The cloning of extracellular Ca(2+)-sensing receptors from parathyroid and kidney: molecular mechanisms of extracellular Ca(2+)-sensing.

The parathyroid cell detects changes in the extracellular ionized calcium concentration (Ca2 + o) with exquisite sensitivity, but the mechanisms through which it senses Ca2 + o have remained obscure. Recently, we isolated a cDNA encoding a Ca2 + o-sensing receptor from bovine parathyroid using expression cloning in Xenopus laevis oocytes. The expressed receptor stimulates phospholipase C and has a pharmacological profile almost identical to that of the native receptor. Furthermore, its deduced amino acid sequence confirms that it belongs to the superfamily of G-protein-coupled receptors. Recep

Investigation of calcium disorders.

The advent of new two-site immunometric assays for intact parathyroid hormone (PTH) measurement has enhanced the interpretation of results in many patients under investigation for hypercalcemia. The aim of this article is to give practical advice on the use of the new intact PTH methods and other tests for the investigation of the more common disorders of calcium metabolism.

Reversible parkinsonism induced by hypercalcemia and primary hyperparathyroidism.

We describe a patient who had the recent onset of both primary hyperparathyroidism and parkinsonism. Surgical removal of a parathyroid adenoma was followed by spontaneous resolution of the parkinsonism. Hypoparathyroidism and other disorders of calcium metabolism are recognized causes of secondary parkinsonism, while hyperparathyroidism is not. We review the relevant literature, which supports the hypothesis that hypercalcemia may have induced cytotoxic changes in the basal ganglia of this patient.

[Functional morphology of parathyroid cells in disorders of calcium metabolism].

Cell composition of the parathyroid gland was studied under normal conditions, after the hypo- or hyperloading with calcium gluconate and at hyperfunction. Three types of parathyrocytes were established that probably reflect different stages of their life cycle: the first--poorly differentiated cell, the second--mature differentiated cell and the third--degenerated cell. The majority of cells were represented by the parathyrocytes of the 2nd type the varieties of which reflect a non-active (A-subtype) and active (B-subtype) phases of secretion. The number of A-cells is increased under the cond

Modified immunoradiometric assay of parathyroid hormone-related protein: clinical application in the differential diagnosis of hypercalcemia.

We have developed a sensitive, specific solid-phase immunoradiometric assay (IRMA) of parathyroid hormone-related protein (PTH-RP) with use of affinity-purified polyclonal immunoglobulins. Antibodies recognizing PTH-RP(37-74) are immobilized to a polystyrene bead to "capture" analytes from the sample; antibodies to epitopes within the 1-36 amino acid region of PTH-RP are labeled with 125I. This IRMA recognizes PTH-RP(1-74) and PTH-RP(1-86) equivalently, but does not detect N-terminal or C-terminal fragments of PTH-RP, intact human parathyrin (PTH), or fragments of PTH. PTH-RP is not stable in

[Perinatal calcium metabolism. Physiology and pathophysiology].

During pregnancy, calcium is continuously transferred directly from the maternal intestine to the fetal bone, a transfer that is mainly induced by the interrelated actions of the calcium-regulating hormones parathyroid hormone (PTH), 1,25-dihydroxyvitamin D (1,25(OH)2D) and calcitonin. It has recently been demonstrated in animals that PTH-related protein (PTHrP) is the fetal equivalent of PTH. Human PTHrP, originally described as a product of a human lung cancer cell line and implicated in the pathogenesis of humoral hypercalcemia of malignancy, is a protein with 141 amino acids, and it has bi

Determination of intact parathyrin by immunoradiometric assay evaluated in normal children and in patients with various disorders of calcium metabolism.

We report the reference values for intact parathyrin (PTH) measured by a two-site immunoradiometric assay (IRMA) during childhood. The study has been carried out in 215 healthy children and adolescents, ages 2.0 to 18.7 years. Some patients with altered mineral homeostasis were also studied to assess the sensitivity of the method in a clinical setting. Mean intact PTH concentrations were 30.8 (SD 9.6) ng/L; the median was 28.5 ng/L. Normal reference values were 16.0-59.0 ng/L (95% confidence interval). The distribution of intact PTH values was nongaussian. We found no significant variations be

Intact parathyroid hormone assay and total calcium concentration in the diagnosis of disorders of calcium metabolism in dogs.

Serum samples from eight dogs with primary hyperparathyroidism, seven dogs with hypercalcemic lymphosarcoma (hypercalcemia of malignancy), and four dogs with primary hypoparathyroidism were submitted to the Animal Health Diagnostic Laboratory at Michigan State University for intact parathyroid hormone (PTH) assay. When compared with the adjusted total serum calcium concentration, the intact PTH concentration was consistent with the correct diagnosis in all cases. Two dogs with hypercalcemic lymphosarcoma were mildly azotemic. In both of these cases the intact PTH concentration was consistent w

Disorders of calcium and bone metabolism.

Calcium levels in blood are maintained within narrow limits by parathyroid hormone, acting on kidney, bone and gut. Our understanding of the mechanisms of these actions has increased greatly in recent years, for example by identification of new hormones (eg 1,25 dihydroxy vitamin D) and by the study of isolated bone cells. Though most clinical disorders of calcium metabolism are now readily diagnosed and many are effectively treated, some serious problems still remain. Foremost amongst these are the early diagnosis of osteoporosis and the treatment of osteoporosis after fractures have occurred

Neurologic complications of endocrine diseases.

The endocrine system maintains a constant internal milieu for proper nervous system functioning. It is not surprising that abnormalities develop anywhere along the neuroaxis when disease of the endocrine organs occurs. Appreciating the neurologic signs and symptoms of endocrine disease is of utmost importance, given the relative ease in confirming the diagnosis, their excellent response to treatment, and the devastating consequences if not appreciated. This article reviews the findings seen in disorders of calcium metabolism, thyroid and adrenal disease, diabetes mellitus, and acromegaly.

Regulation of blood calcium in humans.

Serum calcium in humans is closely regulated, with a maximum excursion of values throughout the day of less than 6%. A number of hormonal controls serve to maintain the stability of the serum calcium value. The three most important hormones--PTH, 1,25-dihydroxyvitamin D, and calcitonin--act chiefly on three organs--intestine, bone, and kidney. A set of simple diagrams that summarize these control mechanisms is presented. Organ diagrams are used to indicate the normal fluxes of calcium (or other ions) and the sites of hormone action, while a feedback loop diagram summarizes the interactions amo

The use of an intact molecule parathyroid hormone assay in disorders of calcium metabolism.

We present the results obtained using a 1-84 human parathyroid hormone (PTH) assay in patients with abnormal calcium metabolism. A normal range was established in a series of healthy volunteers. Patients with surgically proven hyperparathyroidism (HPT) showed increased PTH levels which were separate from the normal range. Patients with probable hyperparathyroidism and milder hypercalcaemia showed a raised median PTH level but the range overlapped with normal. Patients with hypercalcaemia of malignancy showed reduced PTH levels and these patients were readily differentiated from those with prob

Two-site assay of intact parathyroid hormone in the investigation of primary hyperparathyroidism and other disorders of calcium metabolism compared with a midregion assay.

We compared the utility of measurements of serum intact human PTH-(1-84) and midregion human PTH-(44-68) in patients with disorders of extracellular calcium metabolism. Serum midregion PTH was determined by RIA, and serum intact PTH was measured by a sensitive and specific immunoradiometric two-site assay. The serum intact PTH concentrations in 70 patients with primary hyperparathyroidism were above the normal range in 69, and thus widely separated from the levels in 40 patients with hypercalcemia of malignancy, in whom serum intact PTH values were usually below normal. In contrast, both group
