CFTR variant-associated hereditary pancreatitis complicated by diabetes mellitus: a case report and literature review.

Hereditary pancreatitis (HP) is a rare autosomal dominant disorder characterized by recurrent episodes of pancreatic inflammation leading to chronic pancreatic damage. We report a 38-year-old Chinese male with a heterozygous cystic fibrosis transmembrane conductance regulator (CFTR) intronic variant (c.1210-11T>G) who presented with pancreatic diabetes (type 3c diabetes) and an otherwise "silent" pancreatic phenotype. He had a family history of similar manifestations (father and brother with pancreatogenic diabetes). Genetic testing identified the CFTR c.1210-11T>G variant, which is known to a

Prenatal CFTR modulator therapy for fetal cystic fibrosis: Emerging evidence, clinical considerations, and future directions.

BACKGROUND: The consequences of cystic fibrosis (CF) transmembrane conductance regulator (CFTR) protein dysfunction or absence begin during fetal development, with pancreatic, intestinal, hepatobiliary, and reproductive manifestations evident at birth. The advent of CFTR modulator therapies (CFTR modulator)-most notably elexacaftor/tezacaftor/ivacaftor (ETI), approved for children and adults with CF who carry responsive variants, and the recently approved vanzacaftor/tezacaftor/deutivacaftor (VTD), currently indicated for older pediatric and adult populations-has transformed long-term health o

Cystic Fibrosis beyond Childhood: A Case Report of Recurrent Heat-Induced Electrolyte Disturbances Unmasking a Late Diagnosis.

INTRODUCTION: Cystic fibrosis (CF) is an autosomal recessive genetic disorder caused by pathogenic variants in the CFTR gene, leading to impaired chloride and bicarbonate transport across epithelial tissues. While typically diagnosed in infancy due to classic respiratory and gastrointestinal symptoms, atypical forms can present later in life with subtle or isolated manifestations. This case represents a rare monosymptomatic and atypical adult-onset presentation of CF, with electrolyte disturbances as the sole clinical manifestation. CASE PRESENTATION: We present the case of a 31-year-old man

Chitayat Syndrome: A Rare Case of Respiratory Distress in a Preterm Infant.

Childhood interstitial lung diseases (chILDs) are rare respiratory conditions with significant mortality rates in neonates. They can be misdiagnosed as the symptoms overlap with other, more common neonatal diseases. Since chILD can be a manifestation of the underlying syndrome, suggestive physical findings, genetic testing, and practitioners' notion can contribute to better recognition, as in our patient with confirmed Chitayat syndrome. A male preterm infant born at 34 weeks of gestation showed signs of progressive respiratory distress from birth with lung hyperinflation and ground glass opac

Prenatal initiation of elexacaftor/tezacaftor/ivacaftor via carrier mother prevents congenital bilateral absence of vas deferens in a male infant with cystic fibrosis.

BACKGROUND: Prenatal therapy with elexacaftor/tezacaftor/ivacaftor (ETI) has recently emerged as a potential strategy to modify the early natural history of cystic fibrosis (CF). While case reports have described prevention of meconium ileus and pancreatic insufficiency, data on male reproductive outcomes remain extremely limited. CASE PRESENTATION: We report a male infant with CF (homozygous F508del) whose heterozygous carrier mother initiated ETI at 27+4 weeks of gestation after prenatal diagnosis. Pregnancy was uneventful until preterm delivery at 35+2 weeks. The neonate presented with nor

[Allergic bronchopulmonary aspergillosis in internal medicine].

Allergic bronchopulmonary aspergillosis (ABPA) is a rare respiratory disease. Unlike other pulmonary aspergilloses resulting from an invasive infection by Aspergillus fumigatus, ABPA is a symptomatic colonization due to the allergic and inflammatory reaction. The presence of an underlying bronchial disease and an atopic background predispose to the development of ABPA. The most common clinical manifestation is a productive cough with excretion of bronchial mucus, but other presentations are possible, including poorly controlled asthma with corticosteroid dependence. Diagnosis is based on a com

Renal Tubule-Specific Deletion of Nephrocystin 3 (Nphp3) Causes Infantile Nephronophthisis-like Phenotypes in Mice.

Patients with nephronophthisis caused by nephrocystin 3 (NPHP3) variants rapidly progress to end-stage kidney disease. However, existing Nphp3 mouse models fail to fully recapitulate the characteristics of this disease. We generated a renal tubule-specific Nphp3 knockout mouse model that more accurately mirrors the human disease course. The mouse model was first validated by confirming the loss of Nphp3 protein expression in renal tubules. Comprehensive phenotypic analyses were then performed to assess both renal and extrarenal manifestations. The origin of renal cysts was investigated, and th

[Research advances in the diagnosis and treatment of Polycystic kidney disease].

Polycystic kidney disease (PKD) is a group of inherited disorders characterized by cystic lesions in the kidneys and multiple organs, primarily including autosomal dominant polycystic kidney disease (ADPKD) and autosomal recessive polycystic kidney disease (ARPKD). ADPKD is mainly caused by variations in the PKD1 and PKD2 genes. Its clinical manifestations include progressive renal cyst growth, hypertension, and multi-system complications. ARPKD, on the other hand, is primarily caused by mutations in the PKHD1 gene. It commonly occurs in infants and young children, with hepatorenal cystic fibr

Inflammation-induced cysts mimic honeycombing on high-resolution computed tomography in rheumatoid arthritis-associated usual interstitial pneumonia.

BACKGROUND: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) often manifests with a usual interstitial pneumonia (UIP) pattern and has a poor prognosis similar to idiopathic pulmonary fibrosis (IPF). Despite radiological similarity to IPF, RA-UIP typically shows greater inflammation. This study aimed to investigate the formation of cysts mimicking honeycombing radiologically in RA-UIP. We hypothesized that the pathogenesis of these cystic structures may differ from that of IPF. METHODS: We retrospectively analyzed patients with rheumatoid arthritis who had a UIP/probable UIP

Bayesian Inferential Analysis of Genetic Risks Based on Family History.

Classical statistical analysis is a frequently employed methodology in numerous domains of genetic research. In recent times, however, there has been a notable increase in the interest accorded to the deployment of Bayesian statistics in the field of genetics, as it incorporates a priori hypotheses about genetic knowledge into the problem. The potential risk of developing a genetic disease is influenced by the patient's genetics, ethnicity, gender, age, and family history. The objective of this study is there[Formula: see text]fore to provide molecular pathologists working with genetic testing

[Expert consensus on the diagnosis and treatment of allergic bronchopulmonary aspergillosis with integrated traditional Chinese and western medicine (2026 edition)].

Allergic bronchopulmonary aspergillosis (ABPA) is a hypersensitivity lung disease triggered by sensitization to Aspergillus fumigatus or other Aspergillus species, with 1.0%-3.5% incidence in asthma patients and 7%-15% incidence in cystic fibrosis patients. It is more common in patients with asthma or cystic fibrosis. Clinical manifestations include recurrent bronchial obstruction, cough, pulmonary infiltrates, and bronchiectasis. However, its actual incidence is likely underestimated due to complex diagnostic criteria and insufficient clinical awareness. To standardize the integrated traditio

Progress towards regenerative therapies for the cystic fibrosis lung and pancreas: insights from stem cells and organoids.

Cystic fibrosis (CF), caused by mutations in the CFTR gene, is characterised by progressive dysfunction in multiple organs. The lung and pancreas largely define patient morbidity and survival, even in the era of highly effective modulator therapies. In the pancreas, CFTR activity loss obstructs exocrine ducts, leading to acinar atrophy and secondary β-cell dysfunction, culminating in CF-related diabetes. In the lung, impaired CFTR-mediated ion transport disrupts mucociliary clearance, driving chronic infection, inflammation, and airway remodelling. Despite these organ-specific manifestations,

Robotic Heminephrectomy for Xanthogranulomatous Pyelonephritis in a Pediatric Patient.

Xanthogranulomatous pyelonephritis (XGP) is a rare, chronic, destructive inflammatory condition of the kidney characterized by progressive renal parenchymal loss and replacement with lipid-laden macrophages, granulomatous inflammation, and fibrosis. In adults, XGP most commonly affects middle-aged women and is typically associated with urinary tract obstruction, nephrolithiasis, and recurrent infection. In contrast, pediatric XGP is exceptionally uncommon, with reported cases largely limited to isolated case reports and small case series. Pediatric presentations are frequently atypical, often

Further Genetic Unraveling of Persistent Tachypnea of Infancy.

BACKGROUND: Childhood interstitial lung diseases (chILDs) are rare, heterogeneous chronic pulmonary disorders that are often underdiagnosed due to their low prevalence and nonspecific clinical presentation. Persistent tachypnea of infancy (PTI), often referred to as neuroendocrine cell hyperplasia of infancy (NEHI), is 1 of the most frequent forms of chILD, although its underlying etiology remains unknown. RESEARCH QUESTION: Can comprehensive genetic testing significantly improve diagnostic yield in patients with PTI/NEHI compared with limited testing of established genes? STUDY DESIGN AND M

Impact of Genetics on Endocrine Manifestations of Cystic Fibrosis.

Cystic fibrosis (CF) is a monogenic multisystem disorder. Endocrine complications of CF are common and include diabetes, growth disorders, and bone disease among others. CF endocrinopathies may be influenced by 2 classes of genetic factors: (1) disease-causing cystic fibrosis transmembrane receptor (CFTR) variants that determine the level of CFTR dysfunction, and (2) variants in other genes throughout the genome known as genetic modifiers. This review highlights the impact of these genetic factors on selected endocrine manifestations of CF. CF-related diabetes (CFRD): CFRD is dependent on both

Pancreatic cystic lesions in hereditary syndromes: Diagnostic role of endoscopic ultrasound.

Pancreatic cystic lesions (PCLs) may occur in several hereditary syndromes, although their frequency, phenotype, and clinical implications differ considerably across entities. This review summarizes the spectrum of hereditary-associated PCLs, focusing on von Hippel-Lindau syndrome, autosomal dominant polycystic kidney disease, cystic fibrosis, multiple endocrine neoplasia type 1, and other hereditary pancreatic cancer predisposition syndromes. The lesions most commonly encountered are simple cysts, serous cystic neoplasms, and intraductal papillary mucinous neoplasms, whereas cystic pancreatic

Nucleic acid-based therapeutic strategies for modulator-refractory cystic fibrosis-causing variants.

Cystic fibrosis (CF) is a life-limiting autosomal recessive disorder caused by pathogenic variants in the CF transmembrane conductance regulator (CFTR) gene that impair epithelial chloride ion transport, leading to progressive lung dysfunction among other systemic manifestations. Small molecule CFTR modulators have dramatically improved outcomes for people with CF (pwCF) by promoting the trafficking of defective CFTR protein to the cell surface and enhancing its activity. However, a substantial proportion of pwCF harbor genotypes which remain refractory to CFTR modulators and thus lack approve

Complex Genomic Rearrangement Involving the TBX4 Promoter Manifesting With Variable Expressivity in a Five-Generation Family.

Variants involving TBX4 are associated with ischiocoxopodopatellar syndrome with or without pulmonary arterial hypertension (ICPPS; also known as small patella syndrome), pulmonary arterial hypertension (PAH), and lethal lung developmental disorders. The variability of penetrance and expressivity of TBX4 variants remains a prominent challenge in understanding their genotype-phenotype correlations. We investigated a five-generation family with 12 affected individuals presenting with isolated ICPPS, lung-related manifestations with features of ICPPS, and other milder abnormalities. Whole-genome

BACKGROUND: Elexacaftor/tezacaftor/ivacaftor (ETI) has significantly improved lung function and the clinical course of people with cystic fibrosis (PwCF) but also can cause elevations in liver function tests (LFTs). While the prescribing information (PI) provides some recommendations for holding ETI or hepatically adjusting doses based on certain criteria, clinicians are often faced with clinical scenarios that do not meet those criteria (e.g. PwCF with CF liver disease [CFLD, now termed CF hepatobiliary involvement (CFHBI)] that do not meet Child-Pugh Class B or C criteria).
