Targeted Delivery of Bilirubin to Pulmonary Endothelium Mitigates Paraquat-Induced Lung Injury.

INTRODUCTION: Paraquat (PQ) poisoning causes high mortality via acute lung injury (ALI) driven by excessive reactive oxygen species (ROS) in pulmonary microvascular endothelial cells. We developed BR@Lipo-CerTP, a lung-targeted nanotherapeutic using C16-ceramide-binding peptide-modified bilirubin liposomes, to enhance endothelial delivery and treat PQ-induced ALI. METHODS: BR@Lipo-CerTP was characterized for physicochemical properties and cellular uptake in pulmonary microvascular endothelial cells (PMVECs). In vitro efficacy was assessed via cytotoxicity, apoptosis, ROS, antioxidant capacity

Survival after curative hepatectomy for hepatocellular carcinoma in patients with and without MAFLD: a western cohort study.

BACKGROUND: Evidence suggesting that hepatocellular carcinoma (HCC) arising in patients with metabolic-associated fatty liver disease (MAFLD) is associated with better outcomes than HCC related to other liver diseases has been derived largely from non-Western cohorts and has incompletely accounted for the severity of underlying liver disease. Whether MAFLD independently influences survival after hepatic resection, particularly in Western populations, remains uncertain. METHODS: In this retrospective single-center cohort study, adults who underwent curative-intent hepatic resection for HCC at

Reprogramming ligand-receptor interactions via a self-powered molecular engine for proton-responsive regulation of cellular motility.

Manipulation of ligand-receptor interactions (LRIs) in a user-defined manner is one of the effective ways to regulate cell motility and disease progression. Molecular engines orchestrate LRIs to regulate cell motility by harnessing various forms of energy within physiological surroundings. However, engineering chemical engines remains largely unexplored, primarily due to the insufficient chemical cue density, uncertain conformational changes of scaffolds, and a limited repertoire of environment-responsive switchable scaffolds. Enzymatic biofuel cells (EBFCs) are electrochemical devices that co

Frailty before chronic disease: multisystem physiological differences in middle-aged adults from the UK Biobank.

Frailty, defined as increased vulnerability to stressors and impaired recovery of homeostasis, poses a major health challenge among older and multimorbid adults. Frailty is also observed in middle-aged and disease-free adults, though evidence remains scarce. Understanding the physiological characteristics of frail individuals free of chronic diseases may inform about the underlying mechanisms of frailty and its clinical management. We assessed frailty in a large disease-free middle-aged cohort and examined the associations of markers of physiological function with frailty. We analyzed data fro

Machine learning-derived serum carbon dioxide trajectories in predicting prognosis of patients with acute aortic dissection.

Venous partial pressure of carbon dioxide (PvCO 2 ) indicates postoperative adverse outcomes in patients with acute aortic dissection, and its trajectories over time affecting postoperative adverse outcomes remain unclear. This study aimed to investigate the relationship between PvCO 2 trajectories and postoperative adverse outcomes, as well as clinical application through group-based trajectory modeling and machine-learning algorithms. This retrospective cohort study analyzed data from 642 patients with acute aortic dissection treated at the Fourth Hospital of Hebei Medical University from 20

Disrupted terminal bilirubin catabolism links Lachnospiraceae depletion to inflammatory bowel disease.

BACKGROUND: The gut microbiome mediates the reductive catabolism of bilirubin into urobilinoids, yet the bacteria and enzymes responsible for the later steps of this pathway remain largely unknown. METHODS: Here, we combine untargeted UPLC/HRMS metabolomics with shotgun metagenomic sequencing of fecal samples from 119 participants, including patients with Crohn's disease, ulcerative colitis, and healthy controls, to map disruptions in the bilirubin catabolic pathway in inflammatory bowel disease (IBD). RESULTS: We show that stercobilinogen and stercobilin, the terminal metabolites of this pa

A matter of balance: Heme oxygenase-1 at the crossroads of healthy and pathological aging.

Increasing life expectancy has led to a growing prevalence of age-related disorders, including dementia, cardiovascular disease, frailty, osteoporosis, sarcopenia, and cancer. Heme oxygenase-1 (HO-1), a stress-inducible enzyme that degrades heme into carbon monoxide (CO), biliverdin which is then reduced to bilirubin by the biliverdin reductase enzyme, and iron, has emerged as a key regulator of several hallmarks of aging, including oxidative stress, inflammation, autophagy, mitochondrial dysfunction, and cellular senescence. This review evaluates age-associated changes in HO-1 expression acro

Effects of Gilbert syndrome on cardiovascular disease risk reduction: a systematic review.

BACKGROUND: Gilbert syndrome is a human genetic disorder which affects bilirubin metabolism in the liver and results in unconjugated hyperbilirubinemia. While considered a benign condition with only occasional jaundice and possible alterations of metabolism of certain drugs, Gilbert syndrome can reduce cardiovascular disease (CVD) risk. The objective of this review was to summarize the evidence on the potential effect of Gilbert Syndrome on reducing CVD risk via lowering cholesterol and/or lipid levels in the body and is associated with other protective mechanisms which are related to higher b

Case Report: Retransplantation for Adult-Onset Bilirubin Encephalopathy After Auxiliary Partial Liver Transplant for Crigler-Najjar Syndrome Type 1 in Childhood.

Crigler-Najjar syndrome (CNS) type 1 is a genetic disorder that leads to severe unconjugated hyperbilirubinemia. Auxiliary partial orthotopic liver transplantation (APOLT) is preferred for metabolic liver diseases as a portion of the native liver is retained. Reported here are details of a 32-year-old male who developed bilirubin encephalopathy due to graft failure 27 years after APOLT performed at 5-year-old for CNS type 1. Initially, the bilirubin level was low, which then gradually increased to 7.3 mg/dL at age 12. At the age of 31, the bilirubin level began to rise rapidly and we submitted

Severe Hyperferritinemia in Metabolic Dysfunction-Associated Steatotic Liver Disease With Normal Transferrin Saturation and Polyclonal Hypergammaglobulinemia Mimicking Iron Overload.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognized as a systemic immunometabolic disorder characterized by the interplay between insulin resistance, hepatic lipid accumulation, and chronic low-grade inflammation. While mild elevations in serum ferritin are frequently observed in MASLD, markedly elevated ferritin levels often prompt evaluation for iron overload syndromes, inflammatory disorders, or malignancy. The coexistence of polyclonal hypergammaglobulinemia further complicates diagnostic interpretation. We report a 36-year-old female with overweight

Association of albumin-bilirubin score with clinical severity and outcome in patients with decompensated heart failure: a retrospective cohort study.

BACKGROUND: The Albumin-Bilirubin (ALBI) score is a simple marker of hepatic function that may reflect systemic congestion in heart failure. Therefore, the present retrospective cohort study aimed to investigate the association between ALBI score and clinical severity, echocardiographic findings, and hospitalization outcomes in patients admitted with decompensated heart failure. Exploratory analyses were additionally performed to examine potential associations between ALBI score and in-hospital mortality. METHODS: This retrospective cohort study included adults (≥ 18 years) admitted with deco

High- versus low-dose phototherapy for neonatal jaundice.

RATIONALE: Phototherapy is the mainstay of treatment for neonatal hyperbilirubinaemia. Increasing the dosage or intensity of phototherapy by various means is commonly practised for infants with high levels of serum bilirubin. It is unclear whether these measures of increasing phototherapy dosage are beneficial and safe for neonates. OBJECTIVES: To assess the effects of high-dose versus low-dose phototherapy on bilirubin level and associated clinical outcomes that constitute the major conditions in kernicterus spectrum disorder (KSD), such as acute bilirubin encephalopathy and kernicterus, as

Peripheral biochemical parameters for discrimination between bipolar disorder and major depressive disorder in female patients of reproductive age: a CRP-stratified exploratory study.

BACKGROUND: Bipolar disorder (BD) is often misdiagnosed as major depressive disorder (MDD) due to overlapping clinical syndromes. Although numerous studies have investigated biomarkers to differentiate these two disorders, no clinically applicable differential diagnostic panel has been established. Notably, sex-specific differences play a critical role in this context. Females are more prone to experience depressive episodes and exhibit more pronounced immune-inflammatory dysregulation, which further increases the risk of misdiagnosis. This study aimed to explore whether C-reactive protein (CR

Fulminant Acute Chest Syndrome and Multiorgan Failure in Sickle Cell Disease With Chronic Hepatic Vasculopathy: A Fatal Synergy.

Sickle cell disease (SCD) is a multisystem disorder characterized by recurrent vaso-occlusive events and progressive organ damage. Among its chronic complications, sickle hepatopathy, driven by sinusoidal obstruction, ischemia, and cholestasis, remains underrecognized despite its major prognostic significance. During acute crises, hepatic dysfunction may amplify systemic inflammation, impair metabolic reserve, and precipitate rapid multiorgan failure. We report the case of a 28-year-old man with homozygous SCD and biopsy-confirmed chronic hepatic vaso-occlusive disease who presented after disc

Recurrent Drug-Induced Autoimmune-like Hepatitis due to Turmeric Supplementation.

Drug-induced autoimmune-like hepatitis (DI-ALH) is a rare form of drug-induced liver injury (DILI) that mimics autoimmune hepatitis (AIH). Commonly implicated agents include minocycline, nitrofurantoin, and infliximab, though a wide range of other drugs has been reported. We report a 65-year-old woman with a history of suspected turmeric-induced liver injury six years prior who presented with nausea and painless jaundice two weeks after restarting the same turmeric supplement containing black pepper extract. She denied alcohol or hepatotoxic medication use, and family and personal history of a
