This review summarizes the research progress on high-altitude polycythemia (HAPC). At the fundamental theoretical level, it provides an in-depth analysis of the epidemiological characteristics influenced by factors such as altitude, genetics, and gender, as well as the pathological mechanisms triggered by chronic hypoxia, including alterations in gene expression, immune imbalance, and disorders of iron metabolism. In the section on clinical manifestations and diagnosis, the multisystem symptoms are elaborated in detail, diagnostic criteria based on hematological indicators combined with high-a

Microcytic anemia is among the most common hematological abnormalities in clinical practice and is usually attributable to iron deficiency, thalassemia traits, or anemia of inflammation. A small but clinically important subset of patients, however, has inherited disorders of iron metabolism or heme synthesis presenting with persistent, unexplained, familial, or iron-refractory microcytosis. Over the past decade, advances in the understanding of the hepcidin-ferroportin axis, erythroid iron regulation, standardized hepcidin assays, and next-generation sequencing have substantially refined diagn

Hereditary hypophosphatemic rickets/osteomalacia encompasses a group of disorders characterized by impaired bone mineralization resulting from phosphate deficiency, which can lead to skeletal deformities and growth retardation when onset occurs in early childhood. A patient with autosomal recessive hypophosphatemic rickets type 1 (ARHR1) caused by a mutation in the dentin matrix protein 1 (DMP1) gene was admitted to the Department of Endocrinology at the First Medical Center of Chinese PLA General Hospital in December 2024. The patient presented with frontal bossing, short stature, lower limb

Tumor-induced osteomalacia (TIO) is an acquired hypophosphatemic osteodystrophy caused by increased renal phosphate excretion due to tumors. Tenosynovial giant cell tumor (TGCT) is a benign mesenchymal tumor but rarely causes hypophosphatemia. TIO is a clinically rare disease, and its clinical manifestations often overlap with rheumatic diseases such as spondyloarthritis (SpA), leading to misdiagnosis. A 32-year-old female had a prolonged misdiagnosis of undifferentiated spondyloarthritis due to chronic low back pain, limb weakness, and spinal deformity, with poor response to non-steroidal ant

Autosomal recessive hypophosphatemic rickets type 2 (ARHR2) caused by biallelic ENPP1 mutations is a rare disorder with a broad phenotypic spectrum. We describe 3 affected siblings from a consanguineous family who presented with markedly heterogeneous clinical features. The proband exhibited classical signs of rickets with progressive lower-limb deformities, short stature, and elevated alkaline phosphatase. Her older sister demonstrated limited elbow extension, conductive hearing loss, and vascular stenoses, while the youngest sibling developed early biochemical abnormalities before overt skel

We report a rare case of a 60-year-old male patient with X-linked hypophosphatemic rickets (XLH) caused by a PHEX gene mutation complicated with polyostotic fibrous dysplasia (FD). The patient presented with bilateral lower limb deformity for 59 years and recurrent fractures for 30 years. Physical examination revealed short stature (113 cm), multiple skeletal deformities, and limited joint mobility. Laboratory investigations showed persistent hypophosphatemia (0.43 mmol/L), elevated alkaline phosphatase (262 U/L), and increased urinary phosphorus excretion. Imaging studies demonstrated both ty

CONTEXT: Cutaneous skeletal hypophosphatemia syndrome (CSHS) is an ultrarare disorder defined by epidermal and/or melanocytic nevi, mosaic skeletal dysplasia, and FGF23-mediated hypophosphatemia. As in other FGF23-mediated hypophosphatemia conditions, individuals with CSHS have renal phosphate wasting and inappropriately normal or frankly low 1,25-dihydroxyvitamin D levels with resultant hypophosphatemia leading to rickets and osteomalacia. Conventional therapy for FGF23-mediated hypophosphatemia consists of multiple daily doses of oral phosphate and active vitamin D.

BACKGROUND: X-linked hypophosphatemia (XLH) is the most common form of inherited rickets, resulting in short stature despite treatment with oral phosphate and active vitamin D. Detailed data of anthropometric parameters at birth, during infancy, and on the influence of an affected parent on outcome are lacking.
METHODS: In this prospective multicenter observational study, conducted from 1998 to 2023 in Germany, Austria, and Switzerland, body length, body weight, and head circumference were investigated in 198 children with XLH from birth until the age of 18 years, all being only on supplement

OBJECTIVES: Chronic kidney disease is associated with mineral and bone disorders, including abnormalities in calcium, phosphorus, parathyroid hormone, and vitamin D metabolism, as well as bone diseases like renal osteodystrophy. Although renal transplant can improve some bone mineral disorders, osteoporosis and altered bone metabolism remain important issues due to factors like corticosteroid use and persistent hyperparathyroidism. Bone biopsies are the gold standard for diagnosing bone disorders but are invasive, leading to the exploration of noninvasive methods such as bone turnover markers

Background: Inherited metabolic disorders (IMDs) are a heterogeneous group of genetic diseases, some of which require lifelong disease-specific dietary management. Children with phenylketonuria (PKU), organic acidemias (OAs), and glycogen storage diseases (GSDs) are particularly susceptible to nutritional imbalances and metabolic disturbances that may adversely affect bone metabolism and skeletal health. However, data regarding bone turnover markers and bone health in these pediatric IMD populations remain limited. Objective: This study aimed to evaluate bone metabolism by assessing biochemica

Recent advancements have led to the establishment of various animal models for chronic kidney disease-mineral and bone disorder (CKD-MBD), including 5/6 nephrectomy and drug induction. However, these models predominantly focus on kidney and vascular pathology, often neglecting skeletal lesions and exhibiting high mortality rates and unclear osteoporosis. Phosphorus plays a crucial role in bone metabolism, and prolonged high phosphorus intake can disrupt normal bone function. Our study introduces a novel CKD-MBD osteoporosis model using mice fed with adenine at three concentrations, combined wi

Tumor-induced osteomalacia is a rare acquired disorder of phosphate metabolism caused by fibroblast growth factor (FGF)23-secreting mesenchymal tumors. We report the case of a 39-year-old man with a 3-year history of progressive musculoskeletal pain, in whom unrecognized hypophosphatemia led to misdiagnoses, including osteoporosis and inflammatory musculoskeletal disorders, and consequently, to inappropriate management. Biochemical evaluation revealed persistent hypophosphatemia, elevated alkaline phosphatase levels, secondary hyperparathyroidism, and markedly increased serum FGF23 levels. Gal

Iron is an essential element as it participates in a wide variety of metabolic processes, including oxygen transport, deoxyribonucleic acid synthesis and electron transport. However, as iron can form free radicals, its concentration in body tissues must be tightly regulated because, in excessive amounts, it can lead to tissue damage. Disorders of iron metabolism are among the most common diseases of humans with diverse clinical manifestations. Iron deficiency is the most common mineral deficiency, affecting approximately 20% of the population. Patients may present with an iron deficiency witho

INTRODUCTION: Vitamin D-dependent rickets type 2A (VDDR2A) is a rare, autosomal recessive disorder caused by pathogenic variants of the VDR gene encoding the vitamin D receptor. It has been proposed to be a form of parathyroid hormone (PTH)-dependent rickets. Here, we describe in detail a girl with VDDR2A who developed a long-standing, tertiary hyperparathyroidism that did not prevent healing of the rickets nor normalization of hypophosphatemia.
CASE PRESENTATION: A girl who presented with seizures at 9 months of age was diagnosed with VDDR2A. She had poor growth, alopecia, severe hypocalcemi

STUDY QUESTION: Is serum phosphate linked with semen quality and reproductive hormones in infertile men?
SUMMARY ANSWER: Hypophosphatemia is a frequent finding in infertile men and is associated with lower number of motile sperm.
WHAT IS KNOWN ALREADY: Phosphate is available in fluid from all segments of the male reproductive tract in concentrations manyfold higher than in serum. However, the role of phosphate in male fertility is largely unknown.
STUDY DESIGN, SIZE, DURATION: This cross-sectional study included 1242 men referred due to infertility between January 2017 and May 2020 at the D

Rickets in children usually present with skeletal manifestations. However, they can also rarely present with extraskeletal manifestations, one of them being respiratory insufficiency. We present an unusual case of a girl in early childhood with respiratory insufficiency, which turned out to be due to the underlying vitamin D-dependent rickets (VDDR). The girl was born to third-degree consanguineous parentage and had progressive skeletal deformities, acquired loss of motor milestones, recurrent lower respiratory tract infections and florid signs of rickets. She was hospitalised for respiratory

Tumor-induced osteomalacia (TIO) is a rare acquired paraneoplastic syndrome caused by a mesenchymal tumor secreting a phosphaturic hormone called FGF23. Patients present with bone pain, fragility fractures and muscle weakness. Biochemical results show hypophosphatemia, raised serum alkaline phosphatase and reduced calcitriol. We report the case of a 44-year-old man who presented to the Emergency Departement with acute low back pain revealing extensive subchondral fractures between D2 and L5. Investigations showed partial Fanconi syndrome; nevertheless, he had profound hypophosphatemia, low 1,2

PURPOSE: Rickets is a rare bone disorder due to altered calcium, vitamin D, and phosphorus metabolism, caused by nutritional deficiencies or, in 13% of cases, genetic origin. Few data are available on an Italian cohort of rickets.
METHODS: Twenty-four patients with confirmed low serum phosphorus levels and reduced renal tubular phosphate reabsorption were recruited from different tertiary care centres over the last 5 years. Biochemical, clinical, and anamnestic data were also collected. DNA was extracted and subjected to targeted next-generation sequencing.
RESULTS: Twenty-four single-nucleo
