Cervicofacial actinomycosis after partial mandibular resection: case report.

An unusual case of cervicofacial actinomycosis in a 60-year old male Nigerian is reported. The patient had had partial mandibular resection for plexiform ameloblastoma one year earlier with insertion of Kirschner wire to serve as a splint. The authors highlight the possible pathophysiology of this condition. It is suggested that clinicians be aware of the various modes of presentation and natural history of the disease to facilitate prompt diagnosis and appropriate treatment. This will assist in forestalling the numerous complications that may result from actinomycosis.

[Cervicofacial actinomycosis].

Actinomycosis is an infectious disease that appears throught the world, in cattle and human beings. It is a chronic granulomatous and suppurative lesion, usually affecting the cervicofacial area. Definitive diagnosis, by culture or biopsy, is not always possible, so the clinician must suspect it by its clinical presentation. It is the purpose of this article to present the patients diagnosed and treated in our Department during the last 5 years, and to review the ideal therapy.

Cervicofacial actinomycosis: a diagnostic challenge.

Actinomycosis was first described as a clinical entity over 100 years ago. However, the fundamental characteristics of this entity have not been fully discussed, and major questions remain unanswered, such as the highly diversified pathogenicity of the phenomenon according to numerous published case reports and clarification of solid diagnostic criteria. Even the frequency of cervicofacial actinomycosis occurrence is unclear; some authors consider it to be rare and others to be common. We present 11 cases examined and treated in our department within the last 14 years along with a review of th

Cervicofacial actinomycosis with paravertebral spread: a case report.

We report a case of cervicofacial actinomycosis with paravertebral extension in a 60-year-old man who presented with recurrent neck masses. Diagnosis was confirmed on culture and histology of pus and debris obtained from surgical drainage. He improved only after lengthy in-hospital high dose penicillin therapy. He is currently well and is on maintenance doxycycline therapy for 6 months following the high dose penicillin therapy.

[Actinomycosis of the tonsils with a pseudotumoral presentation: a clinical case].

A case of actinomycosis of the tonsils with a neoplasm-like clinical presentation is presented. Tonsillectomy confirmed the diagnosis and the patient's evolution was favorable after postoperative antibiotic therapy. The scant literature on cervicofacial actinomycosis is reviewed. These processes often are confused with neoplasms, as in our case, or with mandibular osteomyelitis. Treatment is surgery and prolonged antibiotic administration. Actinomycosis should be considered in the differential diagnosis of cervicofacial masses, particularly when the biopsy is negative for carcinoma.

[Actinomycosis in otorhinolaryngology. Apropos of a case with localization in the nasal cavity].

The authors report an atypical case of actionomycosis implanted in the nasal cavity and occurring twenty years after septum surgery. Treatment by penicillin and surgical excision gave a good result. Cervicofacial actinomycosis is caused by actinomyctes which usually lives as a saprophyte in the oral cavity. A trauma is often found in the previous history. Diagnosis with tumors and abscess may be difficult. The histopathological examination shows typical aspects of the granuloma (gram+ and Grocott+) and the bacteriological isolation of the germ is difficult to obtain. Penicillin associated with

[Cervicofacial actinomycosis in a patient treated by surgery of the lymph node due to laryngeal cancer: case report].

An actinomycotic cervical abscess was excised in a 55-year-old man who had undergone partial laryngectomy and ipsilateral cervical lymph node dissection 6 years earlier. This rare case is described in detail. We conclude that postoperative structural changes producing the loss of physical and immunologic barriers may be determinant factors in the development of this late actinomycotic infection.

Cervicofacial actinomycosis presenting as acute upper respiratory tract obstruction.

An unusual case of cervicofacial actinomycosis presenting as acute upper airway obstruction and demanding urgent tracheostomy is reported. Diagnosis was established by microscopic examination of the pus and culture of Actinomyces israelii. Repeated surgical drainage of the purulent foci and prolonged treatment with penicillin obtained resolution of the disease. Clinicians dealing with acute head and neck swellings should always consider actinomycosis as a possible diagnosis.

Cervicofacial actinomycosis (a case report).

Actinomycosis is now a very uncommonly diagnosed human disease. Major medical centers report approximately one case a year. The pathogenesis of actinomycosis is unclear, but trauma provides a portal of entry. The cervicofacial form is the most common and usually appears as a chronic swelling with one or more draining sinus tracts. For treatment, penicillin in high doses is suggested. This case report presents a case of cervicofacial actinomycosis in a 14 year old boy which was misdiagnosed for a long time and the treatment of the disease with mezlocillin, a new antibiotic in the treatment of a

Cervicofacial actinomycosis: report of two cases.

This article presents two cases of actinomycosis. Case 1 was a 39-year-old man who was first seen with the chief complaint of swelling around the left submandibular region. Case 2 was a 40-year-old woman who was first seen with the chief complaint of mass formation around the left buccal area. Both cases were initially diagnosed as malignant tumors and were later histopathologically interpreted as actinomycosis because of the presence of sulfur granules.

Cervicofacial actinomycosis resembling a ruptured cyst.

Actinomycosis, a chronic suppurative bacterial infection characterized by draining sinuses that discharge "sulfur granules," can mimic numerous other infectious and noninfectious conditions. We describe a 33-year-old woman with cervicofacial actinomycosis, which, on initial evaluation, was clinically and histologically diagnosed as a ruptured cyst.

[Actinomycosis of the central nervous system: a rare complication of cervicofacial actinomycosis].

Central nervous system actinomycosis is a rare but treatable chronic suppurative bacterial infection. The case of a young immunocompetent male with actinomycosis of the CNS is presented. The abscess originated from a primary cervico-facial infection and was located in the left parasellar region. After excision of the mass, that showed Actinomyces colonies, the patient was treated with intravenous Penicillin for 42 days followed by oral administration of the drug for 30 days. After surgery the patient was left with mild sequelae that had improved by the last follow-up, 7 months later. A new CT

Pediatric cervicofacial actinomycosis.

It is well recognized that cervicofacial actinomycosis is rare in children. We have summarized the cases in the English language literature and reported a recent case from our hospital. In only 10% of clinical situations is actinomycosis the primary diagnosis. Failure to consider actinomycosis in the differential diagnosis of a cervical mass may compromise the ability to make the diagnosis, given the fastidious nature of the most common pathogen, Actinomyces israelii. Indeed, the recovery rate of only 30% in some of the earlier studies of actinomycosis may have been in part due to this oversig

Multimodality imaging of cervicofacial actinomycosis.

Actinomycosis is an uncommon chronic disease usually caused by Actinomyces israelii. It affects the soft tissue mainly but sometimes spreads to involve salivary glands, bone, or even the skin of the face and neck. Five cases have been seen in our department. Several imaging modalities were used to assist in making the diagnosis. The cases are presented and the literature reviewed. Ultrasonography was found to be a useful diagnostic tool especially in developing the differential diagnosis.

Cervicofacial actinomycosis: an unusual cause of submandibular swelling.

Two cases of cervicofacial actinomycosis, resulting in submandibular swelling, are presented. How actinomycosis may present under the guise of malignancy is well demonstrated. Factors predisposing to the condition, difficulties with diagnosis, isolation methods of the causative organism and treatment of the disease, are discussed.

[Cervicofacial actinomycosis. A case study].

A case of cervicofacial actinomycosis, sited in the central hyoid region, is reported. The Authors have emphasized the difficulties of the diagnosis. It's very important hat the clinical diagnosis of actinomycotic infection be confirmed by a positive culture test. Actinomycosis can be suspected if multiple recurrent pustular swellings are present, associated with any trauma or teeth extractions.
