Microenvironment shapes Staphylococcus aureus phenotypes in children with cystic fibrosis over time.

Staphylococcus aureus is a common pathogen in children with cystic fibrosis (CF), frequently preceding or coexisting with Pseudomonas aeruginosa. The impact of transient P. aeruginosa infection and therapeutic intervention on S. aureus phenotypic shaping remains poorly understood. We characterized 546 isolates from a longitudinal cohort of sixteen pediatric patients in CF follow-up, either monoinfected or coinfected, over eighteen months. Isolates were grouped into three phenotypic clusters. Monoinfected patients were enriched in a cluster with high pigment and biofilm, whereas transient P. ae

Design of Smart Antifouling Surfaces Using Enzyme-Responsive Surface Topographies.

Biofilm accumulation on polymeric surfaces remains a persistent challenge in biomedical, industrial, and environmental systems. Here, we report an enzyme-responsive antifouling surface that integrates particle-generated topography with biologically activated surface renewal. Microparticles of poly(ethylene terephthalate) (PET) and the chitinase-responsive polymer PHEVD were generated by mechanical abrasion and embedded into poly(dimethylsiloxane) (PDMS) to create polymer-composite surfaces with three-dimensionally distributed microparticles. PET served as a nonenzyme-responsive control while P

Changes in Gastrointestinal Symptoms Among Children With Cystic Fibrosis During 12 Months of Elexacaftor/Tezacaftor/Ivacaftor Therapy: A Swedish Multicentre Study.

AIM: Elexacaftor/Tezacaftor/Ivacaftor (ETI) has markedly improved clinical outcomes in cystic fibrosis (CF). However, its effects on gastrointestinal (GI) function, nutrition and pancreatic enzyme therapy in children remain insufficiently described. This study aimed to characterise GI-related changes during the first year of ETI treatment in Swedish children with CF. METHODS: In this prospective, multicentre study at three paediatric CF centres, children aged 6-17 years who initiated ETI were assessed at baseline, 6 and 12 months. Outcomes included GI symptoms and the use of GI-related medica

Actin cortex formation is required for integrin clustering but not activation during leukocyte adhesion.

Leukocyte recruitment is critical for tissue homeostasis and inflammatory responses. Chemokine-induced β2 integrin-dependent adhesion is the first and most critical step of leukocyte recruitment. Previous studies have shown that crosstalk between integrins and the actin cytoskeleton is vital for the migration of adherent leukocytes. However, whether and how the actin cytoskeleton contributes to integrin-dependent leukocyte adhesion remains poorly defined. As we recently demonstrated, a defect in integrin clustering, even in the presence of normal integrin activation, is sufficient to cause an

Maternal and neonatal outcomes in pregnancies with cystic fibrosis after lung transplantation or during elexacaftor/tezacaftor/ivacaftor (ETI) therapy: a single-center descriptive cohort.

BACKGROUND: Advances in cystic fibrosis (CF) therapy, particularly CFTR modulators such as elexacaftor/tezacaftor/ivacaftor (ETI), are reshaping reproductive outcomes for women with advanced CF. Historically, lung transplantation (LT) was the only means to achieve sufficient respiratory stability for pregnancy. This study provides real-world clinical data describing pregnancies in women with CF after LT and during ETI therapy. METHODS: We conducted a retrospective descriptive study of all pregnancies resulting in live births among women with CF followed at the Nantes University Hospital CF re

AX-35 Targets the Cytochrome bc1 Complex in Mycobacterium abscessus and Reveals a Novel Resistance Mechanism.

In patients with cystic fibrosis, nontuberculous mycobacteria are opportunistic, intracellular and extracellular pulmonary pathogens that contribute to deterioration of lung function and, ultimately, morbidity from respiratory failure. Among nontuberculous mycobacterial respiratory pathogens, members of the Mycobacterium abscessus complex pose additional treatment complications as they are notoriously drug-resistant. As part of our routine screening against mycobacterial species, one of the lead antituberculosis compounds, the cytochrome bc1 inhibitor AX-35, was found to be active against isol

Paediatric palliative care in advanced cystic fibrosis: from prognosis-based to needs-based care across the disease trajectory.

Advances in cystic fibrosis care, particularly highly effective cystic fibrosis transmembrane conductance regulator modulator therapies, have transformed disease trajectories for many children and adolescents, improving health and survival. However, advanced disease, symptom and treatment burden, psychosocial distress, and uncertainty persist, requiring continued attention to palliative care needs. This narrative review examines palliative care in children and adolescents with advanced cystic fibrosis, focusing on multidimensional symptom burden, communication, shared decision-making, advance

Respiratory microbiome dysbiosis in STAT3 hyper-IgE syndrome (Job syndrome).

STAT3 hyper-IgE syndrome (STAT3-HIES), historically known as Job syndrome, is a rare primary immunodeficiency caused by dominant-negative mutations in STAT3. STAT3-HIES is characterized by impaired mucosal immunity, connective tissue abnormalities, and recurrent cutaneous and pulmonary infections. Although STAT3-dependent immune and epithelial pathways are central to airway host defense, the respiratory microbiome in STAT3-HIES remains poorly defined. To address this question, we profiled upper and lower respiratory tract microbiomes using 16S rDNA gene sequencing of oral rinse samples from ei

p66Shc mediates SUMO2-induced endothelial dysfunction.

Sumoylation is a post-translational modification that can regulate different physiological functions. Increased sumoylation, specifically conjugation of SUMO2/3 (small ubiquitin-like modifier 2/3), is detrimental to vascular health. However, the molecular mechanism mediating this effect is poorly understood. We used cell-based assays and mass spectrometry to show that p66Shc is a direct target of SUMO2 and that SUMO2 regulates p66Shc function via lysine-81 modification. To determine the effects of SUMO2-p66ShcK81 on vascular function, we generated Shc1K81R mice and crossbred them toLdlr-/- mic

Longitudinal changes in bone mineral density among adults with cystic fibrosis: a systematic review and meta-analysis.

BACKGROUND: Improved survival in cystic fibrosis (CF) has increased the burden of cystic fibrosis-related bone disease (CFBD), yet the natural history of bone mineral density (BMD) decline remains poorly defined. We conducted a systematic review and meta-analysis (PROSPERO: CRD420251091118) to quantify longitudinal BMD changes in adults with CF. METHODS: MEDLINE, Embase, Cochrane, and PubMed were searched through March 22, 2025 for observational studies and control arms of randomized trials reporting ≥2 dual-energy X-ray absorptiometry (DXA) scans ≥1 year apart in adults not receiving osteopo

Adverse events during cystic fibrosis transmembrane conductance regulator modulator therapy in children with cystic fibrosis: an observational study.

BACKGROUND: Cystic fibrosis (CF) is a life-limiting genetic disease caused by pathogenic variants in the CFTR gene. CFTR modulators (CFTRms) have substantially improved clinical outcomes, but real-world data on symptoms reported during therapy in pediatric populations remain limited. This study evaluated the frequency and distribution of adverse events (AEs) reported during 12 months of CFTR modulator (CFTRm) therapy in pediatric and adolescent patients with CF. METHODS: This prospective, dual-center observational study conducted in Poland included 74 patients aged 2-18 years receiving lumaca

Young adults' understanding of, and reaction to, those with uncertain genetic results.

BACKGROUND: Genetic advances increase the likelihood that children may receive uncertain diagnoses such as cystic fibrosis screen positive inconclusive diagnosis (CFSPID) from newborn screening. Research suggests a profound impact on parents. Research is needed to understand the impact on the child, especially at key developmental points such as young adulthood, when identity forms. As young adults (YA) are influenced by peer reactions, this novel study explored YAs' sensemaking of CFSPID, and their hypothetical reactions to peers or romantic partners with CFSPID. METHODS: Four focus groups w

Dosing and delivery of bacteriophage therapy in a murine wound infection model.

Lytic bacteriophages, viruses that lyse (kill) bacteria, hold great promise for treating infections, including wound infections caused by antimicrobial-resistant Pseudomonas aeruginosa. However, dosing and delivery strategies for phage therapy remain underdeveloped. In a mouse wound infection model specifically designed to evaluate the treatment of established infections-a critical gap in existing preclinical models-we investigated the impact of administration route, dose, and frequency on phage therapy efficacy. In this model, topical, but not systemic, delivery reduced wound bacterial burden
