MicroRNAs during Echinococcus spp. infection: a comprehensive systematic review and implications for diagnosis and therapy.

BACKGROUND: MicroRNAs (miRNAs) are a subclass of small non-coding RNAs that regulate gene expression and play important roles in infectious diseases. Echinococcosis is a globally distributed disease caused by various species of Echinococcus. The disease can be asymptomatic in the early stages, but if left untreated, it can lead to severe complications. Like other helminths, Echinococcus spp. exhibit immunomodulatory capacities that favour infection establishment and progression. Currently, there is no fully accurate, non-invasive method for early detection of echinococcosis. This systematic re

Roflumilast mitigates cartilage degeneration in osteoarthritis by activating CFTR-mediated anti-apoptotic signaling.

Osteoarthritis (OA) is a chronic degenerative joint disorder marked by progressive cartilage loss, synovial inflammation, and remodeling of the subchondral bone. Although roflumilast, a selective phosphodiesterase-4 inhibitor, has shown clinical benefits in chronic inflammatory diseases, its potential role in OA has not been defined. Here, we examined whether roflumilast can protect chondrocytes against apoptosis and extracellular matrix (ECM) degradation under inflammatory stress. In IL-1β-stimulated primary chondrocytes, roflumilast pretreatment (20 µM) suppressed the expression of matrix-de

Non-eligibility for highly effective modulator therapy with elexacaftor/tezacaftor/ivacaftor in cystic fibrosis: What about Portugal?

BACKGROUND: Cystic fibrosis (CF) is an autosomal recessive disease caused by mutations in cystic fibrosis transmembrane conductance regulator (CFTR) gene. CFTR modulators are drugs developed to target the defective protein. All over the world, there are patients considered non-eligible for this therapy according to local regulatory policies. METHODS: To characterise the Portuguese CF patients defined as non-eligible according to genotype for highly effective modulator therapy (HEMT) with elexacaftor/tezacaftor/ivacaftor (ETI), the authors conducted a multicentre observational study based on a

Bacteriophage Therapy in Cystic Fibrosis: Emerging Strategies for Managing Chronic Pseudomonas aeruginosa Infections.

Pseudomonas aeruginosa is the leading cause of chronic respiratory infection in people with cystic fibrosis (pwCF) and remains a major driver of morbidity and premature mortality. The unique environment of the CF lung promotes persistent colonisation, biofilm formation, and the emergence of multidrug-resistant (MDR) phenotypes, severely limiting the effectiveness of conventional antibiotic therapies. Despite aggressive and prolonged antibiotic regimens, eradication of chronic P. aeruginosa infection is rarely achieved, and repeated exposure accelerates resistance development. Consequently, the

Late-Diagnosed Cystic Fibrosis with Co-Occurring CFTR 3849+10kbC>T and 5T Variants: A Case Report.

Background: The deep-intronic variant 3849+10kbC>T preserves residual CFTR function and is recurrently associated with pancreatic sufficiency, normal or borderline sweat chloride and diagnosis beyond early childhood. A co-occurring poly-T 5T allele adds a second layer of uncertainty: its penetrance depends on the TG repeat carried in cis, and its contribution to the phenotype can be weighed only once its phase relative to the second variant has been established. Case Presentation: A boy with recurrent respiratory infections from the age of three was diagnosed with cystic fibrosis at 12 years,

Tracking eco-evolutionary patterns among oropharyngeal pathobiome members in a pediatric cystic fibrosis cohort.

UNLABELLED: Polymicrobial airway colonization is common in individuals with cystic fibrosis (CF). Pathogen communities typically follow an ecological succession in which early colonizers, such as Haemophilus influenzae and Staphylococcus aureus, are later joined by Pseudomonas aeruginosa. Although adaptation to the lung environment is well described for single pathogens in adult people with CF, much less is known about the role of pathogen interactions and how changes in individual pathogens influence community dynamics at the early stages of disease. To address these questions, we combined ge

Beyond Digestion: Secretin as a Key Regulator of Systemic pH.

Secretin is a broad activator of epithelial bicarbonate secretion and regulates local and systemic pH balance along with metabolic functions. Its receptor, widely expressed across epithelial and neuroendocrine tissues, enables coordinated responses to digestive phases, systemic alkalosis, and volume regulation. This positions secretin as a corrective hormone of the postprandial alkaline tide and a critical signaling pathway in the prevention of excessive systemic alkalosis. Its signaling intersects with the renin-angiotensin-aldosterone system, where secretin, angiotensin II, and aldosterone j

Constrictive pericarditis in cystic fibrosis: a case report of an emerging cardiovascular complication successfully managed with off-pump pericardiectomy.

BACKGROUND: Constrictive pericarditis (CP) is rarely reported in people with cystic fibrosis (CF) and may be overlooked because dyspnoea, fatigue, oedema, and reduced exercise tolerance overlap with pulmonary exacerbation. As survival improves with highly effective CF transmembrane conductance regulator modulator therapy, cardiovascular causes of cardiorespiratory symptoms may become increasingly relevant in adults with CF. This case highlights the importance of recognizing CP in this population and individualizing surgical decision-making. CASE SUMMARY: A 33-year-old woman with homozygous F5

Caustic ingestion in children: A prospective analysis of poor outcome key predictors useful for clinical management optimization.

OBJECTIVES: Endoscopic examination is the main diagnostic test to evaluate the extent and severity of any mucosal lesions after caustic ingestion, in order to target the most appropriate treatment and follow-up. Our main aim was to evaluate clinical, biochemical, and instrumental parameters in order to identify possible key predictors of poor outcome and therefore identify possible subgroups of asymptomatic or paucisymptomatic children who may benefit from the endoscopic examination. METHODS: All children admitted for caustic ingestion were prospectively enrolled. Poor outcome was defined as

Environmental Exposures, Chronic Sinusitis, and Olfaction in People With Cystic Fibrosis.

BACKGROUND: Environmental air pollutants may contribute to chronic rhinosinusitis (CRS), but their relationship with upper airway disease in people with cystic fibrosis (CF) remains poorly understood. This study evaluated associations between long-term ambient air pollutant exposure and CRS severity and olfactory dysfunction in adults with CF and CRS. METHODS: 80 adults with CF and comorbid CRS were prospectively enrolled from 2019 to 2023 at multiple U.S. academic rhinology centers. Long-term exposure to PM2.5, PM10, CO, SO2, NO2, and ozone was estimated using the nearest U.S. Environmental

Assessment of Cystic Fibrosis Lung Disease Using Electrical Impedance Tomography.

INTRODUCTION: Electrical impedance tomography (EIT) is a non-invasive, effort-independent imaging technique for measurement of regional lung ventilation and function that does not utilize ionizing radiation. There are limited data on the use of EIT to assess lung disease in people with CF (PwCF). METHODS: This was a cross-sectional observational cohort study. Study participants performed EIT, multiple breath washout (MBW) and spirometry in a single session. We analyzed EIT results to calculate the global inhomogeneity index (GII) and regional ventilation delay at 40% of maximal inspiratory im

Calcimimetic therapy modified the radiological, surgical, and histopathological features of a parathyroid adenoma.

Calcimimetics are increasingly used to control hypercalcemia before parathyroidectomy. Although treatment-induced degenerative changes have been reported in parathyroid adenomas, their effects on preoperative imaging and surgical planning remain unclear. A 70-year-old woman with primary hyperparathyroidism was administered preoperative evocalcet (2 mg/day). After ∼2 months, serum calcium levels decreased to the hypocalcemic range, prompting drug discontinuation before surgery referral. Upon presentation, corrected serum calcium normalized to 9.1 mg/dL (SI: 2.28 mmol/L; reference range, 8.8-10.

Microencapsulation of an amphotericin B-solubilising mucolytic deep eutectic solvent for enhanced oral delivery to treat cystic fibrosis.

This study aimed to develop an innovative oral formulation of Amphotericin B (AmB) for the treatment of intestinal symptoms related to cystic fibrosis (CF) using microcapsules produced via the prilling/vibration technique. The use of a mucolytic Deep Eutectic Solvent (DESolve) aimed to improve the solubility of AmB and its diffusion through the mucus barrier by reducing its viscosity. Diffusion tests in purified intestinal mucus and mucin confirmed the ability to penetrate the thick mucus barrier typical of CF, enhancing drug availability at the target site. A phase 2a study with SPL84: A novel RNA-targeted approach to restore CFTR function in people with CF carrying the 3849+10 kb C→T mutation.

BACKGROUND: The 3849+10 kb C→T mutation in the Cystic Fibrosis Transmembrane Conductance Regulator (CFTR) gene leads to aberrant splicing and the inclusion of a cryptic exon harboring a premature stop codon, resulting in the production of non-functional CFTR protein. SPL84 is an inhaled antisense oligonucleotide (ASO) designed to correct CFTR splicing and restore functional CFTR expression in people with CF (pwCF) carrying this mutation. METHODS: This Phase 2a randomized, placebo-controlled, double-blind, multiple ascending dose study evaluated safety, tolerability, pharmacokinetics, and expl

Model-informed dose reduction of elexacaftor/tezacaftor/ivacaftor therapy in adult people with cystic fibrosis: A simulation study.

BACKGROUND: Most people with cystic fibrosis (pwCF) use elexacaftor/tezacaftor/ivacaftor (ETI) therapy. While generally efficacious, some pwCF experience adverse events. This is typically managed by empirical dose reduction, but the optimal regimen remains unknown. We performed a population pharmacokinetic (PK) study incorporating a simplified exposure effect approximation to evaluate ETI dose reduction strategies. METHODS: ETI exposure under five dose reduction strategies was simulated using available population PK models. Standard dose ETI PK was simulated, from which individuals were selec

Assessment of CFTR Dysfunction and Responsiveness to CFTR Modulators in COPD Bronchial Air-Liquid Interface Cultures.

Increasing evidence suggests that acquired dysfunction of the cystic fibrosis transmembrane conductance regulator (CFTR) anion channel occurs as a result of cigarette smoke exposure in chronic obstructive pulmonary disease (COPD). CFTR-targeted therapies were developed to treat genetic CFTR defects in cystic fibrosis, but they have not demonstrated consistent clinical efficacy in small trials of patients with COPD. Here, we aimed to characterize CFTR and other ion channel activity and ciliation in primary bronchial epithelial cells (pBECs) from COPD donors compared with healthy controls, and t

The impact of modulator therapy on the Pseudomonas aeruginosa cystic fibrosis lung transcriptome.

Chronic lung infection with Pseudomonas aeruginosa is a major complication of cystic fibrosis (CF), contributing to lung function decline and increased morbidity and mortality. Laboratory models are used to study CF infections, but their relevance depends on how accurately they reproduce microbial physiology during human infection. We previously developed a framework that uses bacterial gene expression in human infections to quantify model accuracy. Here, we applied this framework to determine how a major therapeutic intervention impacts P. aeruginosa physiology in the lungs of people with CF

CFTR Functional Rescue and Heterogeneous Clinical Outcomes Following Elexacaftor/Tezacaftor/Ivacaftor Therapy in People with Cystic Fibrosis Carrying the W1282R Variant.

The clinical response to elexacaftor/tezacaftor/ivacaftor (ETI) in people with cystic fibrosis carrying the rare cystic fibrosis transmembrane conductance regulator (CFTR) variant W1282R has not been well characterized. We retrospectively analyzed 12 people with cystic fibrosis treated with ETI for 3-36 months in addition to ongoing standard CF therapy.
