Outcomes of Patients With AL Amyloidosis and End-Stage Renal Disease After Initiation of Therapy.

In AL amyloidosis, advanced renal disease can impact treatment options and cause significant morbidity. Patients with ESRD requiring dialysis are a group with currently unmet needs. In this collaborative study, we identified 362 patients from three European centers who initiated dialysis during the course of the disease. Median survival after the initiation of dialysis was 40 months. Heart involvement, higher BMPC infiltration at diagnosis, and lambda light chain type were associated with inferior survival. At the time of dialysis initiation, cardiac biomarkers were prognostic for postdialysis

Multimodality Imaging in Hypertrophic Cardiomyopathy: From Phenotyping to Precision Clinical Management.

Hypertrophic cardiomyopathy (HCM) is a genetically and phenotypically heterogeneous myocardial disease characterized by unexplained left ventricular hypertrophy and a broad spectrum of clinical manifestations, ranging from an asymptomatic phenotype to heart failure, atrial and ventricular arrhythmias, sudden cardiac death, and advanced heart failure. Accurate diagnosis and risk stratification therefore require complex assessment. Multimodality cardiovascular imaging provides complementary structural, functional, and tissue characterization that cannot be obtained using a single imaging techniq

Arrhythmias in patients with transthyretin cardiac amyloidosis and the impact of contemporary disease-modifying therapies.

Transthyretin cardiac amyloidosis (ATTR-CM) is an increasingly recognized infiltrative cardiomyopathy characterized by extracellular deposition of misfolded transthyretin fibrils, leading to progressive myocardial stiffening, electrical remodeling, and conduction-system disease. These pathophysiological changes create a complex arrhythmic substrate, resulting in a high burden of atrial, bradyarrhythmia, and ventricular arrhythmias that significantly contribute to morbidity and mortality. Despite advances in disease recognition, the optimal management of arrhythmias in ATTR-CM remains challengi

Renal Outcomes in Hereditary Fibrinogen A Alpha-Chain Amyloidosis.

INTRODUCTION: Hereditary fibrinogen A alpha-chain (AFib) amyloidosis is characterized by progressive proteinuric chronic kidney disease (CKD) and early progression to end-stage renal disease (ESRD). A large series published in 2009 showed renal survival <5 years from diagnosis and renal transplant survival of <10 years. Since then, advances in diagnostic techniques and nephroprotective care have transformed both nephrology and amyloidosis. We evaluated contemporary diagnostic and renal outcomes in the largest cohort of patients with AFib amyloidosis to date. METHODS: A total of 194 patients w

Long-term Nlrp3 gain-of-function mutation promotes cardiac dysfunction independent of inflammasome in mice.

Cryopyrin-associated periodic syndrome (CAPS) is an autoinflammatory disease caused by gain-of-function mutations in human nucleotide-binding domain, leucine-rich repeat-containing protein 3 (NLRP3), which lead to constitutive inflammasome activation and excessive IL-1β secretion. Long-term IL-1 blockade has markedly improved patient outcomes by preventing systemic inflammation and amyloidosis. However, whether chronic NLRP3 mutation exerts inflammasome-independent effects on the cardiac function remains unknown. We generated Nlrp3C257Y/WT knock-in mice as a novel CAPS model. Long-term surviva

SPRY1 suppresses microglial fatty acid oxidation by retaining FATP3 on the endoplasmic reticulum in Alzheimer's disease.

BACKGROUND: Microglial metabolic dysfunction is increasingly recognized as a key driver of neuroinflammation in Alzheimer's disease (AD). However, the specific metabolic regulators linking lipid metabolism to pathological activation remain poorly defined. METHODS: We performed integrative transcriptomic analysis on four human AD brain datasets to identify core metabolic hubs. Sprouty homolog 1 (SPRY1) expression was examined in human post-mortem tissues, and its functional role was validated in primary microglia, Drosophila, and APP/PS1-21 mice using genetic knockdown, behavioral assays, and

Musculoskeletal Manifestations of Light Chain Amyloidosis: A Systematic Review.

INTRODUCTION: Diagnosing amyloid arthropathy remains challenging because its manifestations often mimic those of other inflammatory musculoskeletal diseases. This review aimed to describe the clinical, biological, and imaging characteristics of musculoskeletal involvement in light-chain (AL) Amyloidosis. METHODS: We conducted a review in accordance with the PRISMA guidelines. We searched SCOPUS and MEDLINE for case reports and series published between January 2000 and December 2023 that detailed musculoskeletal manifestations of AL amyloidosis. To minimize the risk of omitting relevant articl

Cystic pulmonary amyloidosis with lymphoid interstitial pneumonia in Sjögren's disease: a case report.

BACKGROUND: Interstitial lung diseases are commonly seen in association with Sjögren's disease, especially lymphoproliferative disorders, such as lymphoid interstitial pneumonia. In occasional and rare cases, pulmonary amyloidosis with cysts, nodules, and/or ground-glass opacites as radiological features, can also become a complication of Sjögren's disease. On the other hand, diffuse cystic lung disease, without nodules, concomitant with chylous fluid accumulation in female leads to initial suspicion of lymphangioleiomyomatosis. Interestingly, it's challenging to differentiate if diffuse cysti

Atrial Cardiomyopathy Through the Pathologist's Lens: From Tissue Remodeling to Clinical Expression.

Atrial cardiomyopathy (ACM) provides a useful framework for understanding structural, architectural, contractile, and electrophysiological disease of the atria. ACM and atrial fibrillation (AF) are closely linked but are not interchangeable: ACM favors arrhythmogenesis, whereas sustained AF can accelerate atrial injury. This review considers ACM from the pathologist's perspective, encompassing primary atrial disease and secondary remodeling associated with valvular disease and classical cardiomyopathies. The histologic spectrum includes cardiomyocyte hypertrophy and myocytolysis, interstitial

Disease progression in transthyretin cardiac amyloidosis treated with disease-modifying therapies: a multicenter real-world experience.

BACKGROUND: Transthyretin cardiac amyloidosis (ATTR-CM) is a progressive disease associated with substantial cardiovascular morbidity and mortality. The multimodal progression criteria proposed in 2025 integrate clinical, biomarker, and functional parameters to improve disease monitoring. However, evidence regarding their applicability in real-world practice remains limited. METHODS: We conducted a multicenter ambispective observational cohort study including 68 consecutive patients with ATTR-CM receiving disease-modifying therapies across hospitals in Castilla-La Mancha, Spain. Molecular Pathogenesis of Cerebral Amyloid Angiopathy in Alzheimer's Disease: The Role of the Central Fibrinolytic Cascade at the Neurovascular Unit.

Alzheimer's disease (AD) is increasingly recognized as a multifactorial neurodegenerative disorder involving complex neurovascular pathology. Departing from classic neurocentric paradigms, this review provides a conceptually novel perspective on how vascular inflammation and dysfunction actively drive AD progression through the lens of cerebral amyloid angiopathy (CAA) pathology. We evaluate the bidirectional cascade between amyloid-β pathology and the neurovascular unit, highlighted by cerebrovascular alterations and blood-brain barrier disruption. Crucial for cerebral homeostasis, the centra

Refining Risk Stratification in Light-Chain Cardiac Amyloidosis: A Single-Center Ambispective Cohort Study.

Background: Recent advances in disease awareness and diagnostic strategies have led to earlier diagnosis and improved survival in patients with light-chain cardiac amyloidosis (AL-CA). However, currently used prognostic staging systems are based on older cohorts and rely mainly on cardiac biomarkers, without incorporating patient-related clinical parameters. Aims: We sought to evaluate contemporary survival outcomes in patients with AL cardiac amyloidosis and to identify clinically available predictors that may improve current risk stratification. Methods: This was a single-center ambispective

The role of cardiac CT with late iodine enhancement in patients with acute cardiac symptoms and troponin elevation in the era of high-sensitivity assays.

OBJECTIVES: Diagnostic workup of patients with acute cardiac symptoms, troponin elevation, and no acute myocardial infarction (AMI) criteria is challenging. Cardiac CT angiography (CCTA) rules out obstructive coronary artery disease (CAD). Adding late iodine enhancement and extracellular volume fraction (CCTA + LIE/ECV) provides insights into myocardial injury. However, the impact on working diagnosis, agreement with discharge diagnosis, and mid-term safety of implementing CCTA + LIE/ECV in the emergency department remains uncertain. MATERIALS AND METHODS: Prospective study on consecutive pat
