotransferases but normal or near-normal total bilirubin (TBL). We analyzed the data from the prospective Drug-Induced Liver Injury Network (DILIN) and the Spanish DILI registries. Patients were categorized based on peak alanine or aspartate aminotransferase (ALT/AST) levels-very high (500-1,000 U/L) or

Introduction Liver function tests (LFTs) are essential for evaluating hepatic injury in steatotic liver disease. Alcoholic fatty liver disease (AFLD) and metabolic-associated steatotic liver disease (MASLD) share steatosis but differ in pathogenesis. This study aimed to compare LFT profiles between AFLD and MASLD to identify potentially distinct biochemical patterns. Methods This cross‑sectional study included 90 participants (30 AFLD, 30 MASLD, 30 healthy controls) at Krishna Institute of Medical Sciences, Karad, India. A comprehensive LFT panel was analyzed, including transaminases (alanine
