Malaria-Associated Acute Kidney Injury: A Key Driver of Mortality in Endemic Regions.

BACKGROUND: Malaria remains a major global health challenge, particularly in endemic regions where severe disease contributes substantially to morbidity and mortality. Among its systemic complications, malaria-associated acute kidney injury (MAKI) is increasingly recognized as a significant yet underappreciated contributor to poor clinical outcomes. Despite advances in antimalarial therapy, MAKI continues to be associated with high mortality and growing evidence suggests a link to long-term renal sequelae, including chronic kidney disease (CKD). This review synthesizes current evidence on the

Growing into gender: Funerary practice of boys and girls in the Bronze Age Maros culture (2100 - 1700 BC).

BACKGROUND: Identification and preservation of the recurrent laryngeal nerve (RLN) is of major concern in surgery of the thyroid gland. The purpose of this study was to review the surgical anatomy of the nerve and to describe its relationship to other important structures. METHODS: A total of 325 patients were accrued in this prospective non-randomized study from January 1999 to December 2000. All patients who had total, subtotal and hemithyroidectomies were included in this study. Each side of the thyroid gland was considered as a separate unit in the analysis of the results. RESULTS: Two h

Aldosterone-identified targets for optimal sodium and potassium supplementation in intestinal failure.

BACKGROUND: Parenteral support (PS) is a life-saving organ replacement therapy for patients with intestinal failure (IF). However, practical strategies to establish optimal PS sodium and potassium remain poorly defined. We investigated whether elevated plasma renin and aldosterone reflect suboptimal electrolyte supplementation and could help guide physiologically optimal sodium and potassium supplementation in patients with IF. METHODS: Real-world monitoring data from adult IF patients receiving long-term PS were analyzed using mixed-effects models for repeated measurements. Associations betw

Calcified Constrictive Pericarditis (Concretio Cordis) in a Renal Transplant Recipient: A Multimodal, Hemodynamic, and Surgical Challenge.

Calcified constrictive pericarditis is an uncommon but severe clinical entity that causes restrictive right-sided heart failure. Its occurrence in renal transplant recipients presents a highly complex scenario due to interactions with immunosuppression, the risk of graft dysfunction caused by hemodynamic instability, and the surgical challenges of decortication. We report a 45-year-old male, renal transplant recipient (2018) on maintenance therapy with tacrolimus, everolimus, and prednisone, who was evaluated for progressive refractory ascites that masked the underlying cardiovascular disease.

Carcinoid syndrome revealed by chronic diarrhea and isolated severe tricuspid regurgitation.

Carcinoid syndrome is an uncommon manifestation of metastatic neuroendocrine tumor (NET) and may present without classic flushing or bronchospasm. We describe a woman in her 50s with hypertension treated with hydrochlorothiazide who presented with confusion, severe hyponatremia with a serum sodium concentration of 114 mmol/L (reference range, 135-145 mmol/L), chronic watery diarrhea, bilateral leg edema, and 40-lb weight loss. Computed tomography (CT) revealed a calcified mesenteric mass and multiple hepatic lesions. Serum chromogranin A was 1434 ng/mL (SI: 1434 µg/L) (reference range, <100 ng

Diagnosis and Management of Acute and Chronic Lithium-Associated Nephrotoxicity.

Lithium remains a mainstay of therapy for bipolar disease and refractory depression, with future potential for expanded use on the basis of novel data demonstrating its immunomodulatory and neuroprotective effects. This drug accumulates intracellularly via sodium transport pathways, and its effects on complex cellular signaling mechanisms, including glycogen synthase kinase-3 β inhibition, form the basis of its therapeutic efficacy and toxicity. Increased intracellular levels disrupt cellular processes and trigger injury via mitochondrial dysfunction and oxidative stress. Lithium has a narrow

Managing Rheumatic Mitral Stenosis Complicated by Shock and Cardiac Cirrhosis Beyond Guidelines.

BACKGROUND: Rheumatic mitral stenosis (MS) is the most common form of valvular heart disease worldwide, which could lead to cardiac cirrhosis when associated with severe tricuspid regurgitation. This report presents how we manage patients with cardiogenic obstructive shock due to severe MS complicated by edema but with intravascular volume depletion due to hypoalbuminemia secondary to cardiac cirrhosis. CASE SUMMARY: A 66-year-old woman who was previously diagnosed with rheumatic MS was evaluated for refractory dyspnea, shock, and peripheral edema. The examination revealed cardiac cirrhosis a

Severe Anemia and Acute Kidney Injury After Pegloticase in a Patient With Glucose-6-Phosphate Dehydrogenase Deficiency.

Pegloticase is an effective therapy for refractory gout but carries a risk of oxidative hemolysis in patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency. Reported cases of hemolytic complications remain limited, particularly those with delayed presentation, where traditional markers such as haptoglobin and bilirubin may have normalized, making diagnosis challenging. We describe a 61-year-old female with known G6PD deficiency who presented with near syncope, nausea, vomiting, and volume depletion four days after receiving her first dose of intravenous pegloticase and recently star

Comment on "Lymph Node Yield Per Neck Level as a Predictor of Recurrence in Oral Tongue Squamous Cell Carcinoma: A Retrospective Observational Study".

Numerous anatomical and functional changes occurring in the aging kidney lead to reduced glomerular filtration rate, lower renal blood flow and impaired renal autoregulation. The elderly are especially vulnerable to the development of renal dysfunction and in this population acute renal failure (ARF) is a common problem. ARF is often iatrogenic and multifactorial; common iatrogenic combinations include pre-existing renal dysfunction and exposure to nephrotoxins such as radiocontrast agents or aminoglycosides, use of NSAIDs in patients with congestive cardiac failure and use of ACE inhibitors a

Serial bedside echocardiography identifies children at risk of recurrent Dengue shock: a prospective cohort study.

BACKGROUND: Dengue shock syndrome (DSS) is characterised by vascular leakage, although myocardial impairment may contribute to haemodynamic instability. The dynamic interplay between volume depletion and cardiac function during resuscitation remains incompletely understood. METHODS: We conducted a prospective cohort study of children aged 6–15 years with DSS admitted to Hospital for Tropical Diseases in Ho Chi Minh City (2018–2020). Serial bedside echocardiography was performed at shock presentation and at 1, 3, 6, 12, and 24 h post-resuscitation, daily until hospital discharge, at follow-up 1

Sulfadiazine-induced crystalline nephropathy: a case report of a reversible cause of acute kidney injury in advanced HIV.

INTRODUCTION: Crystal-induced acute kidney injury (AKI) is caused by the intratubular precipitation of crystals, which results in obstruction. Crystal-induced AKI mostly occurs as a result of acute uric acid nephropathy and following the administration of drugs or toxins that are poorly soluble. CASE DESCRIPTION: A 23-year-old man presented with AIDS and 1 week history of right sided weakness. Brain MRI showed bilateral ring enhancing lesions. Patient was empirically started on pyrimethamine, sulfadiazine and leucovorin. Brain biopsy confirmed the diagnosis of toxoplasmosis. Hospital course wa

Perioperative pharmacokinetics of lenacapavir during massive hemorrhage in a patient with hemophilia a undergoing liver transplantation.

Lenacapavir (LEN) is a long-acting HIV-1 capsid inhibitor administered subcutaneously every six months; however, its pharmacokinetic behavior during massive hemorrhage is unknown. We report a man in his 60s with multidrug-resistant HIV-1 infection and severe hemophilia A who underwent living-donor liver transplantation for HCV-related hepatocellular carcinoma. After a 2-week oral loading phase, LEN (927 mg) was administered four weeks before surgery. Liver transplantation was performed with intraoperative blood loss of 7046 g, approximately 1.5 times the estimated circulating blood volume. Des

Rapid recovery from near-fatal immune reconstitution inflammatory syndrome and acute respiratory distress syndrome following neutropenic recovery by cytokine adsorption.

BACKGROUND: In the presence of clinical or microbiological evidence of pneumonia, emerging leukocytes in recovery from neutropenia undergo substantial migration to the site of infection, where they elicit immunogenic reactions. The administration of granulocyte colony-stimulating factors is a risk factor for dysregulation of this immune response, which can lead to ARDS. However, there is a paucity of knowledge regarding the severe forms of near-fatal IRIS. CASE PRESENTATION: This case report describes the successful treatment of a hematologic patient with life-threatening Immune Reconstitutio

Thiamine responsive lactic acidosis in patients not in shock: A case series and literature review.

Lactic acidosis is a common feature of patients presenting with sepsis, volume depletion due to ketoacidosis, tissue ischemia, shock and liver disease. These cases typically presume tissue hypoxia and anaerobic metabolism leading to lactic acidosis. Thiamine deficiency is an underappreciated cause of type B lactic acidosis, which is not caused by anaerobic metabolism. We present a case series of 11 patients admitted to the medicine service who were not in shock, and who had a lactic acidosis (average of 4.5 mmol/L) that improved after thiamine administration (resolving to <2 mmol/L in 10 of 11

Unique aspects of acute kidney injury associated with infections in the tropics: presentation, challenges, and opportunities.

"The tropics" currently account for around 40% of the world's population and face unique environmental challenges, with limited resources, a deficient health system, and a rudimentary sanitary infrastructure, in addition to a lack of adequate preventive measures and poor surveillance programs. Cases of acute kidney injury in the tropics are to a very large extent driven by infections such as leptospirosis, scrub typhus, dengue, malaria, Ebola virus, yellow fever, and HIV, with a pathogenesis driven by volume depletion, hypotension, and immune-mediated mechanisms. Many of these infections also

Beyond the Battlefield: Examining Acute Kidney Injury in Yemen's Humanitarian Crisis.

BACKGROUND: Acute kidney injury (AKI) represents a growing yet under-recognized public health threat in Yemen, where ongoing conflict has exacerbated vulnerabilities to infectious diseases and waterborne illnesses. Sepsis and severe volume depletion-secondary to diarrheal diseases such as cholera-are leading contributors to AKI incidence, especially in a region with a collapsing health infrastructure and limited access to dialysis. OBJECTIVE: This review consolidates existing evidence on the burden, causes, and outcomes of infection-related and dehydration-induced AKI in Yemen. It emphasizes

Feasibility and Effectiveness of a Low-Code AI Platform for Developing a Neonatal Multimodal Pain Classification Model.

BACKGROUND: Artificial intelligence (AI) has advanced neonatal pain recognition, yet a significant gap persists in translating complex algorithms into practical clinical applications. Low-code AI development platforms, which simplify and automate model creation, offer a potential solution to bridge this gap between research and bedside practice. OBJECTIVE: This study aimed to explore the feasibility of constructing and validating a neonatal multimodal pain classification model using a commercial low-code AI development platform (EasyDL). The objective was to develop an accessible, cost-effect

Impact of peritoneal dialysis on weight gain in a patient with anorexia nervosa.

Anorexia nervosa (AN) is an eating disorder characterized by restriction of energy intake leading to a significantly low body weight, and intense fear of gaining weight. Severe electrolyte changes such as hypokalemia and hypophosphatemia; and alterations in water metabolism such as hyponatremia and edema, can occur in patients with AN. Hypokalemia and chronic volume depletion may lead to acute kidney injury (AKI) and chronic kidney disease (CKD). There are few reports of patients with AN who require maintenance dialysis. Therefore, it is unclear whether hemodialysis (HD) or peritoneal dialysis
